    Challenging Social Cognition Models of Adherence:Cycles of Discourse, Historical Bodies, and Interactional Order

    Attempts to model individual beliefs as a means of predicting how people follow clinical advice have dominated adherence research, but with limited success. In this article, we challenge assumptions underlying this individualistic philosophy and propose an alternative formulation of context and its relationship with individual actions related to illness. Borrowing from Scollon and Scollonā€™s three elements of social action ā€“ ā€œhistorical body,ā€ ā€œinteraction order,ā€ and ā€œdiscourses in placeā€ ā€“ we construct an alternative set of research methods and demonstrate their application with an example of a person talking about asthma management. We argue that talk- or illness-related behavior, both viewed as forms of social action, manifest themselves as an intersection of cycles of discourse, shifting as individuals move through these cycles across time and space. We finish by discussing how these dynamics of social action can be studied and how clinicians might use this understanding when negotiating treatment with patients

    Toward a Harmonization for Using in situ Nutrient Sensors in the Marine Environment

Signiļ¬cant progress has been made over recent decades in improving the analysis and data quality for traditional laboratory measurements of nutrients. Similar efforts are required to establish high-quality data outputs from in situ nutrient sensors, which are rapidly becoming integral components of ocean observing systems. This paper suggests using the good practices routine established for laboratory reference methods to propose a harmonized setofdeploymentprotocolsandofqualitycontrolproceduresfornutrientmeasurements obtained from in situ sensors. These procedures are intended to establish a framework to standardize the technical and analytical controls carried out on the three main types of in situ nutrient sensors currently available (wet chemical analyzers, ultraviolet optical sensors, electrochemical sensors) for their deployments on all kinds of platform. The routine reference controls that can be applied to the sensors are listed for each step of sensor use: initial qualiļ¬cation under controlled conditions in the laboratory, preparation of the sensor before deployment, ļ¬eld deployment and ļ¬nally the sensor recovery. The fundamental principles applied to the laboratory reference method are then reviewed in termsofthecalibrationprotocol,instrumentalinterferences,environmentalinterferences, external controls, and method performance assessment. Data corrections (linearity, sensitivity, drifts, interferences and outliers) are ļ¬nally identiļ¬ed along with the concepts and calculations for qualiļ¬cation for both real time and time delayed data. This paper emphasizes the necessity of future collaborations between research groups, referenceaccredited laboratories, and technology developers, to maintain comparability of the concentrationsreportedforthevariousnutrientparametersmeasuredbyinsitusensors

    Womenā€™s psychosocial outcomes following an emergency caesarean section: a systematic literature review

    BACKGROUND: Given the sudden and unexpected nature of an emergency caesarean section (EmCS) coupled with an increased risk of psychological distress, it is particularly important to understand the psychosocial outcomes for women. The aim of this systematic literature review was to identify, collate and examine the evidence surrounding womenā€™s psychosocial outcomes of EmCS worldwide. METHODS: The electronic databases of EMBASE, PubMed, Scopus, and PsycINFO were searched between November 2017 and March 2018. To ensure articles were reflective of original and recently published research, the search criteria included peer-reviewed research articles published within the last 20 years (1998 to 2018). All study designs were included if they incorporated an examination of womenā€™s psychosocial outcomes after EmCS. Due to inherent heterogeneity of study data, extraction and synthesis of both qualitative and quantitative data pertaining to key psychosocial outcomes were organised into coherent themes and analysis was attempted. RESULTS: In total 17,189 articles were identified. Of these, 208 full text articles were assessed for eligibility. One hundred forty-nine articles were further excluded, resulting in the inclusion of 66 articles in the current systematic literature review. While meta-analyses were not possible due to the nature of the heterogeneity, key psychosocial outcomes identified that were negatively impacted by EmCS included post-traumatic stress, health-related quality of life, experiences, infant-feeding, satisfaction, and self-esteem. Post-traumatic stress was one of the most commonly examined psychosocial outcomes, with a strong consensus that EmCS contributes to both symptoms and diagnosis. CONCLUSIONS: EmCS was found to negatively impact several psychosocial outcomes for women in particular post-traumatic stress. While investment in technologies and clinical practice to minimise the number of EmCSs is crucial, further investigations are needed to develop effective strategies to prepare and support women who experience this type of birth.Madeleine Benton, Amy Salter, Nicole Tape, Chris Wilkinson and Deborah Turnbul

    Measurement of the 18Ne(a,p_0)21Na reaction cross section in the burning energy region for X-ray bursts

    The 18Ne(a,p)21Na reaction provides one of the main HCNO-breakout routes into the rp-process in X-ray bursts. The 18Ne(a,p_0)21Na reaction cross section has been determined for the first time in the Gamow energy region for peak temperatures T=2GK by measuring its time-reversal reaction 21Na(p,a)18Ne in inverse kinematics. The astrophysical rate for ground-state to ground-state transitions was found to be a factor of 2 lower than Hauser-Feshbach theoretical predictions. Our reduced rate will affect the physical conditions under which breakout from the HCNO cycles occurs via the 18Ne(a,p)21Na reaction.Comment: 5 pages, 3 figures, accepted for publication on Physical Review Letter

    The characteristics of sexual abuse in sport: A multidimensional scaling analysis of events described in media reports

    Most research on sexual abuse has been conducted within family settings (Fergusson & Mullen, 1999). In recent years, following several high profile convictions and scandals, research into sexual abuse has also encompassed institutional and community settings such as sport and the church (Gallagher, 2000; Wolfe et al., 2003). Research into sexual abuse in sport, for example, began with both prevalence studies (Kirby & Greaves, 1996; Leahy, Pretty & Tenenbaum, 2002) and qualitative analyses of the processes and experiences of athlete sexual abuse (Brackenridge, 1997; Cense & Brackenridge, 2001, Toftegaard Nielsen, 2001). From such work, descriptions of the modus operandi of abusers in sport, and the experiences and consequences for athlete victims, have been provided, informing both abuse prevention work and coach education. To date, however, no study has provided empirical support for multiple associations or identified patterns of sex offending in sport in ways that might allow comparisons with research-generated models of offending outside sport. This paper reports on an analysis of 159 cases of criminally defined sexual abuse, reported in the print media over a period of 15 years. The main aim of the study was to identify the nature of sex offending in sport focusing on the methods and locations of offences. The data were analysed using multidimensional scaling (MDS), as a data reduction method, in order to identify the underlying themes within the abuse and explore the inter-relationships of behaviour, victim and context variables. The findings indicate that there are specific themes that can be identified within the perpetrator strategies that include ā€˜intimateā€™, ā€˜aggressiveā€™, and ā€˜ā€™dominantā€™ modes of interaction. The same patterns that are described here within the specific context of sport are consistent with themes that emerge from similar behavioural analyses of rapists (Canter & Heritage, 1990; Bishopp, 2003) and child molester groups (Canter, Hughes & Kirby, 1998). These patterns show a correspondence to a broader behavioural model ā€“ the interpersonal circumplex (e.g., Leary 1957).     The effect of N-acetyl-aspartyl-glutamate and N-acetyl-aspartate on white matter oligodendrocytes

    Elevations of the levels of N-acetyl-aspartyl-glutamate (NAAG) and N-acetyl-aspartate (NAA) are associated with myelin loss in the leucodystrophies Canavan's disease and Pelizaeus-Merzbacher-like disease. NAAG and NAA can activate and antagonize neuronal N-methyl-D-aspartate (NMDA) receptors, and also act on group II metabotropic glutamate receptors. Oligodendrocytes and their precursors have recently been shown to express NMDA receptors, and activation of these receptors in ischaemia leads to the death of oligodendrocyte precursors and the loss of myelin. This raises the possibility that the failure to develop myelin, or demyelination, occurring in the leucodystrophies could reflect an action of NAAG or NAA on oligodendrocyte NMDA receptors. However, since the putative subunit composition of NMDA receptors on oligodendrocytes differs from that of neuronal NMDA receptors, the effects of NAAG and NAA on them are unknown. We show that NAAG, but not NAA, evokes an inward membrane current in cerebellar white matter oligodendrocytes, which is reduced by NMDA receptor block (but not by block of metabotropic glutamate receptors). The size of the current evoked by NAAG, relative to that evoked by NMDA, was much smaller in oligodendrocytes than in neurons, and NAAG induced a rise in [Ca^{2+}]i in neurons but not in oligodendrocytes. These differences in the effect of NAAG on oligodendrocytes and neurons may reflect the aforementioned difference in receptor subunit composition. In addition, as a major part of the response in oligodendrocytes was blocked by tetrodotoxin (TTX), much of the NAAG-evoked current in oligodendrocytes is a secondary consequence of activating neuronal NMDA receptors. Six hours exposure to 1 mM NAAG did not lead to the death of cells in the white matter. We conclude that an action of NAAG on oligodendrocyte NMDA receptors is unlikely to be a major contributor to white matter damage in the leucodystrophies
