    Lethal Clostridium difficile Colitis Associated with Paclitaxel and Carboplatin Chemotherapy in Ovarian Carcinoma: Case Report and Review of the Literature

    Clostridium difficile colitis, although rare, could represent a serious complication following chemotherapy. Prior antibiotic use has been considered the single most important risk factor in the development of C. difficile infection. Recently, the association between antineoplastic therapy and C. difficile-associated diarrhea in the absence of a prior antibiotic therapy has become more apparent. A 75-year-old woman with serous adenocarcinoma of the ovary developed lethal pancolitis caused by C. difficile after five cycles of paclitaxel- and carboplatin-based chemotherapy. She presented with diarrhea, coffee-ground emesis, and oliguria and was hospitalized immediately for aggressive treatment. Despite all the medical efforts, her condition worsened and she died after twenty days. We describe the second case reported of a patient developing a severe C. difficile colitis following chemotherapy without any recent antibiotic use and review the data of the literature, emphasizing the need to a prompt diagnosis and management that can significantly decrease the morbidity and life-threatening complications associated with this infection

    Ultrasound diagnosis of serous surface papillary borderline ovarian tumor: A case series with a review of the literature

    Serous surface papillary borderline ovarian tumors (SSPBOTs) are a rare morphologic variant of serous ovarian tumors that are typically confined to the ovarian surface, while the ovaries themselves tend to appear normal in size and shape. In this report, we describe the findings from five premenopausal women diagnosed with SSPBOTs, in whom ultrasound showed grossly normal ovaries that were partially or wholly covered with irregular solid tumors. In all five cases, histologic examination showed evidence of borderline serous tumors. These findings demonstrate that SSPBOTs can be diagnosed on a preoperative sonographic examination, which could facilitate conservative, fertility-sparing surgery in young women affected by this condition. © 2015 Wiley Periodicals, Inc.     Evaluating LoRaWAN connectivity in a marine scenario

    The growing need for interoperability among the different oceanic monitoring systems to deliver services able to answer the requirements of stakeholders and end-users led to the development of a low-cost machine-to-machine communication system able to guarantee data reliability over marine paths. In this framework, an experimental evaluation of the performance of long-range (LoRa) technology in a fully operational marine scenario has been proposed. In-situ tests were carried out exploiting the availability of (i) a passenger vessel and (ii) a research vessel operating in the Ligurian basin (North-Western Mediterranean Sea) both hosting end-nodes, and (iii) gateways positioned on mountains and hills in the inland areas. Packet loss ratio, packet reception rate, received signal strength indicator, signal to noise, and expected signal power ratio were chosen as metrics in line of sight and not the line of sight conditions. The reliability of Long Range Wide Area Network (LoRaWAN) transmission over the sea has been demonstrated up to more than 110 km in a free space scenario and for more than 20 km in a coastal urban environment

    Mitochondria form cholesterol-rich contact sites with the nucleus during retrograde response

    Cholesterol metabolism is pivotal to cellular homeostasis, hormones production, and membranes composition. Its dysregulation associates with malignant reprogramming and therapy resistance. Cholesterol is trafficked into the mitochondria for steroidogenesis by the transduceome protein complex, which assembles on the outer mitochondrial membrane (OMM). The highly conserved, cholesterol-binding, stress-reactive, 18kDa translocator protein (TSPO), is a key component of this complex. Here, we modulate TSPO to study the process of mitochondrial retrograde signalling with the nucleus, by dissecting the role played by cholesterol and its oxidized forms. Using confocal and ultrastructural imaging, we describe that TSPO enriched mitochondria, remodel around the nucleus, gathering in cholesterol-enriched domains (or contact sites). This communication is controlled by HMG-CoA reductase inhibitors (statins), molecular and pharmacological regulation of TSPO. The described Nucleus-Associated Mitochondria (NAM) seem to be implementing survival signalling in aggressive forms of breast cancer. This work therefore provides the first evidence for a functional and bio-mechanical tethering between mitochondria and nucleus, as being the basis of pro-survival mechanisms, thus establishing a new paradigm in cross-organelle communication via cholesterol re-distribution

    Circulating Cell-Free DNA in Dogs with Mammary Tumors: Short and Long Fragments and Integrity Index

    Circulating cell-free DNA (cfDNA) has been considered an interesting diagnostic/prognostic plasma biomarker in tumor-bearing subjects. In cancer patients, cfDNA can hypothetically derive from tumor necrosis/apoptosis, lysed circulating cells, and some yet unrevealed mechanisms of active release. This study aimed to preliminarily analyze cfDNA in dogs with canine mammary tumors (CMTs). Forty-four neoplastic, 17 non-neoplastic disease-bearing, and 15 healthy dogs were recruited. Necrosis and apoptosis were also assessed as potential source of cfDNA on 78 CMTs diagnosed from the 44 dogs. The cfDNA fragments and integrity index significantly differentiated neoplastic versus non-neoplastic dogs (P<0.05), and allowed the distinction between benign and malignant lesions (P<0.05). Even if without statistical significance, the amount of cfDNA was also affected by tumor necrosis and correlated with tumor size and apoptotic markers expression. A significant (P<0.01) increase of Bcl-2 in malignant tumors was observed, and in metastatic CMTs the evasion of apoptosis was also suggested. This study, therefore, provides evidence that cfDNA could be a diagnostic marker in dogs carrying mammary nodules suggesting that its potential application in early diagnostic procedures should be further investigated

    Therapeutic Strategies in HCC: Radiation Modalities

    Patients with hepatocellular carcinoma (HCC) comply with an advanced disease and are not eligible for radical therapy. In this distressed scenario new treatment options hold great promise; among them transarterial chemoembolization (TACE) and transarterial metabolic radiotherapy (TAMR) have shown efficacy in terms of both tumor shrinking and survival. External radiation therapy (RTx) by using novel three-dimensional conformal radiotherapy has also been used for HCC patients with encouraging results while its role had been limited in the past for the low tolerance of surrounding healthy liver. The rationale of TAMR derives from the idea of delivering exceptional radiation dose locally to the tumor, with cell killing intent, while preserving normal liver from undue exposition and minimizing systemic irradiation. Since the therapeutic efficacy of TACE is being continuously disputed, the TAMR with 131I Lipiodol or 90Y microspheres has gained consideration providing adequate therapeutic responses regardless of few toxicities. The implementation of novel radioisotopes and technological innovations in the field of RTx constitutes an intriguing field of research with important translational aspects. Moreover, the combination of different therapeutic approaches including chemotherapy offers captivating perspectives.     Temperature-processing effect on equine semen cryopreservation

    Advances in semen cryopreservation, alongside the implementation of new laboratory techniques to better protect sperm cells viability, have allowed increased quality and fertility of frozen semen in mammals. However, this process can impair the acrosomal integrity, viability, and motility of spermatozoa to some degree. Deleterious effects of cryopreservation are largely due to shock temperature during the freezing process that causes osmotic and oxidative stress. The aim of the study was to assess whether an early semen cooling could influence main parameters of equine frozen-thawed semen. Eight healthy and fertile Sicilian Oriental Purebred stallions underwent semen collection by using a Missouri artificial vagina. Semen samples were processed according to two different freezing protocols. Semen was extended 1:1 ratio (semen:extender) in INRA-96 extender prewarmed at 37°C; thereafter in protocol A diluted samples were placed into conical tubes with 2ml Maxifreeze cushion and centrifuged at room temperature for 10 min at 600g. After removal of supernatant and cushion, the sperm pellet was resuspended with INRA-freeze to a final concentration of &lt;200 × 106 spermatozoa/ml. ..
