    A blood microRNA classifier for the prediction of ICU mortality in COVID-19 patients: a multicenter validation study.

    Background The identification of critically ill COVID-19 patients at risk of fatal outcomes remains a challenge. Here, we first validated candidate microRNAs (miRNAs) as biomarkers for clinical decision-making in critically ill patients. Second, we constructed a blood miRNA classifier for the early prediction of adverse outcomes in the ICU. Methods This was a multicenter, observational and retrospective/prospective study including 503 critically ill patients admitted to the ICU from 19 hospitals. qPCR assays were performed in plasma samples collected within the first 48 h upon admission. A 16-miRNA panel was designed based on recently published data from our group. Results Nine miRNAs were validated as biomarkers of all-cause in-ICU mortality in the independent cohort of critically ill patients (FDR < 0.05). Cox regression analysis revealed that low expression levels of eight miRNAs were associated with a higher risk of death (HR from 1.56 to 2.61). LASSO regression for variable selection was used to construct a miRNA classifier. A 4-blood miRNA signature composed of miR-16-5p, miR-192-5p, miR-323a-3p and miR-451a predicts the risk of all-cause in-ICU mortality (HR 2.5). Kaplan‒Meier analysis confirmed these findings. The miRNA signature provides a significant increase in the prognostic capacity of conventional scores, APACHE-II (C-index 0.71, DeLong test p-value 0.055) and SOFA (C-index 0.67, DeLong test p-value 0.001), and a risk model based on clinical predictors (C-index 0.74, DeLong test-p-value 0.035). For 28-day and 90-day mortality, the classifier also improved the prognostic value of APACHE-II, SOFA and the clinical model. The association between the classifier and mortality persisted even after multivariable adjustment. The functional analysis reported biological pathways involved in SARS-CoV infection and inflammatory, fibrotic and transcriptional pathways.     Entre 1973 y 1992, fueron tratados en nuestra institución 20 casos de fracturas patológicas a nivel de la cadera, secundarias a metástasis esqueléticas. Se realizó tratamiento quirúrgico mediante osteosíntesis o artroplastias según la localización y la extensión de la lesión. En 8 casos, la fractura fué subcapital, en 6 fué pertrocantérea y en otros 6 casos fué subtrocantérea. En el tratamiento quirúrgico, se recalca la importancia de la fijación interna inmediata con la adición de metilmetacrilato y el uso de la radioterapia postoperatoria. Siempre se obtuvo la mejoría del dolor y un resultado funcional bueno en el 70% de los casos. En 7 casos, aparecieron complicaciones locales. El tratamiento quirúrgico debe ser agresivo en estos pacientes, ya que la rehabilitación inmediata es primordial en su calidad de vida.From 1973 to 1992, 20 cases of pathological fractures around the hip secondary to skeletal metastases were treated in our institution. The surgical treatment was performed by internal fixation or arthroplasty according the location and extension of the lesion. In 8 cases the fracture was located in the femoral neck, 6 cases were intertrochanteric fractures and 6 cases were subtrochanteric fractures. In the surgical management the importance of inmediately internal fixation, with the use of methylnietacrilate and post surgical radiotherapy are justified. Always the pain was relieved and the functional result was good in 70%. In 7 cases were local complications. The surgical treatment should be more agresive in these special patients, because the inmediately rehabilitation is essential in your quality lifes

    Generation of Bioelectricity Using Molasses as Fuel in Microbial Fuel Cells

    The large amount of molasses that are generated in sugar-processing companies are not always redistributed for commercialization in by-products. Because of this, the present research uses these wastes as fuel in low-cost, lab-scale, single-chamber microbial fuel cells. Zinc and copper electrodes were used as electrodes and 100 mL of molasse in the chamber as fuel, managing to generate current and voltage peaks of 1.73 ± 0.13 mA and 0.953 ± 0.142 V. In monitoring the conductivity of the substrate, a maximum peak of 111.156 ± 8.45 mS/cm was observed, and a slightly acidic pH was observed throughout the monitoring. It was possible to obtain a power density of 5.45 ± 0.31 W/cm2 for a current density of 308.06 mA/cm2, while the yeast count showed a logarithmic curve throughout the monitoring. Finally, the molecular technique identified 100% of the special C. boidinii present in the anodic electrode. This research will give great benefits to sugar companies because they will be able to generate electricity using the molasses that cannot generate by-products

    Marker-free transgenic rice lines with a defensin gene are potentially active against phytopathogenic fungus Sarocladium oryzae

    In this work it was developed marker-free transgenic indica rice plants (cv J-104) by biolistic co-transformation and segregation approach. We attempted to express the NmDef02 antifungal defensin. Primary transformants were regenerated from embryogenic callus on culture medium with 50 mg/L hygromycin. Screening of hpt-marker-free transgenic lines was made by PCR in T1 progeny lines, germinated on semisolid medium without hygromycin. Relative expression of NmDef02 mRNA was examined by quantitative RT-PCR in marker-free T1 plants. In vitro antifungal test was performed by disk diffusion assay against Sarocladium oryzae. PCR assay verified that 15.12% of T1 plants were marker-free (NmDef02+/hpt−). RT-PCR analysis indicated that NmDef02 gene was successfully transcribed and the transgenic lines displayed different expression levels of the NmDef02 cDNA. Protein extracts of marker-free lines with high relative expression of NmDef02 inhibited fungus mycelial growth around disks. In contrast, it was confirmed fungus proliferation on disks impregnated with protein extracts of non-transgenic plants. The results of the present work demonstrated that the expression of the NmDef02 defensin in transgenic rice plants is effective against the phytopathogenic fungus Sarocladium oryzae under in vitro conditions. Thus, NmDef02 defensin could be a useful tool for J-104 rice improvement

    Magnetic operations: a little fuzzy physics?

    We examine the behaviour of charged particles in homogeneous, constant and/or oscillating magnetic fields in the non-relativistic approximation. A special role of the geometric center of the particle trajectory is elucidated. In quantum case it becomes a 'fuzzy point' with non-commuting coordinates, an element of non-commutative geometry which enters into the traditional control problems. We show that its application extends beyond the usually considered time independent magnetic fields of the quantum Hall effect. Some simple cases of magnetic control by oscillating fields lead to the stability maps differing from the traditional Strutt diagram.Comment: 28 pages, 8 figure
