    Aplicación de un tratamiento cognitivo-comportamental para la fibromiálgia en una Unidad de Salud Mental. Un estudio piloto.

    Introducción: Una Unidad de Salud Mental (USM) es un dispositivo al que acuden con frecuencia pacientes diagnosticados de fibromialgia (FM) derivados por los servicios de atención primaria. Pese a la existencia de tratamientos cognitivo-comportamentales (TCC) específicos para la FM, en las USM no suelen instaurarse este tipo de protocolos. Los pacientes con FM suelen recibir tratamiento dirigido a las psicopatologías conco-mitantes pero no específicamente para la FM. Pensamos que la TCC está especialmente indicada para ser aplicada en las USM, ya que son programas de tratamiento grupales con un número limitado de sesiones, lo que resulta muy atractivo desde un punto de vista de coste-beneficio. Metodología: En el presente estudio, pretendemos compro-bar, en forma de un estudio piloto, la posibilidad de implantar un programa de trata-miento psicológico (TCC) en grupo en una USM, con el fin de explorar si los pacientes con FM pueden lograr beneficios en variables psicológicas que impliquen una mejora en el manejo del dolor crónico y, por tanto, en la calidad de vida. En el estudio partici-paron 13 mujeres que recibieron la TCC en 12 sesiones. Resultados: Los resultados en el post-tratamiento indicaron que el impacto de la FM en la vida de las pacientes, la intensidad e interferencia de la fatiga, la interferencia del dolor y la calidad de vida mejoraron. Además aumentó la persistencia en la tarea y la búsqueda de apoyo social. Conclusiones: Nuestro trabajo aporta datos preliminares que apoyan la utilidad de los TCC en el tratamiento de la FM en el contexto de una USM.Introduction: A Mental Health Unit (MHU) is a service that patients diagnosed with fi-bromyalgia (FM) frequently attend to. These patients are referred to the MHU from their Primary Care Centres (PCC). In spite of the existence of Cognitive Behavioural Thera-pies (CBT) which are specific to FM, it is not usual to establish this kind of protocols in MHU. Patients with FM generally receive treatment focused on the concurrent patholo-gies but not on the treatment of FM itself. We believe that CBT is particularly suitable for application in the MHU, as it is based on group treatment programs with a limited number of sessions. It seems to be very attractive from a cost-efficacy point of view. Method: In the present study we intend to prove, as a pilot study, the possibility of implementing a CBT in groups at USM, in order to explore whether patients with FM can achieve ben-efits in psychological variables as an improvement in the management of chronic pain and therefore the quality of life. 13 women participated in the study, and they all re-ceived CBT group program in 12 sessions. Results: The results at post-treatment indi-cated that the impact of FM in the lives of patients, intensity, and interference of fatigue, pain interference and quality of life improved. Task persistence and seeking social sup-port also increased. Conclusions: Our study provides preliminary data supporting the utility of CBT treating FM in the context of MHU

    Jørgensen examines the emergence of Tranquebar as a heritage town in post-colonial India through the diverse, sometimes competing interests and claims of local residents, state-oriented institutions, scholars and policy makers, non-governmental organisations, and private entrepreneurs. Tharangambadi, Nagapattinam, Tamil Nadu, colloquially known as Tranquebar, is a fishing community of about 23000 people on the Coromandel Coast of India. Tranquebar was one of two trading posts that the Danish East India Company established in the 1600s, and that were taken over by the Danish Crown in 1650. The British East India Company acquired Denmark‟s Indian territories in 1845, and they were subsequently taken over in 1857 by the British Crown when the Company was dissolved. These territories were transferred to the Indian national government in 1947, when India gained independence from the British Crown. Jørgensen investigates the use of the past, that is, the making of Tranquebar into a destination for heritage tourism based on its Danish colonial history. The study takes place in the aftermath of the Indian Ocean earthquake on December 26, 2004. This major seismic event centred in the west coast of Sumatra, resulted in a powerful tsunami that radiated from there toward each country that shares a coastline on the Indian Ocean. The tsunami inundation in turn, caused the destruction of infrastructure, towns and villages, the displacement of coastal communities and the loss of human life. Tranquebar was severely impacted and since the tsunami, initiatives to promote economic growth in the town have intensified

    Probing the Pore Drug Binding Site of Microtubules with Fluorescent Taxanes: Evidence of Two Binding Poses

    The pore site in microtubules has been studied with the use of Hexaflutax, a fluorescent probe derived from paclitaxel. The compound is active in cells with similar effects to paclitaxel, indicating that the pore may be a target to microtubule stabilizing agents. While other taxanes bind microtubules in a monophasic way, thus indicating a single type of sites, Hexaflutax association is biphasic. Analysis of the phases indicates that two different binding sites are detected, reflecting two different modes of binding, which could arise from different arrangements of the taxane or fluorescein moieties in the pore.     PM060184, a new tubulin binding agent with potent antitumor activity including P-glycoprotein over-expressing tumors

    PM060184 belongs to a new family of tubulin-binding agents originally isolated from the marine sponge Lithoplocamia lithistoides. This compound is currently produced by total synthesis and is under evaluation in clinical studies in patients with advanced cancer diseases. It was recently published that PM060184 presents the highest known affinities among tubulin-binding agents, and that it targets tubulin dimers at a new binding site. Here, we show that PM060184 has a potent antitumor activity in a panel of different tumor xenograft models. Moreover, PM060184 is able to overcome P-gp mediated resistance in vivo, an effect that could be related to its high binding affinity for tubulin. To gain insight into the mechanism responsible of the observed antitumor activity, we have characterized its molecular and cellular effects. We have observed that PM060184 is an inhibitor of tubulin polymerization that reduces microtubule dynamicity in cells by 59%. Interestingly, PM060184 suppresses microtubule shortening and growing at a similar extent. This action affects cells in interphase and mitosis. In the first case, the compound induces a disorganization and fragmentation of the microtubule network and the inhibition of cell migration. In the second case, it induces the appearance of multipolar mitosis and lagging chromosomes at the metaphase plate. These effects correlate with prometaphase arrest and induction of caspase-dependent apoptosis or appearance of cells in a multinucleated interphase-like state unrelated to classical apoptosis pathways.     New interfacial microtubule inhibitors of marine origin, PM050489/PM060184, with potent antitumor activity and a distinct mechanism

    We have investigated the target and mechanism of action of a new family of cytotoxic small molecules of marine origin. PM050489 and its dechlorinated analogue PM060184 inhibit the growth of relevant cancer cell lines at subnanomolar concentrations. We found that they are highly potent microtubule inhibitors that impair mitosis with a distinct molecular mechanism. They bind with nanomolar affinity to unassembled αβ-tubulin dimers, and PM050489 binding is inhibited by known Vinca domain ligands. NMR TR-NOESY data indicated that a hydroxyl-containing analogue, PM060327, binds in an extended conformation, and STD results define its binding epitopes. Distinctly from vinblastine, these ligands only weakly induce tubulin self-association, in a manner more reminiscent of isohomohalichondrin B than of eribulin. PM050489, possibly acting like a hinge at the association interface between tubulin heterodimers, reshapes Mg2+-induced 42 S tubulin double rings into smaller 19 S single rings made of 7 ± 1 αβ-tubulin dimers. PM060184-resistant mutants of Aspergillus nidulans map to β-tubulin Asn100, suggesting a new binding site different from that of vinblastine at the associating β-tubulin end. Inhibition of assembly dynamics by a few ligand molecules at the microtubule plus end would explain the antitumor activity of these compounds, of which PM060184 is undergoing clinical trials.We wish to thank J.     How does neighbourhood socio-economic status affect the interrelationships between functioning dimensions in first episode of psychosis? A network analysis approach

    The links between psychosis and socio-economic disadvantage have been widely studied. No previous study has analysed the interrelationships and mutual influences between functioning dimensions in first episode of psychosis (FEP) according to their neighbourhood household income, using a multidimensional and transdiagnostic perspective. 170 patients and 129 controls, participants in an observational study (AGES-CM), comprised the study sample. The WHO Disability Assessment Schedule (WHODAS 2.0) was used to assess functioning, whereas participants' postcodes were used to obtain the average household income for each neighbourhood, collected by the Spanish National Statistics Institute (INE). Network analyses were conducted with the aim of defining the interrelationships between the different dimensions of functioning according to the neighbourhood household income. Our results show that lower neighbourhood socioeconomic level is associated with lower functioning in patients with FEP. Moreover, our findings suggest that “household responsibilities” plays a central role in the disability of patients who live in low-income neighbourhoods, whereas “dealing with strangers” is the most important node in the network of patients who live in high-income neighbourhoods. These results could help to personalize treatments, by allowing the identification of potential functioning areas to be prioritized in the treatment of FEP according to the patient's neighbourhood characteristic

    Polygenic contribution to the relationship of loneliness and social isolation with schizophrenia

    Previous research suggests an association of loneliness and social isolation (LNL-ISO) with schizophrenia. Here, we demonstrate a LNL-ISO polygenic score contribution to schizophrenia risk in an independent case-control sample (N = 3,488). We then subset schizophrenia predisposing variation based on its effect on LNL-ISO. We find that genetic variation with concordant effects in both phenotypes shows significant SNP-based heritability enrichment, higher polygenic contribution in females, and positive covariance with mental disorders such as depression, anxiety, attention-deficit hyperactivity disorder, alcohol dependence, and autism. Conversely, genetic variation with discordant effects only contributes to schizophrenia risk in males and is negatively correlated with those disorders. Mendelian randomization analyses demonstrate a plausible bi-directional causal relationship between LNL-ISO and schizophrenia, with a greater effect of LNL-ISO liability on schizophrenia than vice versa.     Eficàcia i seguretat d’un tractament oral a base de mucopolisacàrids, col·lagen tipus I i vitamina C en pacients amb tendinopaties

    Introducció i objectius La tendinopatia és una lesió freqüent durant la pràctica esportiva que es manifesta amb una alteració estructural del tendó. L’objectiu d’aquest estudi fou avaluar l’eficàcia i la seguretat d’un complement alimentari a base de mucopolisacàrids, col·lagen tipus i i vitamina C (Tendoactive®) sobre l’evolució clínica i estructural de les tendinopaties del tendó d’Aquil·les, del rotular i de l’epicòndil lateral del colze. Material i mètodes Es realitzà un estudi multicèntric prospectiu, de tipus exploratori en fase iv, obert i no comparatiu. S’inclogueren un total de 98 pacients amb tendinopaties (32 d’Aquil·les, 32 de rotular i 34 de l’epicòndil lateral) que reberen una dosi diària de 435 mg de mucopolisacàrids, 75 mg de col·lagen tipus i i 60 mg de vitamina C (equivalent a 3 càpsules al dia de Tendoactive®) durant 90 dies consecutius. Mensualment s’avaluà el dolor en repòs i en activitat mitjançant una escala visual analògica (EVA), la funció articular mitjançant els qüestionaris VISA-A, VISA-P i PRTEE, i el tendó afectat es caracteritzà ecogràficament. Resultats En els 3 tipus de tendinopatia es registrà una reducció significativa del dolor, tant en repòs com en activitat, des de la primera visita de control (dia 30) fins al final de l’estudi (dia 90). Així mateix, el dia 90 es detectà una millora del 38% en VISA-A, del 46% en VISA-P i del 77% en PRTEE (p < 0,001). Simultàniament es registrà una reducció del 12% en el gruix del tendó d’Aquil·les, del 10% en el rotular i del 20% en el tendó de l’epicòndil lateral (p < 0,05). Conclusions Els resultats de l’estudi indiquen que l’administració de Tendoactive® és segura i eficaç per millorar els símptomes clínics i l’evolució estructural de les tendinopaties del tendó d’Aquil·les, del tendó rotular i del tendó de l’epicòndil lateral
