    El desarrollo de la cadena de valor del cacao por décadas en Tabasco ha representado por más de 500 años la sustentabilidad ecológica y económica el quehacer cotidiano de familias (Córdova, 2001), que a través de la organización, han tenido el reto del manejo de plantaciones, producción, comercialización y recientemente la obtención de denominación de origen para el cacao Grijalva con lo cual se fortalece esta actividad. La organización de productores de cacao representa una fortaleza en el desarrollo rural tabasqueño, al crease la Unión de Productores de Cacao en 1960, con injerencia directa del gobierno local y suplantación de decisiones de organización y financiera, lo cual fue un factor de decadencia organizacional y productiva a finales de siglo XX. Con apoyo del gobierno en 1975, se adjudican a productores activos fijos constituyéndose una nueva empresa. Para la primera década del presente siglo, lograron reorganizarse por iniciativa y acompañamiento de ONG’s para producir cacao orgánico y exportar chocolate orgánico por primera vez al mercado español. Se crea la asociación de productores de cacao orgánico de Tabasco, que los representará para realizar esta actividad, observándose que estos esfuerzos se dieron por poco tiempo porque no continuó la exportación. Se perdió el valor de la certificación orgánica y el sistema de producción alternativo con fines de exportación, esto se debió también a que se pagó a un precio igual o menor al del cacao de plantaciones no orgánicas. No obstante, continúan las iniciativas los pequeños productores para el control de plagas, la sustitución de plantaciones y generando nuevas relaciones con el gobierno local, tales como la alianza estratégica desde el 2013 con la Nestlé y de igual forma se impulsó la gestión de la “denominación de origen” obteniéndose en 2015, en el marco de una política pública de reactivación del campo Tabasqueño

    Familial Medullary Thyroid Cancer: Five-year Review of the Most Frequent Mutations in the RET Gene: An Update

    Background: Familial Medullary Thyroid Cancer (FMTC) is hereditary in 25% of cases. Patients with an inherited form of FMTC usually have a germline mutation in the RET proto-oncogene (10q11.2); these mutations generally occur in exons 10 (codons 618 and 620) and 11 (codons 630, 631, and 634). Methods: A narrative review of articles focused on the pathology of familial medullary thyroid cancer was carried out using the next databases PubMed, ScienceDirect, BMC, Springer, Frontiers, PMC, Wiley Online Library, Cold Spring Harbor and ELSEVIER. This search was carried out between August and September 2021. Results: 19 studies were selected in which the following mutations were found: five studies (26.31%) reported mutation in exon 10; three studies (15.78%) in exon 11; three studies in exon 13 (one of them associated with a rare mutation in exon 7) (10.52% plus 5.26%); three studies (15.78%) in exon 14; two studies (10.52%) in exon 15; two (10.52%) in exon 16; and one (5.26%) rare FMTC NO RET. The two most frequent mutations were in codons 620 of exon 10 and 804 of exon 14. Conclusion: The findings of this review are consistent with the medical literature, finding the most common RET mutations in exon 10 and codon 620. It is essential that in patients with a presumptive diagnosis, genetic studies (identification of germline mutations in the RET proto-oncogene, located on chromosome 10q11.2) be performed

    Influenza vaccination hesitancy in large urban centers in South America : qualitative analysis of confidence, complacency and convenience across risk groups

    Influenza vaccination coverage in countries of Latin America is low among priority risk groups, ranging from 5 to 75% among older people. This paper aims to describe and analyze the determinants of influenza vaccination hesitancy through the lens of the 3C model of confidence, complacency and convenience among middle-class, urban risk group populations in Brazil, Chile, Paraguay, Peru, Uruguay, countries in South America with contrasting vaccination coverage. Focus groups were conducted among four risk groups: pregnant women, mothers of children aged =60 years in samples of urban residents. Adults with risk factors expressed the most detailed perceptions about confidence in the vaccine. A wide range of perceptions regarding complacency were expressed across risk groups and countries, with pregnant women and mothers showing greater concerns while convenience had a narrower and generally more positive range of perceptions. Participants from Chile and Paraguay expressed the most contrasts regarding confidence and complacency. Information and communication strategies need to be tailored for risk groups while confidence and complacency should be addressed in synergy

    El clembuterol, Químicamente se describe como polvo blanco, anhidro, muy soluble en agua y altamente estable a temperatura ambiente, su punto de fusión es de 174 a 175.5 ºC. También es un derivado sintético perteneciente a una clase de medicamentos análogos fisiológicamente a la adrenalina, tiene la capacidad de interactuar con receptores adrenérgicos,  generalmente  del  tipo  ß2    (β  agonista),  es  uno  de  los  modificadores metabólicos más conocido en el área de producción de carne, debido al alto grado residual que deja esta sustancia en los tejidos comestibles y sus posibles repercusiones en la salud pública. En este trabajo, se describen los principales actores de la cadena productiva en la engorda  de  ganado  que  deben  participar  para  prevenir  el  uso  de  clembuterol.  La información fue recopilada de información científicas y publicaciones de las autoridades competentes, tales como: SENASICA, SAGARPA, COFEPRIS y Ley Federal de Sanidad Animal

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     OBJETIVO: Evaluar la asociación de la presión diastólica (PAD) y variables metabólicas en embarazadas. METODOLOGÍA: Estudio retrospectivo, transversal en embarazadas de término, mayores de 25 años, con embarazo único, agrupadas por Índice de Masa Corporal (IMC). A todas se les midieron lípidos, glucosa y PAD. Se calculó Odds Ratio (IC 95 %), correlación de Spearman entre variables y Kruskal Wallis utilizando SPSS v21.0. RESULTADOS: Se incluyeron 60 embarazadas normoevolutivas, edad promedio 30.9 ± 4.5 años, obteniendo una prevalencia de 41.6 % para sobrepeso y 31.6 % para obesidad. Los valores de glucemia en ayuno fueron de 102.2 ± 49.8 mg/dL en obesidad, 89.8 ± 28.7 mg/dL en sobrepeso y 84.5 ± 12.3 mg/dL en normopeso. Los valores medios de la PAD y PAS mostraron una correlación positiva con el valor medio de IMC pregestacional y gestacional. Mientras que, para las concentraciones de glucosa y lípidos, se obtuvo una correlación positiva con el valor medio de la PAD. CONCLUSIÓN: Se confirma asociación de la PAD con el IMC pregestacional

    The importance of confidence, complacency, and convenience for influenza vaccination among key risk groups in large urban areas of Peru

    Influenza vaccination has been available under Peru’s national immunization program since 2008, but vaccination coverage has decreased lately. Surveys and focus groups were conducted among four risk groups (pregnant women, mothers of children aged <6 years, adults with risk factors, and adults aged ≥65 years) to identify factors affecting influenza vaccine hesitancy in Peru. The 3Cs model (Confidence, Complacency, and Convenience) was used as a conceptual framework for the study. Most pregnant women and mothers of young children (70.0%), but less than half (46.3%) of older adults and adults with risk factors were vaccinated against influenza. Vaccine confidence and complacency were positively associated with educational level. Complacency was the most deficient of the 3Cs. Pregnant women and mothers were the most informed and least complacent among risk groups. Focus groups revealed the misconceptions behind the high level of complacency observed, including the perception of influenza risk and the role assigned to vaccination in preventing the disease. Interviews with officials identified that most strategies are directed to vaccination availability and hence to convenience, with opportunities for strategies to improve vaccination uptake and community engagement. The results highlight the importance of implementing in Peru communication strategies to increase perceptions of vaccine safety and effectiveness thus improving confidence and reducing complacency. The establishment of explicit incentives should also be considered to increase vaccination uptake, particularly to health personnel

    The risk of COVID-19 death is much greater and age dependent with type I IFN autoantibodies

    International audienceSignificance There is growing evidence that preexisting autoantibodies neutralizing type I interferons (IFNs) are strong determinants of life-threatening COVID-19 pneumonia. It is important to estimate their quantitative impact on COVID-19 mortality upon SARS-CoV-2 infection, by age and sex, as both the prevalence of these autoantibodies and the risk of COVID-19 death increase with age and are higher in men. Using an unvaccinated sample of 1,261 deceased patients and 34,159 individuals from the general population, we found that autoantibodies against type I IFNs strongly increased the SARS-CoV-2 infection fatality rate at all ages, in both men and women. Autoantibodies against type I IFNs are strong and common predictors of life-threatening COVID-19. Testing for these autoantibodies should be considered in the general population
