Reproductive life characteristics in females affected with Parkinson's disease and in healthy control subjects – a comparative study on Polish population
Background
Sex and blood level of sex hormones play a key role not only in the susceptibility to develop Parkinson's disease (PD) but also influence the natural course of the disease. The aim of this study was to compare reproductive lifespan events in females affected with PD and in “non-parkinsonian” age matched subjects and to evaluate whether the whole life endogenous oestrogen level is associated with variables describing the course of the disease.
Materials and methods
Reproductive lifespan, age at menarche, age at menopause, gynaecological interventions and parity were compared in 76 women with idiopathic PD and in the age-adjusted control group of 74 subjects. Affected women underwent neurological and psychological assessment. Data were analysed using Mann–Whitney U Test and Spearman Rank Correlation Test.
Results
Women affected with PD had a shorter reproductive lifespan and experienced final menstruation earlier than the control group. Early menopause was reported by 24% of the patients and only by 16% of the control subjects. Parkinsonian women reported more commonly the history of surgical menopause. Duration of reproductive lifespan, age at menopause and the type of menopause influenced both motor and cognitive functioning of patients.
Conclusions
There may be a relationship between the lifetime average endogenous oestrogen level and the susceptibility to develop PD. Longer reproductive lifespan resulting in higher “whole life” female sex steroids concentrations may exert a protective effect on central nervous system, resulting in milder course of the disease
Selektywne modulatory receptora estrogenowego w terapii osteoporozy postmenopauzalnej
Abstract Postmenopausal osteoporosis is associated with lack of estrogens, therefore, understandably one of the treatment options in osteoporosis is a group of medicines known as selective estrogen receptor modulators (SERMs). They can act as an estrogen receptor agonist in some tissues, whereas as an antagonist in others. In relation to this antago-antagonistic action, SERMs have a positive effect on bones, the serum lipid profile and the cardio-vascular system. Moreover, they can protect against some estrogen-dependent neoplasm development. The first used SERM was tamoxifen, but due to its negative effect on endometrium it is not indicated in osteoporosis. Raloxifen, which is currently in use, besides the reduction of vertebral fractures risk, has beneficial influence on endometrial and breast neoplasm development risk as well. On the other hand, raloxifen intensifies vasomotor symptoms and its bone-protecting effect is limited. At present, new SERMs (ospemifen, lasofoxifen, bazedoxifen, arzoxifen) are being researched in clinical trials. In the current stage of investigations they reveal beneficial influence on skeletal as well as extraskeletal tissues. Implementation of SERMs in combined therapy of osteoporosis is currently under research as well. SERM with parathormone or SERM with bisphosphonate might prove to be an advantageous treatment option for women with severe or resistant osteoporosis. An addition of SERM to conventional hormonal replacement therapy did not bring the anticipated benefits. Future studies on SERMs may result in new preparations adjusted to individual needs of the patients..Streszczenie Osteoporoza u kobiet po menopauzie jest związana z brakiem estrogenów. Jedną z grup preparatów stosowanych w leczeniu osteoporozy są selektywne modyfikatory receptora estrogenowego (SERM). Działają one jako agoniści receptora estrogenowego w pewnych tkankach, w innych natomiast jako antagoniści. Dzięki temu SERM mają korzystny wpływ na szkielet, profil lipidowy i układ sercowo-naczyniowy, a jednocześnie wykazują działanie ochronne przed niektórymi nowotworami estrogenozależnymi. Pierwszym zastosowanym SERM był tamoksifen, jednak ze względu ma niekorzystny wpływ na endometrium nie jest wskazany w osteoporozie. Obecnie w użyciu znajduje się raloksifen, który zmniejsza ryzyko złamań kręgów, a ponadto redukuje ryzyko nowotworów endometrium i sutka. Nasila jednak objawy naczynioruchowe, a jego wpływ na metabolizm kostny jest ograniczony. Nowe SERM przechodzą próby kliniczne, między innymi: ospemifen, lasofoksifen, bazedoksifen i arzoksifen. Na obecnym etapie badań wszystkie wykazują korzystne działanie w osteoporozie oraz maja dodatkowe pozytywne efekty pozakostne. Trwają też badania nad wykorzystaniem SERM w terapii kombinowanej osteoporozy. Użycie płąaczenia SERM z parathormonem lub SERM z bisfosfonianem może być korzystne u pacjentek z ciężką lub oporną na leczenie osteoporozą. Połączenie SERM i hormonalnej terapii zastępczej nie przyniosło oczekiwanych korzyści. Badania nad SERM pozwolą być może w przyszłości na opracowanie preparatów dostosowanych do indywidualnych potrzeb każdego pacjenta
Hormones and the cardiovascular system
Hormony wywierają wpływ na wiele tkanek i narządów, w tym również na układ sercowo-naczyniowy (CVS). W zależności od rodzaju
aktywności w obrębie CVS hormony dzieli się na działające hipotensyjnie lub presyjnie oraz działające chronotropowo dodatnio i ujemnie.
Regulacja endokrynna w układzie krążenia może zachodzić na wielu drogach. Obok hormonów zwyczajowo wiązanych z CVS jest
też wiele innych, niedawno odkrytych. Ich działanie może być bezpośrednie (przez swoiste receptory w ścianie naczyń lub w obrębie
komórek serca) lub pośrednie (przez inne czynniki neuroendokrynne). Ponadto, w przypadku hormonów steroidowych i tarczycy zaangażowane
są nowo poznane, niezależne od regulacji transkrypcji, mechanizmy przekazywania sygnału - tak zwane „niegenomowe”.
Spośród hormonów podwyższających ciśnienie tętnicze należy wymienić: urotensynę II, endoteliny, angiotensynę II, katecholaminy,
aldosteron, hormon antydiuretyczny, glikokortykosteroidy, hormony tarczycy, hormon wzrostu i leptynę. Z kolei obniżają ciśnienie tętnicze
następujące hormony: peptydy natriuretyczne, rodzina peptydu związanego z genem kalcytoniny (CGRP), angiotensyna 1-7, substancja
P, neurokinina A, grelina, hormon podobny do parathormonu (PTHrP), oksytocyna i hormony płciowe.
Hormonami wywołującymi w nadmiarze przyspieszenie czynności serca są: katecholaminy, endoteliny, glikokortykosteroidy, hormony
tarczycy, leptyna i PTHrP. Te, które zwalniają czynność serca to: peptydy natriuretyczne, substancja P, neurokinina A, oksytocyna, angiotensyna
1-7. W pracy przedstawiono współczesne poglądy, dotyczące działania hormonów wpływających na serce i naczynia. Efekt
działania części mediatorów nie zawsze jest taki sam i zależy między innymi od stężenia hormonu, receptora, na który oddziałuje; może
podlegać kontrregulacji. Większość substancji odgrywa ważną rolę w patogenezie chorób CVS, przez co stanowią przedmiot badań

CVS, they can be divided into 4 groups: having hypertensive or hypotensive influence and chronotropic positive or negative action.
Endocrine regulation in CVS may occur in many ways. Apart from hormones usually connected with CVS regulation, other more recently,
discovered ones can act on it. A few of these act directly through specific receptors in heart or vessel wall cells, whereas some act indirectly
- stimulating other neuroendocrine factors. Additionally, novel mechanisms of signal transduction have been discovered for steroid and
thyroid hormones, which are independent of gene transcription regulation and are - known as "nongenomic".
Hormones which increase blood pressure include: urotensin II, endothelins, angiotensin II, catecholamines, aldosterone, antidiuretic
hormone, glucocorticosteroids, thyroid hormones, growth hormone and leptin. On the other hand, blood pressure can be decreased by:
natriuretic peptides, the calcitonin gene-related peptide (CGRP) family, angiotensin 1-7, substance P, neurokinin A, ghrelin, Parathyroid
hormone-related protein (PTHrP), oxytocin, and, sex hormones. Hormones which when appearing in excess increase the heart rate are:
catecholamines, endothelins, glucocorticosteroids, thyroid hormones, leptin and PTHrP. Those which decrease the heart rate include:
natriuretic peptides, substance P, neurokinin A, oxytocin, angiotensin 1-7. This paper describes the contemporary view of the functions of
hormones which act on the vessel tree and heart. The particular effect of mediator depends on many circumstances i.e.: hormone concentration,
receptor type. It may also undergo contraregulation.
The majority of those hormones play an important role in the pathogenesis of CVS diseases’, which can result in the development of new

Hormonalna terapia zastępcza a ryzyko udaru niedokrwiennego mózgu
Kobiety w okresie przed menopauzą cechuje niższe ryzyko wystąpienia
udaru niedokrwiennego mózgu (UNM) oraz zawału serca.
W dużej mierze zjawisko to wiąże się z ochronnym oddziaływaniem
estrogenów na ścianę naczyniową. Po menopauzie ryzyko
wystąpienia chorób naczyniowych wzrasta. Ze względu na
wydłużenie życia populacji szacuje się, że w najbliższych latach
liczba kobiet w okresie pomenopauzalnym będzie się szybko zwiększała.
W związku z tym duże nadzieje pokładano w hormonalnej
terapii zastępczej (HTZ), która teoretycznie mogłaby opóźnić rozwój
związanych z wiekiem powikłań miażdżycy, takich jak UNM
i zawał serca.
Jednak w badaniach epidemiologicznych, przeprowadzonych
w ostatnich latach, wykazano, że HTZ nie tylko nie chroni przed
UNM i zawałem serca, ale może nawet zwiększać ryzyko ich wystąpienia.
Spowodowało to radykalną zmianę poglądów międzynarodowych
grup eksperckich.
W poniższym artykule podano podstawowe informacje o biologii
przekwitania kobiety, wpływie HTZ na układ naczyniowy oraz
wnioski płynące z wieloośrodkowych badań nad profilaktycznymi
zastosowaniami HTZ. Omówiono wady i możliwe korzyści zdrowotne
związane z jej stosowaniem.
W podsumowaniu autorzy cytują zalecenia polskich, europejskich
i amerykańskich grup eksperckich, w których zgodnie stwierdzono,
że obecnie nie zaleca się stosowania HTZ - zarówno w pierwotnej,
jak i wtórnej profilaktyce UNM. Ze względu na złożoność zagadnienia oraz ciągle niepełną wiedzę w tym zakresie konieczne są
dalsze badania nad wpływem terapii hormonalnych na rozwój
miażdżycy i jej powikłania
The role of genes in pathogenesis of obesity. Contemporary view, pathogenesis, clinical aspects
Czynniki genetyczne warunkują trzy rodzaje otyłości: uwarunkowaną
jednogenowo, otyłość będącą elementem zespołu oraz otyłość
powszechnie występującą.
Otyłość uwarunkowana jednogenowo stanowi grupę dość rzadkich
schorzeń. Odpowiadają za nie mutacje genów, których produkty
uczestniczą w regulacji apetytu. Za najpowszechniejszą jej
formę uważa się obecnie mutację genu dla receptora 4. melanokortyny,
chociaż wyniki ostatnich badań wskazują, że defekty dotychczas
uważane za bardzo rzadkie mogą stanowić istotny odsetek
patologicznej otyłości zaczynającej się w dzieciństwie. Jest
to grupa schorzeń o rosnącym znaczeniu ze względu na identyfikację
nowych przypadków oraz istniejące i potencjalne możliwości
celowanej terapii.
Spośród 25 zespołów przebiegających z otyłością do najważniejszych
należą zespoły: Prader-Willi, Bardet-Biedl, Alström i Cohen. Ich
patogenezy nie poznano wystarczająco, ale rzuca ona nowe światło
na wiele aspektów fizjologii człowieka. Ze względu na fenotyp obejmujący
wiele układów wymagają one wielospecjalistycznej opieki.
Otyłość powszechnie występująca ma tło wielogenowe i jest wynikiem
interakcji geny-środowisko. Ze względu na skalę zjawiska
jej poznanie wydaję się najważniejsze dla współczesnej medycyny. Niemniej identyfikacja genetycznego tła wymaga zastosowania
nowych technik genetyki populacyjnej oraz ich rozwoju.
Niniejszy artykuł dostarcza informacji na temat postępów w poznawaniu
genetycznego tła wszystkich 3 form otyłości z uwzględnieniem
patogenezy, aspektów klinicznych oraz perspektyw dotyczących
ich poznawania.The genetic factors contribute to three types of obesity: monogenic
obesity, syndromic obesity and common obesity.
The group of monogenic obesity consists of a group of rare
diseases in which mutations of appetite control genes are involved.
It is believed that the most common form of monogenic
obesity is due to a mutation of the melanocortin 4th receptor.
The latest research suggests, that the number of cases with this
mutation may be more abundant than originally hypothesized,
especially in group of patients with early-onset, morbid obesity.
The importance of this type of obesity is growing due to the
increasing incidence of new cases and the effect that new genetic
research is having on existing and potential therapeutic
approaches.
Taking into account the 25 other cases of syndromic obesity, the
most important syndromes are: Prader-Willi, Bardet-Biedl, Alström
and Cohen Syndromes. The pathogenesis of these syndromes
remains unclear, nevertheless these syndromes reveal new insights
to human physiology. Due to the multisystemic phenotypes of these
syndromes, complex medical care is required. Common obesity is a result of a gene-environment interaction. It is
a widespread form of obesity and it seems, that gaining knowledge
about this particular form of obesity is one of the most baffling challenges
for contemporary medicine. Identifying the extent that genes contribute
to obesity will require new techniques in population genetics.
This review provides information about recent genetic research
progressions in all three types of human obesity, with a focus on
pathophysiology, clinical aspects and future research
The leaching of mineral nitrogen forms from light soil fertilized with compost and sewage sludge
The research was carried out in the years 2003-2013 in lysimeters filled with loamy sand. The leachate was tested in three variants: Z - no fertilization, S - 20 g-m-2 of N delivered annually in sewage sludge and C - 20 g-m-2 of N in the form of compost. The lysimeters were planted with Miscanthus giganteus, which is an energy plant with a high demand for water and nutrients. The amount of leaching of mineral nitrogen forms was determined on the basis of measured volumes of leachates from the soil and volumes of ammonium nitrogen (N-NH4) and nitrate nitrogen (N-NO3) contained in them. The research results showed a significant increase in the average content of mineral nitrogen forms in the effluents from the fertilized soil (S - 6.8 mg-dm-3 of N-NO3 and 0.3 mg-dm-3 of N-NH4, C - 7.8 mg-dm-3 of N-NO3 and 0.4 mg-dm-3 of N-NH4), compared to their concentrations in the leachates from non-fertilized soil (Z - 2.1 mg-dm-3 of N-NO3 and 0.2 mg-dm-3 of N-NH4). The content of mineral forms of nitrogen, in particular N-NO3, were similar in both fertilization variants. The lowest concentrations of mineral nitrogen in the leachates occurred in the third and fourth year after planting Miscanthus giganteus, when it entered the period of the highest yield. In the fifth year, due to a cold, snowless winter, there was a weakened growth of plants, which resulted in an increase in the concentration of mineral nitrogen in the leachates from the fertilized soil. It follows that in addition to the intensity of precipitation, the collection of this component by plants primarily influences nitrogen leaching from the soil. The general amount of mineral nitrogen leached from the soil was not large and amounted Z - 2.5 kg-m-2, S - 6.7 kg-m-2, C - 6.4 kg-m-2. This testifies to the intense collection of this form of nitrogen by Miscanthus giganteus
Density Functional Application to Strongly Correlated Electron Systems
The LSDA+U approach to density functional theory is carefully reanalyzed. Its
possible link to single-particle Green's function theory is occasionally
discussed. A simple and elegant derivation of the important sum rules for the
on-site interaction matrix elements linking them to the values of U and J is
presented. All necessary expressions for an implementation of LSDA+U into a
non-orthogonal basis solver for the Kohn-Sham equations are given, and
implementation into the FPLO solver is made. Results of application to several
planar cuprate structures are reported in detail and conclusions on the
interpretation of the physics of the electronic structure of the cuprates are

Density functional theory calculations on magnetic properties of actinide compounds
We have performed a detailed analysis of the magnetic (collinear and
noncollinear) order and atomic and the electron structures of UO2, PuO2 and UN
on the basis of density functional theory with the Hubbard electron correlation
correction (DFT+U). We have shown that the 3-k magnetic structure of UO2 is the
lowest in energy for the Hubbard parameter value of U=4.6 eV (and J=0.5 eV)
consistent with experiments when Dudarev's formalism is used. In contrast to
UO2, UN and PuO2 show no trend for a distortion towards rhombohedral structure
and, thus, no complex 3-k magnetic structure is to be anticipated in these

The DINAMIC 2 study: aims, assumptions and methods (I)
Większość chorych na cukrzycę typu 2 pozostaje pod
opieką lekarzy podstawowej opieki zdrowotnej, dlatego
istnieje konieczność oceny jakości opieki w cukrzycy
typu 2 w gabinetach lekarzy rodzinnych. Badanie
dotyczyło zarówno oceny wyrównania cukrzycy
na podstawie kryteriów międzynarodowych wytycznych,
jak i oceny sposobu postępowania terapeutycznego.
W badaniach wzięło udział 293 lekarzy
podstawowej opieki zdrowotnej spośród
320 wylosowanych z terenu całej Polski. Każdy z lekarzy
ocenił 10 kolejnych chorych na cukrzycę typu 2.
Łącznie do końcowej oceny zakwalifikowano 2636
osób. Stężenie hemoglobiny glikowanej oznaczono
metodą HPLC w centralnym laboratorium u 94,4%
badanych. Ponadto oceniono wskaźnik masy ciała,
ciśnienie tętnicze, mikroalbuminurię, aktualnie stosowany
schemat leczenia cukrzycy i nadciśnienia
tętniczego, a także edukację i stosowanie samokontroli.
Badanie umożliwiło ocenę stopnia wyrównania
cukrzycy oraz sposoby leczenia, nie tylko w skali
całego kraju, ale także porównawczo w poszczególnych
województwach, co mogłoby dać podstawę do
weryfikacji polityki zdrowotnej w zakresie cukrzycy
typu 2.Most patients with type 2 diabetes mellitus are taken
care of by family physicians. For this reason it is
necessary to evaluate the quality of primary care
among diabetic patients. We analysed diabetes control
based upon the fulfilment of international criteria,
and therapeutic approach. Analysis was carried
out by 293 primary care physicians of 320 randomly
chosen all over Poland. Each physician evaluated
10 consecutive type 2 diabetics. Altogether 2693 patients
were included in the final analysis. Glycated
haemoglobin was measured by HPLC in the central
laboratory in 94.4% of patients. We also assessed
body mass index, blood pressure, microalbuminuria,
mode of treating diabetes and arterial hypertension,
education and self-control. The results of the study
provided a basis for defining the level of diabetes
control and mode of treatment not only in the whole
country but also comparatively in the individual
regions, which could allow for modification of health
policy with respect to type 2 diabetes mellitus
