    Constraints on upper crustal fluid circulation and seismogenesis from in-situ outcrop quantification of complex fault zone permeability

    : The permeability of fault zones plays a significant role on the distribution of georesources and on seismogenesis in the brittle upper crust, where both natural and induced seismicity are often associated with fluid migration and overpressure. Detailed models of the permeability structure of fault zones are thus necessary to refine our understanding of natural fluid pathways and of the mechanisms leading to fluid compartmentalization and possible overpressure in the crust. Fault zones commonly contain complex internal architectures defined by the spatial juxtaposition of "brittle structural facies" (BSF), which progressively and continuously form and evolve during faulting and deformation. We present the first systematic in-situ outcrop permeability measurements from a range of BSFs from two architecturally complex fault zones in the Northern Apennines (Italy). A stark spatial heterogeneity of the present-day permeability (up to four orders of magnitude) even for tightly juxtaposed BSFs belonging to the same fault emerges as a key structural and hydraulic feature. Insights from this study allow us to better understand how complex fault architectures steer the 3D hydraulic structure of the brittle upper crust. Fault hydraulic properties, which may change through space but also in time during an orogenesis and/or individual seismic cycles, in turn steer the development of overpressured volumes, where fluid-induced seismogenesis may localize

    Work Ability in Healthcare: Vulnerable Groups and Organizational Factors

    The recent pandemic, along with heavy workloads and staff shortages, has placed significant pressure on healthcare workers. Maintaining adequate work ability is vital for ensuring favorable working conditions, mitigating stress, preventing related illnesses, and safeguarding worker performance and patient safety. This article assesses the work ability and working conditions of healthcare professionals at the University Hospital of Modena through a questionnaire administered between August 1, 2022, and September 30, 2022, to identify vulnerable groups and organizational factors influencing work ability. Among workers with reduced work ability, the majority are over 45 years old and female, 52% are obese, 64% have 3 or more illnesses, 47% report a poor work-life balance, and 50% have at least one dependent adult. Work characteristics are also highlighted as relevant: supervisor support and cooperation with colleagues, autonomy in decision-making processes, participation in the improvement of work processes, possession of skills appropriate to the tasks required, are associated with high levels of work ability. Finally, nurses and nurses aides are associated with lower work ability. Emergency and medical wards are particularly critical in terms of work ability when gender and age differences are taken into account

    How physical exercise changes rat myotendinous junctions : an ultrastructural study

    Myotendinous junctions can be easily injured by overloading or trauma, and exercise training may be a way of increasing their resistance to mechanical stress. To this end, we examined herein the morphological changes induced by moderate exercise training in the myotendinous junctions of extensor digitorum longus and gastrocnemius muscles in rats. Twelve Sprague-Dawley rats were used in this investigation. Six of them were trained to run on a treadmill for 1 h/day, 3 days/week over 10 weeks in order for them to achieve a running rate of 25 m/min at the end of the training period. Six age-matched sedentary rats were used as controls. The rats were sacrificed 24 h after the final training session, and the extensor digitorum longum (EDL) and the gastrocnemium were excised; the myotendinous junctions (MTJ) were then prepared and observed with electron microscopy. Digitation branching was evaluated by counting the bifurcations in the MTJ protrusions. Our observations indicate that exercise does indeed induce changes in MTJ morphology. In both muscles the number of bifurcated interdigitations increased significantly, as well as, in gastrocnemius, the branching of the finger-like processes. It was demonstrated that the MTJ is able to adapt to an increase in tensile force by enlarging the muscle-tendon contact area and, consequently, mechanical resistance

    Surgical site infection after caesarean section. Space for post-discharge surveillance improvements and reliable comparisons

    Surgical site infections (SSI) after caesarean section (CS) represent a substantial health system concern. Surveying SSI has been associated with a reduction in SSI incidence. We report the findings of three (2008, 2011 and 2013) regional active SSI surveillances after CS in community hospital of the Latium region determining the incidence of SSI. Each CS was surveyed for SSI occurrence by trained staff up to 30 post-operative days, and association of SSI with relevant characteristics was assessed using binomial logistic regression. A total of 3,685 CS were included in the study. A complete 30 day post-operation follow-up was achieved in over 94% of procedures. Overall 145 SSI were observed (3.9% cumulative incidence) of which 131 (90.3%) were superficial and 14 (9.7%) complex (deep or organ/space) SSI; overall 129 SSI (of which 89.9% superficial) were diagnosed post-discharge. Only higher NNIS score was significantly associated with SSI occurrence in the regression analysis. Our work provides the first regional data on CS-associated SSI incidence, highlighting the need for a post-discharge surveillance which should assure 30 days post-operation to not miss data on complex SSI, as well as being less labour intensive

    Benthic foraminiferal ultrastructural alteration induced by heavy metals

    Heavy metals are known to cause deleterious effects on biota because of their toxicity, persistence and bioaccumulation. Here, we briefly document the ultrastructural changes observed in the miliolid foraminifer Pseudotriloculina rotunda (d\u27Orbigny in Schlumberger, 1893) and in the perforate calcareous species Ammonia parkinsoniana (d\u27Orbigny, 1839) induced by exposure to one of three heavy metals (zinc, lead, or mercury). The exposure of these two benthic foraminiferal species to the selected heavy metals appears to promote cytological alterations and organelle degeneration. These alterations include a thickening of the inner organic lining, an increase in number and size of lipid droplets, mitochondrial degeneration, and degradation vacuoles and residual body proliferation. Some of these alterations, including the thickening of the inner organic lining and the proliferation of lipids, might represent defense mechanisms against heavy metal-induced stress

    Colorectal Cancer Stage at Diagnosis Before vs During the COVID-19 Pandemic in Italy

    IMPORTANCE Delays in screening programs and the reluctance of patients to seek medical attention because of the outbreak of SARS-CoV-2 could be associated with the risk of more advanced colorectal cancers at diagnosis. OBJECTIVE To evaluate whether the SARS-CoV-2 pandemic was associated with more advanced oncologic stage and change in clinical presentation for patients with colorectal cancer. DESIGN, SETTING, AND PARTICIPANTS This retrospective, multicenter cohort study included all 17 938 adult patients who underwent surgery for colorectal cancer from March 1, 2020, to December 31, 2021 (pandemic period), and from January 1, 2018, to February 29, 2020 (prepandemic period), in 81 participating centers in Italy, including tertiary centers and community hospitals. Follow-up was 30 days from surgery. EXPOSURES Any type of surgical procedure for colorectal cancer, including explorative surgery, palliative procedures, and atypical or segmental resections. MAIN OUTCOMES AND MEASURES The primary outcome was advanced stage of colorectal cancer at diagnosis. Secondary outcomes were distant metastasis, T4 stage, aggressive biology (defined as cancer with at least 1 of the following characteristics: signet ring cells, mucinous tumor, budding, lymphovascular invasion, perineural invasion, and lymphangitis), stenotic lesion, emergency surgery, and palliative surgery. The independent association between the pandemic period and the outcomes was assessed using multivariate random-effects logistic regression, with hospital as the cluster variable. RESULTS A total of 17 938 patients (10 007 men [55.8%]; mean [SD] age, 70.6 [12.2] years) underwent surgery for colorectal cancer: 7796 (43.5%) during the pandemic period and 10 142 (56.5%) during the prepandemic period. Logistic regression indicated that the pandemic period was significantly associated with an increased rate of advanced-stage colorectal cancer (odds ratio [OR], 1.07; 95%CI, 1.01-1.13; P = .03), aggressive biology (OR, 1.32; 95%CI, 1.15-1.53; P < .001), and stenotic lesions (OR, 1.15; 95%CI, 1.01-1.31; P = .03). CONCLUSIONS AND RELEVANCE This cohort study suggests a significant association between the SARS-CoV-2 pandemic and the risk of a more advanced oncologic stage at diagnosis among patients undergoing surgery for colorectal cancer and might indicate a potential reduction of survival for these patients

    AMO-enhancing practices, open innovation and organizations’ innovation in the European context: testing a mediation model

    The literature has recognized the key role of the HRM practices for enhancing firms’ innovative performance. At the same time, scholars have consistently demonstrated open innovation to be an effective approach for boosting companies’ innovative outcome. Nevertheless, academics have largely overlooked to investigate the complex relationship between HRM practices, open innovation and organizations’ innovativeness. Using the AMO framework as analytical lens and data drawn from the European Company Survey 2019, a large-scale representative dataset at European level, this study investigates the direct and indirect relationship between the ability, motivation and opportunities-enhancing practices and firms’ innovation capacity, hypothesizing a potential mediating role of open innovation. The results show that companies that motivate workers and give them the opportunity to contribute with their skills, knowledge and abilities not only have higher probability to innovate, but also are more inclined to collaborate with external partners. Moreover, open innovation not only enhance the innovation capacity of the firm, but also partially mediates the relationship between HRM and organizations’ innovativeness. These results shed further lights on both “the human side” of open innovation, as well as in the mechanisms linking HRM practices with innovation
