    Clarifying the importance of CYP2C19 and PON1 in the mechanism of clopidogrel bioactivation and in vivo antiplatelet response

    AimsIt is thought that clopidogrel bioactivation and antiplatelet response are related to cytochrome P450 2C19 (CYP2C19). However, a recent study challenged this notion by proposing CYP2C19 as wholly irrelevant, while identifying paraoxonase-1 (PON1) and its Q192R polymorphism as the major driver of clopidogrel bioactivation and efficacy. The aim of this study was to systematically elucidate the mechanism and relative contribution of PON1 in comparison to CYP2C19 to clopidogrel bioactivation and antiplatelet response.Methods and resultsFirst, the influence of CYP2C19 and PON1 polymorphisms and plasma paraoxonase activity on clopidogrel active metabolite (H4) levels and antiplatelet response was assessed in a cohort of healthy subjects (n = 21) after administration of a single 75 mg dose of clopidogrel. There was a remarkably good correlation between H4 AUC (0-8 h) and antiplatelet response (r2 = 0.78). Furthermore, CYP2C19 but not PON1 genotype was predictive of H4 levels and antiplatelet response. There was no correlation between plasma paraoxonase activity and H4 levels. Secondly, metabolic profiling of clopidogrel in vitro confirmed the role of CYP2C19 in bioactivating clopidogrel to H4. However, heterologous expression of PON1 in cell-based systems revealed that PON1 cannot generate H4, but mediates the formation of another thiol metabolite, termed Endo. Importantly, Endo plasma levels in humans are nearly 20-fold lower than H4 and was not associated with any antiplatelet response.ConclusionOur results demonstrate that PON1 does not mediate clopidogrel active metabolite formation or antiplatelet action, while CYP2C19 activity and genotype remains a predictor of clopidogrel pharmacokinetics and antiplatelet response. © 2012 The Author

    Clinical and genetic determinants of warfarin pharmacokinetics and pharmacodynamics during treatment initiation

    Variable warfarin response during treatment initiation poses a significant challenge to providing optimal anticoagulation therapy. We investigated the determinants of initial warfarin response in a cohort of 167 patients. During the first nine days of treatment with pharmacogenetics-guided dosing, S-warfarin plasma levels and international normalized ratio were obtained to serve as inputs to a pharmacokinetic-pharmacodynamic (PK-PD) model. Individual PK (S-warfarin clearance) and PD (I max) parameter values were estimated. Regression analysis demonstrated that CYP2C9 genotype, kidney function, and gender were independent determinants of S-warfarin clearance. The values for I max were dependent on VKORC1 and CYP4F2 genotypes, vitamin K status (as measured by plasma concentrations of proteins induced by vitamin K absence, PIVKA-II) and weight. Importantly, indication for warfarin was a major independent determinant of I max during initiation, where PD sensitivity was greater in atrial fibrillation than venous thromboembolism. To demonstrate the utility of the global PK-PD model, we compared the predicted initial anticoagulation responses with previously established warfarin dosing algorithms.     Background: Antiplatelet therapy reduces the risk of major vascular events for people with occlusive vascular disease, although it might increase the risk of intracranial haemorrhage. Patients surviving the commonest subtype of intracranial haemorrhage, intracerebral haemorrhage, are at risk of both haemorrhagic and occlusive vascular events, but whether antiplatelet therapy can be used safely is unclear. We aimed to estimate the relative and absolute effects of antiplatelet therapy on recurrent intracerebral haemorrhage and whether this risk might exceed any reduction of occlusive vascular events. Methods: The REstart or STop Antithrombotics Randomised Trial (RESTART) was a prospective, randomised, open-label, blinded endpoint, parallel-group trial at 122 hospitals in the UK. We recruited adults (≥18 years) who were taking antithrombotic (antiplatelet or anticoagulant) therapy for the prevention of occlusive vascular disease when they developed intracerebral haemorrhage, discontinued antithrombotic therapy, and survived for 24 h. Computerised randomisation incorporating minimisation allocated participants (1:1) to start or avoid antiplatelet therapy. We followed participants for the primary outcome (recurrent symptomatic intracerebral haemorrhage) for up to 5 years. We analysed data from all randomised participants using Cox proportional hazards regression, adjusted for minimisation covariates. This trial is registered with ISRCTN (number ISRCTN71907627). Findings: Between May 22, 2013, and May 31, 2018, 537 participants were recruited a median of 76 days (IQR 29–146) after intracerebral haemorrhage onset: 268 were assigned to start and 269 (one withdrew) to avoid antiplatelet therapy.     Fact-finding hearings may be held to determine disputed allegations of domestic violence in child contact cases in England and Wales, and can play a vital role for mothers seeking protection and autonomy from violent fathers. Drawing on the author’s empirical study, this article examines the implications for the holding of fact-finding hearings of judges’ and professionals’ understandings of domestic violence and the extent to which they perceive it to be relevant to contact. While more judges and professionals are developing their understanding of domestic violence, the ambit of when and how it is considered relevant to contact has grown increasingly narrow, which suggests that many disputed allegations of domestic violence are disregarded and women and children continue to be put at risk from violent fathers. This bifurcated approach is likely to have significant implications for recent developments in this area of family law which are considered in this article

    The Impact of a Clinical Pharmacist on Patient and Economic Outcomes in a Child and Adolescent Mental Health Unit

    ABSTRACT Background and Objective: It has been shown that clinical pharmacists positively influence quality of care and decrease drug expenditures in hospital settings. This 2-part study was undertaken to evaluate the impact of a clinical pharmacist on patient and economic outcomes in a pediatric mental health setting.Methods: In the first part of the study, a 4-week prospective evaluation period, pharmacist-initiated interventions were documented. This information was distributed to a panel of assessors, who determined the impact of each intervention on patient care. In the second part of the study, a retrospective cost analysis was used to compare drug costs for 2 consecutive years, the 12-month period before and the 12-month period immediately after institution of the clinical pharmacy position. A matched-pair t-test and regression analysis were conducted on the cost data.Results: The pharmacist initiated 48 interventions during the 4-week period, 47 (98%) of which were accepted by the treating physician. Eighty-six percent (38/44) of the interventions were assessed as having a positive effect on patient care. Drug cost per patient-day was 14% lower in the year after implementation of the pharmacy position, and the difference was statistically significant in the last 8 months of that year (p = 0.0019). Cette étude de deux phases a été menée pour évaluer l’incidence des interventions du pharmacien clinicien en termes de résultats thérapeutiques et de répercussions économiques dans un établissement de soins de santé mentale pour enfants.Méthodes : Dans la première phase de l’étude, soit une période d’évaluation prospective de quatre semaines, les interventions entreprises par le pharmacien ont été documentées. L’information ainsi recueillie a été distribuée à un comité qui a évalué l’incidence de chaque intervention sur les soins au patient. Dans la seconde phase de l’étude, une analyse rétrospective des coûts — au moyen d’un test de Student apparié et d’une analyse de régression — a été menée pour comparer le coût des médicaments sur une période de deux années consécutives, soit la période de douze mois avant et celle immédiatement après la mise en place du poste de pharmacien clinicien.Résultats : Des 48 interventions entreprises par le pharmacien durant la période de quatre semaines, 47 (98 %) ont été acceptées par le médecin traitant et 86 % (38/44) se sont révélées avoir un effet positif sur les soins aux patients. Le coût des médicaments par jour-patient était 14 % plus bas dans l’année suivant la mise en place du poste de pharmacien clinicien, et on a observé une différence statistiquement significative dans les huit derniers mois de cette année (p = 0,0019). Le coût total des médicaments a diminué de 21 %, ce qui s’est traduit par des économies de 5 485,80 .Conclusion : Le pharmacien clinicien a eu une incidence positive à la fois sur les résultats thérapeutiques et sur les coûts au sein de cette population d’enfants en milieu de soins de santé mentale
