    Examining gender differences in young men and women’s goals for a technology-driven weight loss intervention

    Background Emerging adults (EA) are at high risk for weight gain and obesity yet are underrepresented in behavioral weight loss (BWL) programs and fare worse than their older counterparts when they join these programs. Further, even within BWL programs adapted specifically for this population, young men are particularly challenging to recruit, representing less than 20% of samples in recent trials. One hypothesis for the poor recruitment of men is that men’s goals for a weight loss program are not described in recruitment messages for these studies. Understanding whether men and women’s personal goals for participating in a weight loss program could be a key way to tailor recruitment messaging. As such, the objective of this study is to compare young men and women’s personal goals for a BWL program. Methods Participants (N=382; 21.9+1.2 years; 83% female; BMI=33.5+4.9 kg/m2) were recruited for a technology-driven weight loss intervention adapted for this age group. At baseline, participants ranked their top 3 personal goals from a list of 15 areas in which they wanted to see change during the program. The most commonly reported areas (n=8) were coded as yes or no if the goal was ranked as one of their top 3. Chi-square tests were conducted to compare men and women on the most commonly reported goals for the program; using Bonferroni correction to adjust for multiple comparisons (p\u3c.006). Results Overall, the most commonly reported areas for wanting to change in the program were: weight (62.7%), physical fitness (43.7%), body fat percentage (25.7%), body shape (25.1%), energy level (24.1%), clothing size (19.6%), confidence (18.8%), and self-esteem (18.0%). Weight was the most commonly ranked as a top goal (43.5%), followed by physical fitness (11.3%) and body fat percentage (8.9%). No significant differences were found between men and women for personal goals. Conclusion Emerging adults have a desire to lose weight and body fat, as well as improve physical fitness when it comes to personal goals for a weight loss program. While there were no gender differences in goals for a weight loss program, this could be due to enrollment of a treatment seeking sample. Coupled with differential enrollment of young men, it is possible the results may not be representative of non-treatment seeking young men.     Outdoor workers and sun protection strategies: Two case study examples in Queensland, Australia

    Introduction: Outdoor workers are at risk of developing skin cancer because they are exposed to high levels of harmful ultraviolet radiation. The Outdoor Workers Sun Protection Project investigated sun protection strategies for high risk outdoor workers in rural and regional Australia. Methods: Fourteen workplaces (recruitment rate 37%) across four industries in rural and regional Queensland, Australia were recruited to the OWSPP. In 2011–2012, data were collected using pre- and post-intervention interviews and discussion groups. This article presents two workplaces as case study examples. Results: The flat organisational structure of workplace 1 supported the implementation of the Sun Safety Action Plan (SSAP), whilst the hierarchical organisational nature of workplace 2 delayed implementation of the SSAP. Neither workplace had an existing sun protection policy but both workplaces adopted one. An effect related to the researchers’ presence was seen in workplace 1 and to a lesser degree in workplace 2. Overt reciprocity was seen between management and workers in workplace 1 but this was not so evident in workplace 2. In both workplaces, the role of the workplace champion was pivotal to SSAP progression. Conclusions: These two case studies highlight a number of contextually bound workplace characteristics related to sun safety. These issues are (1) the structure of workplace, (2) policy, (3) an effect related to the researchers’ presence, (4) the workplace champion and (5) reciprocity. There are several recommendations from this article. Workplace health promotion strategies for sun safety need to be contextualised to individual workplaces to take advantage of the strengths of the workplace and to build capacity

    Recruitment of young adults for weight gain prevention: randomized comparison of direct mail strategies

    Abstract Background Recruiting young adults (ages 18–35 years) into weight gain prevention intervention studies is challenging and men are particularly difficult to reach. This paper describes two studies designed to improve recruitment for a randomized trial of weight gain prevention interventions. Study 1 used a quasi-experimental design to test the effect of two types of direct mailings on their overall reach. Study 2 used a randomized design to test the effect of using targeted messages to increase recruitment of men into the trial. Methods For Study 1, 60,000 male and female young-adult households were randomly assigned to receive either a recruitment brochure or postcard. Visits to recruitment websites during each mailing period were used to assess response to each mailing. Study 2 focused on postcard recruitment only. These households received either a targeted or generic recruitment postcard, where targeted postcards included the word “Men” in the headline text. Response rates to each type of card were categorized based on participant report of mailing received. Results The reach of the postcards and brochures were similar (421 and 386 website visits, respectively; P = 0.22). Individuals who received the brochure were more likely to initiate the online screener than those who received a postcard (P = 0.01). In Study 2, of those who completed the telephone screening, 60.9 % of men (n = 23) had received the targeted postcard as compared to the generic postcard (39.1 %, P = 0.30). The reverse was true for women (n = 62, 38.7 vs. 61.3 %, P = 0.08). Conclusions These studies suggest there was little difference in the reach of postcards versus brochures. However, recipients of brochures were more likely to continue to the next stage of study participation. As expected, men’s response to the weight gain prevention messages was lower than women’s response; but using targeted messages appears to have modestly increased the proportion of male respondents. These studies add to the limited experimental literature on recruitment messaging and provide further indication for using targeted messages to reach underrepresented populations while providing initial evidence on the effect of mailing type on message reach.     Changes in Outdoor Workers' Sun-Related Attitudes, Beliefs, and Behaviors: A Pre–Post Workplace Intervention

    Objective: To evaluate changes in outdoor workers' sun-related attitudes, beliefs, and behaviors in response to a health promotion intervention using a participatory action research process. Methods: Fourteen workplaces across four outdoor industry types worked collaboratively with the project team to develop tailored sun protection action plans. Workers were assessed before and after the 18-month intervention. Results: Outdoor workers reported increases in workplace support for sun protection (P < 0.01) and personal use of sun protection (P < 0.01). More workers reported seeking natural shade (+20%) and wearing more personal protective equipment, including broad-brimmed hats (+25%), long-sleeved collared shirts (+19%), and long trousers (+16%). The proportion of workers reporting sunburn over the past 12 months was lower at postintervention (-14%) (P = 0.03); however, the intensity of reported sunburn increased. Conclusions: This intervention was successful in increasing workers' sun protective attitudes, beliefs, and behaviors

    Effect of teammates on changes in physical activity in a statewide campaign

    Objective—Most Americans do not meet physical activity recommendations. Statewide campaigns can effectively increase activity levels. Reported herein are physical activity outcomes from Shape Up Rhode Island (SURI) 2007, a statewide campaign to increase steps through team-based competition. Given the importance of social networks in behavior change, this paper focused on the effects of team and team characteristics on activity outcomes. Method—For 16-weeks, 5333 adults comprising 652 teams wore pedometers and reported their steps online. Results—Participants’ daily steps increased from 7029(3915) at baseline to 9393(5976) at SURI end (p\u3c.001). There was a significant intraclass correlation for step change among team members (ICC=.09); thus, an individual’s change in steps was influenced by what team they were on. Moreover, baseline team characteristics predicted individual step change; being on a more active team was associated with greater increases in activity for individual members (p\u3c.001), whereas being on a team with a broad range of steps was associated with smaller changes in activity for individual members (p=.02). Conclusion—These findings are the first to suggest that team members influence individual activity outcomes in team-based statewide campaigns. Future research should explore ways to use social network factors to enhance team-based physical activity programs

    A Statewide Intervention Reduces BMI in Adults: Shape Up Rhode Island Results

    Given the epidemic of obesity, approaches to weight loss that can be applied on a community, state, or national level are needed. We report results from Shape Up Rhode Island 2007 (SURI), a state-wide Internet based program involving team-based competition to increase physical activity and achieve weight loss. A total of 4,717 adults (84% female; mean BMI = 29.6 kg/m2 ) enrolled in the 16 week weight loss competition of SURI and 3311 completed at least 12 weeks. Completers reported losing 3.2 ± 3.4 kg, and 30% achieved a clinically significant weight loss of 5% or more. Although modest, these weight losses shifted the BMI distribution from a mean BMI of 29.4 to 28.2 kg/m2 and reduced the population that was obese from 39% to 31%. More conservative intent-to-treat analyses and analysis of 132 participants with objective weights still showed a significant reduction in BMI of −0.8 units. These findings suggest that state-wide weight loss campaigns can produce modest weight losses in large numbers of participants. These data provide a bench-mark that can be used for comparisons with other state-wide campaigns. Research on ways to improve such campaigns is needed

    Randomized trial comparing group size of periodic in-person sessions in a remotely delivered weight loss intervention

    Abstract Background Few randomized studies have examined differential effects of group size in behavioral weight control, especially in hybrid programs that include Internet treatment approaches. Methods Randomized controlled trial (n = 195) comparing a 4 month hybrid internet weight loss program coupled with monthly face to face groups of 100 persons (Large Group, LG; 1 group) or to the same approach with monthly groups of 20 persons (Small Group, SG; 4 groups). Repeated-measures mixed-model analysis with age and race as covariates were used to estimate primary (weight) and secondary outcomes, and to test group differences in change over time. Results The sample was 46.3 years old ±10.4, 90.3% female, and 51.9% non-white, with BMI 37.9 ± 8.4 kg/m2. Participants in the LG were more likely to return for the 4-month assessment visit than those in the SG (p = 0.04). Participants randomized to both the LG and SG conditions experienced significant WL over time (no between group difference: −4.1 kg and −3.7 kg, respectively) and weight loss was positively associated with attendance at monthly meetings and logins to the website. Satisfaction with the program was high and similar in both groups (94.4% reported that they were “satisfied” or “very satisfied”). Conclusions Using a hybrid approach of in-person and online weight loss interventions may be an effective way to reach larger and more diverse populations. Delivering the face to face component of the intervention in groups larger than those traditionally delivered (20–25 people) could increase the cost-effectiveness of group-based behavioral weight loss interventions.     Mass Stranding of Marine Birds Caused by a Surfactant-Producing Red Tide

    In November-December 2007 a widespread seabird mortality event occurred in Monterey Bay, California, USA, coincident with a massive red tide caused by the dinoflagellate Akashiwo sanguinea. Affected birds had a slimy yellow-green material on their feathers, which were saturated with water, and they were severely hypothermic. We determined that foam containing surfactant-like proteins, derived from organic matter of the red tide, coated their feathers and neutralized natural water repellency and insulation. No evidence of exposure to petroleum or other oils or biotoxins were found. This is the first documented case of its kind, but previous similar events may have gone undetected. The frequency and amplitude of red tides have increased in Monterey Bay since 2004, suggesting that impacts on wintering marine birds may continue or increase
