Avoiding Antiplatelet Reversal in Non-Operative Intracranial Hemorrhages: Functional Outcomes of Guideline-Based Practice
Introduction: Intracranial hemorrhage (ICH) is a common, life-threatening neurological pathology in aging patients, many of whom take antiplatelet medications with potential to worsen the hemorrhage. In the event of ICH, Thomas Jefferson University Hospital (TJUH) follows a protocol modeling the 2016 Neurocritical Care Society (NCS) joint guidelines for antiplatelet medication reversal. We analyzed pre- and post-NCS guideline data from TJUH for outcomes of non-operative ICH patients in order to tease out the potential benefits of this protocol.
Methods: This retrospective cohort study took place from January 2016 – Jan. 2018 at a tertiary care center: TJUH. Patients included were ³18 y.o., on antiplatelet therapy who, had CTs available for evaluation of expansion, and did not undergo surgical management. The primary outcomes measured for comparison were both the admission and discharge Glasgow Coma Scores (GCS), admission and discharge modified Rankin Scores (mRS), time to death, hematoma expansion, and in-hospital mortality. T-tests, the Kolmogorov-Smirnov-test, and Chi-Square test for independence were used.
Results: For pre- and post-protocol groups, no significant difference existed for GCS or mRS, at admission and discharge. There were no significant findings for in-hospital mortality and hemorrhage expansion.
Discussion: TJUH established a protocol in line with the 2016 NCS joint guidelines for managing ICH in patients on antiplatelet therapies. This protocol recommends discontinuing antiplatelet therapy and not transfusing platelets in patients not receiving surgical management. We examined the protocol efficacy have found no significant differences in the pre- and post-protocol groups, indicating patient outcomes may be equivalent
Survival after resection of malignant peripheral nerve sheath tumors:Introducing and validating a novel type-specific prognostic model
Background: This study aimed to assess the performance of currently available risk calculators in a cohort of patients with malignant peripheral nerve sheath tumors (MPNST) and to create an MPNST-specific prognostic model including type-specific predictors for overall survival (OS). Methods: This is a retrospective multicenter cohort study of patients with MPNST from 11 secondary or tertiary centers in The Netherlands, Italy and the United States of America. All patients diagnosed with primary MPNST who underwent macroscopically complete surgical resection from 2000 to 2019 were included in this study. A multivariable Cox proportional hazard model for OS was estimated with prespecified predictors (age, grade, size, NF-1 status, triton status, depth, tumor location, and surgical margin). Model performance was assessed for the Sarculator and PERSARC calculators by examining discrimination (C-index) and calibration (calibration plots and observed-expected statistic; O/E-statistic). Internal-external cross-validation by different regions was performed to evaluate the generalizability of the model. Results: A total of 507 patients with primary MPNSTs were included from 11 centers in 7 regions. During follow-up (median 8.7 years), 211 patients died. The C-index was 0.60 (95% CI 0.53-0.67) for both Sarculator and PERSARC. The MPNST-specific model had a pooled C-index of 0.69 (95%CI 0.65-0.73) at validation, with adequate discrimination and calibration across regions. Conclusions: The MPNST-specific MONACO model can be used to predict 3-, 5-, and 10-year OS in patients with primary MPNST who underwent macroscopically complete surgical resection. Further validation may refine the model to inform patients and physicians on prognosis and support them in shared decision-making.</p
The Tudor SND1 protein is an m6A RNA reader essential for replication of Kaposi’s sarcoma-associated herpesvirus
N6-methyladenosine (m6A) is the most abundant internal RNA modification of cellular mRNAs. m6A is recognised by YTH domain-containing proteins, which selectively bind to m6A-decorated RNAs regulating their turnover and translation. Using an m6A-modified hairpin present in the Kaposi’s sarcoma associated herpesvirus (KSHV) ORF50 RNA, we identified seven members from the ‘Royal family’ as putative m6A readers, including SND1. RIP-seq and eCLIP analysis characterised the SND1 binding profile transcriptome-wide, revealing SND1 as an m6A reader. We further demonstrate that the m6A modification of the ORF50 RNA is critical for SND1 binding, which in turn stabilises the ORF50 transcript. Importantly, SND1 depletion leads to inhibition of KSHV early gene expression showing that SND1 is essential for KSHV lytic replication. This work demonstrates that members of the ‘Royal family’ have m6A-reading ability, greatly increasing their epigenetic functions beyond protein methylation
Recognition of depression in people of different cultures: a qualitative study
<p>Abstract</p> <p>Background</p> <p>Many minority group patients who attend primary health care are depressed. To identify a depressive state when GPs see patients from other cultures than their own can be difficult because of cultural and gender differences in expressions and problems of communication. The aim of this study was to explore and analyse how GPs think and deliberate when seeing and treating patients from foreign countries who display potential depressive features.</p> <p>Methods</p> <p>The data were collected in focus groups and through individual interviews with GPs in northern Sweden and analysed by qualitative content analysis.</p> <p>Results</p> <p>In the analysis three themes, based on various categories, emerged; "Realizing the background", "Struggling for clarity" and "Optimizing management". Patients' early life events of importance were often unknown which blurred the accuracy. Reactions to trauma, cultural frictions and conflicts between the new and old gender norms made the diagnostic process difficult. The patient-doctor encounter comprised misconceptions, and social roles in the meetings were sometimes confused. GPs based their judgement mainly on clinical intuition and the established classification of depressive disorders was discussed. Tools for management and adequate action were diffuse.</p> <p>Conclusion</p> <p>Dialogue about patients' illness narratives and social context are crucial. There is a need for tools for multicultural, general practice care in the depressive spectrum. It is also essential to be aware of GPs' own conceptions in order to avoid stereotypes and not to under- or overestimate the occurrence of depressive symptoms</p

Prevalence of carpal tunnel syndrome symptoms among neurosurgical spine surgeons
Background: Neurosurgeons, particularly spine surgeons, have high exposure to vibrations via electric or pneumatic drills and repetitive motion. Although no data exist for the prevalence of carpal tunnel syndrome (CTS) among these surgeons, anecdotal evidence suggests the rate of symptoms is higher than in the general population. Methods: An anonymous questionnaire was developed to assess demographics, practice patterns, presence of CTS symptoms, and treatment (time off, bracing, medication, injections, surgery). The survey was sent via anonymous email link to members of the AANS/CNS Joint Section on Disorders of the Spine and Peripheral Nerve. Results: 101 members responded: 44 reported at least one symptom related to CTS (43.6%). There was no statistically significant relationship between overall or spine case volume, the number of cases performed annually/daily, and CTS symptoms. Respondents working in non-teaching settings were significantly more likely to have CTS symptoms than academic teaching institutions (50.0% v. 45.0%; p = 0.0112). Conclusions: Our survey demonstrated CTS to be more prevalent in spine neurosurgeons (43.6%) than in the general population (1–5%). The lack of significant association between most practice-based metrics and CTS symptoms may indicate that respondents have a minimum case volume that exceeds the amount of vibration exposure/repetitive motion to develop symptoms. The significantly increased prevalence of CTS among neurosurgeons at non-teaching institutions suggests that residents provide operative assistance offsetting the vibration exposure/repetitive motion by attendings. Further research may determine the root cause for the high prevalence of CTS in spine neurosurgeons and devise methods for reducing vibration exposure
