    Objective dysphonia quantification in vocal fold paralysis: comparing nonlinear with classical measures

    Clinical acoustic voice recording analysis is usually performed using classical perturbation measures including jitter, shimmer and noise-to-harmonic ratios. However, restrictive mathematical limitations of these measures prevent analysis for severely dysphonic voices. Previous studies of alternative nonlinear random measures addressed wide varieties of vocal pathologies. Here, we analyze a single vocal pathology cohort, testing the performance of these alternative measures alongside classical measures.

We present voice analysis pre- and post-operatively in unilateral vocal fold paralysis (UVFP) patients and healthy controls, patients undergoing standard medialisation thyroplasty surgery, using jitter, shimmer and noise-to-harmonic ratio (NHR), and nonlinear recurrence period density entropy (RPDE), detrended fluctuation analysis (DFA) and correlation dimension. Systematizing the preparative editing of the recordings, we found that the novel measures were more stable and hence reliable, than the classical measures, on healthy controls.

RPDE and jitter are sensitive to improvements pre- to post-operation. Shimmer, NHR and DFA showed no significant change (p > 0.05). All measures detect statistically significant and clinically important differences between controls and patients, both treated and untreated (p < 0.001, AUC > 0.7). Pre- to post-operation, GRBAS ratings show statistically significant and clinically important improvement in overall dysphonia grade (G) (AUC = 0.946, p < 0.001).

Re-calculating AUCs from other study data, we compare these results in terms of clinical importance. We conclude that, when preparative editing is systematized, nonlinear random measures may be useful UVFP treatment effectiveness monitoring tools, and there may be applications for other forms of dysphonia.

    Cut-off rate calculations for the outer channel in a concatenated cooling system

    Concatenated codes were long used as a practical means of achieving long block or constraint lengths for combating errors on very noisy channels. The inner and outer encoders are normally separated by an interleaver, so that decoded error bursts coming from the inner decoder are randomized before entering the outer decoder. The effectiveness of this interleaver is examined by calculating the cut-off rate of the outer channel seen by the outer decoder with and without interleaving. Interleaving never hurts the performance of a concatenated code, and when the inner code rate is near the cut-off rate of the inner channel, interleaving significantly improves code performance

    Topological Defects in Twisted Bundles of Two-Dimensionally Ordered Filaments

    Twisted assemblies of filaments in ropes, cables and bundles are essential structural elements in wide use in macroscopic materials as well as within the cells and tissues of living organisms. We develop the unique, non-linear elastic properties of twisted filament bundles that derive from generic properties of two-dimensional line-ordered materials. Continuum elasticity reveals a formal equivalence between the elastic stresses induced by bundle twist and those induced by the positive curvature in thin, elastic sheets. These geometrically-induced stresses can be screened by 5-fold disclination defects in lattice packing, and we predict a discrete spectrum elastic energy groundstates associated with integer numbers of disclinations in cylindrical bundles. Finally, we show that elastic-energy groundstates are extremely sensitive to defect position in the cross-section, with off-center disclinations driving the entire bundle to buckle, adopting globally writhing configurations.Comment: 4.1 pages; 3 figure

    Social media brings opportunities for local governments to innovatively engage the public to enhance service delivery. Using the City of Austin, Texas as a backdrop, we assess the Austin Police Department’s utilization of social media in its communication efforts around community policing. This assessment develops a practical model to evaluate its community policing communication policies and practices by way of social media. The findings from this assessment provide several lessons and recommendations for practitioners to use in their efforts to integrate social media into public service delivery. These lessons and recommendations drawn from this case can provide an example of the potential strengths and weaknesses behind incorporating social media within public service delivery

    Leveraging a New Building to Overcome First and Second-Order Barriers to Faculty Technology Integration

    This study uses the context of a School of Education’s transition from an old adapted building to a new dedicated structure to explore ways in which that occasion created an opportunity address first and second-order barriers to faculty technology integration and pedagogical innovation.  Barriers were addressed through the convergence of a purposeful application of an adult learner model to technology support and planning, and the opportunities provided by the move to a new building.  Findings from pre- and post-move faculty and administrator interviews highlight intended and unintended strategic, symbolic, and functional outcomes.

    Predicting Beef Palatability

    The search for factors influencing the palatability of beef continues. The relative importance of variables in the USDA quality grading system that currently predicts acceptability and palatability has been questioned. The effect of marbling on palatability is one of the more controversial aspects. Marbling is one of the prime factors in determining the quality grade of a beef carcass and researchers have shown that other variables may play an equal or more important role influencing the eating quality of beef. The purpose of this study was to evaluate the accuracy of commonly-used predictors of palatability and to consider the possible influence of other carcass measures as predictors of palatability. Palatability, as defined in this study, was measured by the subjective taste panel scores for flavor, juiciness and tenderness.     Diagnostic errors in paediatric cardiac intensive care

    AbstractIntroductionDiagnostic errors cause significant patient harm and increase costs. Data characterising such errors in the paediatric cardiac intensive care population are limited. We sought to understand the perceived frequency and types of diagnostic errors in the paediatric cardiac ICU.MethodsPaediatric cardiac ICU practitioners including attending and trainee physicians, nurse practitioners, physician assistants, and registered nurses at three North American tertiary cardiac centres were surveyed between October 2014 and January 2015.ResultsThe response rate was 46% (N=200). Most respondents (81%) perceived that diagnostic errors harm patients more than five times per year. More than half (65%) reported that errors permanently harm patients, and up to 18% perceived that diagnostic errors contributed to death or severe permanent harm more than five times per year. Medication side effects and psychiatric conditions were thought to be most commonly misdiagnosed. Physician groups also ranked pulmonary overcirculation and viral illness to be commonly misdiagnosed as bacterial illness. Inadequate care coordination, data assessment, and high clinician workload were cited as contributory factors. Delayed diagnostic studies and interventions related to the severity of the patient’s condition were thought to be the most commonly reported process breakdowns. All surveyed groups ranked improving teamwork and feedback pathways as strategies to explore for preventing future diagnostic errors.ConclusionsPaediatric cardiac intensive care practitioners perceive that diagnostic errors causing permanent harm are common and associated more with systematic and process breakdowns than with cognitive limitations.</jats:sec

    DNA Typing of Mycobacterium bovis Isolates from Badgers (Meles meles) Culled from Areas in Ireland with Different Levels of Tuberculosis Prevalence

    Badgers (Meles meles) have been implicated in the transmission of Mycobacterium bovis infection to cattle in Ireland and UK. Recent studies in Ireland have shown that although the disease is endemic in badgers, the prevalence of disease is not uniform throughout the country and can vary among subpopulations. The extent to which the prevalence levels in badgers impact on the prevalence in cattle is not known. Previously, DNA fingerprinting has shown that M. bovis strain types are shared between badgers and cattle, and that there are a large number of strain types circulating in the two species. In this study we have carried out spoligotyping and variable number tandem repeat (VNTR) analysis of M. bovis isolates from two groups of badgers, representing a wide geographic area, with different tuberculosis prevalence levels. The results of the typing show that there is no geographic clustering of strain types associated with prevalence. However, two VNTR profiles were identified that appear to be associated with high- and low-prevalence M. bovis infection levels, respectively. In addition, spoligotyping and VNTR analysis has provided evidence, for the first time, of multiple infections of individual badgers with different M. bovis strains
