    Periodontal condition and recurrence of periodontitis associated with alcohol consumption in periodontal maintenance therapy

    This study followed individuals in periodontal maintenance therapy (PMT) over 6 years and longitudinally evaluated the effects of the frequency of alcohol consumption on the recurrence of periodontitis (RP). From a 6-year follow-up cohort study with 268 individuals under PMT, 142 patients who attended at least one PMT visit within 12 months were determined to be eligible. Based on their alcohol consumption, participants were categorized into 3 groups: none or occasional alcohol use (NA; n=88), moderate alcohol use (MA; n=26) and intense alcohol use (IA; n=24). Complete periodontal examination and alcohol consumption were evaluated at 2 times, T1 (after active periodontal therapy) and T2 (6 years). The frequencies of RP in the NA, MA and IA groups were 46.5%, 57.6%, and 79.1%, respectively. The following variables were significantly associated with RP in final multivariate logistic regression model: age >50 years old (OR = 1.79; 95%CI 1.42-2.91; p=0.002), current smoking (OR = 2.42; 95%CI 1.33-4.31; p=0.001), and intensive alcohol use (OR = 1.96; 95%CI: 1.37-2.64; p=0.024). Interaction between intensive alcohol use and smoking showed a high OR estimate of 3.15 (95%CI 1.29-6.32) for RP. IA individuals undergoing PMT presented worse periodontal condition, higher rates of RP and tooth loss when compared to NA individuals. Additionally, the interaction between intensive alcohol use and smoking significantly increased the risk for RP

    A disciplina de história ambiental, introduzida no meio acadêmico no início da década de 1970, traz consigo uma nova maneira de se estudar a história, rompendo com a ótica dualista e determinista vigente à época, de modo a considerar a inter- relação entre ser humano e meio natural como ponto fundamental para a análise historiográfica. Com o intuito de verificar a relevância da disciplina de história ambiental nos cursos de graduação em história, analisou-se as matrizes curriculares dos cursos de graduação presenciais, ofertados em Goiás, comparando-as às grades dos 10 melhores cursos de graduação em história do país utilizando o ranking elaborado pela Folha de São Paulo, em 2018. Verificou-se que, embora transcorrido 49 anos, desde a primeira vez que a disciplina de história ambiental foi ofertada no mundo, sua inserção nos cursos de graduação em história no Brasil ainda é escassa, estando presente em apenas 20% dos cursos pesquisados, apontando para o desprestígio desta matéria em termos de concepção da relevância de se estudar a história tendo como base a inter-relação entre o ser humano e a natureza no campo historiográfico

    Impact of Routine Use of Drug-eluting Stents in Contemporary Interventional Cardiology at a Tertiary Center: One-decade Experience of the DESIRE Registry

    ABSTRACTBackgroundDrug-eluting stents (DES) have changed contemporary interventional cardiology practice, enabling the approach of increasingly more complex clinical and angiographic scenarios. The objective of this study was to demonstrate the changes observed in the last 10 years in the indication and practice of percutaneous coronary intervention (PCI) at a tertiary private hospital in the State of São Paulo.MethodsDESIRE is a single-center prospective registry aiming at following the acute and late outcomes of consecutive patients treated by DES.ResultsFrom 2002 to 2011, 4,299 patients were included, with mean age of 64.3±11.2years, 23% were female and 30.5 were diabetic. The total number of lesions treated was 6,518 of which 61.5% were type B2/C. During the course of the study, DES were progressively more used, reaching a penetration of 88.4% in 2011. The complexity of PCIs has increased and in the past year 1.76 lesions per patient were treated with an average of 1.89 DES. The SYNTAX score increased from 12.3±4.4 (2002-2006) to 15.7±4.7 (2007-2011). Clinical follow-up was obtained in 98.2% of the patients, with a median of 5.2years, and during this period target-lesion revascularization rate was 5%, myocardial infarction was 6.7% and cardiovascular death was 4.1%. Stent thrombosis was observed in 2.4% of the cases.ConclusionsOur results showed a marked increment in the complexity profile of patients treated in the last 10 years and at the same time confirm the long-term effectiveness of DES, despite the clinical and angiographic profile of patients

    Macroeconomic foundations in the perspectives of Marx and Keynes : contributions to the heterodox thought

    A necessidade de definir fundamentos microeconômicos para os modelos macroeconômicos, presente no pensamento dominante na economia contemporânea, pode ser questionada com base em diferentes abordagens teóricas. Posições como as de Marx e Keynes permitem a apreensão das ações dos agentes econômicos (grupos ou classes) como influenciadas e limitadas pelo ambiente social e macroeconômico, fornecendo visões de fundamentos macroeconômicos da microeconomia, ao invés da visão ortodoxa de fundamentos micro da macroeconomia. Essa forma de apreensão lança luz sobre diferenças de perspectivas da heterodoxia quando se trata de intervir sobre o contexto macroeconômico.The need to define microeconomic foundations for macroeconomic models, which is present in the mainstream thinking in contemporary economics, can be questioned on the basis of different theoretical approaches. Positions like those of Marx and Keynes permit the comprehension of the actions of economic agents (groups or classes) as influenced and limited by the social and macroeconomic environment, providing views of macroeconomic foundations of microeconomics, instead of the orthodox view of microfoundations of macroeconomics. This comprehension sheds light on distinct perspectives in heterodox thought regarding the intervention in the macroeconomic context

    Cobertura vacinal da Pentavalente e da Estratégia de Saúde da Família

    Objective: to evaluate Pentavalent vaccination coverage in children under one year old in the Brazilian regions and capitals and the coverage of the Family Health Strategy (FHS). Method: a study of a historical series with secondary data from national vaccine management systems and the FHS, from 2014 to 2018, analyzed by descriptive statistics. Results: the Brazilian regions have maintained Pentavalent vaccination coverage below 95% since 2017. The scope of the FHS did not significantly impact on the achievement of the recommended vaccination target. Conclusion: low vaccination coverage increases the risk for the reintroduction of controlled diseases. Access to the health services needs to be facilitated and vaccination prioritized when planning the actions of the FHS team. Nursing is able to create local actions to reverse this scenario, with the support of the managers to intensify campaigns and combat the causes that have hindered the achievement of the goals.Objetivo: avaliar a cobertura vacinal da Pentavalente em menores de um ano nas regiões e capitais brasileiras e a cobertura da Estratégia de Saúde da Família (ESF). Método: estudo de série histórica de dados secundários provenientes de sistemas de gestão nacional de vacinas e da ESF, no período de 2014 a 2018, analisados por estatística descritiva. Resultados: as regiões brasileiras mantêm a cobertura vacinal da Pentavalente abaixo de 95%, desde 2017. A abrangência da ESF não impactou significativamente no alcance da meta vacinal preconizada. Conclusão: as baixas coberturas vacinais aumentam o risco de reintrodução de doenças controladas. O acesso aos serviços de saúde precisa ser facilitado e a vacinação priorizada no planejamento das ações da equipe da ESF. A enfermagem tem condições de criar ações locais para reverter este cenário, com o apoio dos gestores para intensificar campanhas e combater as causas que têm prejudicado o alcance das metas
