Measuring physical activity with activity monitors in patients with heart failure: from literature to practice. A position paper from the Committee on Exercise Physiology and Training of the Heart Failure Association of the European Society of Cardiology
The aims of this paper were to provide an overview of available activity monitors used in research in patients with heart failure and to identify the key criteria in the selection of the most appropriate activity monitor for collecting, reporting, and analysing physical activity in heart failure research. This study was conducted in three parts. First, the literature was systematically reviewed to identify physical activity concepts and activity monitors used in heart failure research. Second, an additional scoping literature search for validation of these activity monitors was conducted. Third, the most appropriate criteria in the selection of activity monitors were identified. Nine activity monitors were evaluated in terms of size, weight, placement, costs, data storage, water resistance, outcomes and validation, and cut-off points for physical activity intensity levels were discussed. The choice of a monitor should depend on the research aims, study population and design regarding physical activity. If the aim is to motivate patients to be active or set goals, a less rigorously tested tool can be considered. On the other hand, if the aim is to measure physical activity and its changes over time or following treatment adjustment, it is important to choose a valid activity monitor with a storage and battery longevity of at least one week. The device should provide raw data and valid cut-off points should be chosen for analysing physical activity intensity levels. Other considerations in choosing an activity monitor should include data storage location and ownership and the upfront costs of the device

Intégration des soins aux personnes ùgées fragiles en Belgique : une évaluation de la troisiÚme phase du Protocole 3
27 p.ill.,La convention Protocole 3 a Ă©tĂ© lancĂ©e en 2010 pour soutenir des projets innovants de soins Ă domicile pour des personnes ĂągĂ©es fragiles nĂ©cessitant des soins complexes. Elle a subi quelques remaniements au fil du temps, mais est restĂ©e Ă lâĂ©tat de projet pilote. LâINAMI a demandĂ© au Centre fĂ©dĂ©ral dâExpertise des Soins de santĂ© (KCE) dâen faire une Ă©valuation avant de dĂ©cider de son Ă©ventuelle pĂ©rennisation.
AprĂšs avoir interrogĂ© tous les acteurs impliquĂ©s ainsi que des bĂ©nĂ©ficiaires, le KCE constate que, mĂȘme si les rĂ©sultats sont globalement satisfaisants, les remaniements de la convention mettent en pĂ©ril sa philosophie premiĂšre et risquent de dĂ©motiver les soignants qui sây impliquent. Il recommande que les soins proposĂ©s dans ce cadre soient transformĂ©s en un modĂšle gĂ©nĂ©ralisĂ© de soins intĂ©grĂ©s et bĂ©nĂ©ficient dâun cadre structurel stable. 42 p.ill.,Les incidents CBRNe sont des situations dâurgence caractĂ©risĂ©es par la libĂ©ration de substances chimiques (C) ou biologiques (B) ou par une contamination radiologique/nuclĂ©aire (RN), parfois par le biais dâune explosion (e). Ă titre dâexemples, on peut citer un accident dans une entreprise de produits chimiques ou lors du transport de substances dangereuses, une catastrophe nuclĂ©aire ou encore un attentat terroriste. SollicitĂ© pour rĂ©flĂ©chir aux moyens dâorganiser encore mieux lâaide mĂ©dicale dans ce type de situation, le Centre FĂ©dĂ©ral dâExpertise des Soins de SantĂ© (KCE) a consultĂ© de trĂšs nombreux acteurs de terrain. Il plaide notamment en faveur du transfert de toutes les victimes griĂšvement blessĂ©es vers des hĂŽpitaux spĂ©cialisĂ©s disposant dâune expertise CBRNe suffisante (les « hĂŽpitaux CBRNe de Type I »). Les soins aux victimes moins griĂšvement blessĂ©es, eux, pourraient ĂȘtre administrĂ©s ou poursuivis dans les autres hĂŽpitaux (dits « de type II »). Sur les lieux de la catastrophe, la mesure la plus importante est de procĂ©der Ă une dĂ©contamination la plus rapide possible, doublĂ©e dâune aide mĂ©dicale.
