Impact of use of optical surface imaging on initial patient setup for stereotactic body radiotherapy treatments
Purpose
To evaluate the effectiveness of surface image guidance (SG) for preâimaging setup of stereotactic body radiotherapy (SBRT) patients, and to investigate the impact of SG reference surface selection on this process.
Methods and materials
284 SBRT fractions (SGâSBRT = 113, nonâSGâSBRT = 171) were retrospectively evaluated. Differences between initial (preâimaging) and treatment couch positions were extracted from the recordâandâverify system and compared for the two groups. Rotational setup discrepancies were also computed. The utility of orthogonal kVs in reducing CBCT shifts in the SGâSBRT/nonâSGâSBRT groups was also calculated. Additionally, the number of CBCTs acquired for setup was recorded and the average for each cohort was compared. These data served to evaluate the effectiveness of surface imaging in preâimaging patient positioning and its potential impact on the necessity of including orthogonal kVs for setup. Since reference surface selection can affect SG setup, daily surface reproducibility was estimated by comparing cameraâacquired surface references (VRT surface) at each fraction to the external surface of the planning CT (DICOM surface) and to the VRT surface from the previous fraction.
Results
The reduction in all initialâtoâtreatment translation/rotation differences when using SGâSBRT was statistically significant (RankâSum test, Îą = 0.05). Orthogonal kV imaging kept CBCT shifts below reimaging thresholds in 19%/51% of fractions for SGâSBRT/nonâSGâSBRT cohorts. Differences in average number of CBCTs acquired were not statistically significant. The reference surface study found no statistically significant differences between the use of DICOM or VRT surfaces.
Conclusions
SGâSBRT improved preâimaging treatment setup compared to inâroom laser localization alone. It decreased the necessity of orthogonal kV imaging prior to CBCT but did not affect the average number of CBCTs acquired for setup. The selection of reference surface did not have a significant impact on initial patient positioning
Experiential Career Exploration: Qualitative Examination of a Group-Based Intervention
This study explores the self-reported experience of vocationally stalled adults transitioning from welfare to work as reported on the Session Impact Scale and uses Consensual Qualitative Research (CQR) to analyze their observed in-the-moment interactions in an experiential career preparation intervention. The analysis resulted in five domains, Affiliation, Barriers, New Knowledge, Isolated Noteworthy Events, and Negative Impacts, and 20 categories. Implications for the practice of career counseling as well as the training of those who work with transitioning assistance recipients and vocationally stalled adults are discussed. Suggestions for further research with this population in the context of career preparation are also discussed
Acute reduction in secretory immunoglobulin A following smoking cessation
Smokers report an increase in upper respiratory infections in the early phase of stopping smoking. One possible cause is a depletion in secretory immunoglobulin A (S-IgA) which has been observed in one study. The present study sought to establish this finding in smokers using nicotine patches. Ninety-two smokers, trying to stop smoking, were assessed whilst smoking and for up to six weeks of abstinence. All smokers were prescribed 15 mg 16-h nicotine patches. Among abstinent smokers, changes in S-IgA and saliva volume were assessed. During the preliminary analyses, we observed that for the pre-smoking cessation measure a longer time since the last cigarette was significantly related to Lower S-IgA levels (P = 0.006). Consequently, the main analysis, of changes in S-IgA from pre-cessation to post-cessation, was confined to those who had smoked within 0.5-1.5 h of the pre-cessation measure (n = 51). There was a significant decline in S-IgA, relative to pre-smoking abstinence levels, following abstinence of one day (P = 0.027), but Levels returned to pre-abstinence values after one week. There was no evidence of any significant changes in saliva volume following smoking cessation, relative to pre-cessation levels. Users of 15 mg patches are likely to experience a decline in S-IgA levels on the first day of smoking cessation, independent of saliva volumes, and this decline in S-IgA is Likely to occur acutely, within the first few hours of smoking abstinence. This acute drop in S-IgA appears to stem from a factor other than depletion of nicotine from the body. The observed decrease in S-IgA may help to explain the increased susceptibility of smokers to upper respiratory tract infections in the immediate post-cessation period. (C) 2004 Elsevier Ltd. Magnetic flux reversal in laminated Ni-Fe films
Anomalously fast flux reversal has been observed in films made of Ni-Fe layers separated by SiO. The speed of reversal increases as the number of identical layers of Ni-Fe increases. For a 5-layer film, the anomalous speed is observed in films with the SiO thickness as great as 1600 Ă
. Reversal time curves presented as a family of curves of 1/Ď = fh_â´ with h_s as a parameter have two regions. The high-drive region has a lower slope in the laminated films when compared to the single-layer films. For this family of curves, a switching coefficient S_(w') can be defined, as the inverse slope, in a manner similar to the definition of S_w for 1/Ď = f(h_(s)) with h_â´ as a parameter. For films with from two to five layers, Sw' is constant at 1 x 10^(-3) Îźs and is smaller by an order of magnitude for the single-layer films. A dual loop experiment is used to confirm that coherent rotation is not a dominant mechanism. It is concluded that a model must satisfy the following criteria to successfully describe flux reversal in the laminated films: It must provide rapid flux reversal for fields less than H_k, an insensitivity to transverse fields either constant or pulsed, and an interaction that can survive over a wide range of SiO thicknesses
The two-fluid model with superfluid entropy
The two-fluid model of liquid helium is generalized to the case that the
superfluid fraction has a small entropy content. We present theoretical
arguments in favour of such a small superfluid entropy. In the generalized
two-fluid model various sound modes of HeII are investigated. In a
superleak carrying a persistent current the superfluid entropy leads to a new
sound mode which we call sixth sound. The relation between the sixth sound and

Melting Alpine Glaciers Enrich High-Elevation Lakes with Reactive Nitrogen
Alpine glaciers have receded substantially over the last century in many regions of the world. Resulting changes in glacial runoff not only affect the hydrological cycle, but can also alter the physical (i.e., turbidity from glacial flour) and biogeochemical properties of downstream ecosystems. Here we compare nutrient concentrations, transparency gradients, algal biomass, and fossil diatom species richness in two sets of high-elevation lakes: those fed by snowpack melt alone (SF lakes) and those fed by both glacial and snowpack meltwaters (GSF lakes). We found that nitrate (NO3-) concentrations in the GSF lakes were 1-2 orders of magnitude higher than in SF lakes. Although nitrogen (N) limitation is common in alpine lakes, algal biomass was lower in highly N-enriched GSF lakes than in the N-poor SF lakes. Contrary to expectations, GSF lakes were more transparent than SF lakes to ultraviolet and equally transparent to photosynthetically active radiation.Sediment diatom assemblages had lower taxonomic richness in the GSF lakes, a feature that has persisted over the last century. Our results demonstrate that the presence of glaciers on alpine watersheds more strongly influences NO3- concentrations in high-elevation lake ecosystems than any other geomorphic or biogeographic characteristic

Strontium as a tracer of weathering processes in a silicate catchment polluted by acid atmospheric inputs, Strengbach, France
This paper determines the weathering and atmospheric contributions of Ca in surface water from a small spruce forested silicate catchment (NâE France) receiving acid atmospheric inputs. The bedrock is a granite with K-feldspar and albite as dominant phases. The calcium content in plagioclase is low and the Ca/Na ratio in surface water is high, reflecting other sources of calcium from those expected from the weathering of major mineral phases. The biotite content is low. Only traces of apatite were detected while no calcite was found in spite of a major hydrothermal event having affected the granite. The strontium isotopic ratio 87Sr/86Sr and Sr content was used as a tracer of weathering and was determined in minerals and bulk bedrock, open field precipitation, throughfall, soil solution, spring and stream water. The Sr isotopic ratio of the reacting weathering end-member was predicted by simulating the alteration of the granite minerals by incorporating strontium into the waterârock interaction kinetic code KINDIS. In the early stages of waterârock interaction, K-feldspar and biotite strongly influence the isotopic composition of the weathering solution whereas, the Na-rich plagioclase appears to be the main long-term reactive weathering end-member. Approximately 50% of dissolved Sr in streamwater are atmospherically derived. The 87Sr/86Sr ratios of exchangeable Sr in the fine fraction at 1-m depth from a soil profile indicate that the amount of exchangeable Sr seems essentially controlled by atmospheric inputs. The exception is the deep saprolite where weathering processes could supply the Sr (and Ca). Na-Plagioclase weathering obviously control the chemistry and the isotopic composition of surface waters. The weathering of trace mineral plays a secondary role, the exception is for apatite when plagioclase is absent. Our hydrochemical, mineralogical and isotopic investigations show that a major part of the strong Ca losses detected in catchment hydrochemical budgets that result from the neutralization of acid precipitation has an atmospheric origin. Consequently, in the long term, in such areas, the availability of such an exchangeable base cation might be strongly limited and surface waters consequently acidified
Criticality and Superfluidity in liquid He-4 under Nonequilibrium Conditions
We review a striking array of recent experiments, and their theoretical
interpretations, on the superfluid transition in He in the presence of a
heat flux, . We define and evaluate a new set of critical point exponents.
The statics and dynamics of the superfluid-normal interface are discussed, with
special attention to the role of gravity. If is in the same direction as
gravity, a self-organized state can arise, in which the entire sample has a
uniform reduced temperature, on either the normal or superfluid side of the
transition. Finally, we review recent theory and experiment regarding the heat
capacity at constant . The excitement that surrounds this field arises from
the fact that advanced thermometry and the future availability of a
microgravity experimental platform aboard the International Space Station will
soon open to experimental exploration decades of reduced temperature that were

REFERQUAL: A pilot study of a new service quality assessment instrument in the GP Exercise Referral scheme setting
Background
The development of an instrument accurately assessing service quality in the GP Exercise Referral Scheme (ERS) industry could potentially inform scheme organisers of the factors that affect adherence rates leading to the implementation of strategic interventions aimed at reducing client drop-out.
Methods
A modified version of the SERVQUAL instrument was designed for use in the ERS setting and subsequently piloted amongst 27 ERS clients.
Results
Test re-test correlations were calculated via Pearson's 'r' or Spearman's 'rho', depending on whether the variables were Normally Distributed, to show a significant (mean r = 0.957, SD = 0.02, p < 0.05; mean rho = 0.934, SD = 0.03, p < 0.05) relationship between all items within the questionnaire. In addition, satisfactory internal consistency was demonstrated via Cronbach's 'Îą'. Furthermore, clients responded favourably towards the usability, wording and applicability of the instrument's items.
Conclusion
REFERQUAL is considered to represent promise as a suitable tool for future evaluation of service quality within the ERS community. Future research should further assess the validity and reliability of this instrument through the use of a confirmatory factor analysis to scrutinise the proposed dimensional structure
