    Towards a gender analysis of women and development: a case study of the gender division of labour in a rural black community in South Africa

    This thesis examines the topic of the position of women in rural development. This topic has become the focus of extensive international debate at theoretical and policy levels, but for historical reasons remains relatively undeveloped in South Africa. After reviewing a number of contemporary approaches, the thesis argues in favour of the "Gender and Development" approach, and applies this to a case study of the gender division of labour in a rural black community in South Africa. Chapter one reviews the variants of the "Women in Development" (WID) approaches, as well as the claims of "Global Feminism". It is argued that, although Global Feminism began as a critique of WID approaches, it is in fact similar in many respects to the contemporary "empowerment" focus within the WID tradition. Although useful, these approaches are not sufficiently explicit in their theorizing of gender relations in the context of development. Chapter 2 sets out the key elements of the "Gender and Development" approach, which emphasises the complexity of the issue of women's interests, and warns against assuming a commonality of interests amongst all women. Instead, the GAD approach demands a detailed investigation of socially constructed gender relations in specific communities, with a special focus on the gender division of labour. For GAD theorists such analysis is a requirement of development planning which seeks to advance the position of women. Chapter 3 spells out the methodological implications of the GAD approach, and develops a specific research design, influenced by GAD as well as feminist methodology, for the investigation of the gender-specific needs of women in Merino Walk, a rural black community. Chapter 4 presents a brief overview of the general context of rural women in Southern Africa, and a specific history of the Merino Walk community. Chapter 5 presents the results of the research. In the context of this thesis, the results are presented essentially as an illustration of the application of the GAD principles to a specific South African example. We are producing a demo that can still identify such cybercriminals.     Multi-element soil analysis: an assessment of its potential as an aid to archaeological interpretation

    Multi-element soil analysis is now an established technique in archaeology. It has been used to locate archaeological sites and define the extent of human activity beyond the structural remains, and to aid interpretation of space use in and around archaeological remains. This study aimed to evaluate the consistency of these soil element signatures between sites and hence their potential usefulness in archaeological studies. Known contexts on abandoned farms across the UK were sampled to test the relationships between element concentrations and known functional area and to assess inter-site variability. The results clearly show that there are significant differences in the soil chemistry of contrasting functional areas, particularly for Ba, Ca, P, Zn, Cu, Sr and Pb. Despite significant site specific effects, which appear to reflect individual anthropogenic practices rather than geological influences, there is sufficient similarity in the pattern of element enhancement to allow reliable interpretation of former function using discriminant models. Relating these enhancements to precise soil inputs, however, is more problematic because many important soil inputs do not contain distinct element fingerprints and because there is mixing of materials within the soil. There is also a suggestion that charcoal and bone play an important role in both the loading and post-depositional retention of Ca, Sr, P, Zn, and Cu and thus may be significant in the formation of soil element concentration patterns

    Tell formation processes as indicated from geoarchaeological and geochemical investigations at Xeropolis, Euboea, Greece

    Xeropolis is a tell site on the island of Euboea, Greece just to the east of the village of Lefkandi, and was occupied from the Early Bronze Age to the Early Iron Age. Excavations in recent years have provided an opportunity to investigate site formation processes using geoarchaeological and geochemical techniques. Sediments derived from the tell on the southern side have been lost by coastal erosion whilst those on the north mantle the flanking slope. Of particular interest is a homogeneous and unstratified deposit of over 2 m which overlies the archaeology near the southern perimeter of the summit area. The soil structure as evident in thin sections indicates a high degree of bioturbation, probably stimulated by recent manuring and cultivation. The implication is that tillage erosion has had a major impact on the morphology as well as on the surface soils of the tell. Despite such reworking and redeposition of near surface materials, it is still possible from multi-element analysis to identify the geochemical distinctiveness of six archaeological contexts (pit, house, plaster floor, alley, road and yard); pits and floors have high loadings of all elements except Pb; in contrast pits and floors have the lowest elemental concentrations

    Sequential element extraction of soils from abandoned farms: an investigation of the partitioning of anthropogenic element inputs from historic land use

    Enhanced soil element concentrations may serve as indicators not only of modern pollution, but also of former historic and/or pre-historic human activity. However, there is little consensus over the most appropriate means of extraction for identifying chemical signatures of modern and archaeological pollution. This study addressed this question by using a 5-step sequential extraction to examine the partitioning of elements within the soil. Samples were taken from known functional areas (hearth, house, byre, arable, and grazing areas) on a 19th century abandoned croft (small farm). A hot nitric acid digest and five-stage sequential extraction method were used to examine the partitioning of elements in soil and identify the current elemental distribution of anthropogenic contamination. The results indicate that although a significant proportion of Ca tends to be bound with exchangeable and weak acid soluble fractions, in the hearth and house areas there is also a significant proportion held within the recalcitrant residue. Pb concentrations tend to be associated with organic matter, ammonium oxalate extractable fractions and the residue, whilst Zn generally has a more even partitioning between the six soil fractions. The implications of this for extraction methodology are element and soil specific. However, the presence of a significant proportion of anthropogenically significant elements (including Ca, Pb, Zn, Sr, and Ba) within the resistant residue suggests the use of only a weak acid or an exchangeable fraction extraction would result in the loss of information from contamination resulting from former human activity. Hence, a total or pseudo-total extraction method is recommended for this type of study

    To date, 18 living recommendations for the clinical care of pregnant and postpartum women with COVID-19 have been issued by the National COVID-19 Clinical Evidence Taskforce. This includes recommendations on mode of birth, delayed umbilical cord clamping, skin-to-skin contact, breastfeeding, rooming-in, antenatal corticosteroids, angiotensin-converting enzyme inhibitors, disease-modifying treatments (including dexamethasone, remdesivir and hydroxychloroquine), venous thromboembolism prophylaxis and advanced respiratory support interventions (prone positioning and extracorporeal membrane oxygenation). Through continuous evidence surveillance, these living recommendations are updated in near real-time to ensure clinicians in Australia have reliable, evidence-based guidelines for clinical decision-making.     Association between mortality from suicide in England and antidepressant prescribing: an ecological study

    BACKGROUND: Antidepressant prescribing has been increasing in England. Studies in other countries suggest that while this may be associated with reduced suicide rates, it may also be associated with increased fatal poisoning from antidepressant drugs. We therefore conducted an ecological study to assess the association between prescription rates for antidepressants and suicide or fatal antidepressant-related poisoning in England. METHODS: The Office for National Statistics provided information on the number of suicides, antidepressant-related poisoning deaths and populations for England between 1993 and 2002. The Department of Health supplied data on prescriptions for all antidepressants dispensed in England. Associations between prescriptions and deaths were assessed using Spearman's rank correlation coefficient. RESULTS: There were 46,747 suicides, 3,987 deaths involving tricyclic antidepressants and 430 involving selective serotonin re-uptake inhibitors and other antidepressants. Increased antidepressant prescribing was statistically associated with a fall in suicide rates (Spearman's r(s )= -0.73, p = 0.02) and fatal poisoning involving tricyclic antidepressants (r(s )= -0.64, p = 0.05). In contrast, increased prescribing of selective serotonin re-uptake inhibitors and other antidepressants was statistically associated with an increase in fatal poisoning involving these drugs (r(s )= 0.99, p < 0.001). CONCLUSION: Increased prescribing of antidepressants may indicate improved diagnosis and treatment of depression in primary care. Our analysis suggests that this was accompanied by lower suicide rates. A decrease in poisoning deaths involving tricyclic antidepressants may suggest a change in preference for using serotonin reuptake inhibitors and other antidepressant drugs for high-risk patients. This may also partially explain the increase in deaths involving these drugs. Due to the ecological nature of the design, we cannot say conclusively whether reduced suicide rates are a direct consequence of increased antidepressant prescribing rates. To confirm these associations, individual level data on prescribing and suicide is needed

    Algorithmic approach to finding people with multiple sclerosis using routine healthcare data in Wales

    Background Identification of multiple sclerosis (MS) cases in routine healthcare data repositories remains challenging. MS can have a protracted diagnostic process and is rarely identified as a primary reason for admission to the hospital. Difficulties in identification are compounded in systems that do not include insurance or payer information concerning drug treatments or non-notifiable disease. Aim To develop an algorithm to reliably identify MS cases within a national health data bank. Method Retrospective analysis of the Secure Anonymised Information Linkage (SAIL) databank was used to identify MS cases using a novel algorithm. Sensitivity and specificity were tested using two existing independent MS datasets, one clinically validated and population-based and a second from a self-registered MS national registry. Results From 4 757 428 records, the algorithm identified 6194 living cases of MS within Wales on 31 December 2020 (prevalence 221.65 (95% CI 216.17 to 227.24) per 100 000). Case-finding sensitivity and specificity were 96.8% and 99.9% for the clinically validated population-based cohort and sensitivity was 96.7% for the self-declared registry population. Discussion The algorithm successfully identified MS cases within the SAIL databank with high sensitivity and specificity, verified by two independent populations and has important utility in large-scale epidemiological studies of MS

    A randomised controlled trial of a digital intervention (Renewed) to support symptom management, wellbeing and quality of life in cancer survivors

    Background: Many cancer survivors following primary treatment have prolonged poor quality of life.Aim: To determine the effectiveness of a bespoke digital intervention to support cancer survivors.Design: Pragmatic parallel open randomised trial.Setting: UK general practices.Methods: People having finished primary treatment (&lt;= 10 years previously) for colo-rectal, breast or prostate cancers, with European-Organization-for-Research-and-Treatment-of-Cancer QLQ-C30 score &lt;85, were randomised by online software to: 1) detailed ‘generic’ digital NHS support (‘LiveWell’;n=906), 2) a bespoke complex digital intervention (‘Renewed’;n=903) addressing symptom management, physical activity, diet, weight loss, distress, or 3) ‘Renewed-with-support’ (n=903): ‘Renewed’ with additional brief email and telephone support. Results: Mixed linear regression provided estimates of the differences between each intervention group and generic advice: at 6 months (primary time point: n’s respectively 806;749;705) all groups improved, with no significant between-group differences for EORTC QLQ-C30, but global health improved more in both intervention groups. By 12 months there were: small improvements in EORTC QLQ-C30 for Renewed-with-support (versus generic advice: 1.42, 95% CIs 0.33-2.51); both groups improved global health (12 months: renewed: 3.06, 1.39-4.74; renewed-with-support: 2.78, 1.08-4.48), dyspnoea, constipation, and enablement, and lower NHS costs (generic advice £265: in comparison respectively £141 (153-128) and £77 (90-65) lower); and for Renewed-with-support improvement in several other symptom subscales. No harms were identified.Conclusion: Cancer survivors quality of life improved with detailed generic online support. Robustly developed bespoke digital support provides limited additional short term benefit, but additional longer term improvement in global healthenablement and symptom management, with substantially lower NHS costs.<br/

    Killer immunoglobulin-like receptor and human leukocyte antigen-C genotypes in rheumatoid arthritis primary responders and non-responders to anti-TNF-α therapy

    The identification of patients who will respond to anti-tumor necrosis factor alpha (anti-TNF-α) therapy will improve the efficacy, safety, and economic impact of these agents. We investigated whether killer cell immunoglobulin-like receptor (KIR) genes are related to response to anti-TNF-α therapy in patients with rheumatoid arthritis (RA). Sixty-four RA patients and 100 healthy controls were genotyped for 16 KIR genes and human leukocyte antigen-C (HLA-C) group 1/2 using polymerase chain reaction sequence-specific oligonucleotide probes (PCR-SSOP). Each patient received anti-TNF-α therapy (adalimumab, etanercept, or infliximab), and clinical responses were evaluated after 3 months using the disease activity score in 28 joints (DAS28). We investigated the correlations between the carriership of KIR genes, HLA-C group 1/2 genes, and clinical data with response to therapy. Patients responding to therapy showed a significantly higher frequency of KIR2DS2/KIR2DL2 (67.7% R vs. 33.3% NR; P = 0.012). A positive clinical outcome was associated with an activating KIR–HLA genotype; KIR2DS2(+)HLA-C group 1/2 homozygous. Inversely, non-response was associated with the relatively inhibitory KIR2DS2(–)HLA-C group 1/2 heterozygous genotype. The KIR and HLA-C genotype of an RA patient may provide predictive information for response to anti-TNF-α therapy
