Conformational degrees of freedom and stability of splay-bend ordering in the limit of a very strong planar anchoring
We study the self-organization of flexible planar trimer particles on a
structureless surface. The molecules are made up of two mesogenic units linked
by a spacer, all of which are modeled as hard needles of the same length. Each
molecule can dynamically adopt two conformational states: an achiral
bent-shaped (cis-) and a chiral zigzag (trans-) one.
Using constant pressure Monte Carlo simulations and Onsager-type density
functional theory (DFT), we show that the system consisting of these molecules
exhibits a rich spectrum of (quasi-)liquid crystalline phases. The most
interesting observation is the identification of stable smectic splay-bend
() and chiral smectic A () phases. The phase is also
stable in the limit, where only cis-conformers are allowed. The second phase
occupying a considerable portion of the phase diagram is with chiral
layers, where the chirality of the neighboring layers is of opposite sign. The
study of the average fractions of the trans- and cis-conformers in various
phases shows that while in the isotropic phase all fractions are equally
populated, the phase is dominated by chiral conformers (zigzag), but
the achiral conformers win in the smectic splay-bend phase. To clarify the
possibility of stabilization of the nematic splay bend () phase for
trimers, the free energy of the and phases is calculated
within DFT for the cis-conformers, for densities where simulations show stable
. It turns out that the phase is unstable away from the phase
transition to the nematic phase, and its free energy is always higher than that
of , down to the transition to the nematic phase, although the
difference in free energies becomes extremely small when approaching the
transition

Antihypertensive Efficacy and Safety of Lacidipine in the Treatment of Patients with Mild to Moderate Essential Hypertension - Multi-Center Study POL-LACY
Wstęp Lacydypina jest długodziałającym antagonistą wapnia z grupy pochodnych dihydropirydynowych i charakteryzuje się łagodnym początkiem działania hipotensyjnego i trwającym 24 h efektem hipotensyjnym. W badaniach z użyciem tego leku potwierdzono jej skuteczność hipotensyjną, a także korzystny wpływ na układ sercowo-naczyniowy. Celem wieloośrodkowego badania POL-LACY była ocena skuteczności hipotensyjnej oraz tolerancji lacydypiny podawanej raz dziennie w zakresie dawek
2–6 mg chorym z łagodnym i umiarkowanym nadciśnieniem tętniczym pierwotnym.
Materiał i metody Otwarte badanie o charakterze otwartym przeprowadzono w 37 ośrodkach w Polsce, włączając do niego 470 chorych z łagodnym i umiarkowanym
nadciśnieniem tętniczym pierwotnym. Badanie składało się z dwóch faz. W trakcie 7-dniowego okresu kwalifikacyjnego u chorych odstawiano stosowane wcześniej leki hipotensyjne oraz wykonywano badania laboratoryjne. W trakcie 8-tygodniowego okresu
aktywnego leczenia chorzy otrzymywali lacydypinę w początkowej dawce dobowej 4 mg lub 2 mg u chorych powyżej 65 rż. pacjenci podawanej raz dziennie z możliwością zwiększenia dawki do 6 lub 4 mg na dobę u osób, u których nie uzyskano normalizacji ciśnienia po 4 tygodniach leczenia. Normalizację ciśnienia definiowano jako skurczowe
ciśnienie tętnicze (SBP) o wartości < 140 mm Hg i rozkurczowe ciśnienie tętnicze (DBP) wynoszące < 90 mm Hg. W czasie leczenia chorzy odbyli 2 wizyty kontrolne, po 4 i po 8 tygodniach, w trakcie których dokonywano oceny osiągniętego efektu hipotensyjnego oraz tolerancji leczenia.
Wyniki Badanie ukończyło 446 chorych (255 M, 191 K; średnia wieku 52 ± 13 lat). Pod koniec badania 9,9% chorych leczono dawką 2 mg, 72,9% chorych - dawką 4 mg, a 17,3% chorych - dawką 6 mg lacydypiny. Po 8 tygodniach leczenia tym preparatem w badanej grupie uzyskano znamienne obniżenie SBP o 23 mm Hg (p
< 0,0001) i DBP o 15 mm Hg (p < 0,0001) w pozycji siedzącej. Uzyskany efekty hipotensyjny nie zależał od płci, wieku i wskaźnika masy ciała (BMI). Pełną normalizację wartości ciśnienia uzyskano u 71% chorych. W czasie badania stwierdzono istotne obniżenie
średnich wartości ciśnienia tętna o 8 mm Hg (p < 0,0001). Lacydypina była dobrze tolerowana przez chorych, działania niepożądane wystąpiły u 21% z nich.
Wnioski Lacydypina (Lacipil) stosowana w monoterapii charakteryzuje się wysoką skutecznością hipotensyjną u chorych z łagodnym i umiarkowanym nadciśnieniem tętniczym pierwotnym. Efekt hipotensyjny nie zależy od wieku i płci pacjentów.
Preparat cechuje korzystny wpływ na wartości ciśnienia tętna uważanego za niezależny czynnik ryzyka powikłań sercowo-naczyniowych. Uzyskane wyniki wskazują na dobrą tolerancję leku przez chorych z nadciśnieniem tętniczym. Badanie wskazuje na przydatność
leku w monoterapii nadciśnienia tętniczego.Background Lacidipine is an orally administered calcium channel blocker of the dihydropyridine class, which shows selectivity for vascular smooth muscle and has a long duration of action. The aim of multi-center study POL-LACY was to establish antihypertensive effect and safety of lacidipine given once a day 2 to 6 mg in patient with mild to moderate hypertension.
Material and methods The study was an open, multi-centre study in 37 Polish medical centers. 470 patients with mild to moderate hypertension were included into the study. 7 days qualification period was followed by 8 weeks of active treatment. During qualification period antihypertensive treatment was
withdrawn and patients underwent clinical evaluation. In the phase of active treatment patients were treated with lacidipine (Lacipil®, GlaxoSmithKline Pharmaceuticals)
starting from dose of 4 mg or 2 mg in patients older than 65 years. The dose could be tritated up to 6 or 4 mg if there was no normalization of BP level after 4 weeks of treatment. Normalization of BP was defined as systolic BP < 140 mm Hg and diastolic BP < 90 mm Hg. During phase of active treatment, after 4 and 8 weeks,
efficacy and safety of treatment was evaluated.
Results 446 patients completed the study (255 M, 191 F; mean
age 52 ± 13 years). The mean baseline BP value was 157/98 mm Hg. At the end of the study 9,9% of patients were treated with 2 mg, 72,9% of patients with 4 mg and 17,3% of patients with 6 mg of lacidipine. After 8 weeks lacidipine decreased
significantly systolic BP by 23 mm Hg (p < 0,0001) and diastolic BP by 15 mm Hg (p < 0,0001) in the sitting position. Antihypertensive effect of lacidipine was not influenced by sex, age and BMI. Full normalization of BP was achieved in 71% of patients. Pulse pressure was significantly reduced by 8 mm Hg (p < 0,0001). Lacidipine was well tolerated, 21% of patients experienced adverse events.
Conclusions Lacidipine is a highly effective drug in the monotherapy of mild to moderate essential hypertension. Antihypertensive effect of lacidipine
is not influenced by age, sex and BMI. Lacidipine has a positive effect on pulse pressure. Lacidipine is well tolerated by patients. In conclusion, presented data point at the lacidipine as a useful drug in monotherapy of essential hypertension

Clinical Significance of Peritoneal Fluid Examination for Free Cancer Cells in Patients Qualified for Surgery for Gastric Cancer
Background: Peritoneal lavage cytology in patients with gastric cancer may correlate with an unfavorable prognosis. This study evaluated the presence of free cancer cells in the peritoneal lavage of a population of patients with gastric cancer and its correlation with the outcome of surgical treatment.Methods: One hundred patients diagnosed with gastric or gastrointestinal junction adenocarcinoma underwent surgery. In all patients, a cytological and immunohistochemical analysis of peritoneal lavage was performed. Based on the presence of free cancer cells (fcc) at the cytological and immunohistochemical examination of peritoneal lavage, patients undergoing surgery for gastric cancer were divided into two groups: fcc (+) and fcc (–).Results: A total of 100 patients, 37 women, and 63 men with a median age of 65 years were included in the study. In the entire study group, 16 (16%) patients were positive for the presence of free cancer cells (fcc +) at peritoneal lavage examination. However, in the group of patients who underwent gastrectomy, fcc (+) was found in 10 out of 77 (13%) patients. The presence of cancer cells in peritoneal lavage was a strong predictive factor in an unfavorable outcome after surgery, and 1-year and 2-year patient survival was 34 and 0% in fcc (+) patients and 79 and 59% in fcc (–), respectively. Moreover, the presence of free cancer cells was associated with a five-fold increased risk of death within 2 years after surgery. When analyzing the group of patients undergoing R0/R1 surgery, this difference was even more significant (p < 0.0001).Conclusions: The presence of free cancer cells in peritoneal lavage may significantly affect the outcome of patients with gastric cancer. Radical surgery in patients with free cancer cells in the peritoneal lavage did not result in a survival advantage. Identification of free cancer cells could help for a better stratification of gastric cancer patients, identifying those patients who could better benefit from a radical surgical treatment, finally improving long-term survival
Single tryptophan Y160W mutant of homooligomeric E. coli purine nucleoside phosphorylase implies that dimers forming the hexamer are functionally not equivalent
E. coli purine nucleoside phosphorylase is a homohexamer, which structure, in the apo form, can be described as a trimer of dimers. Earlier studies suggested that ligand binding and kinetic properties are well described by two binding constants and two sets of kinetic constants. However, most of the crystal structures of this enzyme complexes with ligands do not hold the three-fold symmetry, but only two-fold symmetry, as one of the three dimers is different (both active sites in the open conformation) from the other two (one active site in the open and one in the closed conformation). Our recent detailed studies conducted over broad ligand concentration range suggest that protein–ligand complex formation in solution actually deviates from the two-binding-site model. To reveal the details of interactions present in the hexameric molecule we have engineered a single tryptophan Y160W mutant, responding with substantial intrinsic fluorescence change upon ligand binding. By observing various physical properties of the protein and its various complexes with substrate and substrate analogues we have shown that indeed three-binding-site model is necessary to properly describe binding of ligands by both the wild type enzyme and the Y160W mutant. Thus we have pointed out that a symmetrical dimer with both active sites in the open conformation is not forced to adopt this conformation by interactions in the crystal, but most probably the dimers forming the hexamer in solution are not equivalent as well. This, in turn, implies that an allosteric cooperation occurs not only within a dimer, but also among all three dimers forming a hexameric molecule
The I/D ACE gene polymorphism and antihypertensive efficacy of losartan in patients with primary hypertension
Wstęp Polimorfizm insercyjno/delecyjny (I/D)
genu ACE kodującego konwertazę angiotensyny I
jest najchętniej analizowanym i najlepiej przebadanym
wariantem sekwencji w genomie. Jego
istotne znaczenie czynnościowe sprawia, że jest
częstym obiektem badań typu analizy związku
w odniesieniu do praktycznie wszystkich chorób
układu sercowo-naczyniowego i ich powikłań.
Trwają badania dotyczące związku polimorfizmu
I/D genu ACE z pierwotnym nadciśnieniem tętniczym
oraz farmakogenetyką leków hipotensyjnych,
w tym antagonistów receptora angiotensyny II
(ARB). Celem niniejszej pracy była ocena związku
pomiędzy polimorfizmem I/D genu ACE
a działaniem hipotensyjnym losartanu (podawanego
przez 8 tygodni) u chorych na pierwotne nadciśnienie
tętnicze.
Materiał i metody Do badania włączono 50 osób
(40 mężczyzn i 10 kobiet) w wieku średnio 47 ± 8 lat,
z nadciśnieniem tętniczym (stopień I-II wg ESH/
/ESC, 2007). Wszyscy chorzy przez 8 tygodni byli
poddawani leczeniu antagonistą receptora angiotensyny
II - losartanem w dawce 50 mg raz na
dobę. W przypadku braku normalizacji ciśnienia
(< 140/90 mm Hg) po 4 tygodniach zwiększano
dawkę leku do 100 mg raz na dobę. U wszystkich
osób włączonych do badania wykonywano pomiary
ciśnienia tętniczego metodą tradycyjną (przed
oraz po 4 i 8 tygodniach leczenia), całodobową rejestrację
ciśnienia tętniczego (ABPM) i podstawowe
badania biochemiczne przed i po 8 tygodniach
leczenia; oznaczano też stężenia angiotensyny II
w osoczu i aktywność reninową osocza (ARO)
w warunkach podstawowych oraz po 8 tygodniach
terapii. Genotypy I/D ACE określano metodą łańcuchowej
reakcji polimerazy (PCR).
Wyniki W badanej grupie chorych stwierdzono: 10
homozygot II (20%), 25 heterozygot ID (50%) oraz
15 homozygot DD (30%). Częstość występowania allelu
I wynosiła 45%, a allelu D - 55%. Pomiędzy
pacjentami o różnych genotypach nie stwierdzono
istotnych różnic w zakresie wieku, płci, wskaźnika
masy ciała, parametrów przemiany lipidowej oraz
wyjściowych wartości skurczowego (SBP) i rozkurczowego
(DBP) ciśnienia tętniczego. Po 8 tygodniach
leczenia u wszystkich pacjentów stwierdzono
istotne obniżenie zarówno SBP, jak i DBP w pomiarach
tradycyjnych oraz ABPM. Dawkę losartanu
zwiększono do 100 mg u 23 osób, w tym u 7 (70%)
z genotypem II, 12 z genotypem ID (48%) i 4 z genotypem
DD (27%). Po 8 tygodniach leczenia wartość
DBP poniżej 90 mm Hg osiągnięto u wszystkich
chorych z genotypem DD. Zarówno wartości SBP,jak i DBP po 8 tygodniach leczenia były istotnie niższe
u osób z genotypem DD niż u pacjentów z allelem
I (ID lub II).
Wnioski Wyniki badania wskazują na związek pomiędzy
polimorfizmem I/D genu ACE a działaniem
hipotensyjnym wywieranym przez antagonistę receptora

seems the most widely studied sequence variant
of human genome. Although evidence implicates the linkage
of ACE locus and the risk for arterial hypertension, there
are confounding data regarding association of ACE gene
polymorphism with blood pressure (BP) response to antihypertensive
therapy in patients with essential hypertension.
Therefore the aim of our study was to evaluate the association
between I/D ACE polymorphism and antihypertensive
efficacy of an angiotensin II receptor blocker - losartan.
Material and methods Fifty patients (mean age 47 ± 8
years) with essential hypertension (stage I-II , ESH/ESC,
2007) were included into the study. The patients were
treated with losartan starting from dose of 50 mg once
daily. The dose could be increased up to 100 mg once
daily if there was no normalization of blood pressure after
4 weeks of treatment. The ambulatory BP measurements
(ABPM), clinic BP measurements as well as evaluation
of plasma renin activity and serum angiotensin II
concentration measurements were performed before and after 8 weeks of treatment. The ACE genotypes were
identified by PCR method.
Results The frequency distribution of ACE genotypes were as
follows: 20% II homozygotes, 50% ID heterozygotes and 30%
DD homozygotes. There were no significant differences in
clinic characteristics and baseline BP levels among II, ID or
DD patients. After 8 weeks of treatment significant decrease
of BP levels both in clinic measurements and ABPM was
noted regardless of ACE genotype. Losartan dose was titrated
in 7 patients with II genotype (70%), 12 patients with ID
genotype (48%) and in 4 patients with DD genotype (27%).
Patients with DD genotype were characterized by lower
systolic and diastolic BP levels after 8 weeks of treatment as
compared with carriers of the I allele (ID or II).
Conclusions Our results suggest the association between I/D

Interwencja kryzysowa : konteksty indywidualne i społeczne

miała zostać poświęcona interwencji środowiskowej, tzw. nurtowi ekosystemu.
Zamiaru tego nie udało się jednak w pełni zrealizować, z uwagi na
stosunkowo niewielkie zainteresowanie polskich badaczy i praktyków teoretycznymi,
jak i praktycznymi problemami interwencji kryzysowej społeczności
i na rzecz społeczności, oraz jej specyfiką w polskich warunkach i realiach.
Brak zainteresowania wydaje się po części wynikać z małego zaangażowania
polskich służb pomocowych w nurty środowiskowej prewencji i działalności
pomocowej.
Ponad 20 lat rozwoju interwencji kryzysowej w naszym państwie zaowocowało
rozbudową instytucjonalnych struktur interwencyjnych, a także przygotowaniem
i kształceniem kadry interwentów kryzysowych, podejmujących
się pomocy w sytuacjach kryzysowych i traumatycznych, w ramach instytucjonalnych
powiązanych z pomocą społeczną, psychologiczną, rzadziej medyczną.
Prezentowane w naszej monografii opracowania autorskie w pewien
sposób odzwierciedlają problemy angażujące polskich interwentów kryzysu
zarówno w aspekcie teoretycznym, jak praktyki interwencyjnej. Zanim jednak
wprowadzimy Czytelników w problematykę poszczególnych opracowań
monografii, chcielibyśmy pokrótce scharakteryzować stany kryzysowe, będące
przedmiotem interwencyjnych oddziaływań ze współczesnej perspektywy
ekologiczno-systemowej.Publikacja powstała w ramach projektu badawczego Krakowskiej Akademii im. Andrzeja Frycza Modrzewskiego nr WPiNH/DS/3/2015-KO
