Medizinische Genetik â Vom Orchideenfach zur SchlĂŒsseldisziplin : Mit dem Abschluss des Humangenomprojektes im Jahr 2003 wurden grosse Erwartungen an die Rolle der Genetik im medizinischen Alltag geweckt. Haben sich diese heute bereits erfĂŒllt?
Als 2003 das internationale Humangenomprojekt mit der erstmaligen Erstellung einer weitgehend kompletten Sequenz- und Genkarte des Menschen abgeschlossen wurde, war die Medizinische Genetik noch ein Randfach der Medizin [1]. Aus der EntschlĂŒsselung der Genomsequenz und aller Gene, zweifellos ein monumentaler Meilenstein der Wissenschaft, erwuchsen rasch hohe Erwartungen, diese Erkenntnisse in der gesamten Medizin fĂŒr die Diagnostik, Behandlung und PrĂ€vention von Krankheiten anwenden zu können. Die Kenntnis einer Referenzsequenz reichte allerdings hierfĂŒr nicht aus, es mussten zunĂ€chst Sequenzinformation von vielen gesunden und kranken Menschen erhoben werden. Dies wurde erst durch bahnbrechende methodische Weiterentwicklungen ermöglicht, die es erlaubten, diese Daten unter vertretbaren Kosten zu generieren. Dabei spielten insbesondere Mikroarray- und Hochdurchsatzsequenzierungsverfahren eine SchlĂŒsselrolle [2]. Erst die breite Anwendung dieser Analyseverfahren liess das tatsĂ€chliche Ausmass der interindividuellen genetischen VariabilitĂ€t und deren medizinische Bedeutung erahnen und bahnte den Weg fĂŒr eine genombasierte personalisierte Medizin mit einer insbesondere in den letzten fĂŒnf Jahren stetig wachsenden Bedeutung fĂŒr alle klinischen FĂ€cher.

La connaissance dâune sĂ©quence de rĂ©fĂ©rence nâĂ©tait toutefois pas suffisante pour atteindre cet objectif; il fallait tout dâabord collecter des informations sĂ©quentielles de nombreux individus sains et malades. Cela a uniquement Ă©tĂ© possible grĂące aux avancĂ©es mĂ©thodologiques rĂ©volutionnaires, qui ont permis de gĂ©nĂ©rer ces donnĂ©es Ă des coĂ»ts raisonnables. En particulier les procĂ©dĂ©s de puces Ă ADN («microarray») et de sĂ©quençage Ă haut dĂ©bit ont jouĂ© un rĂŽle clĂ© dans ce contexte [2]. Viele haben jedoch nur eine vage Vorstellung, was eine genetische Beratung beinhaltet.

A Gene for Universal Congenital Alopecia Maps to Chromosome 8p21-22
SummaryComplete or partial congenital absence of hair (congenital alopecia) may occur either in isolation or with associated defects. The majority of families with isolated congenital alopecia has been reported to follow an autosomal-recessive mode of inheritance (MIM 203655). As yet, no gene has been linked to isolated congenital alopecia, nor has linkage been established to a specific region of the genome. In an attempt to map the gene for the autosomal recessive form of the disorder, we have performed genetic linkage analysis on a large inbred Pakistani family in which affected persons show complete absence of hair development (universal congenital alopecia). We have analyzed individuals of this family, using >175 microsatellite polymorphic markers of the human genome. A maximum LOD score of 7.90 at a recombination fraction of 0 has been obtained with locus D8S258. Haplotype analysis of recombination events localized the disease to a 15-cM region between marker loci D8S261 and D8S1771. We have thus mapped the gene for this hereditary form of isolated congenital alopecia to a locus on chromosome 8p21-22 (ALUNC [alopecia universalis congenitalis]). This will aid future identification of the responsible gene, which will be extremely useful for the understanding of the biochemistry of hair development
Segment-Wise Genome-Wide Association Analysis Identifies a Candidate Region Associated with Schizophrenia in Three Independent Samples
Recent studies suggest that variation in complex disorders (e.g., schizophrenia) is explained by a large number of genetic variants with small effect size (Odds RatioâŒ1.05â1.1). The statistical power to detect these genetic variants in Genome Wide Association (GWA) studies with large numbers of cases and controls (âŒ15,000) is still low. As it will be difficult to further increase sample size, we decided to explore an alternative method for analyzing GWA data in a study of schizophrenia, dramatically reducing the number of statistical tests. The underlying hypothesis was that at least some of the genetic variants related to a common outcome are collocated in segments of chromosomes at a wider scale than single genes. Our approach was therefore to study the association between relatively large segments of DNA and disease status. An association test was performed for each SNP and the number of nominally significant tests in a segment was counted. We then performed a permutation-based binomial test to determine whether this region contained significantly more nominally significant SNPs than expected under the null hypothesis of no association, taking linkage into account. Genome Wide Association data of three independent schizophrenia case/control cohorts with European ancestry (Dutch, German, and US) using segments of DNA with variable length (2 to 32 Mbp) was analyzed. Using this approach we identified a region at chromosome 5q23.3-q31.3 (128â160 Mbp) that was significantly enriched with nominally associated SNPs in three independent case-control samples. We conclude that considering relatively wide segments of chromosomes may reveal reliable relationships between the genome and schizophrenia, suggesting novel methodological possibilities as well as raising theoretical questions

Bei Betrachtung des gegenwĂ€rtigen Stands der Ausbildung in gewerblich-technischen Berufen zeigt sich, dass es viele gute AnsĂ€tze zur Integration moderner Technologien gibt, um den Lernprozess zu unterstĂŒtzen oder zu vereinfachen (z. B. Fehling, 2017; Bundesministerium fĂŒr Bildung und Forschung, 2019). Die meisten technischen AnsĂ€tze eint, dass sie die Darstellung von Funktions- und WirkzusammenhĂ€ngen von technischen Systemen in Ausbildungs- und Lernsituationen unterstĂŒtzen. Es reicht fĂŒr die zukĂŒnftige Beruflichkeit vor dem Hintergrund verschiedener Szenarien (vgl. Frenz, Heinen & Schlick, 2015) jedoch nicht aus, lediglich Technologien abzubilden. Stattdessen mĂŒssen FachkrĂ€fte bei der Problemlösung im realen Arbeitsprozess gefördert werden (vgl. z. B. Abele, 2014; Rauner, 2017). Im Gegensatz zu reinen Funktions- und WirkzusammenhĂ€ngen liegen hier multiple technische SystemzustĂ€nde vor. Mit den rasanten technologischen Entwicklungen im medizinisch-genomischen Bereich und dem damit verbundenen exponentiellen Kenntniszuwachs hat der Bedarf an genetischer Expertise in praktisch allen medizinischen Disziplinen zugenommen. Die Schweizerische Gesellschaft fĂŒr Medizinische Genetik (SGMG) sieht sich als Fachgesellschaft und Vertreterin einer transversalen medizinischen Disziplin besonders geeignet, die gegebene Situation zu evaluieren und zukĂŒnftige Konzepte zu entwickeln und zu begleiten
SwissGenVar: A Platform for Clinical-Grade Interpretation of Genetic Variants to Foster Personalized Healthcare in Switzerland.
Large-scale next-generation sequencing (NGS) germline testing is technically feasible today, but variant interpretation represents a major bottleneck in analysis workflows. This includes extensive variant prioritization, annotation, and time-consuming evidence curation. The scale of the interpretation problem is massive, and variants of uncertain significance (VUSs) are a challenge to personalized medicine. This challenge is further compounded by the complexity and heterogeneity of the standards used to describe genetic variants and the associated phenotypes when searching for relevant information to support clinical decision making. To address this, all five Swiss academic institutions for Medical Genetics joined forces with the Swiss Institute of Bioinformatics (SIB) to create SwissGenVar as a user-friendly nationwide repository and sharing platform for genetic variant data generated during routine diagnostic procedures and research sequencing projects. Its aim is to provide a protected environment for expert evidence sharing about individual variants to harmonize and upscale their significance interpretation at the clinical grade according to international standards. To corroborate the clinical assessment, the variant-related data will be combined with consented high-quality clinical information. Broader visibility will be achieved by interfacing with international databases, thus supporting global initiatives in personalized healthcare
