    The Visitor Experience: Reimagining the 21st Century Visitor

    This thesis explores a challenge that living history museums currently face of being tied to a specific time and place but needing to attract and interact with changing 21st century visitors. One way that museums combat this challenge is through an engaging visitor experience. To explore this issue, Old Sturbridge Village is used as a case study. It is a living history museum, depicting the 1830s, that is located in central Massachusetts. OSV was founded in 1946. The research is coupled with literature on museum visitor experiences. This mixed methods study utilizes interviews of employees at the museum, participant observation of meetings, and content analysis of social media pages, signage, and magazines/brochures. Through these methods, the balancing act that museums undertake is examined. Finally, recommendations for continued engagement with 21st century visitors are made

    Cranberry metabolites with urinary anti-adhesion activity

    The fruit of the American cranberry (Vaccinium macrocarpon Aiton, Ericaceae) is a comfood and a top selling dietary supplement in the U.S. Cranberry juice is traditionally used for the prevention of urinary tract infections (UTIs), many of which are caused by P-fimbriated Escherichia coli. Human urine produced after cranberry juice consumption can prevent E. coli adhesion, but the urinary metabolites responsible for this activity are currently not known. High-molecular weight components of cranberry juice, specifically proanthocyanidin oligomers (PACs), are currently believed to be responsible for the ability of cranberry juice to prevent UTIs. Cranberry PACs can prevent the adherence of P-fimbriated E. coli to uroepithelial cells in vitro and are hypothesized to act by a similar mechanism of action in vivo. Many questions exist, however, regarding the potential for PACs to be absorbed, metabolized, and excreted into urine. Adult female sows were therefore fed spray-dried cranberry juice powder (5 g/kg/d) and urine was collected via catheter. Urine fractions were tested for anti-adhesion activity using a human red blood cell (HRBC) anti-agglutination assay with uropathogenic P-fimbriated E. coli. Active urine fractions but not control urine or inactive fractions were found to contain complex mixtures of oligosaccharides but not PACs. An octasaccharide, consisting of a tetrameric, β-(1→4)-linked D-glucose backbone and possessing two D-xylose-L-arabinose side chains, was purified from the oligosaccharide mixture of active urine fractions in sufficient quantity to assign the full structure. Oligosaccharide mixtures of similar character were subsequently identified in cranberry products using methods adapted from those applied to urine samples. Cranberry oligosaccharides were enriched in large scale from three sources of cranberry material and were shown to have activity in both the HRBC anti-agglutination assay and a second anti-adhesion assay that used uropathogenic E. coli and uroepithelial cells. The results of these studies indicate that the compounds responsible for the anti-adhesion properties of urine after cranberry juice consumption are oligosaccharides that are structurally related to those found in cranberry. These findings overturn the current paradigm regarding the active components of cranberry juice that prevent UTIs and provide many new directions for future research

    Views of NHS commissioners on commissioning support provision. Evidence from a qualitative study examining the early development of clinical commissioning groups in England

    Objective: The 2010 healthcare reform in England introduced primary care-led commissioning in the National Health Service (NHS) by establishing clinical commissioning groups (CCGs). A key factor for the success of the reform is the provision of excellent commissioning support services to CCGs. The Government's aim is to create a vibrant market of competing providers of such services (from both for-profit and not-for-profit sectors). Until this market develops, however, commissioning support units (CSUs) have been created from which CCGs are buying commissioning support functions. This study explored the attitudes of CCGs towards outsourcing commissioning support functions during the initial stage of the reform. Design: The research took place between September 2011 and June 2012. We used a case study research design in eight CCGs, conducting in-depth interviews, observation of meetings and analysis of policy documents. Setting/participants: We conducted 96 interviews and observed 146 meetings (a total of approximately 439 h). Results: Many CCGs were reluctant to outsource core commissioning support functions (such as contracting) for fear of losing local knowledge and trusted relationships. Others were disappointed by the absence of choice and saw CSUs as monopolies and a recreation of the abolished PCTs. Many expressed doubts about the expectation that outsourcing of commissioning support functions will result in lower administrative costs. Conclusions: Given the nature of healthcare commissioning, outsourcing vital commissioning support functions may not be the preferred option of CCGs. Considerations of high transaction costs, and the risk of fragmentation of services and loss of trusted relationships involved in short-term contracting, may lead most CCGs to decide to form long-term partnerships with commissioning support suppliers in the future. This option, however, limits competition by creating ‘network closure’ and calls into question the Government's intention to create a vibrant market of commissioning support provision

    Inhibition of Melanoma Angiogenesis by Telomere Homolog Oligonucleotides

    Telomere homolog oligonucleotides (T-oligos) activate an innate telomere-based program that leads to multiple anticancer effects. T-oligos act at telomeres to initiate signaling through the Werner protein and ATM kinase. We wanted to determine if T-oligos have antiangiogenic effects. We found that T-oligo-treated human melanoma (MM-AN) cells had decreased expression of vascular endothelial growth factor (VEGF), VEGF receptor 2, angiopoeitin-1 and -2 and decreased VEGF secretion. T-oligos activated the transcription factor E2F1 and inhibited the activity of the angiogenic transcription factor, HIF-1α. T-oligos inhibited EC tubulogenesis and total tumor microvascular density matrix invasion by MM-AN cells and ECs in vitro. In melanoma SCID xenografts, two systemic T-oligo injections decreased by 60% (P<.004) total tumor microvascular density and the functional vessels density by 80% (P <.002). These findings suggest that restriction of tumor angiogenesis is among the host's innate telomere-based anticancer responses and provide further evidence that T-oligos may offer a powerful new approach for melanoma treatment.National Institutes of Health (CA10515); American Skin Associatio

    Engaging GPs in commissioning: realist evaluation of the early experiences of Clinical Commissioning Groups in the English NHS.

    Objectives To explore the 'added value' that general practitioners (GPs) bring to commissioning in the English NHS. We describe the experience of Clinical Commissioning Groups (CCGs) in the context of previous clinically led commissioning policy initiatives. Methods Realist evaluation. We identified the programme theories underlying the claims made about GP 'added value' in commissioning from interviews with key informants. We tested these theories against observational data from four case study sites to explore whether and how these claims were borne out in practice. Results The complexity of CCG structures means CCGs are quite different from one another with different distributions of responsibilities between the various committees. This makes it difficult to compare CCGs with one another. Greater GP involvement was important but it was not clear where and how GPs could add most value. We identified some of the mechanisms and conditions which enable CCGs to maximize the 'added value' that GPs bring to commissioning. Conclusion To maximize the value of clinical input, CCGs need to invest time and effort in preparing those involved, ensuring that they systematically gather evidence about service gaps and problems from their members, and engaging members in debate about the future shape of services

    Pairing in fermionic systems: A quantum information perspective

    The notion of "paired" fermions is central to important condensed matter phenomena such as superconductivity and superfluidity. While the concept is widely used and its physical meaning is clear there exists no systematic and mathematical theory of pairing which would allow to unambiguously characterize and systematically detect paired states. We propose a definition of pairing and develop methods for its detection and quantification applicable to current experimental setups. Pairing is shown to be a quantum correlation different from entanglement, giving further understanding in the structure of highly correlated quantum systems. In addition, we will show the resource character of paired states for precision metrology, proving that the BCS states allow phase measurements at the Heisenberg limit.Comment: 23 pages, 4 figure

    Telehealth Utilization in Response to the COVID-19 Pandemic: Current State of Medical Provider Training

    Background: The COVID-19 pandemic accelerated the development of telehealth services and thus the need for telehealth education and training to support rapid implementation at scale. A national survey evaluating the current state of the telehealth landscape was deployed to organizational representatives, and included questions related to education and training. Materials and Methods: In the summer of 2020, 71 survey participants (31.8%) completed an online survey seeking to determine the utilization of telehealth services across institutional types and locations. This included data collected to specifically compare the rates and types of formal telehealth education provided before and during the pandemic. Results: Thirty percent of organizations reported no telehealth training before COVID-19, with those in suburban/rural settings significantly less likely to provide any training (55% vs. 82%) compared with urban. Pandemic-related training changes applied to 78% of organizations, with more change happening to those without any training before COVID-19 (95%). Generally, organizations offering training before the pandemic reported deploying COVID-19-related telehealth services, while a higher percentage of those without any training beforehand reported that they either did not plan on providing these services or were in the early planning stages. Discussion: Telehealth education is moving from elective to essential based on the need to prepare and certify the workforce to support high-quality telehealth services. Conclusions: As telehealth continues to evolve to meet the future health care service needs of patients and providers, education and training will advance to meet the needs of everyday clinical encounters and broader public health initiatives

    Bayesian High-Redshift Quasar Classification from Optical and Mid-IR Photometry

    We identify 885,503 type 1 quasar candidates to i<22 using the combination of optical and mid-IR photometry. Optical photometry is taken from the Sloan Digital Sky Survey-III: Baryon Oscillation Spectroscopic Survey (SDSS-III/BOSS), while mid-IR photometry comes from a combination of data from the Wide-Field Infrared Survey Explorer (WISE) "ALLWISE" data release and several large-area Spitzer Space Telescope fields. Selection is based on a Bayesian kernel density algorithm with a training sample of 157,701 spectroscopically-confirmed type-1 quasars with both optical and mid-IR data. Of the quasar candidates, 733,713 lack spectroscopic confirmation (and 305,623 are objects that we have not previously classified as photometric quasar candidates). These candidates include 7874 objects targeted as high probability potential quasars with 3.5<z<5 (of which 6779 are new photometric candidates). Our algorithm is more complete to z>3.5 than the traditional mid-IR selection "wedges" and to 2.2<z<3.5 quasars than the SDSS-III/BOSS project. Number counts and luminosity function analysis suggests that the resulting catalog is relatively complete to known quasars and is identifying new high-z quasars at z>3.     Arabinoxyloglucan Oligosaccharides May Contribute to the Antiadhesive Properties of Porcine Urine after Cranberry Consumption

    © 2019 As part of our continuing investigation for interesting biological activities of native medicinal plants, thirty-nine plants, obtained from diverse areas in Saudi Arabia and Yemen, were screened for insecticidal activity against yellow fever mosquito Aedes aegypti (L.). Out of the 57 organic extracts, Saussurea lappa, Ocimum tenuiflorum, Taraxacum officinale, Nigella sativa, and Hyssopus officinalis exhibited over 80% mortality against adult female Ae. aegypti at 5 μg/mosquito. In the larvicidal bioassay, the petroleum ether extract of Aloe perryi flowers showed 100% mortality at 31.25 ppm against 1st instar Ae. aegypti larvae. The ethanol extract of Saussurea lappa roots was the second most active displaying 100% mortality at 125 and 62.5 ppm. Polar active extracts were processed using LC-MS/MS to identify bioactive compounds. The apolar A. perryi flower extract was analyzed by headspace SPME-GC/MS analysis. Careful examination of the mass spectra and detailed interpretation of the fragmentation pattern allowed the identification of various biologically active secondary metabolites. Some compounds such as caffeic and quinic acid and their glycosides were detected in most of the analyzed fractions. Additionally, luteolin, luteolin glucoside, luteolin glucuronide and diglucuronide were also identified as bioactive compounds in several HPLC fractions.     Statistics on indigenous peoples: International effort needed

    In 2007, the UN General Assembly endorsed the United Nations Declaration on the Rights of Indigenous Peoples. In the following years, there has been a strong call from a range United Nations agencies and spokespersons for countries to act to improve their statistics relating to Indigenous peoples as part of their response to the Declaration. These calls have emphasised the need for a holistic approach, describing strengths and resilience of Indigenous peoples and not just a focus on gaps and disadvantage. National responses have been mixed and overall statistics remain inadequate. Significantly, there has been no international statistical effort through the United Nations statistical structures to respond to the Declaration and the increasing array of calls for improved statistics. The United Nations Statistical Commission in particular has an array of mechanisms to study statistical needs and develop solutions across a broad international statistical agenda. It is time for countries to make a concerted effort to improve their own statistics on Indigenous peoples, and to insist that the Statistical Commission work in partnership with the Permanent Forum on Indigenous Issues and other stakeholders to lead a major international drive to improve statistics on and for Indigenous peoples
