    Social distance and wage inequalities for immigrants in Switzerland

    This study investigates the impact of the social distance between immigrants and Swiss natives on wage inequalities. Social distance is assumed to be high when immigrants come from a different culture, a different education system and speak a different language. By combining these three aspects, this article goes beyond existing studies that only focus on cultural differences. The empirical findings indicate that, net of education, immigrants with a high or very high social distance from the receiving society are strongly disadvantaged in terms of their salaries. Additional analyses show that the impact of typical explanatory factors for labour market success such as education, citizenship and length of stay also vary by the degree of social distance between immigrants and their destination country. The analyses are based on data from the Swiss Labour Force Survey for the years 2010 and 2011, which allow for detailed analyses on individual immigrant groups due to oversampling

    Understanding cross-country variation in the long-term consequences of graduating at a bad time: A comparison of five European countries

    This working paper investigates if graduating in a bad economy scars careers of youth cohorts in terms of increased future unemployment and overrepresentation in fixed-term and involuntary part-time work. These dynamics of scarring are explored from a cross-country comparative perspective, focusing on the UK, Germany, Switzerland, Spain and Finland. These countries make up for interesting cases as they differ remarkably on institutional and economic dimensions such as for example the vocational orientation of their education systems, the strictness of employment protection legislation, active labour market policies to support job-search success of jobless young people and the general level of prevalent youth unemployment, which are assumed to be related to cross-nationally distinct patterns in scarring effects. The focus of the empirical analysis is on long-term effects of the level of aggregate youth unemployment at graduation on career evolvement of school-leaver cohorts over 12 years since their graduation, distinguishing between educational groups while allowing for gender effects. All in all we find that bad luck in timing of labour market entry can scar future careers over the long-run. A bad economy at labour market entry may thus be seen as a major risk factor for the future integration of youth cohorts in very different institutional contexts

    Compared to other European countries, Switzerland has demonstrated relative economic stability since the economic crisis in 2008/2009. The employment rate of 15 to 24-year-olds in Switzerland registered a fluctuation of maximum 5% during the period from 2008 to 2015. The lowest rate was observed in the 2nd quarter of 2012 (65.2%) and the highest rate in the 3rd quarter of 2010 (70.2%). In comparison, the fluctuation of the employment rate of the general population (15-64 years of age) is even smaller; 82.2% in the 2nd quarter of 2010 was the lowest rate and 84.6% the highest in the last quarter of 2015. Overall, the employment rate among the younger population is less stable than throughout the general population (Bundesamt für Statistik BFS, 2016). As for fixed-term contracts, the Swiss Federal Office of Statistics registered a shared of 19% of fixed-term contracts among the age group 15-24, which is considerably higher than the share in the general labour force population (8% in 2015) (see section 2). In Switzerland, the youth unemployment rate differs from canton to canton. The French- and Italian-speaking parts show a higher rate than the German-speaking part of Switzerland. According to Eurostat (4Q 2015), on average, 9.8% of the young people between 15-24 years of age were unemployed in 2015, which was about double as high as the unemployment rate of the entire work force population (15-65-year-olds: 4.8%). However, compared to other OECD countries the youth unemployment rate in Switzerland is still relatively low (see section 2). According to the Swiss Federal Statistical Office, the share of people in the age group 18-24 in education or training has not varied much from 2008 (63.5%) to 2014 (64.5%). The lowest participation rate was measured in 2009, when 57.6% were in education or training (Eurostat 2016). The educational level in Switzerland has shown some changes regarding the tertiary education. While in 2010 35.3% of the population declared that they had obtained a tertiary degree as their highest attained education level, it was 40.2% in 2014. Furthermore, there has been a slight increase of people, who have completed at least an upper-secondary education (85.8% in 2010 and 88.0% in 2014) (FSO 2016). The OECD data show that the proportion of students enrolling in the general and in the vocational programmes has hardly changed over the years. In 2008 it was 35:65, and in 2013 it was 34:66 (OECD 2015). The number of persons aged from 18 to 24, who have only completed the lower secondary education, and who are currently not in education or training, has slightly decreased over the years: While it was 7.6% in 2010, it dropped to 6.3% by 2014 (FSO 2016). The overall positive economic situation in Switzerland provides favourable conditions for a stable labour market.     External Validation of the Modified 4C Deterioration Model and 4C Mortality Score for COVID-19 Patients in a Swiss Tertiary Hospital

    Prognostic models to predict the deterioration and mortality risk in COVID-19 patients are utterly needed to assist in informed decision making. Most of these models, however, are at high risk of bias, model overfitting, and unclear reporting. Here, we aimed to externally validate the modified (urea was omitted) 4C Deterioration Model and 4C Mortality Score in a cohort of Swiss COVID-19 patients and, second, to evaluate whether the inclusion of the neutrophil-to-lymphocyte ratio (NLR) improves the predictive performance of the models. We conducted a retrospective single-centre study with adult patients hospitalized with COVID-19. Both prediction models were updated by including the NLR. Model performance was assessed via the models' discriminatory performance (area under the curve, AUC), calibration (intercept and slope), and their performance overall (Brier score). For the validation of the 4C Deterioration Model and Mortality Score, 546 and 527 patients were included, respectively. In total, 133 (24.4%) patients met the definition of in-hospital deterioration. Discrimination of the 4C Deterioration Model was AUC = 0.78 (95% CI 0.73-0.82). A total of 55 (10.44%) patients died in hospital. Discrimination of the 4C Mortality Score was AUC = 0.85 (95% CI 0.79-0.89). There was no evidence for an incremental value of the NLR. Our data confirm the role of the modified 4C Deterioration Model and Mortality Score as reliable prediction tools for the risk of deterioration and mortality.     Glued suture-less peritoneum closure in laparoscopic inguinal hernia repair reduces acute postoperative pain.

    Inguinal hernia repair is performed more than 20 million times per annum, representing a significant health and economic burden. Over the last three decades, significant technical advances have started to reduce the invasiveness of these surgeries, which translated to better recovery and reduced costs. Here we bring forward an innovative surgical technique using a biodegradable cyanoacrylate glue instead of a traumatic suture to close the peritoneum, which is a highly innervated tissue layer, at the end of endoscopy hernia surgery. To test how this affects the invasiveness of hernia surgery, we conducted a cohort study. A total of 183 patients that underwent minimally invasive hernia repair, and the peritoneum was closed with either a conventional traumatic suture (n = 126, 68.9%) or our innovative approach using glue (n = 57, 31.1%). The proportion of patients experiencing acute pain after surgery was significantly reduced (36.8 vs. 54.0%, p = 0.032) by using glue instead of a suture. In accordance, the mean pain level was higher in the suture group (VAS = 1.5 vs. 1.3, p = 0.029) and more patients were still using painkillers (77.9 vs. 52.4%, p = 0.023). Furthermore, the rate of complications was not increased in the glue group. Using multivariate regressions, we identified that using a traumatic suture was an independent predictor of acute postoperative pain (OR 2.0, 95% CI 1.1-3.9, p = 0.042). In conclusion, suture-less glue closure of the peritoneum is innovative, safe, less painful, and possibly leads to enhanced recovery and decreased health costs

    International epidemiological studies indicate that around 1-7% of the population respond with an allergic reaction to a hymenoptera sting, which is frequently associated with admission to an emergency department. This retrospective study included patients admitted between 2009 and 2013 to an emergency department after a hymenoptera sting. In all, 86 (60.1%) men and 57 (39.9%) women were included in the study. The mean age was 43 years, with a range from 19 to 84 years. The most common localisations of a sting were the head (n = 33; 22.5%), the hands (n = 32; 21.9%) and the arms (n = 26; 17.8%). In women, we recorded significantly more stings in distal extremities (p = 0.033) and in men stings in the rump and head were most frequent. Local swellings were observed in 67.1% (n = 96) of patients and 34.3% (n = 49) patients exhibited an anaphylactic reaction. Of these, 21.7% (n = 31) suffered from a grade I, 6.3% (n = 9) grade II, 4.2% (n = 6) grade III and 2.1% (n = 3) grade IV anaphylactic reactions. 46% (66) of the patients were given antihistamines, 45% (64) intravenous glucocorticoids and only 12.5% (16) epinephrine. Most stings were recorded on days without rainfall (p = 0.013), with more hours of sunshine (p = 0.001), low relative humidity (p = 0.006), with mean air pressure above 954.3 hPa and on days with mean temperature above 24.2 °C (p = 0.001). In conclusion, the most hymenoptera stings induced local swelling only; severe reactions were rare. The most dangerous stings are enoral and result from inattentive drinking. Epinephrine was rarely used in anaphylactic reactions

    Response of ocean ecosystems to climate warming

We use vertical velocity, maximum winter mixed layer depth, and sea ice cover to define six biomes. Climate warming leads to a contraction of the highly productive marginal sea ice biome by 42% in the Northern Hemisphere and 17% in the Southern Hemisphere, and leads to an expansion of the low productivity permanently stratified subtropical gyre biome by 4.0% in the Northern Hemisphere and 9.4% in the Southern Hemisphere. In between these, the subpolar gyre biome expands by 16% in the Northern Hemisphere and 7% in the Southern Hemisphere, and the seasonally stratified subtropical gyre contracts by 11% in both hemispheres. The low-latitude (mostly coastal) upwelling biome area changes only modestly. Vertical stratification increases, which would be expected to decrease nutrient supply everywhere, but increase the growing season length in high latitudes. We use satellite ocean color and climatological observations to develop an empirical model for predicting chlorophyll from the physical properties of the global warming simulations. Four features stand out in the response to global warming: (1) a drop in chlorophyll in the North Pacific due primarily to retreat of the marginal sea ice biome, (2) a tendency toward an increase in chlorophyll in the North Atlantic due to a complex combination of factors, (3) an increase in chlorophyll in the Southern Ocean due primarily to the retreat of and changes at the northern boundary of the marginal sea ice zone, and (4) a tendency toward a decrease in chlorophyll adjacent to the Antarctic continent due primarily to freshening within the marginal sea ice zone. We use three different primary production algorithms to estimate the response of primary production to climate warming based on our estimated chlorophyll concentrations. The three algorithms give a global increase in primary production of 0.7% at the low end to 8.1% at the high end, with very large regional differences. The main cause of both the response to warming and the variation between algorithms is the temperature sensitivity of the primary production algorithms.     A cluster of multidrug-resistant Mycobacterium tuberculosis among patients arriving in Europe from the Horn of Africa: a molecular epidemiological study

    SummaryBackground The risk of tuberculosis outbreaks among people fleeing hardship for refuge in Europe is heightened. We describe the cross-border European response to an outbreak of multidrug-resistant tuberculosis among patients from the Horn of Africa and Sudan. Methods On April 29 and May 30, 2016, the Swiss and German National Mycobacterial Reference Laboratories independently triggered an outbreak investigation after four patients were diagnosed with multidrug-resistant tuberculosis. In this molecular epidemiological study, we prospectively defined outbreak cases with 24-locus mycobacterial interspersed repetitive unit-variable number tandem repeat (MIRU-VNTR) profiles; phenotypic resistance to isoniazid, rifampicin, ethambutol, pyrazinamide, and capreomycin; and corresponding drug resistance mutations. We whole-genome sequenced all Mycobacterium tuberculosis isolates and clustered them using a threshold of five single nucleotide polymorphisms (SNPs). We collated epidemiological data from host countries from the European Centre for Disease Prevention and Control. Findings Between Feb 12, 2016, and April 19, 2017, 29 patients were diagnosed with multidrug-resistant tuberculosis in seven European countries. All originated from the Horn of Africa or Sudan, with all isolates two SNPs or fewer apart. 22 (76%) patients reported their travel routes, with clear spatiotemporal overlap between routes. We identified a further 29 MIRU-VNTR-linked cases from the Horn of Africa that predated the outbreak, but all were more than five SNPs from the outbreak. However all 58 isolates shared a capreomycin resistance-associated tlyA mutation. Interpretation Our data suggest that source cases are linked to an M tuberculosis clone circulating in northern Somalia or Djibouti and that transmission probably occurred en route before arrival in Europe. We hypothesise that the shared mutation of tlyA is a drug resistance mutation and phylogenetic marker, the first of its kind in M tuberculosis sensu stricto.
