    Current Status of Antimicrobial Resistance in Taiwan

    While some trends in antimicrobial resistance rates are universal, others appear to be unique for specific regions. In Taiwan, the strikingly high prevalence of resistance to macrolides and streptogramin in clinical isolates of gram-positive bacteria correlates with the widespread use of these agents in the medical and farming communities, respectively. The relatively low rate of enterococci that are resistant to glycopeptide does not parallel the high use of glycopeptides and extended-spectrum beta-lactams in hospitals. The evolving problem of extended-spectrum beta-lactamase-producing Escherichia coli and Klebsiella pneumoniae isolates is substantial, and some unique enzymes have been found. Recently, some gram-negative bacteria (e.g., Pseudomonas aeruginosa and Acinetobacter baumannii) that are resistant to all available antimicrobial agents including carbapenems have emerged

    Perturbated Gradients Updating within Unit Space for Deep Learning

    In deep learning, optimization plays a vital role. By focusing on image classification, this work investigates the pros and cons of the widely used optimizers, and proposes a new optimizer: Perturbated Unit Gradient Descent (PUGD) algorithm with extending normalized gradient operation in tensor within perturbation to update in unit space. Via a set of experiments and analyses, we show that PUGD is locally bounded updating, which means the updating from time to time is controlled. On the other hand, PUGD can push models to a flat minimum, where the error remains approximately constant, not only because of the nature of avoiding stationary points in gradient normalization but also by scanning sharpness in the unit ball. From a series of rigorous experiments, PUGD helps models to gain a state-of-the-art Top-1 accuracy in Tiny ImageNet and competitive performances in CIFAR- {10, 100}.     Pulsed laser deposition of hexagonal GaN-on-Si(100) template for MOCVD applications

    Growth of hexagonal GaN on Si(100) templates via pulsed laser deposition (PLD) was investigated for the further development of GaN-on-Si technology. The evolution of the GaN growth mechanism at various growth times was monitored by SEM and TEM, which indicated that the GaN growth mode changes gradually from island growth to layer growth as the growth time increases up to 2 hours. Moreover, the high-temperature operation (1000°C) of the PLD meant no significant GaN meltback occurred on the GaN template surface. The completed GaN templates were subjected to MOCVD treatment to regrow a GaN layer. The results of X-ray diffraction analysis and photoluminescence measurements show not only the reliability of the GaN template, but also the promise of the PLD technique for the development of GaN-on-Si technology

    Unfolding Collapsed Polyelectrolytes in Alternating-Current Electric Fields

    We investigate the unfolding of single polyelectrolyte (PE) chains collapsed by trivalent salt under the action of alternating-current (AC) electric fields through computer simulations and theoretical scaling. The results show that a collapsed chain can be unfolded by an AC field when the field strength exceeds the direct-current (DC) threshold and the frequency is below a critical value, corresponding to the inverse charge relaxation/dissociation time of condensed trivalent counterions at the interface of the collapsed electrolyte. This relaxation time is also shown to be identical to the DC chain fluctuation time, suggesting that the dissociation of condensed polyvalent counterion on the collapsed PE interface controls the polyelectrolyte dipole formation and unfolding dynamics under an AC electric field.Comment: 18 pages, 5 figures, submitte

    Evaluation of prognostic factors and the role of chemotherapy in unfavorable carcinoma of unknown primary site: a 10-year cohort study

    <p>Abstract</p> <p>Background</p> <p>Carcinoma of unknown primary site (CUP) has a poor prognosis and the prognostic factors in these patients are not well established. Furthermore, there are no selection criteria for patients who should benefit from chemotherapy.</p> <p>Methods</p> <p>The medical records of 179 CUP patients who were treated at Taipei Veterans General Hospital from 2000 to 2009 were reviewed. Factors associated with survival were determined by Kaplan-Meier analysis. Differences between the groups with and without palliative chemotherapy were analyzed.</p> <p>Results</p> <p>Univariate analysis revealed multiple prognostic factors, including performance status, lung metastasis, number of metastatic organs, serum albumin, corrected serum calcium, lactate dehydrogenase (LDH), sodium, and cholesterol levels, palliative chemotherapy, and white blood cell and lymphocyte counts. Multivariate analysis showed that performance status < 2, serum albumin level ≥ 3.5 g/dl, corrected serum calcium level < 10.7 mg/dl, single metastatic organ, and palliative chemotherapy were independent factors of better prognosis. Patients with better performance status, higher serum albumin, and lower serum LDH levels had significantly greater benefit from palliative chemotherapy.</p> <p>Conclusions</p> <p>Certain patients with unfavorable CUP will have better survival. Identification of patients with unfavorable CUP who could benefit from palliative chemotherapy warrants future prospective studies.</p
