    S-Nitroso-N-Acetylcysteine Ameliorates Ischemia-Reperfusion Injury In The Steatotic Liver

    BACKGROUND: Steatosis is currently the most common chronic liver disease and it can aggravate ischemia-reperfusion (IR) lesions. We hypothesized that S-nitroso-N-acetylcysteine (SNAC), an NO donor component, can ameliorate cell damage from IR injury. In this paper, we report the effect of SNAC on liver IR in rats with normal livers compared to those with steatotic livers. METHODS: Thirty-four rats were divided into five groups: I (n=8), IR in normal liver; II (n=8), IR in normal liver with SNAC; III (n=9), IR in steatotic liver; IV (n=9), IR in steatotic liver with SNAC; and V (n=10), SHAN. Liver steatosis was achieved by administration of a protein-free diet. A SNAC solution was infused intraperitoneally for one hour, beginning 30 min. after partial (70%) liver ischemia. The volume of solution infused was 1 ml/100 g body weight. The animals were sacrificed four hours after reperfusion, and the liver and lung were removed for analysis. We assessed hepatic histology, mitochondrial respiration, oxidative stress (MDA), and pulmonary myeloperoxidase. RESULTS: All groups showed significant alterations compared with the group that received SHAN. The results from the steatotic SNAC group revealed a significant improvement in liver mitochondrial respiration and oxidative stress compared to the steatotic group without SNAC. No difference in myeloperoxidase was observed. Histological analysis revealed no difference between the non-steatotic groups. However, the SNAC groups showed less intraparenchymal hemorrhage than groups without SNAC (p=0.02). CONCLUSION: This study suggests that SNAC effectively protects against IR injury in the steatotic liver but not in the normal liver

    Estradiol-17β altera expressão proteica endometrial em fêmeas bovinas tratadas no 17º dia do ciclo estral

    In bovine females the release of prostaglandin F2&#945; (PGF2&#945;) is induced in vivo by estradiol (E2). It is believed that E2 stimulates the synthesis of essential proteins for the production of PGF2&#945;. This study aimed to evaluate the effect of E2 in increasing the concentration of total protein and modifying the protein composition of endometrial explants from bovine females treated with E2 at the 17th day of estrous cycle. Crossbred heifers were treated at 17th day of estrous cycle intravenously with 0 mg (Control Group; n = 6) or 3 mg of E2 (E2 Group; n = 6) and killed two hours after. Endometrial explants were isolated, subjected to extraction of total protein, quantified and were analyzed by one-dimensional electrophoresis on polyacrylamide gel 10% SDS-PAGE. The concentration of total protein did not differ between groups, 6296.10 + 439.90 µg/mL for the Control Group and 8426.56 + 1156.00 µg/mL for E2 Group (p = 0.1158). There was no significant difference (p &gt; 0.05) in the protein profile of endometrial explants in gels stained with Coomasie Blue. In gels stained with Silver Nitrate it was verified in E2 Group greater relative percentage of the bands referring to the molecular weight of 75 to 76 kDa (8.40% vs. 4.89% in E2 Group and Control respectively; p < 0.05) and 108 to 110 Kda (6.85% vs. 3.84% in E2 Group and Control respectively, p < 0.05). It was observed in E2 Group lower relative percentage of the band referring to the molecular weight of 90 kDa (5.78% vs. 9.83% in E2 Group and control respectively; p < 0.05). We concluded that the E2 does not increase the protein concentration in the endometrium, however, it modifies the proteinic composition in the endometrial explants, indicating that E2 alters the expression of specific proteins.Em fêmeas bovinas, a liberação de prostaglandina F2α (PGF2α) é induzida in vivo pelo estradiol (E2). Acredita-se que o E2 estimule a síntese de proteínas essenciais na produção de PGF2α. Objetivou-se avaliar o efeito do E2 no incremento da concentração de proteínas totais e na modificação da composição proteica em explantes endometriais de fêmeas bovinas tratadas com E2 no 17º dia do ciclo estral. Novilhas cruzadas foram tratadas no 17º dia do ciclo estral, via intravenosa, com 0 mg (Grupo Controle; n = 6) ou 3 mg de E2 (Grupo E2; n = 6) e abatidas duas horas após. Explantes endometriais foram isolados, submetidos à extração de proteínas totais, quantificados e avaliados por Eletroforese Unidimensional em gel de poliacrilamida 10% SDS-PAGE. A concentração de proteínas totais não diferiu entre os grupos, 6296,10 + 439,90 µg/mL para o Grupo Controle e 8426,56 + 1156,00 µg/mL para o Grupo E2 (p = 0,1158). Não houve diferença significativa (p &gt; 0,05) no perfil proteico dos explantes endometriais em géis corados com Coomasie Blue. Em géis corados com Nitrato de Prata verificou-se no Grupo E2 maior porcentagem relativa das bandas referentes ao peso molecular de 75 a 76 kDa (8,40% vs. 4,89%; no Grupo E2 e Controle respectivamente; p &lt; 0,05) e 108 a 110 kDa (6,85% vs. 3,84%; no Grupo E2 e Controle respectivamente; p &lt; 0,05). Observou-se no Grupo E2 menor porcentagem relativa da banda referente ao peso molecular de 90 kDa (5,78% vs. 9,83%; no Grupo E2 e Controle respectivamente; p &lt; 0,05). Conclui-se que o E2 não incrementa a concentração de proteínas no endométrio, entretanto, altera a composição proteica nos explantes endometriais, indicando que o E2 altera a expressão de proteínas específicas

    Impacto do rastreamento clínico e genético para neoplasia endócrina múltipla tipo 1

    PURPOSE: To perform clinical and genetic screening for multiple endocrine neoplasia type 1 (MEN1) in patients at the Academic Hospital of the University of São Paulo School of Medicine, and to analyze its impact on clinical management of patients with MEN1. METHODS: The clinical diagnosis of MEN1 was made in accordance with the Consensus on multiple endocrine neoplasias. Mutation analysis of the entire MEN1 tumor suppressor gene and genetic screening of at-risk family members were performed by direct sequencing. To analyze the implementation of genetic diagnosis, the studied patients were separated into 3 groups: MEN1 index cases (group I), clinically diagnosed MEN1 cases (group II), and genetically diagnosed MEN1 cases (group III). RESULTS: In total, 154 individuals were clinically and genetically studied. We identified 12 different MEN1 mutations. Fifty-two MEN1 cases were identified: 13 in group I, 28 in group II, and 11 in group III. The mean age in group III (27.0 years) was significantly lower than in groups I (39.5 years) and II (42.4 years; P = 0.03 and P = 0.01, respectively). Patients in groups I and II mostly presented 2 or 3 MEN1-related tumors, while 81.8% of those in group III presented 1 or no MEN1-related tumor. Additionally, in group III, 45.4% of cases were asymptomatic, and no metastasis or death was verified. Surveillance for MEN1 mutations allowed the exclusion of 102 noncarriers, including a case of MEN1 phenocopy. CONCLUSION: Our data supports the benefits of clinical and genetic screening for multiple endocrine neoplasia type 1 in the management of this syndrome.OBJETIVOS: Realizar rastreamentos clínico e gênico para Neoplasia Endócrina Múltipla tipo 1 (NEM1) e analisar seu impacto no seguimento clínico desses pacientes no Hospital das Clínicas, SP. Os casos afetados foram sub-divididos em 3 grupos e analisados separadamente: casos-índices (grupo I), familiares diagnosticados clinicamente (grupo II) e genicamente (grupo III). RESULTADOS: Um total de 154 casos participou desse estudo, sendo 52 diagnosticados com NEM1: 13 do grupo I, 28 do grupo II e 11 do grupo III. A idade média ao diagnóstico no grupo III (27 anos) foi significativamente menor que a dos grupos I (39,5 anos; p = 0,03) e II (42,4 anos; p = 0,01). A maioria dos pacientes dos grupos I e II apresentou 2 ou 3 tumores, enquanto que 81,8% dos casos do grupo III apresentavam 1 ou nenhum tumor relacionado à NEM1. Além disto, 45,4% dos casos do grupo III eram assintomáticos, não sendo observados nenhuma metástase ou óbito. Os demais 102 familiares sob-risco estudados não herdaram mutação MEN1 e foram excluídos do rastreamento clínico. Um caso de fenocópia NEM1 foi também localizado. DISCUSSÃO: Nossos dados demonstraram importantes benefícios no seguimento dos pacientes NEM1, obtidos pela implementação dos rastreamentos clínico e gênico para essa doença

    First-year experience of a Brazilian tertiary medical center in supporting severely ill patients using extracorporeal membrane oxygenation

    OBJECTIVES: The aim of this manuscript is to describe the first year of our experience using extracorporeal membrane oxygenation support. METHODS: Ten patients with severe refractory hypoxemia, two with associated severe cardiovascular failure, were supported using venous-venous extracorporeal membrane oxygenation (eight patients) or veno-arterial extracorporeal membrane oxygenation (two patients). RESULTS: The median age of the patients was 31 yr (range 14-71 yr). Their median simplified acute physiological score three (SAPS3) was 94 (range 84-118), and they had a median expected mortality of 95% (range 87-99%). Community-acquired pneumonia was the most common diagnosis (50%), followed by P. jiroveci pneumonia in two patients with AIDS (20%). Six patients were transferred from other ICUs during extracorporeal membrane oxygenation support, three of whom were transferred between ICUs within the hospital (30%), two by ambulance (20%) and one by helicopter (10%). Only one patient (10%) was anticoagulated with heparin throughout extracorporeal membrane oxygenation support. Eighty percent of patients required continuous venous-venous hemofiltration. Three patients (30%) developed persistent hypoxemia, which was corrected using higher positive end-expiratory pressure, higher inspired oxygen fractions, recruitment maneuvers, and nitric oxide. The median time on extracorporeal membrane oxygenation support was five (range 3-32) days. The median length of the hospital stay was 31 (range 3-97) days. Four patients (40%) survived to 60 days, and they were free from renal replacement therapy and oxygen support. CONCLUSIONS: The use of extracorporeal membrane oxygenation support in severely ill patients is possible in the presence of a structured team. Efforts must be made to recognize the necessity of extracorporeal respiratory support at an early stage and to prompt activation of the extracorporeal membrane oxygenation team

    Latin Americans show wide-spread Converso ancestry and imprint of local Native ancestry on physical appearance

    Historical records and genetic analyses indicate that Latin Americans trace their ancestry mainly to the intermixing (admixture) of Native Americans, Europeans and Sub-Saharan Africans. Using novel haplotype-based methods, here we infer sub-continental ancestry in over 6,500 Latin Americans and evaluate the impact of regional ancestry variation on physical appearance. We find that Native American ancestry components in Latin Americans correspond geographically to the present-day genetic structure of Native groups, and that sources of non-Native ancestry, and admixture timings, match documented migratory flows. We also detect South/East Mediterranean ancestry across Latin America, probably stemming mostly from the clandestine colonial migration of Christian converts of non-European origin (Conversos). Furthermore, we find that ancestry related to highland (Central Andean) versus lowland (Mapuche) Natives is associated with variation in facial features, particularly nose morphology, and detect significant differences in allele frequencies between these groups at loci previously associated with nose morphology in this sample.Instituto Multidisciplinario de Biología Celula

    Pervasive gaps in Amazonian ecological research

    Biodiversity loss is one of the main challenges of our time,1,2 and attempts to address it require a clear un derstanding of how ecological communities respond to environmental change across time and space.3,4 While the increasing availability of global databases on ecological communities has advanced our knowledge of biodiversity sensitivity to environmental changes,5–7 vast areas of the tropics remain understudied.8–11 In the American tropics, Amazonia stands out as the world’s most diverse rainforest and the primary source of Neotropical biodiversity,12 but it remains among the least known forests in America and is often underrepre sented in biodiversity databases.13–15 To worsen this situation, human-induced modifications16,17 may elim inate pieces of the Amazon’s biodiversity puzzle before we can use them to understand how ecological com munities are responding. To increase generalization and applicability of biodiversity knowledge,18,19 it is thus crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple or ganism groups in a machine learning model framework to map the research probability across the Brazilian Amazonia, while identifying the region’s vulnerability to environmental change. 15%–18% of the most ne glected areas in ecological research are expected to experience severe climate or land use changes by 2050.
