    Enhancing Cognitive Behavioral Therapy and Expressive Arts Therapy by Applying a Hybrid Approach

    Therapists often draw from multiple therapeutic modalities in order to find appropriate treatment options for a client; however, little is written on the subject of how these modalities may be able to enhance each other in a strategic way. When considering the field of expressive arts therapy and the struggle it has faced in the effort to legitimize itself, it is worth considering how other therapeutic modalities could work in tandem with it in order to improve the overall experience. Alternatively, cognitive behavioral therapy is a strong, evidence-based practiced that sometimes lacks in the qualitative experience realm, providing a gap that may be filled by applying unique, expressive arts therapy techniques to achieve therapeutic goals. This literature review explores the possibility of therapeutic enhancement through a hybrid approach to cognitive behavioral therapy and expressive arts therapy. The proposal for a hybrid approach is based on an investigation into the published literature and historical texts of each modality, their underlying theories, an analysis of the strengths and weaknesses, potential overlap of the two modalities, and gaps that may be filled by a tandem application. The final component of this literature review provides samples for application by focusing on adolescents with anxiety as a target population. The research presented here encourages practitioners to perform further research on the utilization of this particular hybrid approach and opens the door for further communication about other potential hybrid approaches

    Prognostic factors for liver, blood and kidney adverse events from glucocorticoid sparing immune-suppressing drugs in immune-mediated inflammatory diseases: a prognostic systematic review

    Background: Immune-suppressing drugs can cause liver, kidney or blood toxicity. Prognostic factors for these adverse-events are poorly understood. Purpose: To ascertain prognostic factors associated with liver, blood or kidney adverse-events in people receiving immune-suppressing drugs. Data sources: MEDLINE, Web of Science, EMBASE and the Cochrane library (01 January 1995 to 05 January 2023), and supplementary sources. Data extraction and synthesis: Data were extracted by one reviewer using a modified CHARMS-PF checklist and validated by another. Two independent reviewers assessed risk of bias using Quality in Prognostic factor Studies tool and assessed the quality of evidence using a Grading of Recommendations Assessment, Development and Evaluation-informed framework. Results: Fifty-six studies from 58 papers were included. High-quality evidence of the following associations was identified: elevated liver enzymes (6 studies) and folate non-supplementation (3 studies) are prognostic factors for hepatotoxicity in those treated with methotrexate; that mercaptopurine (vs azathioprine) (3 studies) was a prognostic factor for hepatotoxicity in those treated with thiopurines; that mercaptopurine (vs azathioprine) (3 studies) and poor-metaboliser status (4 studies) were prognostic factors for cytopenia in those treated with thiopurines; and that baseline elevated liver enzymes (3 studies) are a prognostic factor for hepatotoxicity in those treated with anti-tumour necrosis factors. Moderate and low quality evidence for several other demographic, lifestyle, comorbidities, baseline bloods/serologic or treatment-related prognostic factors were also identified. Limitations: Studies published before 1995, those with less than 200 participants and not published in English were excluded. Heterogeneity between studies included different cut-offs for prognostic factors, use of different outcome definitions and different adjustment factors. Conclusions: Prognostic factors for target-organ damage were identified which may be further investigated for their potential role in targeted (risk-stratified) monitoring.     The Effectiveness of Passive Physical Modalities for the Management of Soft Tissue Injuries and Neuropathies of the Wrist and Hand: A Systematic Review by the Ontario Protocol for Traffic Injury Management (OPTIMa) Collaboration

    AbstractObjectiveThe purpose of this systematic review was to determine the effectiveness of passive physical modalities compared to other interventions, placebo/sham interventions, or no intervention in improving self-rated recovery, functional recovery, clinical outcomes and/or administrative outcomes (eg, time of disability benefits) in adults and/or children with soft tissue injuries and neuropathies of the wrist and hand.MethodsWe systematically searched MEDLINE, EMBASE, PsycINFO, and the Cochrane Central Register of Controlled Trials, accessed through Ovid Technologies, Inc, and CINAHL Plus with Full Text, accessed through EBSCO host, from 1990 to 2015. Our search strategies combined controlled vocabulary relevant to each database (eg, MeSH for MEDLINE) and text words relevant to our research question and the inclusion criteria. Randomized controlled trials, cohort studies, and case-control studies were eligible. Random pairs of independent reviewers screened studies for relevance and critically appraised relevant studies using the Scottish Intercollegiate Guidelines Network criteria. Studies with low risk of bias were synthesized following best evidence synthesis principles.ResultsWe screened 6618 articles and critically appraised 11 studies. Of those, 7 had low risk of bias: 5 addressed carpal tunnel syndrome (CTS) and 2 addressed de Quervain disease. We found evidence that various types of night splints lead to similar outcomes for the management of CTS. The evidence suggests that a night wrist splint is less effective than surgery in the short term but not in the long term. Furthermore, a night wrist splint and needle electroacupuncture lead to similar outcomes immediately postintervention. Finally, low-level laser therapy and placebo low-level laser therapy lead to similar outcomes. The evidence suggests that kinesio tape or a thumb spica cast offers short-term benefit for the management of de Quervain disease. Our search did not identify any low risk of bias studies examining the effectiveness of passive physical modalities for the management of other soft tissue injuries or neuropathies of the wrist and hand.ConclusionsDifferent night orthoses provided similar outcomes for CTS. Night orthoses offer similar outcomes to electroacupuncture but are less effective than surgery in the short term. This review suggests that kinesio tape or a thumb spica cast may offer short-term benefit for the management of de Quervain disease

    Spreading rates of bacterial colonies depend on substrate stiffness and permeability

    The ability of bacteria to colonize and grow on different surfaces is an essential process for biofilm development. Here, we report the use of synthetic hydrogels with tunable stiffness and porosity to assess physical effects of the substrate on biofilm development. Using time-lapse microscopy to track the growth of expanding Serratia marcescens colonies, we find that biofilm colony growth can increase with increasing substrate stiffness, unlike what is found on traditional agar substrates. Using traction force microscopy-based techniques, we find that biofilms exert transient stresses correlated over length scales much larger than a single bacterium, and that the magnitude of these forces also increases with increasing substrate stiffness. Our results are consistent with a model of biofilm development in which the interplay between osmotic pressure arising from the biofilm and the poroelastic response of the underlying substrate controls biofilm growth and morphology

    Cognitive function prior to systemic therapy and subsequent well‐being in older breast cancer survivors: Longitudinal findings from the Thinking and Living with Cancer Study

    ObjectiveTo investigate the relationships between self‐reported and objectively measured cognitive function prior to systemic therapy and subsequent well‐being outcomes over 24 months in older breast cancer survivors.MethodsData were from 397 women aged 60 to 98 diagnosed with non‐metastatic breast cancer in the Thinking and Living with Cancer Study recruited from 2010‐2016. Cognitive function was measured at baseline (following surgery, prior to systemic therapy) using neuropsychological assessments of attention, processing speed, and executive function (APE), learning and memory (LM), and the self‐reported FACT‐Cog scale. Well‐being was measured using the FACT‐G functional, physical, social, and emotional well‐being domain scales at baseline and 12 and 24 months later, scaled from 0 (low) to 100 (high). Linear mixed‐effects models assessed the relationships between each of baseline APE, LM, and FACT‐Cog quartiles with well‐being scores over 24 months, adjusted for confounding variables.ResultsAt baseline, older survivors in the lowest APE, LM, and FACT‐Cog score quartiles experienced poorer global well‐being than those in the highest quartiles. At 24 months, older survivors tended to improve in well‐being, and there were no differences according to baseline APE or LM scores. At 24 months, mean global well‐being was 80.3 (95% CI: 76.2‐84.3) among those in the lowest vs 86.6 (95% CI: 83.1‐90.1) in the highest FACT‐cog quartile, a clinically meaningful difference of 6.3 points (95% CI: 1.5‐11.1).ConclusionsAmong older breast cancer survivors, self‐reported, but not objective cognitive impairments, were associated with lower global well‐being over the first 2 years of survivorship.Peer Reviewedhttps://deepblue.lib.umich.edu/bitstream/2027.42/155908/1/pon5376.pdfhttps://deepblue.lib.umich.edu/bitstream/2027.42/155908/2/pon5376_am.pd

    Oxidation of SQSTM1/p62 mediates the link between redox state and protein homeostasis.

    Cellular homoeostatic pathways such as macroautophagy (hereinafter autophagy) are regulated by basic mechanisms that are conserved throughout the eukaryotic kingdom. However, it remains poorly understood how these mechanisms further evolved in higher organisms. Here we describe a modification in the autophagy pathway in vertebrates, which promotes its activity in response to oxidative stress. We have identified two oxidation-sensitive cysteine residues in a prototypic autophagy receptor SQSTM1/p62, which allow activation of pro-survival autophagy in stress conditions. The Drosophila p62 homologue, Ref(2)P, lacks these oxidation-sensitive cysteine residues and their introduction into the protein increases protein turnover and stress resistance of flies, whereas perturbation of p62 oxidation in humans may result in age-related pathology. We propose that the redox-sensitivity of p62 may have evolved in vertebrates as a mechanism that allows activation of autophagy in response to oxidative stress to maintain cellular homoeostasis and increase cell survival

    Identifying seasonal distribution patterns of fin whales across the Scotia Sea and the Antarctic Peninsula region using a novel approach combining habitat suitability models and ensemble learning methods

    Following their near extirpation by industrial whaling of the 20th century, the population status of Southern Hemisphere fin whales (SHFW) remains unknown. Systematic surveys estimating fin whale abundance in the Southern Ocean are not yet available. Records of fin whale sightings have been collected by a variety of organisations over the past few decades, incorporating both opportunistic data and dedicated survey data. Together, these isolated data sets represent a potentially valuable source of information on the seasonality, distribution and abundance of SHFW. We compiled records across 40 years from the Antarctic Peninsula and Scotia Sea from multiple sources and used a novel approach combining ensemble learning and a maximum entropy model to estimate abundance and distribution of SHFW in this region. Our results show a seasonal distribution pattern with pronounced centres of distribution from January-March along the West Antarctic Peninsula. Our new approach allowed us to estimate abundance of SHFW for discrete areas from a mixed data set of mainly opportunistic presence only data.publishedVersio
