Bin Laden has earned global notoriety because of his role in the September 11th attacks against the United States of America. Yet, Osama does not represent the beginning, nor the end of Muslim radicals. He is only one link in a chain of radical thought. Bin Laden's unorthodox actions and words will leave a legacy, but what factors influenced him? This thesis provides insight into understanding the ideological foundation of Osama bin Laden. It incorporates primary documents from those individuals responsible for indoctrinating the Saudi millionaire, particularly Abdullah Azzam and Ayman al-Zawahiri. Additionally, it identifies key historic figures and events that transformed bin Laden from a modest, shy conservative into a Muslim extremist

    The role of deficient pain modulatory systems in the development of persistent post-traumatic headaches following mild traumatic brain injury: an exploratory longitudinal study

    Background: Post-traumatic headache (PTH) is one of the most common and long-lasting symptoms following mild traumatic brain injury (TBI). However, the pathological mechanisms underlying the development of persistent PTH remain poorly understood. The primary purpose of this prospective pilot study was to evaluate whether early pain modulatory profiles (sensitization and endogenous pain inhibitory capacity) and psychological factors after mild TBI predict the development of persistent PTH in mild TBI patients. Methods: Adult mild TBI patients recruited from Level I Emergency Department Trauma Centers completed study sessions at 1-2 weeks, 1-month, and 4-months post mild TBI. Participants completed the following outcome measures during each session: conditioned pain modulation to measure endogenous pain inhibitory capacity, temporal summation of pain and pressure pain thresholds of the head to measure sensitization of the head, Pain Catastrophizing Scale, Center for Epidemiological Studies - Depression Scale, and a standardized headache survey. Participants were classified into persistent PTH (PPTH) and no-PPTH groups based on the 4-month data. Results: The results revealed that mild TBI patients developing persistent PTH exhibited significantly diminished pain inhibitory capacity, and greater depression and pain catastrophizing following injury compared to those who do not develop persistent PTH. Furthermore, logistic regression indicated that headache pain intensity at 1-2 weeks and pain inhibitory capacity on the conditioned pain modulation test at 1-2 weeks predicted persistent PTH classification at 4 months post injury. Conclusions: Overall, the results suggested that persistent PTH is characterized by dysfunctional alterations in endogenous pain modulatory function and psychological processes in the early stages following mild TBI, which likely exacerbate risk for the maintenance of PTH

    Intensive Goal-Directed Treatments in Enriched Environments Augments Patient Outcomes Post-Stroke

    Objective: Previous research indicates that patients post-stroke, average 400-800 steps within physical therapy sessions and demonstrate heart rate values of 24-35% of HR Max. This dosage and intensity is inadequate to promote neuroplastic changes and maximize recovery. The goal of this study was to quantify and examine the amount of high-intensity stepping practice that was delivered within an Acute Inpatient Rehabilitation (AIR) setting for individuals with sub-acute stroke. Methods: 14 patients with a diagnosis of sub-acute stroke were admitted to AIR. Standardized outcomes included the 6-Minute Walk Test (6MWT), 10-Meter Walk Test (10MWT), the Berg Balance Scale (BBS) and the Postural Assessment Scale for Stroke Patients (PASS). A step activity monitor with an internal accelerometer was applied on the non-paretic extremity from the hours of 7 am to 5 pm. During therapy sessions, subjects were exposed to a plan of care that involved high-intensity, high-frequency stepping practice through Body Weight Supported Treadmill Training (BWSTT), over-ground stepping, stair climbing, obstacle navigation, dynamic standing balance activities and error augmentation tasks. All individuals were continuously monitored with heart rate monitors, and perceived intensity was recorded within 5-minute intervals and after modification of activity with the Borg Rate of Perceived Exertion (RPE) Scale. Target intensity was defined with heart rate values of 70-85% of HR Max and RPE values of 14-20 (hard to maximal exertion). Time spent in target intensity ranges was collected. Each subject was scheduled for 1-2 hours of physical therapy per day. Results: During the 2-month collection period, subjects demonstrated ability to tolerate a high-intensity, high-frequency stepping gait training program within the intensities defined. Stepping data indicated that subjects received an average daily stepping dosage of 2000-8000 steps per day, well above previously reported values. Conclusion: It is possible to implement a high-intensity, high-frequency stepping gait training program within an acute inpatient rehabilitation setting for the stroke population. However, future research concerning therapy intensity and frequency of stepping should be designed with a larger sample size

    The Fifth Data Release of the Sloan Digital Sky Survey

    This paper describes the Fifth Data Release (DR5) of the Sloan Digital Sky Survey (SDSS). DR5 includes all survey quality data taken through June 2005 and represents the completion of the SDSS-I project (whose successor, SDSS-II will continue through mid-2008). It includes five-band photometric data for 217 million objects selected over 8000 square degrees, and 1,048,960 spectra of galaxies, quasars, and stars selected from 5713 square degrees of that imaging data. These numbers represent a roughly 20% increment over those of the Fourth Data Release; all the data from previous data releases are included in the present release. In addition to "standard" SDSS observations, DR5 includes repeat scans of the southern equatorial stripe, imaging scans across M31 and the core of the Perseus cluster of galaxies, and the first spectroscopic data from SEGUE, a survey to explore the kinematics and chemical evolution of the Galaxy. The catalog database incorporates several new features, including photometric redshifts of galaxies, tables of matched objects in overlap regions of the imaging survey, and tools that allow precise computations of survey geometry for statistical investigations.Comment: ApJ Supp, in press, October 2007.     Pan-Cancer Analysis of lncRNA Regulation Supports Their Targeting of Cancer Genes in Each Tumor Context

    Long noncoding RNAs (lncRNAs) are commonly dys-regulated in tumors, but only a handful are known toplay pathophysiological roles in cancer. We inferredlncRNAs that dysregulate cancer pathways, onco-genes, and tumor suppressors (cancer genes) bymodeling their effects on the activity of transcriptionfactors, RNA-binding proteins, and microRNAs in5,185 TCGA tumors and 1,019 ENCODE assays.Our predictions included hundreds of candidateonco- and tumor-suppressor lncRNAs (cancerlncRNAs) whose somatic alterations account for thedysregulation of dozens of cancer genes and path-ways in each of 14 tumor contexts. To demonstrateproof of concept, we showed that perturbations tar-geting OIP5-AS1 (an inferred tumor suppressor) andTUG1 and WT1-AS (inferred onco-lncRNAs) dysre-gulated cancer genes and altered proliferation ofbreast and gynecologic cancer cells. Our analysis in-dicates that, although most lncRNAs are dysregu-lated in a tumor-specific manner, some, includingOIP5-AS1, TUG1, NEAT1, MEG3, and TSIX, synergis-tically dysregulate cancer pathways in multiple tumorcontexts
