    Comparison of Routine Health Screening Rates Between Two EMR Systems

    An EMR, electronic medical record, system refers to the software widely adopted by medical practitioners to reduce the use of hard-copy files, and improve the documentation, storage, and retrieval of patient information. Some EMR systems can generate automatic alerts reminding providers when patients are due for certain preventive services or meet the criteria for various screening measures. Epic, one of the leading EMR systems in use today, contains this additional feature. The screening recommendations built into Epic are derived from a committee of USPSTF, U.S. Preventive Services Task Force, medical experts whose focus is to improve patient health across the nation. In September of 2021, the University of Tennessee-Family Medicine Jackson clinic switched from using the Centricity EMR to Epic EMR. The primary goal of this study was to measure whether there is any quantifiable improvement in USPSTF screening rates after implementing the Epic EMR, which contains the notifications to prevent lapses in patient care. For each EMR, 100 patients were selected at random including those greater than 44 years of age and less than 65 years of age. This study involved checking whether patients were up-to-date on thirteen potential USPSTF screening measures and recording the data in the SPSS statistical software program. Chi-square analysis comparing the data across both Centricity and Epic, revealed no statistical significance between screening rates. However, when accounting for sex (male vs. female) across both EMRs, women in the study had 16% more drug screening, 3% more tobacco screening, and 18% more statin use in comparison to men. Furthermore, statistical significance was found when comparing race (black vs. white). For example, black patients had 10% more syphilis screening and 33% more STD screening, whereas white patients had 28% more lung cancer screening across both EMR systems. Although reassuring to find no statistical significance when comparing screening rates across both EMRs, conclusions to explain the results for specific screening measures pertaining to race and sex cannot be drawn based on the data at this time. Future research analyzing external variables such as patient compliance, provider biases, and population risk, may be useful in providing further insight to explain the statistical significance of this data

    The scaling properties of exchange and correlation holes of the valence shell of second row atoms

    We study the exchange and correlation hole of the valence shell of second row atoms using variational Monte Carlo techniques, especially correlated estimates, and norm-conserving pseudopotentials. The well-known scaling of the valence shell provides a tool to probe the behavior of exchange and correlation as a functional of the density and thus test models of density functional theory. The exchange hole shows an interesting competition between two scaling forms -- one caused by self-interaction and another that is approximately invariant under particle number, related to the known invariance of exchange under uniform scaling to high density and constant particle number. The correlation hole shows a scaling trend that is marked by the finite size of the atom relative to the radius of the hole. Both trends are well captured in the main by the Perdew-Burke-Ernzerhof generalized-gradient approximation model for the exchange-correlation hole and energy.Comment: 18 pages, 8 figure

    We model the exchange-correlation (XC) energy density of the Si crystal and atom as calculated by variational Monte Carlo (VMC) methods with a gradient analysis beyond the local density approximation (LDA). We find the Laplacian of the density to be an excellent predictor of the discrepancy between VMC and LDA energy densities in each system. A simple Laplacian-based correction to the LDA energy density is developed by means of a least square fit to the VMC XC energy density for the crystal, which fits the homogeneous electron gas and Si atom without further effort.Comment: 4 pages, 3 figures, submitted to Phys.     Feminist Perspectives on Disaster, Pandemics, and Intimate Partner Violence

    The COVID-19 pandemic brought international awareness to the likelihood of increased abuse of those in abusive intimate partner relationships because of the forced confinement with their abusers (Bettinger-Lopez and Bro, A double pandemic: domestic violence in the age of COVID 19, Council on Foreign Relations. https://www.cfr.org/in-brief/double-pandemic-domestic-violence-age-covid-19, 2020). While this awareness was much discussed, assistance to survivors of abuse was limited because survivors often could not reach out for help, nor could advocates wishing to offer assistance safely reach in to advise them (Taub, A new Covid-19 crisis: domestic abuse rises worldwide. https://www.nytimes.com/2020/04/06/world/coronavirus-domestic-violence.html, 2020). The ever-present influence of the abuser prevented or limited the delivery of effective aid in many cases (Taub, A new Covid-19 crisis: domestic abuse rises worldwide. https://www.nytimes.com/2020/04/06/world/coronavirus-domestic-violence.html, 2020). But this was not the first disaster to place those experiencing intimate partner abuse at greater risk. Other disasters have in various ways increased women’s safety risks both during and after crisis. Other crises routinely resulted in increased abuse of survivors in intimate partner relationships including sexual and other assault by intimate partners (Rao 2020; Sohrabizadeh Prehosp Disaster Med, 31(4):407–412. https://www.ncbi.nlm.nih.gov/pubmed/27212204, 2016). This chapter briefly identifies some forms of abuse experienced by women outside of the home during times of crisis and their connection to intimate partner abuse. Then, the impact of COVID-19 on intimate partner survivors will be explored and some of their experiences described. Gaps and flaws in nations’ approaches to gender violence are often highlighted during and after disasters. Lack of planning to protect survivors of intimate partner violence and other highly vulnerable populations during crises was exacerbated by the additional dangers brought by COVID-19. Primarily, the heightened dangers resulted from stay-at-home orders, which made services for abused partners more difficult to obtain and highlighted the possibility of unintended consequences resulting from policies designed to protect the public as a whole (Godin 2020). This chapter concludes by considering what has been lacking in strategic disaster planning, including what lessons have been learned thus far through the COVID-19 experience. Recommendations for effective disaster planning to protect survivors of intimate partner abuse, while also protecting other vulnerable populations, are suggested. This information is presented with the caution that as of this writing the global pandemic continues to limit access to hard data.     Evaluation of peroxyacetic acid, liquid buffered vinegar, and cultured dextrose fermentate as potential antimicrobial interventions for raw chicken livers

    This study aimed to evaluate the use of peroxyacetic acid (PAA), buffered vinegar (BV), and cultured dextrose fermentate (CDF) to reduce Salmonella on artificially inoculated raw chicken livers, one of the most consumed offal around the world. Samples were inoculated with a 5-strain cocktail of poultry-borne Salmonella to obtain 106 CFU/g and immersed for 90 s with agitation in one of the following treatments: distilled water (control), 450 ppm PAA, 2.0% (w/v) BV, or 1.5% (w/v) CDF, prior to storing at 4oC. Salmonella was enumerated on XLD agar and monitored for 14 days. Data were analyzed using analysis of covariance. After immersion, there was a significant Salmonella reduction (p \u3c .05) with all treatments, including the control. PAA resulted in the greatest numerical reduction at 0.65 ± 0.12 log; however, there were no significant differences in the reductions among all other treatments (p \u3e .05). After 14 days, higher numerical reductions were observed for PAA, but only when compared to CDF. Although similar reductions (p \u3e .05) were noted after 14 days except for CDF, Salmonella counts were lowest in all timepoints when PAA was used. PAA and CDF inhibited the growth of aerobic bacteria until day 3 while BV inhibited the growth up to 7 days. Regarding objective color, chicken livers immersed in PAA became lighter, but the difference was not sustained over time. No differences were observed in redness or yellowness values across any treatments
