    An invasive and a noninvasive approach for the automatic differentiation of obstructive and central hypopneas

    The automatic differentiation of obstructive and central respiratory events is a major challenge in the diagnosis of sleep-disordered breathing. Esophageal pressure (Pes) measurement is the gold-standard method to identify these events. This study presents a new classifier that automatically differentiates obstructive and central hypopneas with the Pes signal and a new approach for an automatic noninvasive classifierwith nasal airflow. An overall of 28 patients underwent night polysomnography with Pes recording, and a total of 769 hypopneas were manually scored by human experts to create a gold-standard annotation set. Features were automatically extracted fromthe Pes signal to train and test the classifiers (discriminant analysis, support vector machines, and adaboost). After a significantly (p < 0.01) higher incidence of inspiratory flow limitation episodes in obstructive hypopneas was objectively, invasively assessed compared to central hypopneas, the feasibility of an automatic noninvasive classifier with features extracted from the airflow signal was demonstrated. The automatic invasive classifier achieved a mean sensitivity, specificity, and accuracy of 0.90 after a 100-fold cross validation. The automatic noninvasive feasibility study obtained similar hypopnea differentiation results as a manual noninvasive classification algorithm.     Cloud System Evolution in the Trades (CSET): Following the Evolution of Boundary Layer Cloud Systems with the NSFNCAR GV

    The Cloud System Evolution in the Trades (CSET) study was designed to describe and explain the evolution of the boundary layer aerosol, cloud, and thermodynamic structures along trajectories within the North Pacific trade winds. The study centered on seven round trips of the National Science FoundationNational Center for Atmospheric Research (NSFNCAR) Gulfstream V (GV) between Sacramento, California, and Kona, Hawaii, between 7 July and 9 August 2015. The CSET observing strategy was to sample aerosol, cloud, and boundary layer properties upwind from the transition zone over the North Pacific and to resample these areas two days later. Global Forecast System forecast trajectories were used to plan the outbound flight to Hawaii with updated forecast trajectories setting the return flight plan two days later. Two key elements of the CSET observing system were the newly developed High-Performance Instrumented Airborne Platform for Environmental Research (HIAPER) Cloud Radar (HCR) and the high-spectral-resolution lidar (HSRL). Together they provided unprecedented characterizations of aerosol, cloud, and precipitation structures that were combined with in situ measurements of aerosol, cloud, precipitation, and turbulence properties. The cloud systems sampled included solid stratocumulus infused with smoke from Canadian wildfires, mesoscale cloudprecipitation complexes, and patches of shallow cumuli in very clean environments. Ultraclean layers observed frequently near the top of the boundary layer were often associated with shallow, optically thin, layered veil clouds. The extensive aerosol, cloud, drizzle, and boundary layer sampling made over open areas of the northeast Pacific along 2-day trajectories during CSET will be an invaluable resource for modeling studies of boundary layer cloud system evolution and its governing physical processes

    A distributed geotechnical remote analysis of data system (Distributed G-RAD) can benefit both owners and contractors in providing better quality control and assurance on geotechnical projects. The Distributed G-RAD approach involves efficient data acquisition using PDAs with GPS capability, radio frequency identification (RFID) tags for labeling soil samples, laser scanning for measuring lift thickness and volumes of stockpiles and borrow pits. Spatial data storage is provided using a geographic information system (GIS). Portions of this system are already developed while other parts are still being considered. This paper also describes how RFID and laser scanning technologies can be used in the larger Distributed G-RAD system

    Ibrutinib Unmasks Critical Role of Bruton Tyrosine Kinase in Primary CNS Lymphoma.

    Bruton tyrosine kinase (BTK) links the B-cell antigen receptor (BCR) and Toll-like receptors with NF-κB. The role of BTK in primary central nervous system (CNS) lymphoma (PCNSL) is unknown. We performed a phase I clinical trial with ibrutinib, the first-in-class BTK inhibitor, for patients with relapsed or refractory CNS lymphoma. Clinical responses to ibrutinib occurred in 10 of 13 (77%) patients with PCNSL, including five complete responses. The only PCNSL with complete ibrutinib resistance harbored a mutation within the coiled-coil domain of CARD11, a known ibrutinib resistance mechanism. Incomplete tumor responses were associated with mutations in the B-cell antigen receptor-associated protein CD79B

    A 68 ka precipitation record from the hyperarid core of the Atacama Desert in northern Chile

    [Abstract] The Atacama Desert in northern Chile is one of the driest deserts on Earth. Hyperaridity persists at least since the Miocene and was punctuated by pluvial phases. However, very little is known about the timing, regional spread and intensities of precipitation changes. Here, we present a new precipitation record from a sedimentary sequence recovered in a tectonically blocked endorheic basin that is located in the hyperarid core of the Atacama Desert. The chronostratigraphic framework of the record is given by a multi-disciplinary dating approach, suggesting an age of ca. 68 ka BP for the core base. The sequence consists of three sediment types, whose sedimentological and geochemical characteristics suggest different depositional processes that reflect different degrees in humidity. First, particularly fine-grained sediments with high clastic but low calcium sulfate and carbonate contents reflect a particularly dry climate with only sporadic precipitation events and fluvial supply via channel systems. Second, more coarse-grained sediments with lower clastic and higher calcium sulfate and carbonate contents reflect more moist conditions with stronger precipitation events that lead to fluvial activity not restricted to the channels but involving the slopes and plains in the catchment. Third, normally graded layers with an equally high proportion of calcium sulfate and carbonate reflect occasional high-precipitation events that caused sediment supply also from most distant parts of the catchment via severe flash floods. The sedimentary succession suggests that precipitation changes took place on orbital but also on millennial time scales. Rather moist periods occurred during most of MIS 2, several shorter periods within MIS 3 and parts of MIS 4. Comparison of the findings from the Huara record with selected climate records from continental and marine sites in South America suggests a strong precipitation heterogeneity across the Atacama. This heterogeneity is caused by pronounced differences in the dominating climate patterns and a shift from predominant summer rain in the north to winter rain in the south. Precipitation supply to the Huara clay plan is controlled by the atmospheric circulation rather than the surface temperature of the adjacent ocean

    Cuando se realizan valoraciones de cualquier protocolo de evaluaciónuno de los aspectos fundamentales es conocer la validez y fiabilidadde los dispositivos utilizados. El objetivo del presente trabajo fue analizar lavalidez y fiabilidad de la variable de velocidad en el rango isocinético (VRI)en el dispositivo Haefni Health (HHe 1.0). Un total de dos protocolo se diseñaronpara el análisis de la validez y fiabilidad. Los resultados mostraronaltos Índices de Correlación Intraclase (ICC2,1) para las medidas de validezy fiabilidad del dispositivo (0,998 y 0,99, respectivamente). Cuando se analizóla fiabilidad de las medidas por cada una de las condiciones de evaluaciónaltos ICC2,1, además de bajos CV y SEM fueron encontraros (rango0,71–0,99, rango 0,19–3,73 y rango 0,001–0,004 m·s-1, respectivamente).Los resultados obtenidos avalan la validez y fiabilidad del dispositivo HaefniHealth HHe1.0 para la medición de la variable de VRI tanto para la faseconcéntrica como para la fase excéntrica del movimiento
