Analysis of the rotation period of asteroids (1865) Cerberus, (2100) Ra-Shalom, and (3103) Eger - search for the YORP effect
The spin state of small asteroids can change on a long timescale by the
Yarkovsky-O'Keefe-Radzievskii-Paddack (YORP) effect, the net torque that arises
from anisotropically scattered sunlight and proper thermal radiation from an
irregularly-shaped asteroid. The secular change in the rotation period caused
by the YORP effect can be detected by analysis of asteroid photometric
lightcurves. We analyzed photometric lightcurves of near-Earth asteroids (1865)
Cerberus, (2100) Ra-Shalom, and (3103) Eger with the aim to detect possible
deviations from the constant rotation caused by the YORP effect. We carried out
new photometric observations of the three asteroids, combined the new
lightcurves with archived data, and used the lightcurve inversion method to
model the asteroid shape, pole direction, and rotation rate. The YORP effect
was modeled as a linear change in the rotation rate in time d\omega /dt. Values
of d\omega/ dt derived from observations were compared with the values
predicted by theory. We derived physical models for all three asteroids. We had
to model Eger as a nonconvex body because the convex model failed to fit the
lightcurves observed at high phase angles. We probably detected the
acceleration of the rotation rate of Eger d\omega / dt = (1.4 +/- 0.6) x
10^{-8} rad/d (3\sigma error), which corresponds to a decrease in the rotation
period by 4.2 ms/yr. The photometry of Cerberus and Ra-Shalom was consistent
with a constant-period model, and no secular change in the spin rate was
detected. We could only constrain maximum values of |d\omega / dt| < 8 x
10^{-9} rad/d for Cerberus, and |d\omega / dt| < 3 x 10^{-8} rad/d for
Ra-Shalom
Joint system quantum descriptions arising from local quantumness
Bipartite correlations generated by non-signalling physical systems that
admit a finite-dimensional local quantum description cannot exceed the quantum
limits, i.e., they can always be interpreted as distant measurements of a
bipartite quantum state. Here we consider the effect of dropping the assumption
of finite dimensionality. Remarkably, we find that the same result holds
provided that we relax the tensor structure of space-like separated
measurements to mere commutativity. We argue why an extension of this result to
tensor representations seems unlikely
A Randomized Trial of Nighttime Physician Staffing in an Intensive Care Unit
Background
Increasing numbers of intensive care units (ICUs) are adopting the practice of nighttime intensivist staffing despite the lack of experimental evidence of its effectiveness.
Methods
We conducted a 1-year randomized trial in an academic medical ICU of the effects of nighttime staffing with in-hospital intensivists (intervention) as compared with nighttime coverage by daytime intensivists who were available for consultation by telephone (control). We randomly assigned blocks of 7 consecutive nights to the intervention or the control strategy. The primary outcome was patientsâ length of stay in the ICU. Secondary outcomes were patientsâ length of stay in the hospital, ICU and in-hospital mortality, discharge disposition, and rates of readmission to the ICU. For length-of-stay outcomes, we performed time-to-event analyses, with data censored at the time of a patientâs death or transfer to another ICU.
Results
A total of 1598 patients were included in the analyses. The median Acute Physiology and Chronic Health Evaluation (APACHE) III score (in which scores range from 0 to 299, with higher scores indicating more severe illness) was 67 (interquartile range, 47 to 91), the median length of stay in the ICU was 52.7 hours (interquartile range, 29.0 to 113.4), and mortality in the ICU was 18%. Patients who were admitted on intervention days were exposed to nighttime intensivists on more nights than were patients admitted on control days (median, 100% of nights [interquartile range, 67 to 100] vs. median, 0% [interquartile range, 0 to 33]; P\u3c0.001). Nonetheless, intensivist staffing on the night of admission did not have a significant effect on the length of stay in the ICU (rate ratio for the time to ICU discharge, 0.98; 95% confidence interval [CI], 0.88 to 1.09; P=0.72), ICU mortality (relative risk, 1.07; 95% CI, 0.90 to 1.28), or any other end point. Analyses restricted to patients who were admitted at night showed similar results, as did sensitivity analyses that used different definitions of exposure and outcome.
Conclusions
In an academic medical ICU in the United States, nighttime in-hospital intensivist staffing did not improve patient outcomes. (Funded by University of Pennsylvania Health System and others; ClinicalTrials.gov number, NCT01434823.
What are the mechanisms that support healthcare professionals to adopt assisted decision-making practice? A rapid realist review
Background
The United Nations Convention on the Rights of Persons with Disabilities (UNCRPD) establishes a right to legal capacity for all people, including those with support needs. People with disabilities have a legal right to be given the appropriate supports to make informed decisions in all aspects of their lives, including health. In Ireland, the Assisted Decision-Making (Capacity) Act (2015) ratifies the Convention and has established a legal framework for Assisted Decision Making (ADM). The main provisions of the Act are not yet implemented. Codes of Practice to guide health and social care professionals are currently being developed. Internationally, concerns are expressed that ADM implementation is poorly understood. Using realist synthesis, this study aims to identify Programme Theory (PT) that will inform ADM implementation in healthcare.
Methods
A Rapid Realist Review using collaborative methods was chosen to appraise relevant literature and engage knowledge users from Irish health and social care. The review was led by an expert panel of relevant stakeholders that developed the research question which asks, âwhat mechanisms enable healthcare professionals to adopt ADM into practice?â
To ensure the PT was inclusive of local contextual influences, five reference panels were conducted with healthcare professionals, family carers and people with dementia. PT was refined and tested iteratively through knowledge synthesis informed by forty-seven primary studies, reference panel discussions and expert panel refinement and consensus.
Results
The review has developed an explanatory PT on ADM implementation in healthcare practice. The review identified four implementation domains as significant. These are Personalisation of Health & ADM Service Provision, Culture & Leadership, Environmental & Social Re-structuring and Education, Training & Enablement. Each domain is presented as an explanatory PT statement using realist convention that identifies context, mechanism and outcome configurations.
Conclusions
This realist review makes a unique contribution to this field. The PT can be applied by policymakers to inform intervention development and implementation strategy. It informs the imminent policy and practice developments in Ireland and has relevance for other worldwide healthcare systems dealing with similar legislative changes in line with UNCRPD
Asteroids' physical models from combined dense and sparse photometry and scaling of the YORP effect by the observed obliquity distribution
The larger number of models of asteroid shapes and their rotational states
derived by the lightcurve inversion give us better insight into both the nature
of individual objects and the whole asteroid population. With a larger
statistical sample we can study the physical properties of asteroid
populations, such as main-belt asteroids or individual asteroid families, in
more detail. Shape models can also be used in combination with other types of
observational data (IR, adaptive optics images, stellar occultations), e.g., to
determine sizes and thermal properties. We use all available photometric data
of asteroids to derive their physical models by the lightcurve inversion method
and compare the observed pole latitude distributions of all asteroids with
known convex shape models with the simulated pole latitude distributions. We
used classical dense photometric lightcurves from several sources and
sparse-in-time photometry from the U.S. Naval Observatory in Flagstaff,
Catalina Sky Survey, and La Palma surveys (IAU codes 689, 703, 950) in the
lightcurve inversion method to determine asteroid convex models and their
rotational states. We also extended a simple dynamical model for the spin
evolution of asteroids used in our previous paper. We present 119 new asteroid
models derived from combined dense and sparse-in-time photometry. We discuss
the reliability of asteroid shape models derived only from Catalina Sky Survey
data (IAU code 703) and present 20 such models. By using different values for a
scaling parameter cYORP (corresponds to the magnitude of the YORP momentum) in
the dynamical model for the spin evolution and by comparing synthetics and
observed pole-latitude distributions, we were able to constrain the typical
values of the cYORP parameter as between 0.05 and 0.6.Comment: Accepted for publication in A&A, January 15, 201
Overview of systematic reviews of therapeutic ranges : methodologies and recommendations for practice
BACKGROUND: Many medicines are dosed to achieve a particular therapeutic range, and monitored using therapeutic drug monitoring (TDM). The evidence base for a therapeutic range can be evaluated using systematic reviews, to ensure it continues to reflect current indications, doses, routes and formulations, as well as updated adverse effect data. There is no consensus on the optimal methodology for systematic reviews of therapeutic ranges. METHODS: An overview of systematic reviews of therapeutic ranges was undertaken. The following databases were used: Cochrane Database of Systematic Reviews (CDSR), Database of Abstracts and Reviews of Effects (DARE) and MEDLINE. The published methodologies used when systematically reviewing the therapeutic range of a drug were analyzed. Step by step recommendations to optimize such systematic reviews are proposed. RESULTS: Ten systematic reviews that investigated the correlation between serum concentrations and clinical outcomes encompassing a variety of medicines and indications were assessed. There were significant variations in the methodologies used (including the search terms used, data extraction methods, assessment of bias, and statistical analyses undertaken). Therapeutic ranges should be population and indication specific and based on clinically relevant outcomes. Recommendations for future systematic reviews based on these findings have been developed. CONCLUSION: Evidence based therapeutic ranges have the potential to improve TDM practice. Current systematic reviews investigating therapeutic ranges have highly variable methodologies and there is no consensus of best practice when undertaking systematic reviews in this field. These recommendations meet a need not addressed by standard protocols

A controversy at the 2016 IUCN World Conservation Congress on the topic of closing domestic ivory markets (the 007, or so-called James Bond, motion) has given rise to a debate on IUCN's value proposition. A cross-section of authors who are engaged in IUCN but not employed by the organization, and with diverse perspectives and opinions, here argue for the importance of safeguarding and strengthening the unique technical and convening roles of IUCN, providing examples of what has and has not worked. Recommendations for protecting and enhancing IUCN's contribution to global conservation debates and policy formulation are given
Belle II Technical Design Report
The Belle detector at the KEKB electron-positron collider has collected
almost 1 billion Y(4S) events in its decade of operation. Super-KEKB, an
upgrade of KEKB is under construction, to increase the luminosity by two orders
of magnitude during a three-year shutdown, with an ultimate goal of 8E35 /cm^2
/s luminosity. To exploit the increased luminosity, an upgrade of the Belle
detector has been proposed. A new international collaboration Belle-II, is
being formed. The Technical Design Report presents physics motivation, basic
methods of the accelerator upgrade, as well as key improvements of the
