    Mapping the autistic advantage from the accounts of adults diagnosed with autism: A qualitative study

Strengths and weaknesses have traditionally been conceptualized as dichotomous. Methods: We conducted 28 semi-structured interviews with autistic adults. Maximum variation sampling was used to ensure diversity in relation to support needs. We asked which personal traits adults attributed to their autism, and how these have helped in the workplace, in relationships, and beyond. Data were collected in two stages. Responses were analyzed using content and thematic techniques. Results: The ability to hyperfocus, attention to detail, good memory, and creativity were the most frequently described traits. Participants also described specific qualities relating to social interaction, such as honesty, loyalty, and empathy for animals or for other autistic people. In thematic analysis we found that traits associated with autism could be experienced either as advantageous or disadvantageous dependent on moderating influences. Moderating influences included the social context in which behaviors occurred, the ability to control behaviors, and the extent to which traits were expressed. Conclusions: Separating autistic strengths from weaknesses may be a false dichotomy if traits cannot be isolated as separate constructs of strengths or deficits. If attempts to isolate problematic traits from advantageous traits are ill conceived, there may be implications for interventions that have reduction in autistic traits as a primary outcome measure.Wellcome Trus

    Sol-gel based materials for biomedical applications

    Sol-gel chemistry offers a flexible approach to obtaining a diverse range of materials. It allows differing chemistries to be achieved as well as offering the ability to produce a wide range of nano-/micro-structures. The paper commences with a generalized description of the various sol-gel methods available and how these chemistries control the bulk properties of the end products. Following this, a more detailed description of the biomedical areas where sol-gel materials have been explored and found to hold significant potential. One of the interesting fields that has been developed recently relates to hybrid materials that utilize sol-gel chemistry to achieve unusual composite properties. Another intriguing feature of sol-gels is the unusual morphologies that are achievable at the micro- and nano-scale. Subsequently the ability to control pore chemistry at a number of different length scales and geometries has proven to be a fruitful area of exploitation, that provides excellent bioactivity and attracts cellular responses as well as enables the entrapment of biologically active molecules and their controllable release for therapeutic action. The approaches of fine-tuning surface chemistry and the combination with other nanomaterials have also enabled targeting of specific cell and tissue types for drug delivery with imaging capacity

    Bereavement and the role of religious and cultural factors.

    yesThe aim of this article is to give an overview of some of the key dimensions of variation in cultural and religious rituals during the immediate period after a death and in the longer term, in order to inform service delivery in multi-cultural societies. For each area we give examples of different customs, and consider their functions and possible impact. Dimensions considered in the immediate period after bereavement are: The time and space given to formal rituals, expression of feelings, assertion of status and disposal of the body. In the longer term, we look at variations in remembering the deceased and in continuing bonds. Throughout we consider the interplay between individual responses and the personÂżs cultural and religious context. Our objective is to provide an accessible introduction for practitioners new to working with bereavement and provide a succinct reference point for more experienced bereavement workers.Full text made available 1st May 2014 at the end of the publisher's embargo period

    Multidetector row CT for imaging the paediatric tracheobronchial tree

    The introduction of multidetector row computed tomography (MDCT) scanners has altered the approach to imaging the paediatric thorax. In an environment where the rapid acquisition of CT data allows general hospitals to image children instead of referring them to specialist paediatric centres, it is vital that general radiologists have access to protocols appropriate for paediatric applications. Thus a dramatic reduction in the delivered radiation dose is ensured with optimal contrast bolus delivery and timing, and inappropriate repetition of the scans is avoided. This article focuses on the main principles of volumetric CT imaging that apply generically to all MDCT scanners. We describe the reconstruction techniques for imaging the paediatric thorax and the low-dose protocols used in our institution on a 16-slice detector CT scanner. Examples of the commonest clinical applications are also given

    Molybdenum isotope and trace metal signals in an iron-rich Mesoproterozoic ocean: A snapshot from the Vindhyan Basin, India

    Fundamental questions persist regarding the redox structure and trace metal content of the Mesoproterozoic oceans. Multiple lines of evidence suggest more widespread anoxia in the deep oceans compared to today, and iron speciation indicates that anoxia was largely accompanied by dissolved ferrous iron (ferruginous conditions) rather than free sulfide (euxinia). Still, exceptions exist—euxinic conditions have been reported from some ocean margin and epeiric sea settings, and oxic conditions were reported in one deeper water environment and are also known from shallow waters. Constraining the temporal evolution of Mesoproterozoic marine redox structure is critical because it likely governed redox-sensitive trace metal availability, which in turn played a significant role in marine diazotrophy and the evolution of early eukaryotes. Here, we present a new, multi-proxy geochemical dataset from the ~1.2 Ga Bijaygarh Shale (Kaimur Group, Vindhyan Basin, India) emphasizing total organic carbon, iron speciation, and trace metal concentrations, as well as sulfur, nitrogen, and molybdenum isotopes. This unit was deposited in an open shelf setting near or just below storm wave base. Taken together, our data provide a unique snapshot of a biologically important shallow shelf setting during the Mesoproterozoic Era, which includes: 1) locally ferruginous waters below the zone of wave mixing, 2) muted enrichment of trace metals sensitive to general anoxia (e.g., chromium) and variable enrichment of trace metals sensitive to euxinia (e.g., molybdenum and, to a lesser extent, vanadium), 3) general sulfate limitation, and 4) nitrogen fixation by molybdenum-nitrogenase and a dominantly anaerobic nitrogen cycle in offshore settings. Differential patterns of trace metal enrichment are consistent with data from other basins and suggest a largely anoxic ocean with limited euxinia during the Mesoproterozoic Era. Our new molybdenum isotope data—the first such data from unambiguously marine shales deposited between 1.4 and 0.75 Ga—record values up to +1.18 ± 0.12‰ that are analogous to data from other Mesoproterozoic shale units. Ultimately, this study provides a broad, multi-proxy perspective on the redox conditions that accompanied early eukaryotic evolution

    Filamin A (FLNA) mutation—A newcomer to the childhood interstitial lung disease (ChILD) classification

    AIM: Interstitial lung disease (ILD) in infants represents a rare and heterogenous group of disorders, distinct from those occurring in adults. In recent years a new entity within this category is being recognized, namely filamin A (FLNA) mutation related lung disease. Our aims are to describe the clinical and radiological course of patients with this disease entity to aid clinicians in the prognostic counseling and management of similar patients they may encounter. METHOD: A retrospective case note review was conducted of all patients treated at our institution (a specialist tertiary referral childrens’ center) for genetically confirmed FLNA mutation related lung disease. The clinical presentation, evolution, management and radiological features were recorded and a medical literature review of Medline indexed articles was conducted. RESULTS: We present a case series of four patients with interstitial lung disease and genetically confirmed abnormalities within the FLNA gene. Their imaging findings all reveal a pattern of predominantly upper lobe overinflation, coarse pulmonary lobular septal thickening and diffuse patchy atelectasis. The clinical outcomes of our patients have been variable ranging from infant death, lobar resection and need for supplemental oxygen and bronchodilators. CONCLUSION: The progressive nature of the pulmonary aspect of this disorder and need for early aggressive supportive treatment make identification crucial to patient management and prognostic counseling

    Sleep habits and sleep disturbances in Dutch children: a population-based study

    Sleep disorders can lead to significant morbidity. Information on sleep in healthy children is necessary to evaluate sleep disorders in clinical practice, but data from different societies cannot be simply generalized. The aims of this study were to (1) assess the prevalence of sleep disturbances in Dutch healthy children, (2) describe sleep habits and problems in this population, (3) collect Dutch norm data for future reference, and (4) compare sleep in children from different cultural backgrounds. A population-based descriptive study was conducted using the Children’s sleep habits questionnaire and the sleep self-report. One thousand five hundred seven proxy-reports and 262 self-reports were analyzed. Mean age was 8.5 years (95% confidence interval, 8.4–8.6), 52% were boys. Sleep problems in Dutch children were present in 25%, i.e., comparable to other populations. Sleep habits were age-related. Problem sleepers scored significantly higher on all scales. Correlations between parental and self-assessments were low to moderate. Dutch children had significantly more sleep disturbances than children from the USA and less than Chinese children. Cognitions and attitudes towards what is considered normal sleep seem to affect the appraisal of sleep, this probably accounts partly for cultural differences. For a better understanding of cultural influences on sleep, more information on these determinants and the establishment of cultural norms are mandatory
