    Pressure Losses in Multiple-Elbow Paths and in V-Bends of Hydraulic Manifolds

    Hydraulic manifolds are used to realize compact circuit layouts, but may introduce high pressure losses in the system because their design is usually oriented to achieving minimum size and weight more than reducing the pressure losses. The purpose of this work is to obtain the pressure losses when the internal connections within the manifold are creating complex paths for the fluid and the total loss cannot be calculated simply as the sum of the single losses. To perform the analysis both Computational Fluid Dynamic (CFD) analysis and experimental tests have been executed. After the comparison between numerical and experimental results, it was possible to assess that the numerical analysis developed in this work is able to depict the correct trends of the pressure losses also when complex fluid path are realized in the manifold. Successively, the numerical analysis was used to calculate the pressure loss for inclined connections of channels (or V-bends), a solution that is sometimes adopted in manifolds to meet the design requirements aimed towards the minimum room-minimum weight objective

    Design Of Two-Stage On/Off Cartridge Valves For Mobile Applications

    Cartridge valves are widely used in mobile applications, where they are screwed in manifolds, to realize opportune circuit layouts. These valves are quite simple in operation but require a sophisticated design in order to meet all the requirements needed in the mobile machines. Typically, the design process is developed realizing a first design concept and some prototypes and experimentally testing them; after this, the designer chases the optimal performances requested to the valve with a trial and error approach on the prototypes, involving high time and cost resources. In this paper an alternative design procedure is proposed, which involves dedicated simulations to analyze the main critical issues regarding the cartridge valve object of the study. Modelling and simulations here have been considered as steps into the design process of a new valve, which satisfies the requirements and well adapt to the necessities to operate at higher flow and pressure levels without compromising its performances. In that way, the number of prototypes, realized to validate the numerical results and verify the design process, has been considerably reduced, together with related time and costs

    Hydraulic manifolds are used to realize compact circuit layout, but may introduce a high pressure drop in the system. Their design is in fact oriented more toward achieving minimum size and weight than to reducing pressure losses. This work studies the pressure losses in hydraulic manifolds using different methods: Computational Fluid Dynamic (CFD) analysis; semi-empirical formulation derived from the scientific literature, when available; and experimental characterization. The purpose is to obtain the pressure losses when the channels' connections within the manifold are not ascribable to the few classic cases studied in the literature, in particular for 90° bends (elbows) with expansion/contraction and offset intersection of channels. Moreover, since CFD analysis is used to predict pressure losses, general considerations of the manifold design may be outlined and this will help the design process in the optimization of flow passages. The main results obtained show how CFD analysis overestimates the experimental results; nevertheless, the numerical analysis represents the correct trends of the pressure losses

    The Importance of Reading the Skin: Cutaneous Metastases of Pancreatic Cancer, a Systematic Review

    Background: Pancreatic cancer is notorious for its aggressive nature and low survival rate, with less than 10% of patients surviving beyond five years. Early detection is difficult, but skin metastases can be a rare but significant indicator. This systematic review focuses on the epidemiology, clinical features, and histology of skin metastases from pancreatic cancer to determine their importance in early diagnosis and overall management of the disease. Materials and methods: Following PRISMA guidelines, we conducted an exhaustive search of MEDLINE/PubMed, EMBASE, and SCOPUS databases up to June 2023, using specific keywords. Four independent investigators screened the studies using predefined criteria, and two investigators checked the accuracy and consistency of the data extraction. We assessed the quality of the trials using adapted criteria from the Joanna Briggs Institute. A narrative synthesis rather than a meta-analysis was chosen because of the different study designs. Results: The final analysis included 57 patients with skin metastases from pancreatic cancer. Cutaneous metastases, although rare, presented with approximately equal gender distribution and a mean age of 63.4 years. Predominantly non-umbilical (77%), these metastases showed clinical diversity, ranging from asymptomatic nodules to painful or ulcerated lesions. Notably, skin metastases often preceded the diagnosis of primary pancreatic cancer (58%). Primary tumor characteristics revealed different localizations, with adenocarcinoma being the most prevalent histological type (77%). A significant association (p = 0.008) was observed between pancreatic tumor location and the timing of presentation of skin metastases. Tumors located in the body and tail of the pancreas were more likely to manifest skin metastases as an initial clinical manifestation (62.2%) than those in the head of the pancreas (20.8%). Conclusions: In conclusion, although skin metastases are rare, they are important indicators of pancreatic cancer, highlighting the need for multidisciplinary healthcare collaboration and thorough skin examination. Recognizing them could lead to earlier diagnosis, which is crucial in a cancer with limited treatment options

    Development and validation of a new prognostic system for patients with hepatocellular carcinoma.

    Background Prognostic assessment in patients with hepatocellular carcinoma (HCC) remains controversial. Using the Italian Liver Cancer (ITA.LI.CA) database as a training set, we sought to develop and validate a new prognostic system for patients with HCC. Methods and Findings Prospective collected databases from Italy (training cohort, n = 3,628; internal validation cohort, n = 1,555) and Taiwan (external validation cohort, n = 2,651) were used to develop the ITA.LI.CA prognostic system. We first defined ITA.LI.CA stages (0, A, B1, B2, B3, C) using only tumor characteristics (largest tumor diameter, number of nodules, intra- and extrahepatic macroscopic vascular invasion, extrahepatic metastases). A parametric multivariable survival model was then used to calculate the relative prognostic value of ITA.LI. CA tumor stage, Eastern Cooperative Oncology Group (ECOG) performance status, Child\u2013 Pugh score (CPS), and alpha-fetoprotein (AFP) in predicting individual survival. Based on the model results, an ITA.LI.CA integrated prognostic score (from 0 to 13 points) was constructed, and its prognostic power compared with that of other integrated systems (BCLC, HKLC, MESIAH, CLIP, JIS). Median follow-up was 58 mo for Italian patients (interquartile range, 26\u2013106 mo) and 39 mo for Taiwanese patients (interquartile range, 12\u201361 mo). The ITA.LI.CA integrated prognostic score showed optimal discrimination and calibration abilities in Italian patients. Observed median survival in the training and internal validation sets was 57 and 61 mo, respectively, in quartile 1 (ITA.LI.CA score 1), 43 and 38 mo in quartile 2 (ITA.LI.CA score 2\u20133), 23 and 23 mo in quartile 3 (ITA.LI.CA score 4\u20135), and 9 and 8 mo in quartile 4 (ITA.LI.CA score > 5). Observed and predicted median survival in the training and internal validation sets largely coincided. Although observed and predicted survival estimations were significantly lower (log-rank test, p < 0.001) in Italian than in Taiwanese patients, the ITA.LI.CA score maintained very high discrimination and calibration features also in the external validation cohort. The concordance index (C index) of the ITA.LI.CA score in the internal and external validation cohorts was 0.71 and 0.78, respectively. The ITA.LI.CA score\u2019s prognostic ability was significantly better (p < 0.001) than that of BCLC stage (respective C indexes of 0.64 and 0.73), CLIP score (0.68 and 0.75), JIS stage (0.67 and 0.70), MESIAH score (0.69 and 0.77), and HKLC stage (0.68 and 0.75). The main limitations of this study are its retrospective nature and the intrinsically significant differences between the Taiwanese and Italian groups. Conclusions The ITA.LI.CA prognostic system includes both a tumor staging\u2014stratifying patients with HCC into six main stages (0, A, B1, B2, B3, and C)\u2014and a prognostic score\u2014integrating ITA.LI.CA tumor staging, CPS, ECOG performance status, and AFP. The ITA.LI.CA prognostic system shows a strong ability to predict individual survival in European and Asian populations

    Direct-acting antivirals and hepatocellular carcinoma in chronic hepatitis C: A few lights and many shadows

    With the introduction of direct-acting antiviral agents (DAA), the rate of sustained virological response (SVR) in the treatment of hepatitis C virus (HCV) has radically improved to over 95%. Robust scientific evidence supports a beneficial role of SVR after interferon therapy in the progression of cirrhosis, resulting in a decreased incidence of hepatocellular carcinoma (HCC). However, a debate on the impact of DAAs on the development of HCC is ongoing. This review aimed to analyse the scientific literature regarding the risk of HCC in terms of its recurrence and occurrence after the use of DAAs to eradicate HCV infection. Among 11 studies examining HCC occurrence, the de novo incidence rate ranged from 0 to 7.4% (maximum follow-up: 18 mo). Among 18 studies regarding HCC recurrence, the rate ranged from 0 to 54.4% (maximum "not well-defined" followup: 32 mo). This review highlights the major difficulties in interpreting data and reconciling the results of the included studies. These difficulties include heterogeneous cohorts, potential misclassifications of HCC prior to DAA therapy, the absence of an adequate control group, short follow-up times and different kinds of follow-up. Moreover, no clinical feature-based scoring system accounts for the molecular characteristics and pathobiology of the tumours. Nonetheless, this review does not suggest that there is a higher rate of de novo HCC occurrence or recurrence after DAA therapy in patients with previous HCV infection. \ua9 2018 The Author(s).     Platelets and hepatocellular cancer: Bridging the bench to the clinics

    Growing interest is recently being focused on the role played by the platelets in favoring hepatocellular cancer (HCC) growth and dissemination. The present review reports in detail both the experimental and clinical evidence published on this topic. Several growth factors and angiogenic molecules specifically secreted by platelets are directly connected with tumor progression and neo-angiogenesis. Among them, we can list the platelet-derived growth factor, the vascular endothelial growth factor, the endothelial growth factor, and serotonin. Platelets are also involved in tumor spread, favoring endothelium permeabilization and tumor cells' extravasation and survival in the bloodstream. From the bench to the clinics, all of these aspects were also investigated in clinical series, showing an evident correlation between platelet count and size of HCC, tumor biological behavior, metastatic spread, and overall survival rates. Moreover, a better understanding of the mechanisms involved in the platelet-tumor axis represents a paramount aspect for optimizing both current tumor treatment and development of new therapeutic strategies against HCC

    Lumped parameter model of vane pumps developed in OpenModelica environment

    In this paper, the authors present a 0D fluid dynamic model of a vane pump used to refill tanks with fuel. The model is entirely developed in OpenModelica environment, where the authors have created specific libraries of elements suitable for the physical modelling of fluid power components and systems. Among the different approaches, the zero-dimension (0D) fluid-dynamic modelling of positive displacement machines is suitable to study many aspects as: the instantaneous flow rate, pressure and torque transients, the fluid borne noise related to the flow rate and pressure irregularity, the dynamic behaviour of the variable displacement control. Overall, this approach in modelling allows to link the geometrical features of the machine with its dynamic behaviour and for this reason is particularly useful in guiding the design. The model of the vane pump is described together with the main design features that can be analysed in terms of their influence on the pump behaviour. Besides the specific results obtained regarding the design of the pump, the paper also demonstrates the use of OpenModelica language and environment, and its efficacy, into the applications of fluid power modelling and simulation

    Money, privacy, anonimity: what experiments tell us

    The attention paid to the role of money as a store of plivacy is increasing. In a monetmy transaction, full plivacy protection coincides with anonymity. In such situations, an empilical question arises: Is anonymity relevant in shaping the demand for money? We attempt to answer this question through laborat01Y experiments. The results show that anonymity matters and increases the overall appeal of a medium of payment, and that this effect is stronger for lisk-prone individuals. Moreover, the trade-off between the two propelTies of liquidity and retum is relatively high — to accept higher illiquidity risks, individuals require a more-than-propolTional increase in the expected retum. In general, the expeliments suggest that the future attractiveness of altemative cunencies depends on whether the three propelTies of money are mixed in a way that is consistent with the individual's preferences
