    Skin manifestations following anti-COVID-19 vaccination: A multicentricstudy from Turkey

    Purpose: After the emergence of the pandemic caused by the COVID-19 virus, vaccination with various vaccines has started to be implemented across the world. To identify dermatological reactions developing after the COVID-19 vaccines administered in Turkey and determine their clinical features and risk factors that may play a role in their development. Materials and Methods: The study included patients aged ≥18 years, who presented to 13 different dermatology clinics in Turkey between July 2021 and September 2021 after developing dermatological reactions following the administration of the COVID-19 vaccine. After providing written consent, the patients were asked to complete a standard survey including questions related to age, gender, occupation, comorbidities, the regular medication used, the onset of cutaneous reactions after vaccination, and localization of reactions. Dermatological reactions were categorized according to whether they developed after the first or second dose of the vaccine or whether they occurred after the inactivated or messenger RNA (mRNA) vaccine. The relationship between dermatological reactions and some variables such as gender and comorbidities was also evaluated. Results: A total of 269 patients [116 women (43.1%), 153 men (56.9%)] were included in the study. It was observed that the dermatological diseases and reactions that most frequently developed after vaccination were urticaria (25.7%), herpes zoster (24.9%), maculopapular eruption (12.3%), and pityriasis rosea (4.5%). The rate of dermatological reactions was 60.6% after the administration of the mRNA vaccine and 39.4% after that of the inactivated vaccine. There was a statistically significantly higher number of reactions among the patients that received the mRNA vaccine (p = 0.001). Conclusion: The most common reactions in our sample were urticaria, herpes zoster, and maculopapular eruption. Physicians should know the dermatological side effects of COVID-19 vaccines and their clinical features

    In a real-life setting, direct-acting antivirals to people who inject drugs with chronic hepatitis c in Turkey

    Background: People who inject drugs (PWID) should be treated in order to eliminate hepatitis C virus in the world. The aim of this study was to compare direct-acting antivirals treatment of hepatitis C virus for PWID and non-PWID in a real-life setting. Methods: We performed a prospective, non-randomized, observational multicenter cohort study in 37 centers. All patients treated with direct-acting antivirals between April 1, 2017, and February 28, 2019, were included. In total, 2713 patients were included in the study among which 250 were PWID and 2463 were non-PWID. Besides patient characteristics, treatment response, follow-up, and side effects of treatment were also analyzed. Results: Genotype 1a and 3 were more prevalent in PWID-infected patients (20.4% vs 9.9% and 46.8% vs 5.3%). The number of naïve patients was higher in PWID (90.7% vs 60.0%), while the number of patients with cirrhosis was higher in non-PWID (14.1% vs 3.7%). The loss of follow-up was higher in PWID (29.6% vs 13.6%). There was no difference in the sustained virologic response at 12 weeks after treatment (98.3% vs 98.4%), but the end of treatment response was lower in PWID (96.2% vs 99.0%). In addition, the rate of treatment completion was lower in PWID (74% vs 94.4%). Conclusion: Direct-acting antivirals were safe and effective in PWID. Primary measures should be taken to prevent the loss of follow-up and poor adherence in PWID patients in order to achieve World Health Organization’s objective of eliminating viral hepatitis

    Outcomes of high-risk breast lesions diagnosed using image-guided core needle biopsy: results from a multicenter retrospective study

    PURPOSEThe clinical management of high-risk lesions using image-guided biopsy is challenging. This study aimed to evaluate the rates at which such lesions were upgraded to malignancy and identify possible predictive factors for upgrading high-risk lesions.METHODSThis retrospective multicenter analysis included 1.343 patients diagnosed with high-risk lesions using an image-guided core needle or vacuum-assisted biopsy (VAB). Only patients managed using an excisional biopsy or with at least one year of documented radiological follow-up were included. For each, the Breast Imaging Reporting and Data System (BI-RADS) category, number of samples, needle thickness, and lesion size were correlated with malignancy upgrade rates in different histologic subtypes. Pearson’s chi-squared test, the Fisher–Freeman–Halton test, and Fisher’s exact test were used for the statistical analyses.RESULTSThe overall upgrade rate was 20.6%, with the highest rates in the subtypes of intraductal papilloma (IP) with atypia (44.7%; 55/123), followed by atypical ductal hyperplasia (ADH) (38.4%; 144/375), lobular neoplasia (LN) (12.7%; 7/55), papilloma without atypia (9.4%; 58/611), flat epithelial atypia (FEA) (8.7%; 10/114), and radial scars (RSs) (4.6%; 3/65). There was a significant relationship between the upgrade rate and BI-RADS category, number of samples, and lesion size Lesion size was the most predictive factor for an upgrade in all subtypes.CONCLUSIONADH and atypical IP showed considerable upgrade rates to malignancy, requiring surgical excision. The LN, IP without atypia, pure FEA, and RS subtypes showed lower malignancy rates when the BI-RADS category was lower and in smaller lesions that had been adequately sampled using VAB. After being discussed in a multidisciplinary meeting, these cases could be managed with follow-up instead of excision

    Age-related and sex-related changes in the intervertebral discs and endplates of rats at all spinal levels

    WOS: 000256162100007The intervertebral disc (IVD) and endplates change throughout life, eventually resulting in tissue degeneration, which may have clinical significance. In this study, we analyzed age-related changes in the IVD and endplate of newborn (1 mo), middle-aged (12 mo), and older (24 mo) male and female Sprague-Dawley rats. We evaluated the presence of embryonic disc cells, chondrocyte proliferation, cell death, appearance of clefts/tears, granular degeneration, cell proliferation, and cartilage disorganization in the IVD and endplate under light microscopy, according to age and sex Chondrocyte proliferation was greater in male rats than in female rats across all age groups in the cervical region and at 1, and 24 months in the thoracic region. A strong correlation between aging and cell death in female rats was observed.. In females, cell death was significantly elevated in 12 and 24-month-old rats compared with 1-month-old rats. Examining the histologic changes with aging in the IVD and endplates at all levels of the spine is obviously difficult in humans. Animal research is essential for making the transition from scientific concepts to clinical applications. The aging spine shows the greatest changes in the cervical and thoracic regions in male rats, whereas it mostly affects the lumbar region in females

    Amaç: Bu çalışmada Konya Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Göğüs Cerrahisi Kliniği'nde yatarak tedavi gören hastaların depresyon durumu ve yaşam kaliteleri değerlendirildi. Çalışma planı: Bu kesitsel, analitik çalışmaya 15 Haziran 2012-15 Şubat 2013 tarihleri arasında Göğüs Cerrahisi kliniğinde yatarak tedavi gören 170 hasta (124 erkek, 46 kadın, ort. yaş 45.718.2 yıl; dağılım 7-80 yıl) dahil edildi. Depresyon durumu Beck Depresyon Ölçeği ile yaşam kalitesi ise 36 maddelik Kısa Form Sağlık Anketi (kısa form 36) ile değerlendirildi. Bulgular: Beck depresyon puanı ortalaması 11.068.79 idi. Kısa form 36'nın alt ölçeklerinden fiziksel rol kısıtlılıkları, emosyonel güçlük ve genel sağlık puan ortalamaları düşük; mental sağlık ve sosyal fonksiyon puan ortalamaları en yüksek idi. Yaşam kalitesi puanları ile depresyon durumu karşılaştırıldığında, kısa form 36'nın tüm alt ölçeklerinde depresyonu olanlarla olmayanlar arasında istatistiksel olarak anlamlı farklılıklar var idi. Depresyonu olmayanlarda yaşam kalitesi puanları daha yüksek idi. Hastalar akciğer kanseri, akciğer enfeksiyonu ve göğüs acilleri olarak üç tanı grubunda incelendiğinde, tüm gruplarda erkek hasta sayısı kadınlara göre daha yüksek idi; bu durum, tanı ile cinsiyet arasında anlamlı ilişki olduğuna işaret ediyor idi (p0.004). Akciğer kanseri tanısı en çok fiziksel fonksiyon ve emosyonel güçlüğü etkiler iken, göğüs acilleri en çok genel sağlığı, yaşama gücünü ve emosyonel güçlüğü etkiliyor idi. Akciğer enfeksiyonları ise en çok emosyonel güçlüğü etkiliyor idi. Sonuç: Tüm kronik hastalıklarda yaşam kalitesi, sosyal ve fiziksel işlevsellik olumsuz etkilenmekte ve bireylerin yaşamdan aldıkları doyum azalmaktadır. Hastaların depresyon düzeylerinin ve yaşam kalitelerinin ölçülmesi hastalığı daha iyi tanımamıza yardımcı olmakla kalmayıp tedavi yanıtlarının da daha iyi değerlendirilmesini sağlayacaktır.Background: This study aims to evaluate the depression status and quality of life of the patients at the Department of Thoracic Surgery of Necmettin Erbakan University, Meram Medical School. Methods: This cross-sectional, analytic study included 170 inpatients (124 males, 46 females, mean age 45.7±18.2 years; range 7 to 80 years) at the Department of Thoracic Surgery between 15 June 2012 and 15 February 2013. Depression status was evaluated with Beck Depression Inventory, quality of life was evaluated with 36-Item Short Form Health Survey (short form-36). Results: Mean Beck depression score was 11.06±8.79. Of subscales of short form-36; mean scores of physical role limitations, emotional difficulties and general health were low; and mean scores of mental health and social functioning were the highest. When scores of quality of life were compared with depression status, there were statistically significant differences in all subscales of short form-36 between those with and without depression. Scores of quality of life were higher in those without depression. When patients were analyzed in three diagnosis groups as lung cancer, lung infection and thoracic emergencies, number of male patients in all groups was higher than females, indicating a significant relationship between diagnosis and sex (p0.004). While the diagnosis of lung cancer affected physical functioning and emotional difficulties the most, thoracic emergencies affected general health, vitality and emotional difficulties the most. However, lung infections affected emotional difficulties the most. Conclusion: The quality of life, social and physical functionality are negatively affected in all chronic diseases, and the individuals' satisfaction from life decreases. Measuring the depression levels and quality of life of patients will not only assist us in having a better understanding of the disease, but also provide better evaluation of treatment responses
