    Further insights on predictors of environmental tobacco smoke exposure during the pediatric age

    Background: The smoking ban in public places has reduced Environmental Tobacco Smoke (ETS) exposure for non-smokers, but despite this, domestic environments still remain places at high risk of exposure, and, today, about 40% of children worldwide are exposed to ETS at home. The aims of the study are to investigate the contribution of several factors on ETS exposure among a group of Italian children and to evaluate the changes in smoking precautions adopted at home when the smoker is the mother, the father, or both parents, respectively. Methods: A cross-sectional study was performed on a sample of 519 Italian schoolchildren. Information was collected via a questionnaire. Results: 41.4% of the participants lived with at least one smoker. Almost half of the children exposed to ETS lived with one or more smokers who do not observe any home smoking ban. Lower maternal or paternal educational levels significantly increase the risk of ETS exposure at home and the “worst case” is represented by both parents who smoke. Conclusions: More effective preventive interventions are needed to protect children from ETS exposure. Some interventions should be specifically dedicated to smokers with a low educational level and to mothers that smoke

    Isotopic Grand Unification with the Inclusion of Gravity (revised version)

    We introduce a dual lifting of unified gauge theories, the first characterized by the isotopies, which are axiom- preserving maps into broader structures with positive-definite generalized units used for the representation of matter under the isotopies of the Poincare' symmetry, and the second characterized by the isodualities, which are anti-isomorphic maps with negative-definite generalized units used for the representation of antimatter under the isodualities of the Poincare' symmetry. We then submit, apparently for the first time, a novel grand unification with the inclusion of gravity for matter embedded in the generalized positive-definite units of unified gauge theories while gravity for antimatter is embedded in the isodual isounit. We then show that the proposed grand unification provides realistic possibilities for a resolution of the axiomatic incompatibilities between gravitation and electroweak interactions due to curvature, antimatter and the fundamental space-time symmetries.Comment: 20 pages, Latex, revised in various details and with added reference

    Protective Coatings Based on PMMA–Silica Nanocomposites Reinforced with Carbon Nanotubes

    Polymethylmethacrylate–silica hybrids have been prepared using the sol–gel route by the radical polymerization of methyl methacrylate(MMA) using benzoyl peroxide (BPO) as a thermal initiator and 3-(trimethoxysilyl)propyl methacrylate(MPTS) as a coupling agent, followed by acid-catalyzed hydrolytic condensation of tetraethoxysilane (TEOS). Carbon nanotubes (CNTs) were first dispersed either by surfactant addition or by functionalization with carboxyl groups and then added at a carbon (CNT) to silicon (TEOS and MPTS) molar ratio (CCNT/SiHybrid) of 0.05% to two different hybrid matrices prepared at BPO/MMA molar ratios of 0.01 and 0.05. Films of 2–7 ÎŒm thickness deposited onto carbon steel by dip-coating were characterized in terms of their microstructure and their mechanical, thermal and anticorrosive behavior. Atomic force microscopy and optical microscopy confirmed that there was a homogeneous dispersion of CNTs in the nanocomposites and that the surfaces of the films were very smooth. X-ray photoelectron spectroscopy (XPS) confirmed the nominal composition of the films while nuclear magnetic resonance showed that the connectivity of the silica network was unaffected by CNT loading. Thermogravimetric analysis and mechanical measurements confirmed an increase of thermal stability, hardness, adhesion and scratch resistance of CNT-loaded coatings relative to those without CNTs. Electrochemical impedance spectroscopy measurements in 3.5% NaCl solution interpreted in terms of equivalent circuits showed that the reinforced hybrid coatings, prepared at the higher BPO/MMA molar ratio used in this work, act as a very efficient anticorrosive barrier, with an impedance modulus up to 109 Ω cm2

    Subjects with temporomandibular joint disc displacement and body posture assessment via rasterstereography: a pilot case-control study

    Objective: The possible relationship between temporomandibular disorders (TMDs) and body posture is still controversial. Rasterstereography has been introduced as a radiation-free, reliable and non-invasive method to analyze three-dimensional spinal posture. The aim of this case-control study is to evaluate, through rasterstereography, body posture parameters in a group of patients with reducible unilateral dislocation of the articular disc, compared to healthy volunteers. Patients and methods: Rasterstereographic recordings obtained were compared between the two groups with a paired t-student test. Furthermore, the relationship between Rasterstereographic recordings and clinical data in the TMD group were analyzed by means of multiple regression analysis. Results: Only lateral deviation was statistically significant different between the two groups (rms VPDM Control group 40% > TMD group, p=0.02; 43% control group VPDM max > TMD group, p<0.02). In the TMD group, a significant relationship (p<0.05) was found out between lateral and rotational deviations of the column and muscular pain, therefore suggesting a possible overactivity of the masticatory muscles, especially of lateral pterygoids' bilaterally and the left masseter. Conclusions: Patients with reducible unilateral disc displacement showed limited postural alterations compared to healthy volunteers, only lateral deviations (VPDM rms and VPDM-max) were statistically significant (ƒ <0.05) between the two groups

    On the structure of high performance anticorrosive PMMA–siloxane–silica hybrid coatings

    Environmentally compliant organic–inorganic hybrid coatings for efficient corrosion protection of metallic surfaces are potential alternatives to the current method based on chromate passivation. In this context PMMA–siloxane–silica (PMMA–SS) hybrid films were prepared using the sol–gel process from the radical copolymerization of methyl methacrylate and 3-(trimethoxysilyl)propyl methacrylate followed by acidic hydrolysis and polycondensation of tetraethoxysilane (TEOS), under variation of the ethanol to H2O ratio (0.0–1.0). The structural properties of about 2 ÎŒm thick coatings, deposited by dip-coating onto carbon steel, were related with their corrosion protection efficiency. The correlation of data obtained by X-ray photoelectron spectroscopy, nuclear magnetic resonance and small angle X-ray scattering has shown for intermediate ethanol to H2O ratios the highest connectivity (∌83%) of the inorganic phase, bonded covalently to organic moieties, yielding a dense and homogeneous nanocomposite structure with high thermal stability, very good adhesion to the metallic substrate and excellent barrier properties. The electrochemical impedance spectroscopy measurements have shown for coatings prepared at intermediate EtOH/H2O ratios a high corrosion resistance of almost 10 GΩ cm2, which remained unchanged for more than 6 months in contact with 3.5% NaCl solution and more than 3 months exposed to an acidic NaCl environment

    Efficacy of hyaluronate injections in rotator cuff disorders: a level-I meta-analysis

    Background: Rotator cuff disease is the most common cause of shoulder pain and weakness. Conservative treatment is the first choice of shoulder pain management. Viscosupplementation of hyaluronic acid (HA) seems to be effective for management of tendon disorders. The objective of this study was to evaluate the scientific evidence reported in literature according to HA shoulder injection in rotator cuff disorders treatment. Methods: An English-language systematic literature search was performed by two independent researchers; data sources included the following databases: MEDLINE, Embase, CINAHL, Google scholar web, Ovid database, Physiotherapy Evidence Database (PEDro), and the Cochrane Library. We performed a broad research for relevant study up to February 2017. Articles were included if they reported data on clinical and functional outcomes in patients who had undergone HA injection for management of rotator cuff pathology compared to placebo, corticosteroid injection and/or physical therapies. Methodological quality was assessed with the PEDro rating scale. The outcomes were improvement of symptoms (assessed by VAS scale) and shoulder function (assessed through DASH and ASES Score). Results: 5 RCTs studies (990 patients) were pooled in the Meta-analysis. The PEDro rating scale ranged from 2 to 8. Two studies compared HA injection with corticosteroid injections, patients were injected once a week for three weeks. Four studies compared HA injection with placebo injection, of which two used 3 weekly injections and two used 5 weekly injections. Significant difference was found in pain reduction between HA and placebo group at 26 weeks follow-up (MD= -0.51, 95% CI -0.96 to -0.07), p=0.02. Conclusion: HA injections might be a valuable safe alternative to other conservative methods for the treatment of rotator cuff disorders. Nowadays, few and low quality randomized controlled trials have been published. Therefore, to reach an overall conclusion about the effect of HA injection in rotator cuff we need more high quality studies. Level of evidence: Therapeutic Level I.     Straighten your back! Self-correction posture and postural balance in 'non rehabilitative instructed' multiple sclerosis patients

    BACKGROUD: Patients with MS, regardless of the complexity of the activity or sensory conditions, commonly present a significant postural control deficit compared to healthy subjects. OBJECTIVE: To investigate which postural self-correction strategies are adopted by patients with Multiple Sclerosis versus a group of healthy-subjects and how self-correction can influence the control of postural balance. METHODS: A case-control prospective observational study was conducted. Person with Multiple Sclerosis and a group of healthy volunteers were enrolled. Patients included were instructed with vocal commands, to reach a self-correction posture, and they were compared to healthy subjects. Clinical assessments including Balance, Stabilometry and Postural evaluation of the spine were performed. RESULTS: Sixty patients (30: control-group; 30: treatment-group) were enrolled. In the treatment group, the analysis reported a significant statistical difference between path length and center of pressure speed in self-correction posture with closed-eyes (p=0,049; 0,047) and an improvement in C7 and L3 levels in self-correction posture (p<0,01-C7; p<0,01-L3). There are significant statistical differences about path length between the two groups in all examined conditions (p=0,0001). At sagittal plane evaluation, results show an increase of all measurements in both posture (C7-neutral posture p=0,0001; L3-neutral posture p=0,0001; C7-self-correction posture p=0,0001; L3-self-correction posture p=0,0001). CONCLUSION: Further study should investigate dynamic situations and different Multiple Sclerosis forms to complete balance analysis and to establish a correct rehabilitative program with self-correction exercise as powerful focus

    inborn errors of immunity associated with atopy (IEIs-A) are a group of inherited monogenic disorders that occur with immune dysregulation and frequent skin involvement. several pathways are involved in the pathogenesis of these conditions, including immune system defects, alterations of skin barrier and metabolism perturbations. current technological improvements and the higher accessibility to genetic testing, recently allowed the identification of novel molecular pathways involved in IEIs-A, also informing on potential tailored therapeutic strategies. compared to other systemic therapy for skin diseases, biologics have the less toxic and the best tolerated profile in the setting of immune dysregulation. Here, we review IEIs-A with skin involvement focusing on the tailored therapeutic approach according to their pathogenetic mechanism
