    Characterization of varroa destructor mites in Cuba using mitochondrial and nuclear markers

    Varroa destructor has been present in Cuba since 1996, but without the use of acaricidal infestation rates remain at very low levels. The presence of Korean haplotype mites was described in 2007, but there is no information regarding the introgression of the less virulent Japanese haplotype that could account for a low pathogenicity of the mite. In this research, we carried out molecular characterization of Cuban Varroa mites through mitochondrial DNA and hypervariable nuclear loci. We applied an alternative RFLP tech-nique and found that all the analyzed samples corresponded to Korean haplotypes. We analyzed the three STRs loci VD112, VD114 and VD016, previously described as highly variable and found new alleles in all of them, with an absolute allele size very different to those reported worldwide. We also detected genic and genotypic differentiation be-tween samples from two nearby locations (P=0.08).     Malignant pheochromocytoma treated with minimally invasive surgery

Objetivo: Describir 2 casos de Feocromocitoma maligno tratados con cirugía mínimamente invasiva. Casos clínicos: Presentamos 2 pacientes de sexo femenino, de 31 y 51 años respectivamente. La primera con historia de hipertensión arterial de larga data y uso de múltiples fármacos. La segunda paciente debuta con dolor abdominal. En ambos casos los estudios radiológicos y metabólicos confirmaron el diagnóstico de Feocromocitoma. A la primera paciente se le realizó un procedimiento laparoscópico puro, mientras que a la segunda paciente se le realizó un procedimiento laparoscópico asistido por robot. En ambos casos fue necesario realizar adrenalectomía en bloque con el riñón, por compromiso del hilio renal. No hubo complicaciones severas intra ni post-operatorias, como tampoco conversión a cirugía abierta. La histopatología confirmó el diagnóstico de Feocromocitoma maligno con metástasis ganglionares regionales y márgenes quirúrgicos negativos. Ambas se mantienen en control, sin evidencia de recurrencia. Conclusión: El Feocromocitoma maligno puede ser manejado de forma mínimamente invasiva, con morbilidad y resultados oncológicos comparables a cirugía abierta.Introduction: Pheochromocytoma is a tumor derived from chromaffin cells that produces cathecolamines. Malignant pheochromocytomas are extremely rare. Aim: To describe 2 cases of malignant Pheochromocytoma treated with pure laparoscopic and robot assisted surgery. Clinical cases: Two female patients, 31 and 51 years of age, presented with a history of high blood pressure and abdominal pain. Radiologic and metabolic studies confirmed the diagnosis of Pheochromocytoma in both patients. A pure laparoscopic approach was performed in the first patient while a robotic assisted laparoscopy was done in the second patient. In both cases, adrenalectomy with in bloc nephrectomy was performed due to compromise of the renal sinus. There were no complications or conversion to open surgery. Histopathology confirmed the diagnosis of malignant Pheochromocytoma with metastasis to regional nodes and negative surgical margins. At follow-up, there is no evidence of recurrence. Conclusion: Malignant Pheochromocytoma can be treated with minimally invasive surgery, with morbidity and oncological outcomes similar to open approach.http://ref.scielo.org/ksq86

    Enhancing dendritic cell immunotherapy for melanoma using a simple mathematical model

    ABSTRACT Background: The immunotherapy using dendritic cells (DCs) against different varieties of cancer is an approach that has been previously explored which induces a specific immune response. This work presents a mathematical model of DCs immunotherapy for melanoma in mice based on work by Experimental Immunotherapy Laboratory of the Medicine Faculty in the Universidad Autonoma de Mexico (UNAM). Method: The model is a five delay differential equation (DDEs) which represents a simplified view of the immunotherapy mechanisms. The mathematical model takes into account the interactions between tumor cells, dendritic cells, naive cytotoxic T lymphocytes cells (inactivated cytotoxic cells), effector cells (cytotoxic T activated cytotoxic cells) and transforming growth factor β cytokine (TGF − β). The model is validated comparing the computer simulation results with biological trial results of the immunotherapy developed by the research group of UNAM. Results: The results of the growth of tumor cells obtained by the control immunotherapy simulation show a similar amount of tumor cell population than the biological data of the control immunotherapy. Moreover, comparing the increase of tumor cells obtained from the immunotherapy simulation and the biological data of the immunotherapy applied by the UNAM researchers obtained errors of approximately 10 %. This allowed us to use the model as a framework to test hypothetical treatments. The numerical simulations suggest that by using more doses of DCs and changing the infusion time, the tumor growth decays compared with the current immunotherapy. In addition, a local sensitivity analysis is performed; the results show that the delay in time “τ ”, the maximal growth rate of tumor “r” and the maximal efficiency of tumor cytotoxic cells rate “aT” are the most sensitive model parameters. Conclusion: By using this mathematical model it is possible to simulate the growth of the tumor cells with or without immunotherapy using the infusion protocol of the UNAM researchers, to obtain a good approximation of the biological trials data. It is worth mentioning that by manipulating the different parameters of the model the effectiveness of the immunotherapy may increase. This last suggests that different protocols could be implemented by the Immunotherapy Laboratory of UNAM in order to improve their results

    More and more information resources exist in electronic format, such as online databases, journals, books, etc., instead of the traditional print format. Advances in information technology have introduced both new capabilities and interesting challenges in the access to medical literature. This has influenced dramatically medical libraries which have changed and will continue to do so. Studies of the impact of the internet on information retrieval behaviour of users in medical environments could provide very valuable information for medical libraries trying to adapt to this rapid evolution. This study aims to explore the real information needs of physicians at the hospital and their medical library use. A review was carried out of literature relating to health libraries in Pubmed, LISA, CINAHL, and Google Scholar. An ad hoc questionnaire was sent to the different hospital services. A systematic sample was made. Results of 118 completed questionnaires collected from 34 different services were analysed. Items regarding: medical information needs; change of information seeking behaviour; use of library services and services offered by the hospital librarian such as quality filtering and users training. Most users are familiar with information technologies such as Internet, online journals, webs, etc., but at the same time traditional systems are used. The assistance of professional medical librarians retrieving new and relevant information from databases and the Internet and teaching skills to retrieve and select information is considered necessary.En los últimos años se han producido grandes cambios en la forma de acceder a la información en medicina y esto ha incidido profundamente en las bibliotecas médicas y en el modo de organizar sus servicios. Los bibliotecarios médicos han dedicado un especial interés en tratar de adaptarse al cambiante entorno de la información y satisfacer las demandas de sus usuarios. El presente estudio, aparte de realizar una revisión del estado de la cuestión, pretende averiguar las necesidades reales de información de los médicos de un hospital de más de 1.400 camas. Para ello se realizó una amplia revisión de la literatura a través de las bases de datos Pubmed, LISA, CINAHL y Google Scholar con el fin de conocer la actualidad de los sistemas de acceso a la información en Biomedicina. También se confeccionó un cuestionario ad hoc, con el objetivo de recabar datos concretos sobre necesidades y hábitos actuales de información de los médicos del Hospital. Los cuestionarios se distribuyeron al azar entre los distintos profesionales de la división médica del Hospital. Se obtuvieron 118 cuestionarios completos de 34 servicios diferentes, cuyas preguntas se basaban en los siguientes apartados principales: utilización de los servicios de la biblioteca, tipos de documentos que se utilizan, necesidades y hábitos de consumo de información detectadas, utilización de tecnologías y selección de la información y formación de usuarios. Se concluye que la mayoría de usuarios ha incorporado los actuales sistemas de acceso a la información a la vez que siguen utilizando los tradicionales. Las tecnologías han multiplicado las posibilidades de acceso a la información, pero también han añadido nuevas necesidades a los lectores. La participación del bibliotecario se hace cada vez más activa en sus tareas de selección y organización de la información ya sea en bibliotecas presenciales como en bibliotecas virtuales

    A critical review of challenges and opportunities for the design and operation of offshore structures supporting renewable hydrogen production, storage, and transport

    The climate emergency has prompted rapid and intensive research into sustainable, reliable, and affordable energy alternatives. Offshore wind has developed and exceeded all expectations over the last 2 decades and is now a central pillar of the UK and other international strategies to decarbonise energy systems. As the dependence on variable renewable energy resources increases, so does the importance of the necessity to develop energy storage and nonelectric energy vectors to ensure a resilient whole-energy system, also enabling difficult-to-decarbonise applications, e.g. heavy industry, heat, and certain areas of transport. Offshore wind and marine renewables have enormous potential that can never be completely utilised by the electricity system, and so green hydrogen has become a topic of increasing interest. Although numerous offshore and marine technologies are possible, the most appropriate combinations of power generation, materials and supporting structures, electrolysers, and support infrastructure and equipment depend on a wide range of factors, including the potential to maximise the use of local resources. This paper presents a critical review of contemporary offshore engineering tools and methodologies developed over many years for upstream oil and gas (O&amp;G), maritime, and more recently offshore wind and renewable energy applications and examines how these along with recent developments in modelling and digitalisation might provide a platform to optimise green hydrogen offshore infrastructure. The key drivers and characteristics of future offshore green hydrogen systems are considered, and a SWOT (strength, weakness, opportunity, and threat) analysis is provided to aid the discussion of the challenges and opportunities for the offshore green hydrogen production sector.</p

    Identity management strategies among HIV-positive Colombian gay men in London

    This study set out to explore the social-psychological aspects of living with HIV among a group of HIV-positive Colombian gay men in London, and the strategies that they deployed to manage ensuing threats to their identities. Focus group and individual interview data were collected from 14 Colombian gay men living with HIV, and were analysed using qualitative thematic analysis and identity process theory. The following themes are discussed: (1) identity struggles and conflicts in Colombia, (2), managing multiple layers of social stigma in England, and (3) changing interpersonal and intergroup dynamics, which highlight the inter-connections between sexual prejudice, sexual risk-taking and HIV stigma. Identity may be chronically threatened due to the multiple layers of stigma, which can limit the coping strategies available to individuals. Findings strongly support the need for action and programmes to highlight and tackle both racism and HIV stigma on the gay scene and to fund more specific resources for sub-communities of gay, bisexual and other men who have sex with men, which employ appropriately trained and culturally competent staff

    Immobilization of the white-rot fungus Anthracophyllum discolor to degrade the herbicide atrazine

    Herbicides cause environmental concerns because they are toxic and accumulate in the environment, food products and water supplies. There is a need to develop safe, efficient and economical methods to remove them from the environment, often by biodegradation. Atrazine is such herbicide. White-rot fungi have the ability to degrade herbicides of potential utility. This study formulated a novel pelletized support to immobilize the white-rot fungus Anthracophyllum discolor to improve its capability to degrade the atrazine using a biopurification system (BS). Different proportions of sawdust, starch, corn meal and flaxseed were used to generate three pelletized supports (F1, F2 and F3). In addition, immobilization with coated and uncoated pelletized supports (CPS and UPS, respectively) was assessed. UPS-F1 was determined as the most effective system as it provided high level of manganese peroxidase activity and fungal viability. The half-life (t1/2) of atrazine decreased from 14 to 6 days for the control and inoculated samples respectively. Inoculation with immobilized A. discolor produced an increase in the fungal taxa assessed by DGGE and on phenoloxidase activity determined.     Health-related quality of life (HRQoL) in a population at risk of type 2 diabetes: a cross-sectional study in two Latin American cities

    PURPOSE: The purpose of this study was to describe the health-related quality of life (HRQoL) characteristics in a population at risk of developing type 2 diabetes in Barranquilla and Bogotá, Colombia. METHODS: A cross-sectional study with 1135 participants older than 30 years-of-age recruited in Bogotá D.C., and Barranquilla by cluster sampling in 2018 to 2019. The Finnish Diabetes Risk Score (FINDRISC) was used to detect participants at risk of developing type 2 diabetes (T2D). HRQoL was assessed using the EQ-5D-3L questionnaire. Unadjusted and adjusted logistic regression models were used to calculate odds ratios (OR) and their corresponding 95% confidence intervals CI). RESULTS: Moderate or extreme problems appeared more frequently in the dimensions of Pain/Discomfort (60.8%) and Anxiety/Depression (30.8%). The mean score of the EQ-VAS was 74.3 (± 17.3), significantly larger in the state of complete health (11111) compared with those with problems in more than one of the quality-of-life dimensions. Being female and living in Bogota D.C., were associated with greater odds of reporting problems in the Pain (OR 1.6; 95% CI 1.2-2.2) and Discomfort dimensions (OR 1.6; 95% CI 1.2-2.0) respectively and Anxiety/Depression (OR 1.9; 95% CI 1.3-2.7), (OR 9.1; 95% CI 6.6-12.4), respectively. CONCLUSIONS: As living place and sex were associated with dimensions of Pain/Discomfort and Anxiety/Depression in the HRQoL in people at risk of T2D, greater attention should be paid to these determinants of HRQoL to design and reorient strategies with a territorial and gender perspective to achieve better health outcomes. Diabetes is one of the four non-communicable diseases with increasing prevalence in the world, which has made it a serious public health problem. In Colombia, in 2019 diabetes affected 8.4% of the Colombian adult population and more than one million Colombian adults of this age group have hidden or undetected diabetes. This disease is not only characterized by increased premature mortality, loss of productivity, and economic impact, but it also involves a deterioration in the quality of life of people with diabetes with their respective families. However, very Little is known about health-related quality of life (HRQoL) in a population at risk or with prediabetes. This study has evaluated the quality of life in patients at risk of diabetes and their behavior with some variables as sociodemographic, lifestyle, history, and established their difference in two territories of the Colombian Caribbean. The results of this study indicate that the HRQoL of people at risk of type 2 diabetes is affected by factors such as gender, city, dysglycemia, medication for hypertension and education level. Therefore, greater attention should be paid to these determinants of HRQL to design and implement strategies that reduce this risk of developing type 2 diabetes, prevent prediabetes and improve the quality of life in prediabetic or diabetic patients

    Monitoring the antimicrobial susceptibility of Gramnegative organisms involved in intraabdominal and urinary tract infections recovered during the SMART study (Spain, 2016 and 2017)

    OBJECTIVE: Continuous antimicrobial resistance surveillance is recommended by Public Health authorities. We up-dated data from the SMART (Study for Monitoring Antimicrobial Resistance Trends) surveillance study in Spain. METHODS: The antimicrobial susceptibility data and extended-spectrum beta-lactamase (ESBL) production in isolates recovered from intra-abdominal (IAI) (n=1,429) and urinary tract (UTI) (n=937) infections during the 2016- 2017 SMART study in 10 Spanish hospitals were analysed. RESULTS: Escherichia coli was the most frequently microorganism isolated (48.3% and 53.7%) followed by Klebsiella spp. (11.5% and 21.9%) in IAIs and UTIs, respectively. Figures for Pseudomonas aeruginosa were 9.0% and 6.1%, being more frequently recovered from patients with nosocomial infections. Overall, 9.9% (IAI) and 14.0% (UTI) of E. coli, Klebsiella spp. and Proteus mirabilis isolates were ESBL-producers, being Klebsiella pneumoniae (34.5%) from UTI of nosocomial origin the most frequent. ESBL-producers were higher in patients >60 years in both IAIs and UTIs. As in previous years, amikacin (96.3%-100% susceptibility), ertapenem (84.2%-100%) and imipenem (70.3%- 100%) were the most active antimicrobials tested among Enterobacterales species. The activity of amoxicillin-clavulanic, piperacillin-tazobactam, and ciprofloxacin susceptibility was lower, particularly among ESBL-producers. Ertapenem susceptibility (88.9%-100%) was retained in ESBL-E. coli isolates that were resistant to these antimicrobials but decreased (28.6%-100%) in similar isolates of K. pneumoniae. CONCLUSIONS: Continuous antimicrobial resistance surveillance from the SMART study reveals overall maintenance of ESBL-producers in Spain, although with higher presence in isolates from UTIs than from IAIs. Moreover, ertapenem activity was high in E. coli irrespective of ESBL production but decreased in K. pneumoniae, particularly among ESBL-producers
