The rate and cost of nurse turnover in Australia
© 2014 Australian College of Nursing Ltd. Nurse turnover is a critical issue facing workforce planners across the globe, particularly in light of protracted and continuing workforce shortages. An ageing population coupled with the rise in complex and chronic diseases, have contributed to increased demands placed on the health system and importantly, nurses who themselves are ageing. Costs associated with nurse turnover are attracting more attention; however, existing measurements of turnover show inconsistent findings, which can be attributed to differences in study design, metrics used to calculate turnover and variations in definitions for turnover. This paper will report the rates and costs of nurse turnover across three States in Australia
Instability in patient and nurse characteristics, unit complexity and patient and system outcomes
© 2014 John Wiley & Sons Ltd. Aims: To explore key factors related to nursing unit instability, complexity and patient and system outcomes. Background: The relationship between nurse staffing and quality of patient outcomes is well known. The nursing unit is an important but different aspect that links to complexity and to system and patient outcomes. The relationship between the instability, complexity and outcomes needs further exploration. Design: Descriptive. Methods: Data were collected via a nurse survey, unit profile and review of patient records on 62 nursing units (wards) across three states of Australia between 2008-2010. Two units with contrasting levels of patient and nurse instability and negative system and patient outcomes, were profiled in detail from the larger sample. Results: Ward A presented with greater patient stability (low occupancy, high planned admissions, few ICU transfers, fewer changes to patient acuity/work re-sequencing) and greater nurse instability (nurses changing units, fewer full-time staff, more temporary/casual staff) impacting system outcomes negatively (high staff turnover). In contrast, Ward B had greater patient instability, however, more nurse stability (greater experienced and permanent staff, fewer casuals), resulting in high rates for falls, medication errors and other adverse patient outcomes with lower rates for system outcomes (lower intention to leave). Conclusion: Instability in patient and nurse factors can contribute to ward complexity with potentially negative patient outcomes. The findings highlight the variation of many aspects of the system where nurses work and the importance of nursing unit managers and senior nurse executives in managing ward complexity
ciliaFA : a research tool for automated, high-throughput measurement of ciliary beat frequency using freely available software
Background: Analysis of ciliary function for assessment of patients suspected of primary ciliary dyskinesia (PCD) and
for research studies of respiratory and ependymal cilia requires assessment of both ciliary beat pattern and beat
frequency. While direct measurement of beat frequency from high-speed video recordings is the most accurate and
reproducible technique it is extremely time consuming. The aim of this study was to develop a freely available
automated method of ciliary beat frequency analysis from digital video (AVI) files that runs on open-source software
(ImageJ) coupled to Microsoft Excel, and to validate this by comparison to the direct measuring high-speed video
recordings of respiratory and ependymal cilia. These models allowed comparison to cilia beating between 3 and 52 Hz.
Methods: Digital video files of motile ciliated ependymal (frequency range 34 to 52 Hz) and respiratory epithelial cells
(frequency 3 to 18 Hz) were captured using a high-speed digital video recorder. To cover the range above between 18
and 37 Hz the frequency of ependymal cilia were slowed by the addition of the pneumococcal toxin pneumolysin.
Measurements made directly by timing a given number of individual ciliary beat cycles were compared with those
obtained using the automated ciliaFA system.
Results: The overall mean difference (± SD) between the ciliaFA and direct measurement high-speed digital imaging
methods was −0.05 ± 1.25 Hz, the correlation coefficient was shown to be 0.991 and the Bland-Altman limits of
agreement were from −1.99 to 1.49 Hz for respiratory and from −2.55 to 3.25 Hz for ependymal cilia.
Conclusions: A plugin for ImageJ was developed that extracts pixel intensities and performs fast Fourier
transformation (FFT) using Microsoft Excel. The ciliaFA software allowed automated, high throughput measurement of
respiratory and ependymal ciliary beat frequency (range 3 to 52 Hz) and avoids operator error due to selection bias. We
have included free access to the ciliaFA plugin and installation instructions in Additional file 1 accompanying this
manuscript that other researchers may use

We uplift 5-dimensional super-Yang-Mills theory to a 6-dimensional gauge
theory with the help of a space-like constant vector , whose norm
determines the Yang-Mills coupling constant. After the localization of
the 6D gauge theory acquires Lorentzian invariance as well as scale invariance.
We discuss KK states, instantons and the flux quantization. The 6D theory

Unexpected features of branched flow through high-mobility two-dimensional electron gases
GaAs-based two-dimensional electron gases (2DEGs) show a wealth of remarkable
electronic states, and serve as the basis for fast transistors, research on
electrons in nanostructures, and prototypes of quantum-computing schemes. All
these uses depend on the extremely low levels of disorder in GaAs 2DEGs, with
low-temperature mean free paths ranging from microns to hundreds of microns.
Here we study how disorder affects the spatial structure of electron transport
by imaging electron flow in three different GaAs/AlGaAs 2DEGs, whose mobilities
range over an order of magnitude. As expected, electrons flow along narrow
branches that we find remain straight over a distance roughly proportional to
the mean free path. We also observe two unanticipated phenomena in
high-mobility samples. In our highest-mobility sample we observe an almost
complete absence of sharp impurity or defect scattering, indicated by the
complete suppression of quantum coherent interference fringes. Also, branched
flow through the chaotic potential of a high-mobility sample remains stable to

Inspiratory muscle training reduces blood lactate concentration during volitional hyperpnoea
Although reduced blood lactate concentrations ([lac−]B) have been observed during whole-body exercise following inspiratory muscle training (IMT), it remains unknown whether the inspiratory muscles are the source of at least part of this reduction. To investigate this, we tested the hypothesis that IMT would attenuate the increase in [lac−]B caused by mimicking, at rest, the breathing pattern observed during high-intensity exercise. Twenty-two physically active males were matched for 85% maximal exercise minute ventilation (V˙Emax) and divided equally into an IMT or a control group. Delayed gastric emptying and reduced postprandial small bowel water content of equicaloric whole meal bread versus rice meals in healthy subjects: novel MRI insights
BACKGROUND/OBJECTIVES: Postprandial bloating is a common symptom in patients with functional gastrointestinal (GI) diseases. Whole meal bread (WMB) often aggravates such symptoms though the mechanisms are unclear. We used magnetic resonance imaging (MRI) to monitor the intragastric fate of a WMB meal (11% bran) compared to a rice pudding (RP) meal.
SUBJECTS/METHODS: 12 healthy volunteers completed this randomised crossover study. They fasted overnight and after an initial MRI scan consumed a glass of orange juice with a 2267 kJ WMB or an equicaloric RP meal. Subjects underwent serial MRI scans every 45 min up to 270 min to assess gastric volumes and small bowel water content and completed a GI symptom questionnaire.
RESULTS: The MRI intragastric appearance of the two meals was markedly different. The WMB meal formed a homogeneous dark bolus with brighter liquid signal surrounding it. The RP meal separated into an upper, liquid layer and a lower particulate layer allowing more rapid emptying of the liquid compared to solid phase (sieving). The WMB meal had longer gastric half emptying times (132±8 min) compared to the RP meal (104±7 min), P<0.008. The WMB meal was associated with markedly reduced MRI-visible small bowel free mobile water content compared to the RP meal, P<0.0001.
CONCLUSIONS: WMB bread forms a homogeneous bolus in the stomach which inhibits gastric sieving and hence empties slower than the equicaloric rice meal. These properties may explain why wheat causes postprandial bloating and could be exploited to design foods which prolong satiation
The role of regulatory B cells on hepatocellular carcinoma progression
Poster PresentationCongress Theme: Translating Discoveries into Prevention and CuresPURPOSE: Regulatory B cells (Bregs) play important roles in autoimmune diseases, but their function in hepatocellular carcinoma (HCC) progression remains unclear. This study attempted to unveil the role of Bregs on HCC progression. EXPERIMENTAL DESIGN: This study examined the distribution of intrahepatic B cells and circulating Bregs population at the level of phenotypes as well as functionality in HCC patients. The mechanisms of Bregs regulating liver tumor cells were further explored in a series of in vitro and in vivo functional studies. RESULTS: The percentage of B cells at tumor margin region was significantly higher than that in tumor or non-tumor region. Increased intrahepatic B cells at tumor margin were positively associated with tumor invasive features and more tumor recurrence. Besides, HCC patients had a significant higher percentage of circulating Bregs than healthy people. Increased circulating Bregs were positively correlated with advanced tumor staging, tumor multiplicity and venous infiltration. Next, our in vivo study firstly revealed that human Bregs promoted HCC tumor growth independent of Tregs in SCID mice. The migration of Bregs into tumor in mice was further confirmed by in vivo imaging and histology. Finally, the molecular mechanism of Bregs promoted proliferation and migration of HCC cells was proved by direct cell-cell interaction via CD40/CD154 signaling in vitro. Coculture of Bregs and HCC cells induced CD40/CD154-dependent cytokines secretion. CONCLUSION: Human Bregs promoted HCC growth and invasiveness by interacting with HCC tumor cells through CD40/CD154 signaling pathway. Bregs might be both a prognostic marker and a therapeutic target for HCC.published_or_final_versio
