A Schumpeterian Growth Model with Heterogenous Firms
A common assumption in the Schumpeterian growth literature is that the innovation size is constant and identical across industries. This is in contrast with the empirical evidence which shows that: (i) the innovation size is far from being identical across industries; and (ii) the size distribution of profit returns from innovation is highly skewed toward the low value side, with a long tail on the high value side. In the present paper, we develop a Schumpeterian growth model that is consistent with this evidence. In particular, we assume that when a firm innovates, the size of its quality improvement is the result of a random draw from a Pareto distribution. This enables us to extend the class of quality-ladder growth models to encompass firm heterogeneity. We study the policy implications of this new set-up numerically and find that it is optimal to heavily subsidize R&D for plausible parameter values. Although it is optimal to tax R&D for some parameter values, this case only occurs when the steady-state rate of economic growth is very low.Schumpeterian Growth, R&D, optimal policy
Contemporary surgical practice in the management of anal fistula: results from an international survey
Background Management of anal fistula (AF) remains challenging with many controversies. The purpose of this study was to explore current surgical practice in the management of AF with a focus on technical variations among surgeons. Methods An online survey was conducted by inviting all surgeons and physicians on the membership directory of European Society of Coloproctology and American Society of Colon and Rectal Surgeons. An invitation was extended to others via social media. The survey had 74 questions exploring diagnostic and surgical techniques. Results In March 2018, 3572 physicians on membership directory were invited to take part in the study 510 of whom (14%) responded to the survey. Of these respondents, 492 (96%) were surgeons. Respondents were mostly colorectal surgeons (84%) at consultant level (84%), ageââ„â40 years (64%), practicing in academic (53%) or teaching (30%) hospitals, from the USA (36%) and Europe (34%). About 80% considered fistulotomy as the gold standard treatment for simple fistulas. Endorectal advancement flap was performed using partial- (42%) or full-thickness (44%) flaps. Up to 38% of surgeons performed ligation of the intersphincteric fistula tract (LIFT) sometimes with technical variations. Geographic and demographic differences were found in both the diagnostic and therapeutic approaches to AF. Declared rates of recurrence and fecal incontinence with these techniques were variable and did not correlate with surgeonsâ experience. Only 1â4% of surgeons were confident in performing the most novel sphincter-preserving techniques in patients with Crohnâs disease. Conclusions Profound technical variations exist in surgical management of AF, making it difficult to reproduce and compare treatment outcomes among different centers
Hellenic karst waters: geogenic and anthropogenic processes affecting their geochemistry and quality
Karst hydrosystems represent one of the largest global drinking water resources, but they are
extremely vulnerable to pollution. Climate change, high population density, intensive industrial, and
agricultural activities are the principal causes of deterioration, both in terms of quality and quantity,
of these resources. Samples from 172 natural karst springs were collected in the whole territory of
Greece. To identify any geogenic contamination and/or anthropogenic pollution, analyses of their
chemical compositions, in terms of major ions and trace elements, were performed and compared to
the EU limits for drinking water. Based on chloride content, the collected karst springs were divided
into two groups: low-chloride (< 100 mg Lâ
1) and high-chloride content (> 100 mg Lâ
1). An additional
group of springs with calcium-sulfate composition was recognised. Nitrate concentrations were
always below the EU limit (50 mg Lâ
1), although some springs presented elevated concentrations.
High contents in terms of trace elements, such as B, Sr, As, and Pb, sometimes exceeding the limits,
were rarely found. The Greek karst waters can still be considered a good quality resource both for
human consumption and for agriculture. The main issues derive from seawater intrusion in the
aquifers along the coasts. Moreover, the main anthropogenic pollutant is nitrate, found in higher
concentrations mostly in the same coastal areas where human activities are concentrated. Finally,
high levels of potentially harmful trace elements (e.g. As, Se) are very limited and of natural origin
(geothermal activity, ore deposits, etc.)
Zero-Emission Regional Aviation in Sweden
Presented at the 33rd ICAS Congress, Sweden (2022).Regional air operations, which can be defined as the transportation of passengers using smaller aircraft over short distances, have been overlooked in recent years by airlines focusing on high volume and profitable routes between large airports. Despite this shift of focus, the airport infrastructure still exists in many smaller communities between which demand for air travel exists. The emergence of new air vehicles designed for shorter routes could stimulate efficient and profitable operations, especially if they leverage currently underutilized and paid-for airports. However, new regional air operations need to be sustainable to be successful in a world striving for a carbon-neutral future, especially since air travel over short distances can be substituted by other means of transportation with a smaller environmental footprint such as cars, trains, or buses. Many different paths are envisioned to reach zero-emission goals. These range from technology advancements to new powertrain configurations, and from new transportation policies to new emission offsetting schemes. It is however not clear how these different paths interact and how solutions could be optimally combined. Analyses are therefore required to estimate future demand for air travel and to assess the feasibility of zero-emission regional aviation with the objective to support decision-making about viable and sustainable paths for new regional air operations. The developed modeling environment is implemented in Sweden and allows for an environmental assessment of various scenarios. Significant untapped demand is uncovered between smaller markets, and given fuel and energy consumption for these operations, it is likely that sustainable advanced regional air mobility will be possible in Sweden provided technology transitions can be made

Pro-Coagulant Endothelial Dysfunction Results from EHEC Shiga Toxins and Host Damage-Associated Molecular Patterns
Hemolytic uremic syndrome (HUS) from enterohemorrhagic Escherichia coli infection is a leading cause of kidney failure in otherwise healthy U.S. children. The bacterial Shiga toxins (Stx) induce the characteristic coagulopathy of HUS, but the damage to toxin-receptor expressing cells and organ injury due to ischemia likely also releases inflammatory damage-associated molecular patterns (DAMPs), which may exacerbate injury along with the toxins. To examine this, human aortic and renal glomerular cell anti-coagulant and barrier functions were studied after in vitro challenge with Stx1, Stx2, and DAMPs. There was significant loss of surface anti-coagulant protein C pathway molecules, increased expression of pro-thrombotic PAR1 and reduced protein C activation capability by 15â27%. Histones nearly completely prevented the activated protein C protection of endothelial cells from thrombin-induced permeability. In mice, lethal Stx2 challenge elevated plasma HMGB1 (day 2, 321 ± 118%; p < 0.01) and extracellular histones (day 3, 158 ± 62%; p < 0.01). Mice colonized with Stx2-expressing Citrobacter rodentium developed increased HMGB1 (day 5, 155 ± 55%; p < 0.01) and histones (day 3, 378 ± 188%; p < 0.01). Anti-histone antibody reduced both DAMPs to baseline, but was not sufficient to improve survival outcome or kidney function. Together, these data suggest a potential role Stx to produce DAMPs, and DAMPs to produce endothelial injury and a pro-thrombotic environment

A cross-sectional exploratory analysis between pet ownership and sleep, exercise, health and neighbourhood perceptions: The Whitehall II cohort study

Background: To explore associations between pets, and specifically dog ownership and sleep, health, exercise and neighbourhood.
Methods: Cross sectional examination of 6575 participants of the Whitehall II study aged between 59-79 years. We used self-assessed measurement scales of the Short Form (SF36), General Health Questionnaire (GHQ), Control, Autonomy, Self-realisation and Pleasure (CASP), Centre for Epidemiologic Studies Depression Scale (CES-D), sleep, exercise, and perceptions of local neighbourhood. In addition the Mini Mental State Examination which is administered to test global cognitive status (MMSE)
Results: We found 2/7 people owned a pet and of those 64% were âveryâ attached to their pet. Mild exercise in metabolic equivalents (MET-hours) was significantly higher in pet owners than non-owners (median 27.8 (IQR 18.1 to 41.8) vs 25.7 (IQR 16.8 to 38.7), p=0.0001), and in dog owners than other pets (median 32.3 (IQR 20.8 to 46.1) vs 25.6 (IQR 16.8 to 38.5), p<0.0001). Moderate exercise was also significantly higher in pet owners than non pet owners (median 11.8 (IQR 4.2 to 21.9) vs 9.8 (IQR 2.8 to 19.5), p<0.0001), and dog owners than owners of other pets (median 12.3 (IQR 4.2 to 22.2) vs 10.1 (3.1 to 20.0), p=0.0002) but there were no significant differences with vigorous exercise. We found that pet owners were significantly more positive about their neighbourhood than non-owners on 8/9 questions, while dog owners were (significantly) even more positive than owners of other pets on 8/9 questions. Associations with sleep were mixed, although dog owners had less trouble falling asleep than non-dog owners, with borderline statistical significance.
Conclusion: Dog owners feel more positive about their neighbourhood, do more exercise, and fall asleep more easily than non-dog owners. These results suggest that dog owners could be more likely to exercise by walking their dogs and therefore may be more familiar and positive about the area in which they walk their dog
