    The multiplicity fraction in 202 open clusters from Gaia

    In this study, we estimate the fraction of binaries with high mass ratios for 202 open clusters in the extended solar neighbourhood (closer than 1.5 kpc from the Sun). This is one of the largest homogeneous catalogues of multiplicity fractions in open clusters to date, including the unresolved and total (close-binary) multiplicity fractions of main-sequence systems with mass ratio larger than 0.6−0.15+0.050.6_{-0.15}^{+0.05}. The unresolved multiplicity fractions are estimated applying a flexible mixture model to the observed Gaia colour-magnitude diagrams of the open clusters. Then we use custom Gaia simulations to account for the resolved systems and derive the total multiplicity fractions. The studied open clusters have ages between 6.6 Myr and 3.0 Gyr and total high-mass-ratio multiplicity fractions between 6% and 80%, with a median of 18%. The multiplicity fractions increase with the mass of the primary star, as expected. The average multiplicity fraction per cluster displays an overall decreasing trend with the open cluster age up to ages about 100 Myr, above which the trend increases. Our simulations show that most of this trend is caused by complex selection effects (introduced by the mass dependence of the multiplicity fraction and the magnitude limit of our sample). Furthermore, the multiplicity fraction is not significantly correlated with the clusters' position in the Galaxy. The spread in multiplicity fraction decreases significantly with the number of cluster members (used as a proxy for cluster mass).     Long-term results of a clinical trial of nadolol with or without isosorbide mononitrate for primary prophylaxis of variceal bleeding in cirrhosis

    It is clearly established that beta-blockers decrease the risk of a first variceal bleeding in cirrhosis. We have recently shown that the addition of isosorbide mononitrate to nadolol decreases the rate of variceal bleeding in patients with cirrhosis and varices, compared with nadolol alone, after a median follow-up of 30 months. It is not established if the long-term treatment with the combination continues to be beneficial. Therefore, we assessed the long-term effect of this combination on first variceal bleeding, complications, and death. One hundred forty-six cirrhotic patients with esophageal varices included in a previously published multicenter, randomized study comparing nadolol (40-160 mg/d) with the combination nadolol plus isosorbide mononitrate (10-20 mg 3 times per day) were followed up for up to 7 years (median follow-up, 55 months). The primary end-point was variceal bleeding of any severity. Twenty-four patients (16 in the nadolol group, and 8 in the combination group) experienced variceal bleeding (log rank test, P =.02). Cumulative risk of bleeding was 29% and 12%, respectively (95% CI for the difference, 1%-23%). Two and 4 patients, respectively, had bleeding from portal hypertensive gastropathy (log rank test, P =.20). Thirty and 25 patients, respectively, died during follow-up (log rank test, P =.13). Twelve and 10 patients, respectively, had de novo occurrence of ascites during follow-up (log rank test, P =.29). In conclusion, nadolol plus isosorbide mononitrate is significantly more effective than nadolol alone in the long-term use. Side effects are few, and no deleterious effects on ascites occurrence or on survival occur after long-term use of this combination

    Business Model Design: Lessons Learned from Tesla Motors

    International audienceElectric vehicle (EV) industry is still in the introduction stage in product life cycle, and dominant design remains unclear. EV companies, both incumbent from the car industry and new comers, have long taken numerous endeavors to promote EV in the niche market by providing innovative products and business models. While most carmakers still take 'business as usual' approach for developing their EV production and offers, Tesla Motors, an EV entrepreneurial firm, stands out by providing disruptive innovation solutions. We review the business model approach in the literature, then classify the innovation dimensions in the EV ecosystem. We study Tesla Motors in terms of: (i) innovation related to the vehicle, (ii) innovation related to the battery (iii) innovation concerning the recharging system, and (iv) innovation toward the EV ecosystem. Lessons for incumbent carmakers for their EV business model design: Tesla Motors 1) holds a product strategy entering from high-end market and moving to mass market, with a high level of innovation adaptation and learning by doing; 2) pays considerable attention to reduce range anxiety by high performance supercharger station network and high capacity battery; 3) shows a very high level of integration of information technology into many aspects of the EV business model, such as advanced in-car services and digital distribute channel; 4) shows a new value configuration which involving in high level of vertical integration towards battery and recharging network. All these lessons of this chapter would be worth the attention of the carmakers if the disruptive choices of Tesla succeed in challenging the dominant design
