    The early mortality from colorectal tumours 1969 - 1972

    In order to compare the mortality from colorectal tumours in Malta with that in other centres, the survival of patients with large bowel tumours from the cases recorded in the Tumour Registry between the years 1969 to 1972 was studied. It was only possible to follow up the patients till July 1973. The deaths occurring up to this period will be discussed in relation to the time in which they occurred and to their causes. It is immediately apparent that our patients are at an advanced stage of malignancy at the time of operation. Aggressive surgery in these groups of individuals is not only futile but also contributes to an earlier demise. This is only too obvious when two thirds of the deaths recorded in the survey occurred within six months of surgical treatment. The crux of the whole problem appears to be the delay between the commencement of symptoms and the surgical treatment. On this account the patients themselves may be partly to blame, minor symptoms are often ignored and not brought up for medical advice.peer-reviewe

    The incidence of colorectal tumours in the Maltese islands

    Colorectal neoplasm is one of the commoner forms of malignant disease; according to the Annual Report Registrar General, 1970 its incidence in England and Wales is second only to that of lung cancer. A survey of the hundred and seventy three patients documented in the Malta Tumour Registry as suffering from colorectal neoplasms between the years 1969-1972 (inclusive) was carried out. Several findings emanate from this survey such as the incidence in the younger age group, the presence of significant nausea and vomiting in fifty percent of the patients of the left hemicolon and twenty percent of cases with a tumour in the right hemicolon. Moreover, this survey has shown that there is an average delay of about six months from the time the patient first presents to his own practitioner until his hospitalization for investigation with yet further delays before surgery is attempted.peer-reviewe

    The virtual human face – superimposing the simultaneously captured 3D photorealistic skin surface of the face on the untextured skin image of the CBCT Scan

    The aim of this study was to evaluate the impact of simultaneous capture of the three-dimensional (3D) surface of the face and cone beam computed tomography (CBCT) scan of the skull on the accuracy of their registration and superimposition. 3D facial images were acquired in 14 patients using the Di3d (Dimensional Imaging, UK) imaging system and i-CAT CBCT scanner. One stereophotogrammetry image was captured at the same time as the CBCT and another one hour later. The two stereophotographs were then individually superimposed over the CBCT using VRmesh. Seven patches were isolated on the final merged surfaces. For the whole face and each individual patch; maximum and minimum range of deviation between surfaces, absolute average distance between surfaces, and standard deviation for the 90th percentile of the distance errors were calculated. The superimposition errors of the whole face for both captures revealed statistically significant differences (P=0.00081). The absolute average distances in both separate and simultaneous captures were 0.47mm and 0.27mm, respectively. The level of superimposition accuracy in patches from separate captures ranged between 0.3 and 0.9mm, while that of simultaneous captures was 0.4mm. Simultaneous capture of Di3d and CBCT images significantly improved the accuracy of superimposition of these image modalities

    SALT observations of the Chromospheric Activity of Transiting Planet Hosts: Mass Loss and Star Planet Interactions

    We measured the chromospheric activity of the four hot Jupiter hosts WASP-43, WASP-51/HAT-P-30, WASP-72 & WASP-103 to search for anomalous values caused by the close-in companions. The Mount Wilson Ca II H&K S-index was calculated for each star using observations taken with the Robert Stobie Spectrograph at the Southern African Large Telescope. The activity level of WASP-43 is anomalously high relative to its age and falls among the highest values of all known main sequence stars. We found marginal evidence that the activity of WASP-103 is also higher than expected from the system age. We suggest that for WASP-43 and WASP-103 star-planet interactions (SPI) may enhance the Ca II H&K core emission. The activity levels of WASP-51/HAT-P-30 and WASP-72 are anomalously low, with the latter falling below the basal envelope for both main sequence and evolved stars. This can be attributed to circumstellar absorption due to planetary mass loss, though absorption in the ISM may contribute. A quarter of known short period planet hosts exhibit anomalously low activity levels, including systems with hot Jupiters and low mass companions. Since SPI can elevate and absorption can suppress the observed chromospheric activity of stars with close-in planets, their Ca II H&K activity levels are an unreliable age indicator. Systems where the activity is depressed by absorption from planetary mass loss are key targets for examining planet compositions through transmission spectroscopy

However, the authors feel that there are distinct benefits to serving within the Army, and that it provides effective occupational medical, MH and welfare support. This research study explores potential benefits and stressors of being in the Army and provides an overview of Army mental health services (AMHS) through the perspectives of AMHS personnel, 84% of which were nurses. The study indicated that the Army can provide a protective community, sharing a bond based on common values and experiences. The Army can provide soldiers with career opportunities that are not available in civilian life, and there are opportunities to develop an employment profile, enhanced by internal and external educational training, and encapsulated within a progressive career pathway. The Army can also be seen to offer an escape route, preventing soldiers entering a life of crime, and supplying the stable family these soldiers had never experienced. The provision of leadership, within an environment where soldiers are valued and stigma is not tolerated can potentially shield against MH problems

    Intestinal transplantation in composite visceral grafts or alone

    Under FK 506-based immunosuppression, the entire cadaver small bowel except for a few proximal and distal centimeters was translated to 17 randomly matched patients, of whom two had antigraft cytotoxic antibodies (positive cross-match). Eight patients received the intestine only, eight had intestine in continuity with the liver, and one received a full multivisceral graft that included the liver, stomach, and pancreas. One liver-intestine recipient died after an intestinal anastomotic leak, sepsis, and graft- versus-host disease. The other 16 patients are alive after 1 to 23 months, in one case after chronic rejection, graft removal, and retransplantation. Twelve of the patients have been liberated from total parenteral nutrition, including all whose transplantation was 2 months or longer ago. The grafts have supported good nutrition, and in children, have allowed growth and weight gain. Management of these patients has been difficult and often complicated, but the end result has been satisfactory in most cases, justifying further clinical trials. The convalescence of the eight patients receiving intestine only has been faster and more trouble free than after liver-intestine or multivisceral transplantation, with no greater difficulty in the control of rejection

    Intra-Organ Variation in Age-Related Mutation Accumulation in the Mouse

    Using a transgenic mouse model harboring chromosomally integrated lacZ mutational target genes, we previously demonstrated that mutations accumulate with age much more rapidly in the small intestine than in the brain. Here it is shown that in the small intestine point mutations preferentially accumulate in epithelial cells of the mucosa scraped off the underlying serosa. The mucosal cells are the differentiated villus cells that have undergone multiple cell divisions. A smaller age-related increase, also involving genome rearrangements, was observed in the serosa, which consists mainly of the remaining crypts and non-dividing smooth muscle cells. In the brain we observed an accumulation of only point mutations in no other areas than hypothalamus and hippocampus. To directly test for cell division as the determining factor in the generation of point mutations we compared mutation induction between mitotically active and quiescent embryonic fibroblasts from the same lacZ mice, treated with either UV (a point mutagen) or hydrogen peroxide (a clastogen). The results indicate that while point mutations are highly replication-dependent, genome rearrangements are as easily induced in non-dividing cells as in mitotically active ones. This strongly suggests that the point mutations found to have accumulated in the mucosal part of the small intestine are the consequence of replication errors. The same is likely true for point mutations accumulating in hippocampus and hypothalamus of the brain since neurogenesis in these two areas continues throughout life. The observed intra-organ variation in mutation susceptibility as well as the variation in replication dependency of different types of mutations indicates the need to not only extend observations made on whole organs to their sub-structures but also take the type of mutations and mitotic activity of the cells into consideration. This should help elucidating the impact of genome instability and its consequences on aging and disease

    The doubly eclipsing quintuple low-mass star system 1SWASP J093010.78+533859.5

    Our discovery of 1SWASP J093010.78+533859.5 as a probable doubly eclipsing quadruple system, containing a contact binary with P ~ 0.23 d and a detached binary with P ~ 1.31 d, was announced in 2013. Subsequently, Koo and collaborators confirmed the detached binary spectroscopically, and identified a fifth set of static spectral lines at its location, corresponding to an additional non-eclipsing component of the system. Here we present new spectroscopic and photometric observations, allowing confirmation of the contact binary and improved modelling of all four eclipsing components. The detached binary is found to contain components of masses 0.837 ± 0.008 and 0.674 ± 0.007M⊙, with radii of 0.832 ± 0.018 and 0.669 ± 0.018R⊙ and effective temperatures of 5185+25-20 and 4325+20-15 K, respectively; the contact system has masses 0.86 ± 0.02 and 0.341 ± 0.011M⊙ , radii of 0.79 ± 0.04 and 0.52 ± 0.05R⊙, respectively, and a common effective temperature of 4700 ± 50 K. The fifth star is of similar temperature and spectral type to the primaries in the two binaries. Long-term photometric observations indicate the presence of a spot on one component of the detached binary, moving at an apparent rate of approximately one rotation every two years. Both binaries have consistent system velocities around −11 to −12 km s-1, which match the average radial velocity of the fifth star; consistent distance estimates for both subsystems of d = 78 ± 3 and d = 73 ± 4 pc are also found, and, with some further assumptions, of d = 83 ± 9 pc for the fifth star. These findings strongly support the claim that both binaries – and very probably all five stars – are gravitationally bound in a single system. The consistent angles of inclination found for the two binaries (88.2 ± 0.3°and 86 ± 4°) may also indicate that they originally formed by fragmentation (around 9–10 Gyr ago) from a single protostellar disk, and subsequently remained in the same orbital plane
