Carboxyhaemoglobin levels and their determinants in older British men
Background: Although there has been concern about the levels of carbon monoxide exposure, particularly among older people, little is known about COHb levels and their determinants in the general population. We examined these issues in a study of older British men.Methods: Cross-sectional study of 4252 men aged 60-79 years selected from one socially representative general practice in each of 24 British towns and who attended for examination between 1998 and 2000. Blood samples were measured for COHb and information on social, household and individual factors assessed by questionnaire. Analyses were based on 3603 men measured in or close to (< 10 miles) their place of residence.Results: The COHb distribution was positively skewed. Geometric mean COHb level was 0.46% and the median 0.50%; 9.2% of men had a COHb level of 2.5% or more and 0.1% of subjects had a level of 7.5% or more. Factors which were independently related to mean COHb level included season (highest in autumn and winter), region (highest in Northern England), gas cooking (slight increase) and central heating (slight decrease) and active smoking, the strongest determinant. Mean COHb levels were more than ten times greater in men smoking more than 20 cigarettes a day (3.29%) compared with non-smokers (0.32%); almost all subjects with COHb levels of 2.5% and above were smokers (93%). Pipe and cigar smoking was associated with more modest increases in COHb level. Passive cigarette smoking exposure had no independent association with COHb after adjustment for other factors. Active smoking accounted for 41% of variance in COHb level and all factors together for 47%.Conclusion: An appreciable proportion of men have COHb levels of 2.5% or more at which symptomatic effects may occur, though very high levels are uncommon. The results confirm that smoking (particularly cigarette smoking) is the dominant influence on COHb levels
Starting to Stop: Young Offenders' Desistance from Crime
This paper explores the complexities of the interplay between structural and agentic changes in 21 young offenders' lives as they start to stop offending. The young people's ability to desist from crime was dependent upon their engagement with a 'hook for change', their development of prosocial relationships and 'knifing off' of elements of their offending past, the extent of their identity change, and their confidence about desistance. Desistance was less likely in the absence of a 'hook' and where offenders were running a 'condemnation script'. The study challenges previous research that argues that desistance from crime in adolescence is unlikely

Observation of an Efimov spectrum in an atomic system
In 1970 V. Efimov predicted a puzzling quantum-mechanical effect that is
still of great interest today. He found that three particles subjected to a
resonant pairwise interaction can join into an infinite number of loosely bound
states even though each particle pair cannot bind. Interestingly, the
properties of these aggregates, such as the peculiar geometric scaling of their
energy spectrum, are universal, i.e. independent of the microscopic details of
their components. Despite an extensive search in many different physical
systems, including atoms, molecules and nuclei, the characteristic spectrum of
Efimov trimer states still eludes observation. Here we report on the discovery
of two bound trimer states of potassium atoms very close to the Efimov
scenario, which we reveal by studying three-particle collisions in an ultracold
gas. Our observation provides the first evidence of an Efimov spectrum and
allows a direct test of its scaling behaviour, shedding new light onto the

'Keeping busy' as agency in early desistance
Agency in desistance research has often been understood as deliberate action
undertaken in pursuit of a desisting identity. Through a micro-longitudinal
approach, this research focuses on the early desistance experiences of a
number of mainly white British female participants. Agency was exhibited not
with a new identity in mind, but instead through 'keeping busy'. The surprising
lack of identity concerns may be due to the early stages of the participants'
desistance experiences, with new identities emerging later in the process.
Alternatively, it may indicate a fundamental difference to the classic desistance
narrative, linked to the differences between this sample and the frequently
researched, Western, male, high-frequency offender. Finally, important aspects
of the cultures surrounding desistance research may have shaped the
narratives of desisters and the biases of researchers towards finding a concern
for identity when this is not necessarily experienced in the everyday lives of
desisters
An exploration of parentsβ preferences for foot care in juvenile idiopathic arthritis: a possible role for the discrete choice experiment
Background:
An increased awareness of patientsβ and parentsβ care preferences regarding foot care is desirable from a clinical perspective as such information may be utilised to optimise care delivery. The aim of this study was to examine parentsβ preferences for, and valuations of foot care and foot-related outcomes in juvenile idiopathic arthritis (JIA).<p></p>
Methods:
A discrete choice experiment (DCE) incorporating willingness-to-pay (WTP) questions was conducted by surveying 42 parents of children with JIA who were enrolled in a randomised-controlled trial of multidisciplinary foot care at a single UK paediatric rheumatology outpatients department. Attributes explored were: levels of pain; mobility; ability to perform activities of daily living (ADL); waiting time; referral route; and footwear. The DCE was administered at trial baseline. DCE data were analysed using a multinomial-logit-regression model to estimate preferences and relative importance of attributes of foot care. A stated-preference WTP question was presented to estimate parentsβ monetary valuation of health and service improvements.<p></p>
Results:
Every attribute in the DCE was statistically significant (pβ<β0.01) except that of cost (pβ=β0.118), suggesting that all attributes, except cost, have an impact on parentsβ preferences for foot care for their child. The magnitudes of the coefficients indicate that the strength of preference for each attribute was (in descending order): improved ability to perform ADL, reductions in foot pain, improved mobility, improved ability to wear desired footwear, multidisciplinary foot care route, and reduced waiting time. Parentsβ estimated mean annual WTP for a multidisciplinary foot care service was Β£1,119.05.<p></p>
Conclusions:
In terms of foot care service provision for children with JIA, parents appear to prefer improvements in health outcomes over non-health outcomes and service process attributes. Cost was relatively less important than other attributes suggesting that it does not appear to impact on parentsβ preferences.<p></p>
Multisensory information facilitates reaction speed by enlarging activity difference between superior colliculus hemispheres in rats
Animals can make faster behavioral responses to multisensory stimuli than to unisensory stimuli. The superior colliculus (SC), which receives multiple inputs from different sensory modalities, is considered to be involved in the initiation of motor responses. However, the mechanism by which multisensory information facilitates motor responses is not yet understood. Here, we demonstrate that multisensory information modulates competition among SC neurons to elicit faster responses. We conducted multiunit recordings from the SC of rats performing a two-alternative spatial discrimination task using auditory and/or visual stimuli. We found that a large population of SC neurons showed direction-selective activity before the onset of movement in response to the stimuli irrespective of stimulation modality. Trial-by-trial correlation analysis showed that the premovement activity of many SC neurons increased with faster reaction speed for the contraversive movement, whereas the premovement activity of another population of neurons decreased with faster reaction speed for the ipsiversive movement. When visual and auditory stimuli were presented simultaneously, the premovement activity of a population of neurons for the contraversive movement was enhanced, whereas the premovement activity of another population of neurons for the ipsiversive movement was depressed. Unilateral inactivation of SC using muscimol prolonged reaction times of contraversive movements, but it shortened those of ipsiversive movements. These findings suggest that the difference in activity between the SC hemispheres regulates the reaction speed of motor responses, and multisensory information enlarges the activity difference resulting in faster responses
Rapid Change in Articulatory Lip Movement Induced by Preceding Auditory Feedback during Production of Bilabial Plosives
BACKGROUND: There has been plentiful evidence of kinesthetically induced rapid compensation for unanticipated perturbation in speech articulatory movements. However, the role of auditory information in stabilizing articulation has been little studied except for the control of voice fundamental frequency, voice amplitude and vowel formant frequencies. Although the influence of auditory information on the articulatory control process is evident in unintended speech errors caused by delayed auditory feedback, the direct and immediate effect of auditory alteration on the movements of articulators has not been clarified. METHODOLOGY/PRINCIPAL FINDINGS: This work examined whether temporal changes in the auditory feedback of bilabial plosives immediately affects the subsequent lip movement. We conducted experiments with an auditory feedback alteration system that enabled us to replace or block speech sounds in real time. Participants were asked to produce the syllable /pa/ repeatedly at a constant rate. During the repetition, normal auditory feedback was interrupted, and one of three pre-recorded syllables /pa/, /Ξ¦a/, or /pi/, spoken by the same participant, was presented once at a different timing from the anticipated production onset, while no feedback was presented for subsequent repetitions. Comparisons of the labial distance trajectories under altered and normal feedback conditions indicated that the movement quickened during the short period immediately after the alteration onset, when /pa/ was presented 50 ms before the expected timing. Such change was not significant under other feedback conditions we tested. CONCLUSIONS/SIGNIFICANCE: The earlier articulation rapidly induced by the progressive auditory input suggests that a compensatory mechanism helps to maintain a constant speech rate by detecting errors between the internally predicted and actually provided auditory information associated with self movement. The timing- and context-dependent effects of feedback alteration suggest that the sensory error detection works in a temporally asymmetric window where acoustic features of the syllable to be produced may be coded
