    Group Education for patients with rheumatoid arthritis

    Patients with rheumatoid arthritis must learn to adjust their exercise, rest and medication to the varying activity of the disease. Patient education can help patients in making the right decisions about adjustments in their treatment regimen and in attaining ¿self-management¿ behaviors. We developed a group education program based on social learning theory and the `Arthritis Self Management Course¿ developed in the USA by Lorig. Goal of the program is the strengthening of self-efficacy, outcome expectations and self-management behaviors of RA patients which may lead to better health status. The program has been evaluated in an experimental design. We established significant positive effects of the group training on functional disability, joint tenderness, practice of relaxation and physical exercises, self-management behavior, outcome expectations, self-efficacy function and knowledge. After 14 months we still found effects on practice of physical exercises, self-efficacy function and knowledge

    Patient education and disease activity: A study among rheumatoid arthritis patients

    Objective: To determine whether patients experiencing high disease activity derive more benefit from patient education than those experiencing low disease activity. - \ud Methods: Data from a randomized study on the effects of a program of patient education were analyzed retrospectively. Four subgroups were studied: the high disease activity subgroup of patients who had participated in the educational program, the complementary low disease activity subgroup, the high disease activity subgroup of controls, and its low disease activity complement. Patients with erythrocyte sedimentation rate >28 mm/first hour were classified as having high disease activity. Effects on frequency of physical exercises, endurance exercises, and relaxation exercises and effects on health status (Modified Health Assessment Questionnaire, Dutch Arthritis Impact Measurement Scales [AIMS]) were measured. - \ud Results: There were no significant differences between the adherence parameters of the various pairs of groups. Four months after the educational program began, anxiety and depression scores on the Dutch-AIMS had increased among participating patients who were experiencing high disease activity and decreased among those who were experiencing low disease activity. - \ud Conclusions: Patients experiencing high disease activity did not derive more benefit from patient education than those experiencing low disease activity. On the contrary, an increase of anxiety and depression is found in these patients. Further study is needed to confirm our findings

    Spatial analysis of BSE cases in the Netherlands

    <p>Abstract</p> <p>Background</p> <p>In many of the European countries affected by Bovine Spongiform Encephalopathy (BSE), case clustering patterns have been observed. Most of these patterns have been interpreted in terms of heterogeneities in exposure of cattle to the BSE agent. Here we investigate whether spatial clustering is present in the Dutch BSE case data.</p> <p>Results</p> <p>We have found three spatial case clusters in the Dutch BSE epidemic. The clusters are geographically distinct and each cluster appears in a different birth cohort. When testing all birth cohorts together, only one significant cluster was detected. The fact that we found stronger spatial clustering when using a cohort-based analysis, is consistent with the evidence that most BSE infections occur in animals less than 12 or 18 months old.</p> <p>Conclusion</p> <p>Significant spatial case clustering is present in the Dutch BSE epidemic. The spatial clusters of BSE cases are most likely due to time-dependent heterogeneities in exposure related to feed production.</p

    Lymphocyte Defect in Plasmacytoma-bearing Mice

    Multiple myeloma is often associated with humoral immunodepression in both man and mouse. When mice bearing the humorally immunodepressive plasmacytomas TEPC-183 and SPQC-11 were injected with SRBC, the rise of serum haemolysins was significantly less than that of non-tumour-bearing mice. Mice with the plasmacytomas MPC-11 and MOPC-315 have an antibody response similar to normal mice when injected with SRBC. Following immunization, normal mice and those bearing MPC-11 showed a 2- to 3-fold increase in total spleen lymphocytes. Mice bearing TEPC-183 or SPQC-11, the plasmacytomas causing an impaired antibody response, has significant increase in spleen lymphocytes under the same conditions. Mice bearing MOPC-315 had a very high initial count of spleen lymphocytes, which did not further increase upon immune stimulation

    Mortality following a brain tumour diagnosis in patients with multiple sclerosis

    Objectives: As brain tumours and their treatment may theoretically have a poorer prognosis in inflammatory central nervous system diseases such as multiple sclerosis (MS), all-cause mortality following a brain tumour diagnosis was compared between patients with and without MS. The potential role of age at tumour diagnosis was also examined. Setting: Hospital inpatients in Sweden with assessment of mortality in hospital or following discharge. Participants: Swedish national registers identified 20 543 patients with an MS diagnosis (1969-2005) and they were matched individually to produce a comparison cohort of 204 163 members of the general population without MS. Everyone with a primary brain tumour diagnosis was selected for this study: 111 with MS and 907 without MS. Primary and secondary outcome measures: 5-year mortality risk following brain tumour diagnosis and age at brain tumour diagnosis. Results: A non-statistically significant lower mortality risk among patients with MS (lower for those with tumours of high-grade and uncertain-grade malignancy and no notable difference for low-grade tumours) produced an unadjusted HR (and 95% CI) of 0.75 (0.56 to 1.02). After adjustment for age at diagnosis, grade of malignancy, sex, region of residence and socioeconomic index, the HR is 0.91 (0.67-1.24). The change in estimate was largely due to adjustment for age at brain tumour diagnosis, as patients with MS were on average 4.7 years younger at brain tumour diagnosis than those in the comparison cohort (p<0.001). Conclusions: Younger age at tumour diagnosis may contribute to mortality reduction in those with highgrade and uncertain-grade brain tumours. Survival following a brain tumour is not worse in patients with MS; even after age at brain tumour diagnosis and grade of malignancy are taken into account

    Two wheeled lunar dumptruck

    The design of a two wheel bulk material transport vehicle is described in detail. The design consists of a modified cylindrical bowl, two independently controlled direct drive motors, and two deformable wheels. The bowl has a carrying capacity of 2.8 m (100 ft) and is constructed of aluminum. The low speed, high HP motors are directly connected to the wheels, thus yielding only two moving parts. The wheels, specifically designed for lunar applications, utilize the chevron tread pattern for optimum traction. The vehicle is maneuvered by varying the relative angular velocities of the wheels. The bulk material being transported is unloaded by utilizing the motors to oscillate the bowl back and forth to a height at which dumping is achieved. The analytical models were tested using a scaled prototype of the lunar transport vehicle. The experimental data correlated well with theoretical predictions. Thus, the design established provides a feasible alternative for the handling of bulk material on the moon

Een aantal bedrijven volgt daartoe een zogeheten alternatief spoor. Deze strategie bestaat uit een combinatie van maatregelen als een verlaging van de kunstmestgift en het voeren van een eiwitarm en structuurrijk rantsoen. De hoofdvraag binnen dit onderzoek is of toepassing van het alternatieve spoor, inclusief het bovengronds aanwenden van mest, een even lage ammoniakemissie oplevert in vergelijking met bedrijven die dit spoor niet volgen en mest emissiearm aanwenden

    Magnetic phase transition in V2O3 nanocrystals

    V2O3 nanocrystals can be synthesized through hydrothermal reduction of VO(OH)2 using hydrazine as a reducing agent. Addition of different ligands to the reaction produces nanoparticles, nanorods and nanoplatelets of different sizes. Small nanoparticles synthesized in this manner show suppression of the magnetic phase transition to lower temperatures. Using muon spin relaxation spectroscopy and synchrotron x-ray diffraction, it is determined that the volume fraction of the high-temperature phase, characterized by a rhombohedral structure and paramagnetism, gradually declines with decreasing temperature, in contrast to the sharp transition observed in bulk V2O3.Comment: 6 pages, 6 figure

    Influence of air mass origin on microphysical properties of low-level cloudsin a subarctic environment

    In this work, an analysis was performed to investigate how different long-range transport air masses can affect the microphysical properties of low-level clouds in a clean subarctic environment. The cloud measurements included in situ and remote sensing ground-based techniques and were conducted during eight Pallas Cloud Experiments (PaCEs) held in the autumn between 2004 and 2019. Each PaCE was carried out at the Pallas Atmosphere-Ecosystem Supersite, located in the Finnish subarctic region. Two cloud spectrometer ground setups were installed on the roof of the station to measure cloud microphysical properties: the cloud, aerosol and precipitation spectrometer (CAPS) and the forward-scattering spectrometer probe (FSSP). Air mass histories were analyzed using the Lagrangian FLEXible PARTicle dispersion model (FLEXPART) in order to investigate the differences between five distinct source regions ( "Arctic ", "Eastern ", "Southern ", "Western " and "Local "). We observed clear differences in the cloud microphysical properties for the air mass source regions. Arctic air masses were characterized by low liquid water content (LWC), low cloud droplet number concentration (Nc) and comparatively large median volume and effective droplet diameter. The Western region (marine North Atlantic) differed from the Arctic by both higher Nc and LWC. The Eastern region (continental Eurasia) only had a little higher LWC than the Arctic but substantially higher Nc and a smaller droplet diameter. The Southern region (continental Europe) had high Nc and LWC and a very similar droplet diameter to the Eastern region. Finally, the relationship between Nc and droplet size (i.e., the Twomey effect) was characterized for the different source regions, indicating that all region clouds were sensitive to increases in Nc.Peer reviewe
