    EC02-179 Managing Livestock Manure to Protect Environmental Quality

    This book covers the land application part of manure management. With increasing regulations, the livestock producer needs to understand the scientific principles that affect manure transformations and how to use these principles to manage the manure for maximum fertilizer value with minimal environmental impact. Improved land application of manure is one part of the solution, but we suggest that the producer evaluate the quantity of nutrients arriving on the farm as feed, animals, and fertilizer compared to the total that is exported. Achieving a nutrient balance will reduce potential environmental hazards often associated with animal agriculture

    Comparison of hydrologic calibration of HSPF using automatic and manual methods

    The automatic calibration software Parameter Estimation (PEST) was used in the hydrologic calibration of Hydrological Simulation Program-Fortran (HSPF), and the results were compared with a manual calibration assisted by the Expert System for the Calibration of HSPF (HSPEXP). In this study, multiobjective functions based on the HSPEXP model performance criteria were developed for use in PEST, which allowed for the comparison of the calibration results of the two methods. The calibrated results of both methods were compared in terms of HSPEXP model performance criteria, goodness-of-fit measures (R-2, E, and RMSE), and base flow index. The automatic calibration results satisfied most of the HSPEXP model performance criteria and performed better with respect to R2, E, RMSE, and base flow index than manual calibration results. The results of the comparison with the manual calibration suggest that the automatic method using PEST may be a suitable alternative to manual method assisted by HSPEXP for calibration of hydrologic parameters for HSPF. However, further research of the weights used in the objective functions is necessary to provide guidance when applying PEST to surface water modeling

    Development and validation of a multivariable risk prediction model for serious infection in patients with psoriasis receiving systemic therapy

    BACKGROUND: Patients with psoriasis are often concerned about the risk of serious infection associated with systemic psoriasis treatments. OBJECTIVES: To develop and externally validate a prediction model for serious infection in patients with psoriasis within 1 year of starting systemic therapies. METHODS: The risk prediction model was developed using the British Association of Dermatologists Biologic Interventions Register (BADBIR), and the German Psoriasis Registry PsoBest was used as the validation dataset. Model discrimination and calibration were assessed internally and externally using the C-statistic, the calibration slope and the calibration in the large. RESULTS: Overall 175 (1·7%) out of 10 033 participants from BADBIR and 41 (1·7%) out of 2423 participants from PsoBest developed a serious infection within 1 year of therapy initiation. Selected predictors in a multiple logistic regression model included nine baseline covariates, and starting infliximab was the strongest predictor. Evaluation of model performance showed a bootstrap optimism-corrected C-statistic of 0·64 [95% confidence interval (CI) 0·60-0·69], calibration in the large of 0·02 (95% CI -0·14 to 0·17) and a calibration slope of 0·88 (95% CI 0·70-1·07), while external validation performance was poor, with C-statistic 0·52 (95% CI 0·42-0·62), calibration in the large 0·06 (95% CI -0·25 to 0·37) and calibration slope 0·36 (95% CI -0·24 to 0·97). CONCLUSIONS: We present the first results of the development of a multivariable prediction model. This model may help patients and dermatologists in the U.K. and the Republic of Ireland to identify modifiable risk factors and inform therapy choice in a shared decision-making process

    A TRPV Channel Modulates C. elegans Neurosecretion, Larval Starvation Survival, and Adult Lifespan

    For most organisms, food is only intermittently available; therefore, molecular mechanisms that couple sensation of nutrient availability to growth and development are critical for survival. These mechanisms, however, remain poorly defined. In the absence of nutrients, newly hatched first larval (L1) stage Caenorhabditis elegans halt development and survive in this state for several weeks. We isolated mutations in unc-31, encoding a calcium-activated regulator of neural dense-core vesicle release, which conferred enhanced starvation survival. This extended survival was reminiscent of that seen in daf-2 insulin-signaling deficient mutants and was ultimately dependent on daf-16, which encodes a FOXO transcription factor whose activity is inhibited by insulin signaling. While insulin signaling modulates metabolism, adult lifespan, and dauer formation, insulin-independent mechanisms that also regulate these processes did not promote starvation survival, indicating that regulation of starvation survival is a distinct program. Cell-specific rescue experiments identified a small subset of primary sensory neurons where unc-31 reconstitution modulated starvation survival, suggesting that these neurons mediate perception of food availability. We found that OCR-2, a transient receptor potential vanilloid (TRPV) channel that localizes to the cilia of this subset of neurons, regulates peptide-hormone secretion and L1 starvation survival. Moreover, inactivation of ocr-2 caused a significant extension in adult lifespan. These findings indicate that TRPV channels, which mediate sensation of diverse noxious, thermal, osmotic, and mechanical stimuli, couple nutrient availability to larval starvation survival and adult lifespan through modulation of neural dense-core vesicle secretion

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

The goal of every irrigator should be to apply the right amount of water uniformly to meet crop needs. To do this, irrigators need to know how much water is applied and where it goes. In other words, they need to know how uniformly the irrigation water infiltrates into the soil profile. Achieving a uniform water application is not easy when using furrow irrigation. However, with a better understanding of how irrigation system management affects water distribution and a willingness to make management changes, the uniformity and efficiency of most systems can be improved. Achieving a uniform water application is not easy when using furrow irrigation. However, with a better understanding of how irrigation system management affects water distribution and a willingness to make management changes, the uniformity and efficiency of most systems can be improved. This paper outlines the use of the "cutoff ratio" and how irrigators can use this management parameter to evaluate irrigation system performance
