MR angiography with blood pool contrast agents

Contrast-enhanced magnetic resonance angiography (CE-MRA) with standard extracellular contrast material is well established for vascular imaging. Recently, the first blood pool contrast agent (BPA) has become clinically available. This paper reviews characteristics and classification of BPA as well as first clinical experience in various vascular territories. BPAs comprise gadolinium-based compounds, synthetic compounds, and ultrasmall superparamagnetic iron-oxide (USPIO) particles. Such BPAs are retained in blood with a prolonged time-window of enhancement as compared to extracellular gadolinium chelates. Promising results from USPIO at first-pass and steady-state angiography have been published, but no USPIO is approved yet. Gadofosveset is the first clinically approved BPA. After bolus injection, gadofosveset binds noncovalently to serum-albumine, thus enhancing relaxivity. First published results from carotid, coronary, renal, and peripheral angiography are encouraging; particularly helpful is prolonged enhancement during steady state. More BPAs have been clinically evaluated, but no approval has been granted. Bolus-injectable BPAs allow for first-pass CE-MRA similar to standard extracellular contrast media, but with higher relaxivity, allowing lower doses and reduced injection rates. The Freshwater Information Platform: a global online network providing data, tools and resources for science and policy support

Freshwaters are among the most complex, dynamic, and diverse ecosystems globally. Despite their small share of the earth’s surface (less than 1%) they are home to over 10% of all known animal species. Biodiversity decrease in general and freshwater biodiversity decline in particular have recently received increasing attention, and various policy instruments are now targeting the conservation, protection and enhancement of biodiversity and associated ecosystem services. Surveillance programs as well as a variety of research projects have been producing a tremendous amount of freshwater-related information. Though there have been various attempts to build infrastructures for online collection of such data, tools and reports, they often provide only limited access to resources that can readily be extracted for conducting large scale analyses. Here, we present the Freshwater Information Platform, an open system of relevant freshwater biodiversity-related information. We provide a comprehensive overview of the platform’s core components, highlight their values, present options for their use, and discuss future developments. This is complemented by information on the platform’s current management structure, options for contributing data and research results and an outlook for the future

Transarticular screw fixation for osteoarthritis of the atlanto axial segment

Atlantoaxial (C1-C2) facet joint osteoarthitis is a distinct clinical syndrome that often goes unrecognized. Severe pain resistant to conservative treatment that is corroborated by the radiographic findings represents the indication for surgery. The aim of this study was to retrospectively examine the long-term outcome [after an average 6.5years (SD 4.0)] of C1-2 fusion for osteoarthritis of the atlantoaxial segment in 35 consecutive patients [25 male, 10 female; aged 62 (SD 15)years]. At follow-up, clinical outcome and radiological status was examined in 27/35 (77%) patients, and self-rated pain and disability (Neck Pain and Disability Scale; NPDS) in 29/35 (83%) patients. In 27/35 patients (77%), 2 screws were inserted; in 7 patients (20%), only 1 screw; and in 1 patient (3%), no screws. 11% of the patients had late complications requiring revision surgery. All patients showed solid fusion at the long-term follow-up. 26% patients showed an improvement in sensory disturbances, 63% no change, and 11% a worsening. 89% were pain-free or had markedly reduced pain. The average score on the NPDS (0-100) was 34 (SD 27), representing ‘mild' neck problems, and the average pain intensity (0-5 VAS) was 1.5 (SD 1.5). Eighty-five percent of the patients declared that they would make the same decision again to undergo surgery. In conclusion, in a group of patients with a painful and debilitating degenerative disorder of C1-2, posterior transarticular atlantoaxial fusion proved to be an effective treatment with a low rate of serious complication

Open Source Geospatial Applications to support River Basin Management in Kharaa River Basin, Mongolia

An open access geoportal, developed using open source technologies, delivers a comprehensive overview of all geodata available for the Kharaa River Basin in Mongolia. The Water Quality Monitoring Database is made available in PostgreSQL/PostGIS format and embedded in the geoportal. Web maps in the Geoportal can be linked to monitoring data using interactive queries to the Water Quality Database enabling water managers to create thematic maps on demand for the development of a comprehensive river basin management plan for the Kharaa River Basin
