A Randomized Controlled Trial to Compare Computer-assisted Motivational Intervention with Didactic Educational Counseling to Reduce Unprotected Sex in Female Adolescents
Study Objective: To examine a computer-assisted, counselor-guided motivational intervention (CAMI) aimed at reducing the risk of unprotected sexual intercourse.
Design, Setting, Participants, Interventions, and Main Outcome Measures: We conducted a 9-month, longitudinal randomized controlled trial with a multisite recruitment strategy including clinic, university, and social referrals, and compared the CAMI with didactic educational counseling in 572 female adolescents with a mean age of 17 years (SD = 2.2 years; range = 13-21 years; 59% African American) who were at risk for pregnancy and sexually transmitted diseases. The primary outcome was the acceptability of the CAMI according to self-reported rating scales. The secondary outcome was the reduction of pregnancy and sexually transmitted disease risk using a 9-month, self-report timeline follow-back calendar of unprotected sex.

Conclusion: Among those who completed the intervention, the CAMI reduced unprotected sex among an at-risk, predominantly minority sample of female adolescents. Modification of the CAMI to address methodological issues that contributed to a high drop-out rate are needed to make the intervention more acceptable and feasible for use among sexually active predominantly minority, at-risk, female adolescents
Correlates of Complete Childhood Vaccination in East African Countries.
Despite the benefits of childhood vaccinations, vaccination rates in low-income countries (LICs) vary widely. Increasing coverage of vaccines to 90% in the poorest countries over the next 10 years has been estimated to prevent 426 million cases of illness and avert nearly 6.4 million childhood deaths worldwide. Consequently, we sought to provide a comprehensive examination of contemporary vaccination patterns in East Africa and to identify common and country-specific barriers to complete childhood vaccination. Using data from the Demographic and Health Surveys (DHS) for Burundi, Ethiopia, Kenya, Rwanda, Tanzania, and Uganda, we looked at the prevalence of complete vaccination for polio, measles, Bacillus Calmette-Guérin (BCG) and DTwPHibHep (DTP) as recommended by the WHO among children ages 12 to 23 months. We conducted multivariable logistic regression within each country to estimate associations between complete vaccination status and health care access and sociodemographic variables using backwards stepwise regression. Vaccination varied significantly by country. In all countries, the majority of children received at least one dose of a WHO recommended vaccine; however, in Ethiopia, Tanzania, and Uganda less than 50% of children received a complete schedule of recommended vaccines. Being delivered in a public or private institution compared with being delivered at home was associated with increased odds of complete vaccination status. Sociodemographic covariates were not consistently associated with complete vaccination status across countries. Although no consistent set of predictors accounted for complete vaccination status, we observed differences based on region and the location of delivery. These differences point to the need to examine the historical, political, and economic context of each country in order to maximize vaccination coverage. Vaccination against these childhood diseases is a critical step towards reaching the Millennium Development Goal of reducing under-five mortality by two-thirds by 2015 and thus should be a global priority
A Personalized Self-Management Rehabilitation System for Stroke Survivors: A Quantitative Gait Analysis Using a Smart Insole.
Background: In the United Kingdom, stroke is the single largest cause of adult disability and results in a cost to the economy of £8.9 billion per annum. Service needs are currently not being met; therefore, initiatives that focus on patient-centered care that promote long-term self-management for chronic conditions should be at the forefront of service redesign. The use of innovative technologies and the ability to apply these effectively to promote behavior change are paramount in meeting the current challenges.
Objective: Our objective was to gain a deeper insight into the impact of innovative technologies in support of home-based, self-managed rehabilitation for stroke survivors. An intervention of daily walks can assist with improving lower limb motor function, and this can be measured by using technology. This paper focuses on assessing the usage of self-management technologies on poststroke survivors while undergoing rehabilitation at home.
Methods: A realist evaluation of a personalized self-management rehabilitation system was undertaken in the homes of stroke survivors (N=5) over a period of approximately two months. Context, mechanisms, and outcomes were developed and explored using theories relating to motor recovery. Participants were encouraged to self-manage their daily walking activity; this was achieved through goal setting and motivational feedback. Gait data were collected and analyzed to produce metrics such as speed, heel strikes, and symmetry. This was achieved using a “smart insole” to facilitate measurement of walking activities in a free-living, nonrestrictive environment.
Results: Initial findings indicated that 4 out of 5 participants performed better during the second half of the evaluation. Performance increase was evident through improved heel strikes on participants’ affected limb. Additionally, increase in performance in relation to speed was also evident for all 5 participants. A common strategy emerged across all but one participant as symmetry performance was sacrificed in favor of improved heel strikes. This paper evaluates compliance and intensity of use.
Conclusion: Our findings suggested that 4 out of the 5 participants improved their ability to heel strike on their affected limb. All participants showed improvements in their speed of gait measured in steps per minute with an average increase of 9.8% during the rehabilitation program. Performance in relation to symmetry showed an 8.5% average decline across participants, although 1 participant improved by 4%. Context, mechanism, and outcomes indicated that dual motor learning and compensatory strategies were deployed by the participants
Assessment of Filled Buprenorphine Prescriptions for Opioid Use Disorder During the Coronavirus Disease 2019 Pandemic

To diminish potential barriers to treatment access, 3 days later, federal guidelines on telemedicine use were released, providing authorized practitioners increased flexibility to prescribe buprenorphine to patients with opioid use disorder (OUD) during this public health emergency.3 Other local, state, and federal policy initiatives have also attempted to preserve access to medication treatment for OUD, yet the cumulative outcome of these undertakings is not clear
Identifying Optimal Methods for Addressing Confounding Bias When Estimating the Effects of State-Level Policies
Background: Policy evaluation studies that assess how state-level policies
affect health-related outcomes are foundational to health and social policy
research. The relative ability of newer analytic methods to address
confounding, a key source of bias in observational studies, has not been
closely examined. Methods: We conducted a simulation study to examine how
differing magnitudes of confounding affected the performance of four methods
used for policy evaluations: (1) the two-way fixed effects (TWFE)
difference-in-differences (DID) model; (2) a one-period lagged autoregressive
(AR) model; (3) augmented synthetic control method (ASCM); and (4) the doubly
robust DID approach with multiple time periods from Callaway-Sant'Anna (CSA).
We simulated our data to have staggered policy adoption and multiple
confounding scenarios (i.e., varying the magnitude and nature of confounding
relationships). Results: Bias increased for each method: (1) as confounding
magnitude increases; (2) when confounding is generated with respect to prior
outcome trends (rather than levels), and (3) when confounding associations are
nonlinear (rather than linear). The AR and ASCM have notably lower root mean
squared error than the TWFE model and CSA approach for all scenarios; the
exception is nonlinear confounding by prior trends, where CSA excels. Coverage
rates are unreasonably high for ASCM (e.g., 100%), reflecting large model-based
standard errors and wide confidence intervals in practice. Conclusions: Our
simulation study indicated that no single method consistently outperforms the
others. But a researcher's toolkit should include all methodological options.
Our simulations and associated R package can help researchers choose the most

Developing and applying heterogeneous phylogenetic models with XRate
Modeling sequence evolution on phylogenetic trees is a useful technique in
computational biology. Especially powerful are models which take account of the
heterogeneous nature of sequence evolution according to the "grammar" of the
encoded gene features. However, beyond a modest level of model complexity,
manual coding of models becomes prohibitively labor-intensive. We demonstrate,
via a set of case studies, the new built-in model-prototyping capabilities of
XRate (macros and Scheme extensions). These features allow rapid implementation
of phylogenetic models which would have previously been far more
labor-intensive. XRate's new capabilities for lineage-specific models,
ancestral sequence reconstruction, and improved annotation output are also
discussed. XRate's flexible model-specification capabilities and computational
efficiency make it well-suited to developing and prototyping phylogenetic

Ripple oscillations in the left temporal neocortex are associated with impaired verbal episodic memory encoding
Background: We sought to determine if ripple oscillations (80-120Hz),
detected in intracranial EEG (iEEG) recordings of epilepsy patients, correlate
with an enhancement or disruption of verbal episodic memory encoding. Methods:
We defined ripple and spike events in depth iEEG recordings during list
learning in 107 patients with focal epilepsy. We used logistic regression
models (LRMs) to investigate the relationship between the occurrence of ripple
and spike events during word presentation and the odds of successful word
recall following a distractor epoch, and included the seizure onset zone (SOZ)
as a covariate in the LRMs. Results: We detected events during 58,312 word
presentation trials from 7,630 unique electrode sites. The probability of
ripple on spike (RonS) events was increased in the seizure onset zone (SOZ,
p<0.04). In the left temporal neocortex RonS events during word presentation
corresponded with a decrease in the odds ratio (OR) of successful recall,
however this effect only met significance in the SOZ (OR of word recall 0.71,
95% CI: 0.59-0.85, n=158 events, adaptive Hochberg p<0.01). Ripple on
oscillation events (RonO) that occurred in the left temporal neocortex non-SOZ
also correlated with decreased odds of successful recall (OR 0.52, 95% CI:
0.34-0.80, n=140, adaptive Hochberg , p<0.01). Spikes and RonS that occurred
during word presentation in the left middle temporal gyrus during word
presentation correlated with the most significant decrease in the odds of
successful recall, irrespective of the location of the SOZ (adaptive Hochberg,
p<0.01). Conclusion: Ripples and spikes generated in left temporal neocortex
are associated with impaired verbal episodic memory encoding
Reflections on a 'virtual' practice development unit: changing practice through identity development
Aims. This paper draws together the personal thoughts and critical reflections of key people involved in the establishment of a ‘virtual’ practice development unit of clinical nurse specialists in the south of England. Background. This practice development unit is ‘virtual’ in that it is not constrained by physical or specialty boundaries. It became the first group of Trust-wide clinical nurse specialists to be accredited in the UK as a practice development unit in 2004. Design and methods. The local university was asked to facilitate the accreditation process via 11 two-hour audio-recorded learning sessions. Critical reflections from practice development unit members, leaders and university staff were written 12 months after successful accreditation, and the framework of their content analysed. Findings and discussion. Practice development was seen as a way for the clinical nurse specialists to realize their potential for improving patient care by transforming care practice in a collaborative, interprofessional and evolutionary manner. The practice development unit provided a means for these nurses to analyse their role and function within the Trust. Roberts’ identity development model for nursing serves as a useful theoretical underpinning for the reflections contained in this paper. Conclusions. These narratives provide another example of nurses making the effort to shape and contribute to patient care through organizational redesign. This group of nurses began to realize that the structure of the practice development unit process provided them with the means to analyse their role and function within the organization and, as they reflected on this structure, their behaviour began to change. Relevance to clinical practice. Evidence from these reflections supports the view that practice development unit participants have secured a positive and professional identity and are, therefore, better able to improve the patient experience
