    Updated estimated incidence and prevalence of serious fungal infections in Trinidad and Tobago

    Objective: : To estimate the incidence and prevalence of serious fungal infections in Trinidad and Tobago (T&T), with a population of 1 394 973 million. Methods: : The medical literature was searched to obtain published data on the incidence and prevalence of fungal infections in the Caribbean. If data were unavailable, estimations were performed using the frequencies of fungal infection in populations at risk. Asthma and pulmonary tuberculosis rates were used to derive the prevalence of allergic bronchopulmonary aspergillosis (ABPA), severe asthma with fungal sensitization (SAFS), and chronic pulmonary aspergillosis (CPA). Results: : The estimated annual burden of fungal infections was 46 156 persons (3.3% of the population), including 21 455 women with recurrent vulvovaginal candidiasis, 118 persons with invasive aspergillosis, 3637 adults with ABPA, 4800 with SAFS, and 178 with CPA. Annually, we estimated 70 cases of candidemia and 14 647 cases of tinea capitis in children. Of the 11 000 persons living with HIV/AIDS, it was estimated that there were 40 cases of cryptococcal meningitis, 88 cases of disseminated histoplasmosis, and 124 cases of Pneumocystis pneumonia. Conclusion: : There seems to be an extensive burden of fungal infections in T&T. Hence, targeted interventions are required to improve clinical and laboratory diagnosis and a national surveillance system should be implemented.S

    Racial Comparisons in Police Officer Bench Press Strength over 12.5 Years

    International Journal of Exercise Science 7(2) : 140-151, 2014. Strength, when considering gender and race, provides a basis for training, hiring, and retention for police officers. The purpose of this study is to identify muscular strength differences among racial groups from initial-recruit to in-service tests. Strength variables included bench press, bench press/lean mass and bench press/body mass. Scores were retrieved for the 1990 to 1995 recruit classes and were paired to the 2006 in-service fitness log. Sample included 309 officers: 30 females (13 black, 17 white) and 279 males (41 black, 238 white). Mean age of recruit was 24.6 Ā± 3.4 years and for in-service was 37.1 Ā± 3.7 years. Time between tests was 12.5 Ā± 2.0 years. Bench press strength significantly increased for all gender and racial groups. Black males were significantly stronger in bench press at initial-recruit and at in-service than white males. All racial and gender groups increased in bench press/lean mass, however, the increase in white females was not significant. No differences were found between black and white females in all strength variables at both testing periods. Black males were significantly stronger than white males in bench press/body mass only at the initial-recruit test. White males showed a significant increase in bench press/body mass over time, but they did not reach the strength level of the black males at in-service. Police departments, with a properly designed physical fitness program, can expect to see increases in strength of personnel over the first half of their careers; however, there are gender and racial differences

    Muscular Strength and Body Composition Comparison Between the Charlotte-Mecklenburg Fire and Police Departments

    Firefighters and police have different physical job requirements and selection processes. Firefighters have on-duty exercise time and police do not. This could affect body composition and strength. The purpose of this investigation was to compare body composition and bench press strength between firefighters and police officers in the Charlotte-Mecklenburg departments. Current 2006 fitness records were analyzed to compare firefighters and police (N = 2330). Variables included: gender, age, body mass, percent body-fat, fat mass, lean mass, bench press strength, bench press/lean mass, bench press/body mass, and percent that were obese. Significant (p ā‰¤ 0.05) differences were found between male firefighters and police in age (37.7 vs. 36.9 yrs), body mass (91.5 vs. 93.2 kg), percent body-fat (17.8% vs.18.5%), fat mass (16.9 vs. 18.0 kg), bench press strength (93.4 vs. 96.3 kg), bench press/body mass (1.03 vs.1.05) and percent that were obese (10% vs.17%), respectively. Significant differences between female firefighters and police were found in age (42.4 vs. 37.4 yrs), body mass (77.5 vs. 71.8 kg), lean mass (54.4 vs. 51.2 kg), bench press strength (52.3 vs. 43.9 kg), bench press/lean mass (0.99 vs. 0.86) and bench press/body mass (0.69 vs. 0.62), respectively. Male police were younger, weighed more, had higher percent body fat, higher percent of obesity, and had greater upper body strength than firefighters. Female police were younger, weighed less, had less lean mass and less upper body strength than their firefighter counterparts. Contributing factors might include pre-employment selection, recruit training, in-service physical fitness testing, and counseling provided by departments

    To play or not to play: Can an instrument really impact lip and tongue performance?

(1) Background: Increasing tongue and lip strength may help improve various speech and swallowing disorders, but it is unclear if instrumentalists who use these muscle groups for long periods of time have greater strength and endurance compared to controls. It is also unclear if instrumentalists can more accurately estimate various exercise intensities. The purpose of this study was to determine differences in lip and tongue strength and endurance between instrumentalists and non-instrumentalists (controls). A secondary purpose was to assess differences in ability to estimate various exercise intensities between the two groups. (2) Methods: Instrumentalists and controlsā€™ maximum strength and endurance were measured using the IOPI Pro medical device. In addition, 40%, 60% and 80% of maximum strength were estimated in a randomized order. (3) Results: No significant differences were found between instrumentalists and controls in strength or endurance or the ability to estimate various intensities. Overall, participants were better at estimating tongue strength at moderate intensities and lip strength at higher intensities. (4) Conclusion: Tongue and lip strength and endurance and the ability to estimate exercise intensities are not impacted by years of instrumentalist training compared to healthy controls

    Different Sites of Alcohol Action in the NMDA Receptor GluN2A and GluN2B Subunits

    The NMDA receptor is a major target of alcohol action in the CNS, and recent behavioral and cellular studies have pointed to the importance of the GluN2B subunit in alcohol action. We and others have previously characterized four amino acid positions in the third and fourth membrane-associated (M) domains of the NMDA receptor GluN2A subunit that influence both ion channel gating and alcohol sensitivity. In this study, we found that substitution mutations at two of the four corresponding positions in the GluN2B subunit, F637 and G826, influence ethanol sensitivity and ion channel gating. Because position 826 contains a glycine residue in the native protein, we focused our attention on GluN2B(F637). Substitution mutations at GluN2B(F637) significantly altered ethanol IC50 values, glutamate EC50 values for peak (Ip) and steady-state (Iss) current, and steady-state to peak current ratios (Iss:Ip). Changes in apparent glutamate affinity were not due to agonist trapping in desensitized states, as glutamate Iss EC50 values were not correlated with Iss:Ip values. Ethanol sensitivity was correlated with values of both Ip and Iss glutamate EC50, but not with Iss:Ip. Values of ethanol IC50, glutamate EC50, and Iss:Ip for mutants at GluN2B(F637) were highly correlated with the corresponding values for mutants at GluN2A(F636), consistent with similar functional roles of this position in both subunits. These results demonstrate that GluN2B(Phe637) regulates ethanol action and ion channel function of NMDA receptors. However, despite highly conserved M domain sequences, ethanol\u27s actions on GluN2A and GluN2B subunits differ

    Loneliness, social support and cardiovascular reactivity to laboratory stress

    Self-reported or explicit loneliness and social support have been inconsistently associated with cardiovascular reactivity (CVR) to stress. The present study aimed to adapt an implicit measure of loneliness, and use it alongside the measures of explicit loneliness and social support, to investigate their correlations with CVR to laboratory stress. Twenty-five female volunteers aged between 18 and 39 years completed self-reported measures of loneliness and social support, and an Implicit Association Test (IAT) of loneliness. The systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR) reactivity indices were measured in response to psychosocial stress induced in the laboratory. Functional support indices of social support were significantly correlated with CVR reactivity to stress. Interestingly, implicit, but not explicit, loneliness was significantly correlated with DBP reactivity after one of the stressors. No associations were found between structural support and CVR indices. Results are discussed in terms of validity of implicit versus explicit measures and possible factors that affect physiological outcomes

    Tourmaline Reference Materials for the In Situ Analysis of Oxygen and Lithium Isotope Ratio Compositions

    Three tourmaline reference materials sourced from the Harvard Mineralogical and Geological Museum (schorl 112566, dravite 108796 and elbaite 98144), which are already widely used for the calibration of in situ boron isotope measurements, are characterised here for their oxygen and lithium isotope compositions. Homogeneity tests by secondary ion mass spectrometry (SIMS) showed that at subā€nanogram test portion masses their 18O/16O and 7Li/6Li isotope ratios are constant within Ā± 0.27ā€° and Ā± 2.2ā€° (1s), respectively. The lithium mass fractions of the three materials vary over three orders of magnitude. SIMS homogeneity tests showed variations in 7Li/28Si between 8% and 14% (1s), which provides a measure of the heterogeneity of the Li contents in these three materials. Here we provide recommended values for Ī“18O, Ī”ā€™17O and Ī“7Li for the three Harvard tourmaline reference materials based on results from bulk mineral analyses from multiple, independent laboratories using laserā€ and stepwise fluorination gas mass spectrometry (for O), and solution multiā€collector inductively coupled plasmaā€mass spectroscopy (for Li). These bulk data also allow us to assess the degree of interā€laboratory data that might be present in such datasets. This work also reā€evaluates the major element chemical composition of the materials by electron probe microanalysis and investigates the presence of a chemical matrix effect on SIMS instrumental mass fractionation with regards to Ī“18O determinations, which was found to be < 1.6ā€° between these three materials. The final table presented here provides a summary of the isotope ratio values that we have determined for these three materials. Depending on their starting mass either 128 or 256 splits have been produced of each material, assuring their availability for many years into the future
