    The fuel moisture index based on understorey Hygrochron iButton humidity and temperature measurements reliably predicts fine fuel moisture content in Tasmanian Eucalyptus forests

    Fine fuel moisture content (FFMC) is a key determinant of wildfire occurrence, behaviour, and pyrogeographic patterns. Accurate determination of FFMC is laborious, hence managers and ecologists have devised a range of empirical and mechanistic measures for FFMC. These FFMC measures, however, have received limited field validation against field-based gravimetric fuel moisture measurements. Using statistical modelling, we evaluate the use of the relationship between gravimetric FFMC and the Fuel Moisture Index (FMI), based on Hygrochron iButton humidity and temperature dataloggers. We do this in Tasmanian wet and dry Eucalyptus forests subjected to strongly contrasting disturbance histories and, hence, percentage of canopy cover. We show that 24 h average FMI based on data from Hygrochron iButtons 0.75 m above the forest floor provides reliable estimates of gravimetric litter fuel moisture (c. 1 h fuels) that are strongly correlated with near surface gravimetric fuel moisture sticks (c. 10 h fuels). We conclude FMI based on Hygrochron iButton data provides ecologists with an economic and effective method to retrospectively measure landscape patterns in fuel moisture in Tasmanian forests

    Low energy H+CO scattering revisited - CO rotational excitation with new potential surfaces

    Context. A recent modeling study of brightness ratios for CO rotational transitions in gas typical of the diffuse ISM by Liszt found the role of H collisions to be more important than previously assumed. This conclusion was based on recent quantum scattering calculations using the so-called WKS potential energy surface (PES) which reported a large cross section for the important 0 → 1 rotational transition. This result is in contradiction to one obtained using the earlier BBH PES for which the cross section is quite small and which is consistent with an expected homonuclear-like propensity for even ∆J transitions. Aims. We revisit this contradiction with new scattering calculations using two new ab initio PESs that focus on the important long- range behavior and explore the validity of the apparent departure from the expected even ∆J propensity in H-CO rotational excitation obtained with the WKS PES. Methods. Close-coupling (CC) rigid-rotor calculations for CO(v = 0, J = 0) excitation by H are performed on four different PESs. Two of the PESs are obtained in this work using state-of-the-art quantum chemistry techniques at the CCSD(T) and MRCI levels of theory. Results. Cross sections for the J = 0 → 1, as well as other odd ∆J, transitions are significantly suppressed compared to even ∆J transitions in thermal energy CC calculations using the CCSD(T) and MRCI surfaces. This is consistent with the expected even ∆J propensity and in contrast to CC calculations using the WKS PES which predict a dominating 0 → 1 transition. Conclusions. Inelastic collision cross section calculations are sensitive to fine details in the anisotropic components of the PES and its long-range behavior. The current results obtained with new surfaces for H-CO scattering suggest that the original astrophysical assumption that excitation of CO by H2 dominates the kinetics of CO in diffuse ISM gas is likely to remain valid

    Predicting incident delirium diagnoses using data from primary-care electronic health records

    Importance risk factors for delirium in hospital inpatients are well established, but less is known about whether delirium occurring in the community or during an emergency admission to hospital care might be predicted from routine primary-care records. Objectives identify risk factors in primary-care electronic health records (PC-EHR) predictive of delirium occurring in the community or recorded in the initial episode in emergency hospitalisation. Test predictive performance against the cumulative frailty index. Design Stage 1: case-control; Stages 2 and 3: retrospective cohort. Setting clinical practice research datalink: PC-EHR linked to hospital discharge data from England. Subjects Stage 1: 17,286 patients with delirium aged ≥60 years plus 85,607 controls. Stages 2 and 3: patients ≥ 60 years (n = 429,548 in 2015), split into calibration and validation groups. Methods Stage 1: logistic regression to identify associations of 110 candidate risk measures with delirium. Stage 2: calibrating risk factor weights. Stage 3: validation in independent sample using area under the curve (AUC) receiver operating characteristic. Results fifty-five risk factors were predictive, in domains including: cognitive impairment or mental illness, psychoactive drugs, frailty, infection, hyponatraemia and anticholinergic drugs. The derived model predicted 1-year incident delirium (AUC = 0.867, 0.852:0.881) and mortality (AUC = 0.846, 0.842:0.853), outperforming the frailty index (AUC = 0.761, 0.740:0.782). Individuals with the highest 10% of predicted delirium risk accounted for 55% of incident delirium over 1 year. Conclusions a risk factor model for delirium using data in PC-EHR performed well, identifying individuals at risk of new onsets of delirium. This model has potential for supporting preventive interventions

    Smoke pollution must be part of the savanna fire management equation: a case study from Darwin, Australia

    Savanna fire management is a topic of global debate, with early dry season burning promoted as a large-scale emissions reduction opportunity. To date, discussions have centred on carbon abatement efficacy, biodiversity and cultural benefits and/or risks. Here we use a case study of Darwin, Australia to highlight smoke pollution as another critical consideration. Smoke pollution from savanna fires is a major public health issue, yet absent so far from discussions of program design. Here, we assess the likely impacts of increased early dry season burning on smoke pollution in Darwin between 2004 and 2019, spanning the introduction and expansion of carbon abatement programs. We found increased smoke pollution in the early dry season but little change in the late dry season, contributing to a net annual increase in air quality standard exceedances. Geospatial analysis suggests this relates to increased burning in the path of early dry season trade winds. This study highlights the complex health trade-offs involved with any large-scale prescribed burning, including for carbon abatement

There is renewed concern that core-sets of outcome measures do not capture what is important. To inform debate about the coverage of back pain outcome measure core-sets, and to suggest areas worthy of exploration within healthcare consultations, we have synthesised the qualitative literature on the impact of low back pain on people’s lives. Methods - Two reviewers searched CINAHL, Embase, PsycINFO, PEDro, and Medline, identifying qualitative studies of people’s experiences of non-specific LBP. Abstracted data were thematic coded and synthesised using a meta-ethnographic, and a meta-narrative approach. Results - We included 49 papers describing 42 studies. Patients are concerned with engagement in meaningful activities; but they also want to be believed and have their experiences and identity, as someone ‘doing battle’ with pain, validated. Patients seek diagnosis, treatment, and cure, but also reassurance of the absence of pathology. Some struggle to meet social expectations and obligations. When these are achieved, the credibility of their pain/disability claims can be jeopardised. Others withdraw, fearful of disapproval, or unable or unwilling to accommodate social demands. Patients generally seek to regain their pre-pain levels of health, and physical and emotional stability. After time, this can be perceived to become unrealistic and some adjust their expectations accordingly. Conclusions - The social component of the biopsychosocial model is not well represented in current core-sets of outcome measures. Clinicians should appreciate that the broader impact of low back pain includes social factors; this may be crucial to improving patients’ experiences of health care.     Routine Antenatal Anti-D Prophylaxis in Women Who Are Rh(D) Negative: Meta-Analyses Adjusted for Differences in Study Design and Quality

    Background: To estimate the effectiveness of routine antenatal anti-D prophylaxis for preventing sensitisation in pregnant Rhesus negative women, and to explore whether this depends on the treatment regimen adopted. Methods: Ten studies identified in a previous systematic literature search were included. Potential sources of bias were systematically identified using bias checklists, and their impact and uncertainty were quantified using expert opinion. Study results were adjusted for biases and combined, first in a random-effects meta-analysis and then in a random-effects metaregression analysis. Results: In a conventional meta-analysis, the pooled odds ratio for sensitisation was estimated as 0.25 (95 % CI 0.18, 0.36), comparing routine antenatal anti-D prophylaxis to control, with some heterogeneity (I 2 = 19%). However, this naĂŻve analysis ignores substantial differences in study quality and design. After adjusting for these, the pooled odds ratio for sensitisation was estimated as 0.31 (95 % CI 0.17, 0.56), with no evidence of heterogeneity (I 2 = 0%). A meta-regression analysis wa

    Saddle point localization of molecular wavefunctions

    The quantum mechanical description of isomerization is based on bound eigenstates of the molecular potential energy surface. For the near-minimum regions there is a textbook-based relationship between the potential and eigenenergies. Here we show how the saddle point region that connects the two minima is encoded in the eigenstates of the model quartic potential and in the energy levels of the [H, C, N] potential energy surface. We model the spacing of the eigenenergies with the energy dependent classical oscillation frequency decreasing to zero at the saddle point. The eigenstates with the smallest spacing are localized at the saddle point. The analysis of the HCN???HNC isomerization states shows that the eigenstates with small energy spacing relative to the effective (v1, v3, l) bending potentials are highly localized in the bending coordinate at the transition state. These spectroscopically detectable states represent a chemical marker of the transition state in the eigenenergy spectrum. The method developed here provides a basis for modeling characteristic patterns in the eigenenergy spectrum of bound states

    A powerful intervention: general practitioners' use of sickness certification in depression

    <b>Background</b> Depression is frequently cited as the reason for sickness absence, and it is estimated that sickness certificates are issued in one third of consultations for depression. Previous research has considered GP views of sickness certification but not specifically in relation to depression. This study aimed to explore GPs views of sickness certification in relation to depression.<p></p> <b>Methods</b> A purposive sample of GP practices across Scotland was selected to reflect variations in levels of incapacity claimants and antidepressant prescribing. Qualitative interviews were carried out between 2008 and 2009.<p></p> <b>Results</b> A total of 30 GPs were interviewed. A number of common themes emerged including the perceived importance of GP advocacy on behalf of their patients, the tensions between stakeholders involved in the sickness certification system, the need to respond flexibly to patients who present with depression and the therapeutic nature of time away from work as well as the benefits of work. GPs reported that most patients with depression returned to work after a short period of absence and that it was often difficult to predict which patients would struggle to return to work.<p></p> <b>Conclusions</b> GPs reported that dealing with sickness certification and depression presents distinct challenges. Sickness certificates are often viewed as powerful interventions, the effectiveness of time away from work for those with depression should be subject to robust enquiry
