    Giant cell reparative granuloma of the hallux following enchondroma

    Giant cell reparative granuloma (GCRG) is a rare, benign intra osseous lytic lesion occurring especially in gnathis bone but also seen in feet and hands. It has similar clinical and radiological presentations than giant cell tumor, chondroblastoma, aneurysmal bone cyst, and hyperparathyroidism brown tumors but with specific histological findings We report a case of a GCRG of hallux phalanx in 18 years old patient appearing many years after enchondroma curettage and grafting. Radiographs showed a multiloculated osteolytic lesions involving whole phalanx with cortical thinning and without fluid-fluid levels in CT view. Expected to be an enchondroma recurrence, second biopsy confirmed diagnosis of GCRG with specific histological findings. Although if aetiopathogeny remains unknown, GCRG is reported to be a local non neoplasic reaction to an intraosseous hemorrhage. Our exceptional case claims that this tumor can appear in reaction to cellular disturbance primary or secondary

    Decompression of Facial Nerve after Temporal Bone Fracture: about 20 Cases

    Background: Seven to ten percent of facial nerve paralysis occurs in patients with temporal bone fracture. It has become increasingly common due to the development of human activity. One of the main topics of discussion is facial nerve injury management resulting from temporal bone trauma. The purpose of this study is to report the paralysis of facial nerve after temporal bone fractures. Aim: In this study, our goal was to record the facial profile of rhinoplasty applicants in Shiraz. Methods: This retrospective study analyzed 20 cases of traumatic paralysis of facial nerve, which underwent facial nerve decompression. Results: Recovery rate was correlated to the delay of surgery. 83.3% of patients who underwent decompression surgery within the first 2 weeks after trauma had an excellent therapeutic outcome (HB grading I-II) and the difference was statistically significant (p=0.000). Conclusion: Facial nerve paralysis surgical management after bone fracture is controversial. The decision must be taken according to the type of paralysis and the radiological electrophysiological and evolutionary data

    Fertility sparing surgery for stage Ic ovarian cancer: An eight case series

    Introduction. One case out of 3 cases of primary ovarian cancer is diagnosed at an early stage. Almost 15% of ovarian cancers are diagnosed in women who are within the range of procreation age, raising the debate on the feasibility of fertility-sparing surgery (FSS) for those tumors. In the case the patient is admitted with stage 1A or 1B in most histologic types, it is still controversial in stage 1C. Results and Discussions. This is an eight-patient-case series. The median age is 27 years old (ranging from 18 to 42). The main clinical manifestation was pelvic pain (87.5%). Four patients (50%) presented with tumor torsion. All patients underwent FSS; five of them (62.5%) had conservative staging, two (25%) underwent salpingooophorectomy and one (12.5%) ovariectomy, type histo + stade Figo. The chemotherapeutic treatment was administered to four patients (50%) following surgery without major side effects. One patient (12.5%) presented a loco regional recurrence that occurred 14 months later. She was treated with combined chemotherapy and radical surgery. One patient out of eight (12.5%) achieved a natural pregnancy after the treatment and another one (12.5%) was submitted to an assisted reproductive procedure which did not result in a viable pregnancy. Conclusion. FSS seems to be a suitable approach for stage 1C ovarian cancer. However, more case series and meta-analyses should be conducted

    Psychosocial Effects and Use of Communication Technologies during Home Confinement in the First Wave of the COVID-19 Pandemic in Italy and The Netherlands

    (1) Background: The COVID-19 pandemic forced people from all around the globe to strongly modify their daily routines, putting a significant strain on the social aspects of daily lives. While the first wave of the pandemic was a very challenging time in all countries, it is still uncertain whether various lockdown intensities and infection rates differed regarding their psychosocial impact. This work therefore aimed to investigate (i) the psychosocial effects of home confinement in two European countries that underwent different lockdown intensities: Italy and the Netherlands and (ii) the role of communication technology in relation to feelings of loneliness. (2) Methods: A cross-sectional online survey inquiring about different psychosocial variables and the use of and satisfaction towards communication technology was circulated among the general public during the first wave of the COVID-19 pandemic. In total, 629 participants (66% female, 68% from the Netherlands) answered each question twice, referring to “before” and “during” the pandemic. (3) Results: We found significant negative effects of COVID-19 home confinement on depressive feelings (p < 0.001, %∆ = +54%), loneliness (p < 0.001, %∆ = +37.3%), life satisfaction (p < 0.001, %∆ = −19.8%) and mental wellbeing (p < 0.001, %∆ = −10.6%) which were accompanied with a significantly increased need for psychosocial support (p < 0.001, %∆ = +17.3%). However, the magnitude of psychosocial impact did not significantly differ between residents undergoing a more intense (Italy) versus a less intense (Netherlands) lockdown, although the decrease in social participation was found to be significantly different for both countries (z = −7.714, p < 0.001). Furthermore, our findings demonstrate that the increase in loneliness was associated with the adoption of new digital communication tools (r = 0.21, p < 0.001), and significantly higher for individuals who started to adopt at least one new digital communication tool during confinement than for those who did not (z = −4.252, p < 0.001). (4) Conclusions: This study highlights that, although COVID-19 home confinement significantly impacted psychosocial wellbeing during the first wave of the pandemic, this impact did not differ based on lockdown intensity. Recognizing the increasing adoption of digital communication technology in an attempt to reduce lockdown loneliness, future studies should investigate what is needed from the technology to achieve this effect

    Exploratory study of the acute and mid-term effects of using a novel dynamic meeting environment (AerisÂź) on cognitive performance and neurophysiological responses

    The purpose of the present study was to assess the acute and mid-term effects of the dynamic aerisÂź-meeting- environment on brain activity, cognitive performance, heart rate variability (HRV), sleepiness, mental workload (EEG-MWI), as well as local experienced discomfort (LED) in healthy adults. Twenty-four healthy adults (16 females, age: 25.2 ± 3.1 years old) were randomly assigned to either the control (i.e., conventional meeting environment, CG) or experimental (AerisÂź dynamic meeting-environment, DG) group with a 1:1 allocation. Participants reported to the laboratory on two test sessions separated by a 2-week intervention period (5 meetings of 90 min each week). Spontaneous resting EEG and HRV activities, as well as attentional (D2-R test) and vigilance (PVT) cognitive performances, sleepiness perceptions, and EEG-MWI, were recorded at the beginning of each test session and immediately following the 90-min meeting. The LED was measured pre- and post-intervention. The changes (Δ) from pre- to post-90 min meeting and from pre- to post- intervention were computed to further examine the acute and mid-term effects, respectively. Compared to the CG, the DG showed higher Δ (pre-post 90 min-meeting) in fronto-central beta (z = −2.41, p = 0.016, d = 1.10) and gamma (z = −2.34, p = 0.019, d = 0.94) frequencies at post-intervention. From pre- to post-intervention, only the DG group showed a significant increase in fronto-central gamma response (Δ) to the meeting session (z = −2.09, p = 0.04, d = 1.08). The acute use of the AerisÂź-meeting-environment during the 90-min meeting session seems to be supportive for (i) maintaining vigilance performance, as evidenced by the significant increase in N-lapses from pre- to post-90 min session only in the CG (p = 0.04, d = 0.99, Δ = 2.5 ± 3 lapses), and (ii) improving alertness, as evidenced by the lower sleepiness score (p = 0.05, d = −0.84) in DG compared to CG. The mid-term use of such an environment showed to blind the higher baseline values of EEG-MWI recorded in DG compared to CG (p = 0.01, d = 1.05) and may prevent lower-back discomfort (i.e., a significant increase only in CG with p = 0.05 and d = 0.78), suggesting a less mentally and physically exhausting meeting in this environment. There were no acute and/or mid-term effects of the dynamic meeting environment on any of the HRV parameters. These findings are of relevance in the field of neuroergonomics, as they give preliminary support to the advantages of meeting in a dynamic office compared to a static office environment

    Effects of pomegranate juice supplementation on oxidative stress biomarkers following weightlifting exercise

    The aim of this study was to test the hypothesis that pomegranate juice supplementation would blunt acute and delayed oxidative stress responses after a weightlifting training session. Nine elite weightlifters (21.0 ±1 years) performed two Olympic-Weightlifting sessions after ingesting either the placebo or pomegranate juice supplements. Venous blood samples were collected at rest and 3 min and 48 h after each session. Compared to the placebo condition, pomegranate juice supplementation attenuated the increase in malondialdehyde (-12.5%; p < 0.01) and enhanced the enzymatic (+8.6% for catalase and +6.8% for glutathione peroxidase; p < 0.05) and non-enzymatic (+12.6% for uric acid and +5.7% for total bilirubin; p < 0.01) antioxidant responses shortly (3 min) after completion of the training session. Additionally, during the 48 h recovery period, pomegranate juice supplementation accelerated (p < 0.05) the recovery kinetics of the malondialdehyde (5.6%) and the enzymatic antioxidant defenses compared to the placebo condition (9 to 10%). In conclusion, supplementation with pomegranate juice has the potential to attenuate oxidative stress by enhancing antioxidant responses assessed acutely and up to 48 h following an intensive weightlifting training session. Therefore, elite weightlifters might benefit from blunted oxidative stress responses following intensive weightlifting sessions, which could have implications for recovery between training sessions

    Effects of home confinement on mental health and lifestyle behaviours during the COVID-19 outbreak:insights from the ECLB-COVID19 multicentre study

    Although recognised as effective measures to curb the spread of the COVID-19 outbreak, social distancing and self-isolation have been suggested to generate a burden throughout the population. To provide scientific data to help identify risk factors for the psychosocial strain during the COVID-19 outbreak, an international cross-disciplinary online survey was circulated in April 2020. This report outlines the mental, emotional and behavioural consequences of COVID-19 home confinement. The ECLB-COVID19 electronic survey was designed by a steering group of multidisciplinary scientists, following a structured review of the literature. The survey was uploaded and shared on the Google online survey platform and was promoted by thirty-five research organizations from Europe, North Africa, Western Asia and the Americas. Questions were presented in a differential format with questions related to responses “before” and “during” the confinement period. 1047 replies (54% women) from Western Asia (36%), North Africa (40%), Europe (21%) and other continents (3%) were analysed. The COVID-19 home confinement evoked a negative effect on mental wellbeing and emotional status (P < 0.001; 0.43 ≀ d ≀ 0.65) with a greater proportion of individuals experiencing psychosocial and emotional disorders (+10% to +16.5%). These psychosocial tolls were associated with unhealthy lifestyle behaviours with a greater proportion of individuals experiencing (i) physical (+15.2%) and social (+71.2%) inactivity, (ii) poor sleep quality (+12.8%), (iii) unhealthy diet behaviours (+10%), and (iv) unemployment (6%). Conversely, participants demonstrated a greater use (+15%) of technology during the confinement period. These findings elucidate the risk of psychosocial strain during the COVID-19 home confinement period and provide a clear remit for the urgent implementation of technology-based intervention to foster an Active and Healthy Confinement Lifestyle AHCL)

    Globally altered sleep patterns and physical activity levels by confinement in 5056 individuals:ECLB COVID-19 international online survey

    Symptoms of psychological distress and disorder have been widely reported in people under quarantine during the COVID-19 pandemic; in addition to severe disruption of peoples' daily activity and sleep patterns. This study investigates the association between physical-activity levels and sleep patterns in quarantined individuals. An international Google online survey was launched in April 6th, 2020 for 12-weeks. Forty-one research organizations from Europe, North-Africa, Western-Asia, and the Americas promoted the survey through their networks to the general society, which was made available in 14 languages. The survey was presented in a differential format with questions related to responses "before" and "during" the confinement period. Participants responded to the Pittsburgh Sleep Quality Index (PSQI) questionnaire and the short form of the International Physical Activity Questionnaire. 5056 replies (59.4% female), from Europe (46.4%), Western-Asia (25.4%), America (14.8%) and North-Africa (13.3%) were analysed. The COVID-19 home confinement led to impaired sleep quality, as evidenced by the increase in the global PSQI score (4.37 +/- 2.71 before home confinement vs. 5.32 +/- 3.23 during home confinement) (p &lt; 0.001). The frequency of individuals experiencing a good sleep decreased from 61% (n = 3063) before home confinement to 48% (n = 2405) during home confinement with highly active individuals experienced better sleep quality (p &lt; 0.001) in both conditions. Time spent engaged in all physical-activity and the metabolic equivalent of task in each physical-activity category (i.e., vigorous, moderate, walking) decreased significantly during COVID-19 home confinement (p &lt; 0.001). The number of hours of daily-sitting increased by similar to 2 hours/days during home confinement (p &lt; 0.001). COVID-19 home confinement resulted in significantly negative alterations in sleep patterns and physical-activity levels. To maintain health during home confinement, physical-activity promotion and sleep hygiene education and support are strongly warranted.</p
