    La artroplastia de Neer en las fracturas complejas del húmero proximal

    Se estudian los resultados obtenidos después del tratamiento agudo de una serie de 32 pacientes diagnosticados de fractura en cuatro fragmentos del húmero proximal a los que se les implantó una hemiartroplastia tipo Neer I. Se llevó a cabo una evaluación clínica-radiológica de los pacientes y para la evaluación de los resultados se aplicaron los criterios de Neer. Los resultados globales obtenidos son de un 69% entre buenos y regulares y de un 31% con resultado malo. El anclaje de las tuberosidades concreta el mayor índice de complicaciones, que junto a otras totalizan un 18% de los casos. El éxito en dicho anclaje se ha correlacionado significativamente con el resultado funcional (p=0,0019). La técnica meticulosa junto con el adecuado protocolo rehabilitador se señalan como puntos esenciales para el éxito.We study the results obtained after acute treatment of a group of 32 patients diagnosed of four-fragment fracture of the proximal humerus in whom a Neer I hemiarthroplasty was used. The patients were assessed with clinic and radiological parameters, and for the evaluation of the results Neer criteria were used. The global results obtained were 69% of good and fair and 31% of bad results. The tuberosities attachment concentrate the great complication index, that together with other complications acconist for 18% of the cases. The success of the attachment had a positive correlation with functional results (p=0.0019). A meticulous technique joined with a good rehabilitation program is pointed as essential for being successful

    Phosphogypsum waste lime as a promising substitute of commercial limes : a rheological approach

    This paper presents the rheological properties of three types of lime putty, specifying the influence of their origin. The study aims to compare a special lime putty prepared from phosphogypsum with a commercial lime powder and an aged lime putty. The results obtained in terms of chemical composition, crystalline structure, grain size and rheological characterization, (linear viscoelasticity, shear rate and time-dependent flow behaviour) are presented in the study. Putties studied present a similar rheological response, which mainly depends on the particle size and water content. Lower values of the linear viscoelastic functions and viscosity were found for the phosphogypsum lime putty, in agreement with the higher particle size. Transient flow tests reveal a predominant elastic response with no significant shear-induced structural perturbations. However, either a thickening phenomenon over time, i.e. rheopexy, favoured at low shear rates, or a viscosity decrease, i.e. thixotropy, favoured at high shear rates, was observed.The authors would like to thank the aid of CITIUS at the University of Seville for the use of their laboratories for the characterization analyses.     Study of Phases Evolution in High-coercive MnAl Powders Obtained Through Short Milling Time of Gas-atomized Particles

    Gas-atomized Mn54Al46 particles constituted nominally of only ε- and γ2-phases, i.e. no content of the ferromagnetic L10-type τ-phase, have been used to study the evolution of phases during short time of high-energy milling and subsequent annealing. Milling for 3 min is sufficient to begin formation of the τ-MnAl phase. A large coercivity of 4.9 kOe has been obtained in milled powder after annealing at 355 °C for 10 min. The large increase in coercivity, by comparison with the lower value of 1.8 kOe obtained for the starting material after the same annealing conditions, is attributed to the combined formation of the τ-MnAl and β-Mn phases and the creation of a very fine microstructure with grain sizes on the order of 20 nm.     Comportamiento agronómico de diferentes cvs. de alcachofa multiplicadas por semilla

    Se estudia el comportamiento agronómico y las características morfológicas de una colección de 12 cultivares (cvs) de alcachofa (Cynara scolymus) multiplicadas por semilla, comparadas con los cvs. Blanca de Tudela y Violeta de Provence, de multiplicación vegetativa. Los cvs. multiplicados por semilla se sembraron el 14 de junio de 2005, transplantando toda la experiencia el 1 de agosto. Todas las plantas fueron sometidas a tres tratamientos sucesivos con ácido giberélico con una concentración de 30 ppm. con abono foliar al 0,1%, aplicaciones que se realizaron los días 20 de septiembre, 5 y 20 de octubre. El mayor rendimiento comercial se obtuvo con el cv. Madrigal que fue también el más tardío. La mayor precocidad correspondió al cv. Blanca de Tudela, sin diferencias estadísticamente significativas (e.s.) en la producción acumulada en enero respecto a los cvs. A-106, Victoria y Lorca. El peso medio de los capítulos osciló entre los 184 y 183 g de los cvs. AR-1 y Madrigal durante el mes de marzo, frente a 97 y 99 g de los cvs. Victoria y Lorca, en ese mismo mes

    Breast cancer PAM50 signature: correlation and concordance between RNA-Seq and digital multiplexed gene expression technologies in a triple negative breast cancer series

    [Background]: Full RNA-Seq is a fundamental research tool for whole transcriptome analysis. However, it is too costly and time consuming to be used in routine clinical practice. We evaluated the transcript quantification agreement between RNA-Seq and a digital multiplexed gene expression platform, and the subtype call after running the PAM50 assay in a series of breast cancer patients classified as triple negative by IHC/FISH. The goal of this study is to analyze the concordance between both expression platforms overall, and for calling PAM50 triple negative breast cancer intrinsic subtypes in particular.[Results]: The analyses were performed in paraffin-embedded tissues from 96 patients recruited in a multicenter, prospective, non-randomized neoadjuvant triple negative breast cancer trial (NCT01560663). Pre-treatment core biopsies were obtained following clinical practice guidelines and conserved as FFPE for further RNA extraction. PAM50 was performed on both digital multiplexed gene expression and RNA-Seq platforms. Subtype assignment was based on the nearest centroid classification following this procedure for both platforms and it was concordant on 96% of the cases (N = 96). In four cases, digital multiplexed gene expression analysis and RNA-Seq were discordant. The Spearman correlation to each of the centroids and the risk of recurrence were above 0.89 in both platforms while the agreement on Proliferation Score reached up to 0.97. In addition, 82% of the individual PAM50 genes showed a correlation coefficient > 0.80.[Conclusions]: In our analysis, the subtype calling in most of the samples was concordant in both platforms and the potential discordances had reduced clinical implications in terms of prognosis.     Safety study of transcranial static magnetic field stimulation (tSMS) of the human cortex

    Transcranial static magnetic field stimulation (tSMS) in humans reduces cortical excitability. Objective: The objective of this study was to determine if prolonged tSMS (2 h) could be delivered safely in humans. Safety limits for this technique have not been described. Methods: tSMS was applied for 2 h with a cylindric magnet on the occiput of 17 healthy subjects. We assessed tSMS-related safety aspects at tissue level by measuring levels of neuron-specific enolase (NSE,a marker of neuronal damage) and S100 (a marker of glial reactivity and damage). We also included an evaluation of cognitive side effects by using a battery of visuomotor and cognitive tests. Results: tSMS did not induce any significant increase in NSE or S100. No cognitive alteration was detected. Conclusions: Our data indicate that the application of tSMS is safe in healthy human subjects, at least within these parameter

    Health-related quality of life with palbociclib plus endocrine therapy versus capecitabine in postmenopausal patients with hormone receptor–positive metastatic breast cancer: Patient-reported outcomes in the PEARL study

    Background: The PEARL study showed that palbociclib plus endocrine therapy (palbociclib/ET) was not superior to capecitabine in improving progression-free survival in postmenopausal patients with metastatic breast cancer resistant to aromatase inhibitors, but was better tolerated. This analysis compared patient-reported outcomes. Patients and methods: The PEARL quality of life (QoL) population comprised 537 patients, 268 randomised to palbociclib/ET (exemestane or fulvestrant) and 269 to capecitabine. Patients completed the European Organisation for Research and Treatment of Cancer QLQ-C30 and QLQ-BR23 and EQ-5D-3L questionnaires. Changes from the baseline and time to deterioration (TTD) were analysed using linear mixed-effect and stratified Cox regression models, respectively. Results: Questionnaire completion rate was high and similar between treatment arms. Significant differences were observed in the mean change in global health status (GHS)/QoL scores from the baseline to cycle 3 (2.9 for palbociclib/ET vs. -2.1 for capecitabine (95% confidence interval [CI], 1.4–8.6; P = 0.007). The median TTD in GHS/QoL was 8.3 months for palbociclib/ET versus 5.3 months for capecitabine (adjusted hazard ratio, 0.70; 95% CI, 0.55–0.89; P = 0.003). Similar improvements for palbociclib/ET were also seen for other scales as physical, role, cognitive, social functioning, fatigue, nausea/vomiting and appetite loss. No differences were observed between the treatment arms in change from the baseline in any item of the EQ-5D-L3 questionnaire as per the overall index score and visual analogue scale. Conclusion: Patients receiving palbociclib/ET experienced a significant delay in deterioration of GHS/QoL and several functional and symptom scales compared with capecitabine, providing additional evidence that palbociclib/ET is better tolerated.
