THE ETIOLOGY AND MANAGEMENT IN SYMPTOMATIC PSYCHOSES

Psihoze simptomatice includ stări psihice, care apar în timpul sau în urma unor boli somatice, intoxicații, infecții. Conform conceptului lui K. Bonhoeffer, creierul răspunde la pericolele exogene cu 5 tipuri de reacții nespecifice: delirium, tulburări crepusculare, amenție, oneiroid, halucinoză. Scopul lucrării. Elucidarea etiologiei și dezvoltării psihozelor simptomatice și managementul psihiatric în psihozele simptomatice. Material și metode. Studiul a fost realizat în urma unei analize teoretică și a sintezei bazată pe bibliografia oficială (77 de surse). Materialul a fost prezentat de un spectru de probleme psihiatrice ce privește asocierea cu patologii de diverse origini: somatice, neinfecțioase, infecțioase, neurologice, endocrinologice, vasculare, intoxicații. Rezultate. Etiologia corelează și este direct proporțională cu trei factori principali: maladii somatice, boli infecțioase, neinfecțioase. Psihozele simptomatice rezultă în urma acumulării unui bagaj de factori de risc în instalarea acestora, descriși ca fiind: expunerea încontinuu a organismului la stres, substanțe chimice nocive și atitudinea indiferentă a indivizilor referitor la propria sănătate. Managementul și tratamentul psihozelor simptomatice depind în totalmente de patologia de bază diagnosticată anterior, urmat ulterior de psihofarmacologia conform sindrom/simptom. Abordarea pacientului trebuie să fie bio-psiho-socială, dar inițierea tratamentului depinde de etiologia psihozei. Concluzii. Școlarizarea populației despre sănătatea mentală, presupune diagnosticarea precoce și stoparea dezvoltării psihozelor simptomatice. Managementul trebuie să fie elaborate în mai multe etape: tratamentul maladiei primare, tratamentul simptomelor psihiatrice, psihoterapia și reîncadrarea în societate.Background. Symptomatic psychoses include mental states, which appear during or following some diseases, intoxications, infections. The concept of Bonhoeffer K. defined that the brain responds to exogenous dangers with 5 types of non-specific reactions: delirium, crepuscular disorders, amnetia, oneiroid, hallucinosis. Objective of the study. Elucidation of the etiology of the development of symptomatic psychoses and psychiatric management in symptomatic psychoses. Material and methods. The study was carried out following a theoretical analysis based on the official bibliography (77 sources). The material was presented by a spectrum of psychiatric problems in association with pathologies of various origins: somatic, non-infectious, infectious, neurological, endocrinological, vascular, intoxications. Results. The etiology correlates to three main factors: somatic diseases, infectious and non-infectious diseases. Symptomatic psychoses result from the accumulation of favorable factors in their installation, described as: continuous exposure of the body to stress, harmful chemicals, and the indifferent attitude of individuals regarding their own health. The management and treatment of symptomatic psychoses depend entirely on the previously diagnosed pathology followed later by psychopharmacology according to syndrome/symptom. The approach to the patient must be bio-psycho-social, but the initiation of treatment depends on the etiology of the psychosis. Conclusion. Educating the population about mental health would lead both to early diagnosis and to stopping the development of symptomatic psychoses. The management must be developed in several stages: treatment of the primary disease, of psychiatric symptoms, psychotherapy, and reintegration into society
Why is a noble metal catalytically active? The role of the O-Ag interaction in the function of silver as an oxidation catalyst
Extensive density-functional theory calculations, and taking into account
temperature and pressure, affords a comprehensive picture of the behavior and
interaction of oxygen and Ag(111), and provides valuable insight into the
function of silver as an oxidation catalyst. The obtained phase-diagram reveals
the most stable species present in a given environment and thus identifies (and
excludes) possibly active oxygen species. In particular, for the conditions of
ethylene epoxidation, a thin oxide-like structure is most stable, suggesting
that such atomic O species are actuating the catalysis, in contrast to hitherto

MENTAL SYMPTOMS IN PARKINSON’S DISEASE, MANAGEMENT AND TREATMENT

Articolul este dedicat simptomelor psihiatrice comorbide în boala Parkinson, specifice de debut, dezvoltarea tabloului clinic, diagnosticul, prognosticul și tratamentul specific. Boala Parkinson este o boală neurodegenerativă relativ frecventă, cu o prezentare clinică specifică. Scopul lucrării. Studiul prezentării clinice și neuropsihiatrice în boala Parkinson, problemele de diagnostic. Prognosticul, managementul și tratamentul. Material și metode. Cercetarea a fost bazată pe studiile și analizele din sursele științifice (PubMed, Medscape, Academie Nationale de Medicine, Google academic). Pentru studierea temei au fost luate în considerație doar articolele publicate în ultimii 23 de ani. Rezultate. Tabloul clinic se caracterizează printr-o triadă clasică, simptome neuropsihiatrice grupate în: Non-motorii: patologia afectivă (anxietate, stări obsesiv-compulsive, apatie, anhedonie, depresie), tulburări de somn, comportament impulsiv, simptome somatice (tulburări de urinare sau constipație), manifestări cutanate, tulburări autonome (hipotensiune ortostatică, disfuncție erectilă și transpirație excesivă), cognitive (demență, dereglarea atenției, concentrației, memoriei), de gândire, de conștiință (delirium), de percepție: halucinații, iluzii, parestezii, cenestopatii, dezorientare, stări psihotice; Psihomotorii și neurologice: disfagie, dizartrie, disfonie, algii, tulburări olfactive, excitație psihomotorie. Concluzii. Tulburările comportamentale, neurotiforme și psihice, motorie și non-motorie în boala Parkinson se răsfrâng direct asupra calității vieții pacienților și familiilor. S-a efectuat analiza manifestărilor clinice comorbide neuropsihiatrice, managementul și metodele specifice de tratament.Background. The article is dedicated to comorbid psychiatric symptoms in Parkinson’s disease, specific onset, development of the clinical picture, diagnosis, prognosis, and specific treatment. Parkinson’s disease is a relatively common neurodegenerative disease with a specific clinical presentation. Objective of the study. Study of clinical and neuropsychiatric presentation in Parkinson’s disease, diagnostic problems. Prognosis, management, and treatment. Material and methods. The research was based on studies and analyzes from scientific sources (PubMed, Medscape, National Academy of Medicine, Google academic). For the study of the theme, only articles published in the last 23 years were considered. Results. The clinical picture is characterized by a classic triad, neuropsychiatric symptoms grouped in: Non-engines: affective pathology (anxiety, obsessive-compulsive states, apathy, anhedonia, depression), sleep disorders, impulsive behavior, somatic symptoms (urinary disorders or constipation), skin manifestations, autonomic disorders (orthostatic hypotension, erectile dysfunction and excessive sweating), cognitive (dementia, dysregulation of attention, concentration, memory), of thinking, of consciousness (delirium), of perception: hallucinations, illusions, paraesthesias, cenestopathies, disorientation, psychotic states; Psychomotor and neurological: dysphagia, dysarthria, dysphonia, algae, olfactory disorders, psychomotor excitement. Conclusion. Behavioral, neurotiform and mental, motor and non-motor disorders in Parkinson’s disease directly affect the quality of life of patients and families. Mental disorders in meningitis and encephalitis
Introducere. Meningita și encefalita de diferită etiologie
au adesea tulburări neuropsihiatrice, modificări psihice patologice
în perioada acută și în perioada cu consecințe pe
termen lung. Sunt tratați de specialiști în boli infecțioase,
neurologi, terapeuți, dar prezența simptomelor mentale necesită
implicarea unui psihiatru. Scopul lucrării. Studiul tabloului
clinic neuropsihiatric comorbid și consecințelor tulburărilor
psihice acute și cronice în meningită și encefalită.
Managementul psihiatric în bolile infecțioase ale creierului
în meningită și encefalită. Material și metode. S-au căutat
surse în baze de date: PubMed, MedScape, Google-scholar.
Au fost selectate 60 de surse cu etiopatogenia tulburărilor
neuropsihiatrice în meningită și encefalită, criteriile de diagnostic.
O caracteristică de selecție a fost manifestările comorbide
neuropsihiatrice în meningoencefalită. Sunt incluse
articole, rezumate și studii publicate în ultimii 23 de ani
în limba engleză. Rezultate. Meningoencefalita poate provoca
simptomatologie neuropsihiatrică. Sunt posibile: convulsii,
tulburări motorii, afective, calitative și cantitative ale
conștiinței, uneori patologie ireversibilă a cogniției, echivalente
psihosomatice, patologia activității sistemului nervos
central și autonom. Starea de conștiență se poate manifesta
prin somnolență ușoară sau poate ajunge la stări severe
de comă sau delirium. Pot fi asociate simptome psihiatrice
grave, tulburări afective severe, de comportament, deficite
cognitive, episoade psihotice sau anomalii ale sistemului
autonom. Recunoașterea tardivă a encefalitei și meningitei
duce la riscuri potențial fatale, consecințe neuropsihiatrice
întârziate și prognostic nefavorabil. Concluzii. Sunt enumerați
factorii etiologici ai meningoencefalitei, mecanismele
patogenetice ale manifestărilor neuropsihice: infecția țesutului
cerebral, dereglarea SNV, furtunile de citokine, hipoxie,
fenomene psihoneurologice ale meningocefalitei.Introduction. Meningitis and encephalitis of various etiologies
often have neuropsychiatric disorders, pathological
mental changes in the acute period and in the long-term
consequences. Specialists in infectious diseases, neurologists,
therapists, treat them but the presence of mental
symptoms requires the involvement of a psychiatrist. The
purpose of the work. Study of the comorbid neuropsychiatric
clinical picture and the consequences of acute and
chronic mental disorders in meningitis and encephalitis.
Psychiatric management of infectious brain diseases in meningitis
and encephalitis. Material and methods. Sources
were searched in databases: PubMed, MedScape, Google-scholar.
60 sources were selected with etiopathogenesis, neuropsychiatric
disorders in meningitis and encephalitis,
diagnostic criteria. A selection feature was comorbid neuropsychiatric
manifestations in meningoencephalitis. Articles,
abstracts, and studies published in the last 23 years
in English are included. Results. Meningoencephalitis can
cause neuropsychiatric symptoms. Possible: convulsions,
motor, affective, qualitative and quantitative disorders of
consciousness, sometimes irreversible pathology of cognition,
psychosomatic equivalents, pathology of the activity
of the central and autonomic nervous system. The state of
consciousness can be manifested by mild drowsiness or can
reach severe states of coma or delirium. Serious psychiatric
symptoms, severe affective and behavioral disorders, cognitive
deficits, psychotic episodes, or abnormalities of the
autonomic system may be associated. Late recognition of
encephalitis and meningitis leads to potentially fatal risks,
delayed neuropsychiatric sequelae, and poor prognosis.
Conclusion. The etiological factors of meningoencephalitis,
the pathogenetic mechanisms of neuropsychic manifestations
are listed: brain tissue infection, SNS deregulation, cytokine
storms, hypoxia, psychoneurological phenomena of
meningocephalitis
Synthesis and Properties of Ag / CuO / SiO2 Nanocomposites
In this paper the composites Ag / SiO2 with regularly distributed in bulk matrix silver and copper oxide nanoparticles were synthesized. Herewith, copper ions was introduced into porous support at the stage of sol-gel synthesis. Sample Ag / CuO / SiO2 was tested by the catalytic reaction of CO oxidation and com-pared with Ag / SiO2. It was revealed that sample with introduced copper show lower activity .This fact can be explained by formation of silver cuprate during preparation of composite Ag / CuO / SiO2. Treatment by reaction mixture (CO and O2) led to release of silver in ionic, clusters and metal states that increased cata-lytic activity of the composite.

Thermal Behavior of Fluorinated Double-Walled Carbon Nanotubes
Double-walled carbon nanotubes (DWNTs), produced by a catalytic chemical vapor deposition method, have been fluorinated using a volatile mixture of BrF3 and Br2. Optical absorption spectroscopic study on the product detected nonfluorinated nanotubes, which could correspond to the inner walls of DWNTs. The fluorinated DWNTs have been annealed in vacuum at fixed temperatures, and X-ray photoelectron spectroscopy showed almost no fluorine in the sample heated to 300 °C. Comparison between X-ray
fluorescent C KR spectra of the pristine DWNT sample and the annealed fluorinated sample revealed change of the atomic structure of graphitic shells in the process of thermal defluorination

Catedra Psihiatrie, Narcologie şi Psihologie Medicală USMF ” Nicolae TestemiţanuThis research paper approaches the problem of mental disorders uith puerperal.It defines
the transitional state, which shows, as a rule, evolutionary states still have a benign prognosis
and is constant in evnvironment 80% of cases (baby chevies).Depressions, including psychotic
states may have an evolutionary continuity; have a specific picture in terms of clinical and
subsequent relapse of bipolar affective or catatonic state. Predisposing factors are listed for the
emergence of postpartum mental disorders including family history, first birth , young age, and
stressors as triggering mechanism of the hypothalamus-pituitary-adrenal, hormonal changes
after childbirth and onset of mental disorders.
În lucrare se abordează problema tulburărilor psihice asociate cu puerperiul. Se definesc
stări tranzitorii, care nu prezinta, ca regulă, stări evolutive în continuare, au un pronostic benign
şi se constată în mediu 80 % cazuri ( baby chevies ). Stările depresive , inclusiv stările psihotice
pot avea o continuitate evolutivă, au un tablou specific clinic şi recăderi ulterioare sub aspectul
unor stări catatonice sau afective bipolare. Sunt enumeraţi factori predispozanţi pentru apariţia
tulburărilor psihice după naştere inclusiv antecedente heredocolaterale, prima naştere, vîrsta
tânără, factori stresanţi prin declanşarea mecanismului hipotalamus-hipofiza-suprarenale,
schimbarea profilului hormonal după naştere şi declanşarea tulburărilor psihice
