    Logistics process improvement of Kapikule border crossing

It is obvious that, any decrease in trade time is expected to realize positive impact on the logistics competitiveness of a specific country. In order to reduce time across all dimensions of the border process, the improvement of physical infrastructure and the proliferation of procedures are the most important issues that should be focused on. The aim of the study is to improve the Kapikule border crossing by reducing the process time. For this purpose, first, in-depth interviews were conducted with the Kapikule customs gate authorities in order to reveal explicitly the process flows. Subsequently, the current situation at the gate was simulated using the Arena simulation software. Finally, several improvement scenarios were tested to identify the ones that will have the highest impact on reducing the queues and the delays at the border

    Who are Sleeping in Sleep Laboratory? A Retrospective Study

    OBJECTIVE: Aim of this study is to compare the results of gold standard in diagnosing sleep disorders polysomnography (PSG) with the physician’s preliminary diagnosis and complaints of patients in our data of sleep laboratory. METHODS: 656 patients who made PSG were included in the study. All of the patients age, gender, comorbid chronic disease, complaints, preliminary diagnosis and PSG diagnosis were evaluated retrospectively. RESULTS: In our study, the average age of patients was 56 and 43% women 57% were male. Complaints of patients were snoring, fatigue, stopped breath during sleep, insomnia, headache, daytime sleepiness, restless legs and abnormal behaviors during sleep. According to preliminary diagnoses and PSG diagnoses; Obstructive Sleep Apnea Syndrome (OSAS), narcolepsy, REM behavior disorder (RBD) reduced (p 0.05). Sleep disorders in patients was accompanied by chronic diseases, hypertension (34.3%), diabetes (12.8%), Chronic Obstructive Pulmonary Disease (COPD) (1.2%), epilepsy (1.8%), Parkinson's disease (3.5%), dementia (3.2%), depression (18.4%), cardiovascular disease (13.3%) and cerebrovascular disease (4.9%). 9%). CONCLUSION: We are offering a good clinical history and physical examination with the correct interpretation of PSG for the differential diagnosis can be made true, accurate diagnoses and appropriate treatment modalities in our patients

    Malignant Transformation of Parotid Gland Non-sebaceous Lymphadenoma: Case Report and Review of Literature

    Non-sebaceous lymphadenoma is a sporadic benign tumor of salivary glands. Histopathologic and immunohistochemical properties, diagnostic criteria, and theories for the histologic origin of the disease have been defined and well-discussed in the literature. However, none of the cases showed malignant transformation to date. We reported a case of 54 years old female patient with a right preauricular mass. Magnetic resonance imaging demonstrated a 2 cm, well-defined contrast-enhanced mass in the right parotid gland. Fine needle aspiration cytology was undiagnostic but suspicious for malignancy. Total parotidectomy with facial nerve preservation was done. In the histopathological examination, non-sebaceous lymphadenoma regions and malignant cells with abundant cytoplasm, large vesicular nuclei, and prominent nucleoli, which occupied approximately 70% of the mass, were seen. The diagnosis was undifferentiated carcinoma arisen from non-sebaceous lymphadenoma. Adjuvant radiotherapy was given. No recurrence was detected during ten months of follow-up. This case is the first case of a malignancy developed from non-sebaceous lymphadenoma

    Assessment of general health status and hematological parameters usually precedes the use of invasive diagnostic and therapeutic procedures in critically ill patients. Angiography can be effective and safe as a substitute for major surgical procedures, or as a bridging therapy in such cases. We present a critically ill patient with hemolytic anemia that underwent splenic artery embolization as a bridging therapy.     A Rare Adrenal Incidentaloma: Adrenal Schwannoma

    Adrenal schwannoma is an extremely uncommon cause of incidentaloma. It originates from neural sheath Schwann cells of the adrenal gland. We report the case of a left adrenal schwannoma incidentally discovered in a 32-year-old woman during examination of bloated feeling and stomach ache. The patient was incidentally found to have a left adrenal mass of 9 cm on abdominal ultrasonography. Computed tomography (CT) of the abdomen and [18F] fluorodeoxyglucose positron emission tomography (PET) were also performed. Metabolic evaluation was unremarkable. Due to the large size of the tumor, left adrenalectomy was performed. The postoperative course was uneventful. Histological examination established the diagnosis of schwannoma. This diagnosis was supported by immunohistochemistry of S-100 and vimentin positivity. In conclusion, adrenal schwannoma is an extremely rare entity and can grow considerably in size. The present case report emphasizes that clinicians should be aware of the possibility of retroperitoneal schwannoma. Total excision of benign schwannoma is associated with a favorable outcome. To our knowledge, there are case reports of schwannoma with CT and magnetic resonance imaging findings in the literature, although this is the first schwannoma case with PET-CT imaging

    Malign melanoma of gall bladder: Case report

    Malign melanom is a disease affecting many organs but rarely seen in gallbladder. Distinction of isolated tumors from primary gallbladder tumors or metastatic disease can not be made in most cases. Mainly the complaints of acute cholecystitis appears. Mostly this clinical condition is not doubted when the primary tumor is not found. In cases with poor prognosis, very few patients benefit from surgery. The role of surgery is limited in malign melanoma of gallbladder because of low experience. Different surgical procedures should be applied to primary gallbladder melanoma and metastatic disease. Patients should be evaluated in a multidisciplinary manner with new therapeutic methods. We report here on an unusual case of gallbladder melanoma that was diagnosed by pathological examination in 46-year-old woman
