    Health Assessment as a Socio-material Process:: A Praxeography of Public Community Mental Healthcare Services in Berlin

    Historically, social security has been related to medical knowledge in many ways. For instance, medical expertise is incorporated into the granting and assessment of welfare state support measures and social policy fields of application shape medical knowledge practice. This paper deals with health assessment practices in Berlin community mental healthcare institutions. Based on a praxeographic approach and ethnographic research conducted between 2011 and 2017, two characteristics of the assessment process are elaborated: on the one hand, the assessment is distributed over different persons, forms of knowledge and artefacts as well as over time. On the other, it focuses on the beneficiary as a case and tailors the assessment of the person and his or her need for support to existing care services.The aim of this contribution is to first locate health assessment processes as situated and contingent knowledge practices in the context of welfare state support measures and second to clarify their effects on the establishment of living conditions of psychiatric service users.  Historically, social security has been related to medical knowledge in many ways.     Distributing Reflexivity through Co-laborative Ethnography

    In ethnographic research and analysis, reflexivity is vital to achieving constant coordination between field and concept work. However, it has been conceptualized predominantly as an ethnographer’s individual mental capacity. In this article, we draw on ten years of experience in conducting research together with partners from social psychiatry and mental health care across different research projects. We unfold three modes of achieving reflexivity co-laboratively: contrasting and discussing disciplinary concepts in interdisciplinary working groups and feedback workshops; joint data interpretation and writing; and participating in political agenda setting.     Examining Cardiomyocyte Dysfunction Using Acute Chemical Induction of an Ageing Phenotype

    Much effort is focussed on understanding the structural and functional changes in the heart that underlie age-dependent deterioration of cardiac performance. Longitudinal studies, using aged animals, have pinpointed changes occurring to the contractile myocytes within the heart. However, whilst longitudinal studies are important, other experimental approaches are being advanced that can recapitulate the phenotypic changes seen during ageing. This study investigated the induction of an ageing cardiomyocyte phenotypic change by incubation of cells with hydroxyurea for several days ex vivo. Hydroxyurea incubation has been demonstrated to phenocopy age- and senescence-induced changes in neurons, but its utility for ageing studies with cardiac cells has not been examined. Incubation of neonatal rat ventricular myocytes with hydroxyurea for up to 7 days replicated specific aspects of cardiac ageing including reduced systolic calcium responses, increased alternans and a lesser ability of the cells to follow electrical pacing. Additional functional and structural changes were observed within the myocytes that pointed to ageing-like remodelling, including lipofuscin granule accumulation, reduced mitochondrial membrane potential, increased production of reactive oxygen species, and altered ultrastructure, such as mitochondria with disrupted cristae and disorganised myofibres. These data highlight the utility of alternative approaches for exploring cellular ageing whilst avoiding the costs and co-morbid factors that can affect longitudinal studies

    Video enhancement using adaptive spatio-temporal connective filter and piecewise mapping

    This paper presents a novel video enhancement system based on an adaptive spatio-temporal connective (ASTC) noise filter and an adaptive piecewise mapping function (APMF). For ill-exposed videos or those with much noise, we first introduce a novel local image statistic to identify impulse noise pixels, and then incorporate it into the classical bilateral filter to form ASTC, aiming to reduce the mixture of the most two common types of noises - Gaussian and impulse noises in spatial and temporal directions. After noise removal, we enhance the video contrast with APMF based on the statistical information of frame segmentation results. The experiment results demonstrate that, for diverse low-quality videos corrupted by mixed noise, underexposure, overexposure, or any mixture of the above, the proposed system can automatically produce satisfactory results

    Why is the tropical cyclone boundary layer not "well-mixed"?

    Plausible diagnostics for the top of the tropical cyclone boundary layer include (i) the top of the layer of strong frictional inflow and (ii) the top of the “well-mixed” layer; that is, the layer over which potential temperature θ is approximately constant. Observations show that these two candidate definitions give markedly different results in practice, with the inflow layer being roughly twice the depth of the layer of nearly constant θ. Here, we will present an analysis of the thermodynamics of the tropical cyclone boundary layer derived from an axisymmetric model. We show that the marked dry static stability in the upper part of the inflow layer is due largely to diabatic effects. The radial wind varies strongly with height, and therefore so does radial advection of θ. This process also stabilizes the boundary layer, but to a lesser degree than diabatic effects. We also show that this differential vertical advection contributes to the observed superadiabatic layer adjacent to the ocean surface, where the vertical gradient of the radial wind is reversed, but that the main cause of this unstable layer is heating from turbulent dissipation. The top of the “well-mixed” layer is thus distinct from the top of the boundary layer in tropical cyclones. The top of the inflow layer is a better proxy for the top of the boundary layer, but is not without limitations. These results may have implications for boundary-layer parameterisations that diagnose the boundary layer depth from thermodynamic, or partly thermodynamic, criteria

    Direct measurement of the muonic content of extensive air showers between 2× 10\u3csup\u3e17\u3c/sup\u3e and 2×\u3csup\u3e1018\u3c/sup\u3e eV at the Pierre Auger Observatory

    The hybrid design of the Pierre Auger Observatory allows for the measurement of the properties of extensive air showers initiated by ultra-high energy cosmic rays with unprecedented precision. By using an array of prototype underground muon detectors, we have performed the first direct measurement, by the Auger Collaboration, of the muon content of air showers between 2 × 10 17 and 2 × 10 18 eV. We have studied the energy evolution of the attenuation-corrected muon density, and compared it to predictions from air shower simulations. The observed densities are found to be larger than those predicted by models. We quantify this discrepancy by combining the measurements from the muon detector with those from the Auger fluorescence detector at 1017.5eV and 1018eV. We find that, for the models to explain the data, an increase in the muon density of 38 % ± 4 % (12 %) ±18%21% for EPOS-LHC, and of 50 % (53 %) ± 4 % (13 %) ±20%23% for QGSJetII-04, is respectively needed

    A combined fit of energy spectrum, shower depth distribution and arrival directions to constrain astrophysical models of UHECR sources

    The combined fit of the measured energy spectrum and distribution of depths of shower maximum of ultra-high-energy cosmic rays is known to constrain the parameters of astrophysical scenarios with homogeneous source distributions. Further measurements show that the cosmic-ray arrival directions agree better with the directions and fluxes of catalogs of starburst galaxies and active galactic nuclei than with isotropy. Here, we present a novel combination of both analyses. For that, a three-dimensional universe model containing a nearby source population and a homogeneous background source distribution is built, and its parameters are adapted using a combined fit of the energy spectrum, depth of shower maximum distribution and energy-dependent arrival directions. The model takes into account a symmetric magnetic field blurring, source evolution and interactions during propagation. We use simulated data, which resemble measurements of the Pierre Auger Observatory, to evaluate the method’s sensitivity. With this, we are able to verify that the source parameters as well as the fraction of events from the nearby source population and the size of the magnetic field blurring are determined correctly, and that the data is described by the fitted model including the catalog sources with their respective fluxes and three-dimensional positions. We demonstrate that by combining all three measurements we reach the sensitivity necessary to discriminate between the catalogs of starburst galaxies and active galactic nuclei

    Presence of intestinal Mycobacterium avium subspecies paratuberculosis (MAP) DNA is not associated with altered MMP expression in ulcerative colitis

    <p>Abstract</p> <p>Background</p> <p><it>Mycobacterium avium </it>subspecies <it>paratuberculosis </it>(MAP) is suspected to be a causative agent in human Crohn's disease (CD). Recent evidence suggests that pathogenic mycobacteria and MAP can induce the expression of Matrix Metalloproteinases (MMP), which are the main proteases in the pathogenesis of mucosal ulcerations in inflammatory bowel disease (IBD). Within this study we assessed the prevalence of intestinal MAP specific DNA in patients with Crohn's disease, ulcerative colitis (UC), and healthy controls. We further analysed regulation patterns of MMPs in mucosal tissues of UC patients with and without intestinal MAP DNA detection.</p> <p>Methods</p> <p>Colonic biopsy samples were obtained from 63 Norwegian and German IBD patients and 21 healthy controls. RNA was quantified by quantitative real-time polymerase chain reaction (PCR) to study MMP gene expression in both pathological and healthy mucosal specimens. The presence of MAP DNA in colonic mucosa was examined using MAP specific PCR.</p> <p>Results</p> <p>MAP DNA was detected in 20% of UC patients and 33% of healthy controls but only in 7% of patients with CD. UC patients treated with corticosteroids exhibited a significantly increased frequency of intestinal MAP DNA compared to those not receiving corticosteroids. Expression of MMP-1, -2, -7, -9, -13, -19, -28 and TNF-α did not differ between UC patients with presence of intestinal MAP DNA compared to those without. MMP-2, MMP-9 and MMP-13 were significantly decreased in UC patients receiving corticosteroids.</p> <p>Conclusions</p> <p>The presence of intestinal MAP specific DNA is not associated with altered MMP expression in UC <it>in vivo</it>. Corticosteroids are associated with increased detection of intestinal MAP DNA and decreased expression of certain MMPs. Frequent detection of MAP DNA in healthy controls might be attributable to the wide environmental distribution of MAP and its presence in the food-chain.</p
