Socioeconomic inequalities in food outlet access through an online food delivery service in England: A cross-sectional descriptive analysis.
Online food delivery services facilitate 'online' access to food outlets selling food prepared away-from-home. Online food outlet access has not previously been investigated in England or across an entire country. Systematic differences in online food outlet access could exacerbate existing health inequalities, which is a public health concern. However, this is not known. Across postcode districts in England (nĀ =Ā 2118), we identified and described the number of food outlets and unique cuisine types accessible online from the market leader (Just Eat). We investigated associations with area-level deprivation using adjusted negative binomial regression models. We also compared the number of food outlets accessible online with the number physically accessible in the neighbourhood (1600m Euclidean buffers of postcode district geographic centroids) and investigated associations with deprivation using an adjusted general linear model. For each outcome, we predicted means and 95% confidence intervals. In November 2019, 29,232 food outlets were registered to accept orders online. Overall, the median number of food outlets accessible online per postcode district was 63.5 (IQR; 16.0-156.0). For the number of food outlets accessible online as a percentage of the number accessible within the neighbourhood, the median was 63.4% (IQR; 35.6-96.5). Analysis using negative binomial regression showed that online food outlet access was highest in the most deprived postcode districts (nĀ =Ā 106.1; 95% CI: 91.9, 120.3). The number of food outlets accessible online as a percentage of those accessible within the neighbourhood was highest in the least deprived postcode districts (nĀ =Ā 86.2%; 95% CI: 78.6, 93.7). In England, online food outlet access is socioeconomically patterned. Further research is required to understand how online food outlet access is related to using online food delivery services

The effect of autonomous and controlled motivation on selfācontrol performance and the acute cortisol response
Autonomously regulated self-control typically does not reduce over time as much, compared with self-control underpinned by controlled motivation. The proposed study tested whether an acute stress response is implicated in this process. Utilizing a framework grounded in self-determination theory, this study examined whether participants' motivational regulation would influence repeated self-control performance and acute stress levels, measured by the stress hormone cortisol. A single-blind randomized experimental design incorporating two motivational conditions (autonomous regulation and controlled regulation) tested these hypotheses. Participants (female = 28; male = 11; Mage = 22.33) performed three sequential self-control tasks; a modified Stroop task followed by two "wall sit" postural persistence tasks. Salivary cortisol was measured at baseline and after each of the wall sits. A repeated measures ANCOVA unexpectedly revealed that participants in the controlled regulation condition recorded greater wall sit performance in the first and second wall sits, compared with the autonomous condition. A repeated measures ANCOVA also revealed a significant quadratic interaction for cortisol. Controlled regulation was associated with an increase, and autonomous regulation condition a decrease, in cortisol that subsided at timepoint two. Results imply autonomous motivation facilitates an adaptive stress response. Performance on the self-control tasks was contrary to expectations, but may reflect short-term performance benefits of controlled motivation
The relationship between multidimensional motivation and endocrine-related responses: a systematic review
Multidimensional motivational theories postulate that the type of motivation is as important as the quantity of motivation, with implications for human functioning and well-being. An extensive amount of research has explored how constructs contained within these theories relate to the activation of the endocrine system. However, research is fragmented across several theories, and determining the current state of the science is complicated. In line with contemporary trends for theoretical integration, this systematic review aims to evaluate the association between multidimensional motivational constructs and endocrine-related responses to determine which theories are commonly used and what inferences can be made. Forty-one studies were identified incorporating five distinct motivation theories and multiple endocrine-related responses. There was evidence across several theories that high-quality motivation attenuated the cortisol response in evaluative environments. There was also evidence that motivational needs for power and affiliation were associated with lower and higher levels of salivary immunoglobulin A, respectively. The need for power may play a role in increasing testosterone when winning a contest; however, this evidence was not conclusive. Overall, this review can shape the future integration of motivational theories by characterizing the nature of physiological responses to motivational processes and examining the implications for well-being
A novel rare event approach to measure the randomness and concentration of road accidents
BACKGROUND:
Road accidents are one of the main causes of death around the world and yet, from a timespace
perspective, they are a rare event. To help us prevent accidents, a metric to determine
the level of concentration of road accidents in a city could aid us to determine whether
most of the accidents are constrained in a small number of places (hence, the environment
plays a leading role) or whether accidents are dispersed over a city as a whole (hence, the
driver has the biggest influence).
METHODS:
Here, we apply a new metric, the Rare Event Concentration Coefficient (RECC), to measure
the concentration of road accidents based on a mixture model applied to the counts of road
accidents over a discretised space. A test application of a tessellation of the space and
mixture model is shown using two types of road accident data: an urban environment
recorded in London between 2005 and 2014 and a motorway environment recorded in
Mexico between 2015 and 2016.
FINDINGS:
In terms of their concentration, about 5% of the road junctions are the site of 50% of the
accidents while around 80% of the road junctions expect close to zero accidents. Accidents
which occur in regions with a high accident rate can be considered to have a strong component
related to the environment and therefore changes, such as a road intervention or a
change in the speed limit, might be introduced and their impact measured by changes to the
RECC metric. This new procedure helps us identify regions with a high accident rate and
determine whether the observed number of road accidents at a road junction has decreased
over time and hence track structural changes in the road accident settings
Symbolic play in congenitally blind children
There is controversy over the existence and nature of blind children's limitations in symbolic play. In this study we tested 13 5- to 9-year-old congenitally blind children for the ability to symbolize when an adult provided scaffolding for their play. The blind children were selected on the basis that they did not have the syndrome of autism, and they comprised two groups matched for age (MA) and verbal ability on the Wechsler Intelligence Scale for Children who contrasted in their ability to engage in social relations. We also tested a group of sighted children who were MA matched with the more socially able blind children. On the basis of an hypothesis about the social-developmental sources of symbolic play deficits in congenitally blind children, we predicted differences between the socially able and socially impaired groups of blind children in the following respects: the attribution of symbolic meanings to play materials, the ascription of individual roles to play figures, and the anchorage of play in the scenario as presented by the adult. The results accorded with these predictions. Whereas the more socially able blind children showed symbolic play that was very similar to that of sighted children, the MA- and IQ-matched socially impaired blind children were limited in the three aspects of their play. On the other hand, even the socially impaired children showed instances of symbolizing. The findings suggest a way to reconcile conflicting reports of symbolic play deficits in young blind children, and may be relevant for explaining the association between autism and congenital blindness
Temporal and spatial analysis of the media spotlight
Items featured in the news usually have a particular novelty or describe events which result in severe impact. Here, the length of time that a story remains in the media spotlight is investigated as well as the scaling with population size of the amount of attention that the media gives to stories from different cities. Based on Twitter feeds, the media coverage from the major online newspapers in Mexico is analysed over a period either side of a recent powerful earthquake. The amount of coverage given to earthquake-related stories had an initial peak and then exhibited an exponential decay, dropping by half every eight days. Furthermore, the coverage per person usually exhibits a superlinear scaling with population size, so that stories about larger cities are more likely to appear in the news. However, during the immediate post-earthquake weeks, the scaling was no longer superlinear. The observed trends can be interpreted as a fundamental switch in the emergent collective behaviour of media producers and consumers

Which symptoms are linked to a delayed presentation among melanoma patients? A retrospective study
Background: The incidence of melanoma is rising. Early detection is associated with a more favourable outcome. The factors that influence the timing of a patientās presentation for medical assessment are not fully understood. The aims of the study were to measure the nature and duration of melanoma symptoms in a group of patients diagnosed with melanoma within the preceding 18 months and to identify the symptoms and barriers associated with a delay in presentation. Methods: A questionnaire was distributed to a random sample of 200 of the 963 melanoma patients who had participated in the Cancer Patient Experience Survey 2010 and were known to be alive 1 year later. Data were collected on symptoms, duration of symptoms prior to presentation and the reasons for not attending a doctor sooner. Results: A total of 159 patients responded to the questionnaire; 74 (47%) were men; mean age was 62 (range 24ā90) years. Of the 149 patients who reported a symptom, 40 (27%) had a delayed presentation (i.e. >3 months). A mole growing bigger was the most common symptom and reporting this symptom was significantly associated with a delayed presentation (odds ratio (OR) 2.04, 95% confidence interval (95% CI) 1.14ā5.08). Patients aged ā„65 years were less likely to report a barrier to presentation and were less likely to delay than those under 40, although this was of borderline statistical significance (OR 0.28, 95% CI 0.08ā1.00). Conclusions: This study highlights that an enlarging mole is a significant symptom influencing the timing of presentation. Increasing public awareness of the signs of melanoma and of the importance of early presentation is desirable. Health professionals should take advantage of the opportunity to educate patients on such symptoms and signs where feasible. Further exploration of the barriers to presentation in younger people should be considered
Polychlorinated Biphenyls and Biotransformation Enzymes in Three Species of Sea Turtles from the Baja California Peninsula of Mexico
Concentrations of polychlorinated biphenyls (PCBs) as well as the expression patterns of cytochrome P450 (CYP) enzymes and glutathione-S-transferase (GST) activities were measured in livers of loggerhead (Caretta caretta), green (Chelonia mydas), and olive ridley (Lepidocheyls olivacea) sea turtles from the Baja California peninsula of Mexico. The mean concentrations of total PCBs were 18.1, 10.5, and 15.2Ā ng/g wet weight (ww) respectively for the three species and PCB 153 was the dominant congener in all samples. Total PCB concentrations were dominated by penta- and hexa-chlorinated biphenyls. The mean estimated TEQs were 42.8, 22.9, and 10.4Ā pg/g (ww) for loggerhead, green, and olive ridley, respectively, and more than 70% was accounted for by non-ortho PCBs. Western blots revealed the presence of hepatic microsomal proteins that cross-reacted with anti-CYP2K1 and anti-CYP3A27 antibodies but not with anti-CYP1A antibody. There were no significant differences in GST activities between species. Grouping congeners based on structureāactivity relationships for CYP isoenzymes suggested limited activity of CYP1A contribution to PCB biotransformation in sea turtles. These results suggest potential accumulation of PCBs that are CYP1A substrates and provide evidence for biotransformation capacity, which differs from known animal models, highlighting the need for further studies in reptiles, particularly those threatened with extinction
