    Bu araştırmanın genel amacı literatürdeki bilgilerden yola çıkarak müzik terapi uygulamasının kişilerin stres, mental iyi oluş, olumlu ve olumsuz duygudurumlarına olan etkisini gözlemlemektir. Araştırmada 27 kontrol ve 28 deney olmak üzere toplamda 58 katılımcıya Edvar Müzik Terapi yöntemi uygulanmıştır. Algılanan Stres Ölçeği, Warwick-Edinburg mental İyi Oluş Ölçeği ve PANAS Olumlu/Olumsuz Duygudurum Ölçeği uygulama öncesi ve uygulama sonrası ön-test ve son-test şeklinde alınarak gerçek deneysel desen kullanılmıştır. Verilerin analiz aşamasında SPSS 22 paket programı kullanılarak bağımlı ve bağımsız örneklem t testi (student’s-t testi) gerçekleştirilmiştir. Analiz sonuçlarına göre deney grubundaki katılımcıların stres ve olumsuz duygu durum düzeyinde azalma; mental iyi oluş ve olumlu duygu durum düzeyinde ise artış görülmüştür. Katılımcıların stres ve olumsuz duygu durum düzeyindeki azalmanın istatistiksel olarak anlamlı olduğu bulgusu elde edilmiştir.The purpose of this recearch is investigating the effect of music therapy treatment on participants stress, mental well being, positive/negative mood level based on the knowledge of literature. There are totally 58 participants were investigated whose are 27 person control group and 28 person experiment group. In our research, we used the Perceived Stress Scale, Warwick-Edinburg Mental Well Being Scale, and Positive and Negative Affect Schedule to measure the dependent variables. SPSS 22 program is used to analyze the data and statistically important difference is observed in participants stress level and negative mood. By this finding, we might say that the music therapy which is used to flourish participants psychological level is found influential by paralel with the literature

    Dokuz Tip Mizaç Ölçeği'nin Geçerlik ve Güvenirliği

    The aim of this study is to develop a scale compatible with the Nine Types Temperament Model (NTTM), which did not have any prior measurement tools to scientifically prove its reliability and validity. NTTM is created by re-evaluating the Enneagram System –a system that defines nine personality types- used for analyzing and comprehending ego mechanisms. Nine Types Temperament Scale (NTTS) which is a self-rated instrument composed of 91 items with three-point Likert type was developed from this model and applied to 990 participants. Confirmatory factor analyses were carried out in order to evaluate whether the scale fits to the model related to the temperament model. In exploratory factor analyses of the scale eigen values of nine factors vary between 8.089 and 1.661, and represent 39.04% of the total variance. In confirmatory analyses of the scale CFI value is 0.88, GFI value is 0.845, IFI value is 0.88 and RMSEA value is 0.054. Test-retest reliability of the scale was evaluated with 46 participants. Cronbach alpha value of the whole scale is 0.75, while Cronbach alpha values for every temperament type were calculated as 0.77, 0.79, 0.68, 0.71, 0.80, 0.74, 0.71, 0.83 and 0.77 respectively. Concurrent validity was performed with Cloninger's TCI (Temperament and Character Inventory) and Akiskal's TEMPS-A (Temperament Evaluation of Memphis, Pisa, Paris and San Diego Autoquestionnaire Version). The types of NTTM showed significant correlations with TCI and TEMPS-A. Results of the study support that NTTS is a reliable and valid scale.Bu çalışmada, benlik mekanizmalarını dokuz kişilik tipi ile açıklayan Enneagram Sistemi'nin yeniden yorumlanması ile oluşturulan ve günümüzde geçerlik ve güvenirliği bilimsel açıdan kanıtlanmış bir ölçüm aracı bulunmayan Dokuz Tip Mizaç Modeli (DTMM) ile uyumlu bir ölçek geliştirilmesi amaçlanmıştır. 91 maddeden oluşan ve üçlü Likert tipi bir öz bildirim ölçeği olan Dokuz Tip Mizaç Ölçeği (DTMÖ) 990 üniversite öğrencisine uygulanmıştır. Ölçeğe uygulanan açıklayıcı faktör analizlerinde dokuz faktörün öz değerleri 8.08 ve 1.66 arasında değişmekte ve toplam varyansın %39.04'ünü temsil etmektedir. Ölçeğin modele uygunluğunu saptamak için doğrulayıcı faktör analizi uygulanmıştır. Ölçeğin doğrulayıcı faktör analizi sonuçlarına göre CFI değeri 0.88, GFI değeri 0.845, IFI değeri 0.88 ve RMSEA değeri 0.054'tür. Ölçeğin test-tekrar test güvenirliği 46 katılımcıyla sınanmıştır. Ölçeğin tümü için Cronbach alfa değeri 0.75, tipler için sırasıyla 0.77, 0.79, 0.68, 0.71, 0.80, 0.74, 0.71, 0.83, 0.77’dir. Ölçeğin eş zamanlı geçerliği Cloninger'in MKE (Mizaç ve Karakter Envanteri) ve Akiskal'in TEMPSA( Temparement Evaluation of Memphis, Pisa, Paris and San Diego- Autoquestionnaire) ölçekleriyle sınanmıştır.     The comparison of local tumor control after microwave ablation, surgical resection and combined treatment for colorectal liver metastases

    Aim. We aimed to compare the local therapeutic efficiency of microwave ablation (MWA), surgical resection, and combined treatment, assess the outcomes, and identify predictive factors for local treatment response in colorectal liver metastases (CLMs). Methods. From March 2013 to September 2019, a total of 54 patients with 302 CLMs were enrolled in this retrospective study. Eleven patients (20.4%) were treated with MWA, 9 patients (16.7%) with surgery, and 34 patients (63%) with the combined method. Univariate and multivariate analyses were performed to investigate overall survival (OS) and hepatic progression-free survival (HPFS) using the Cox proportional hazard regression model. The logistic regression analysis was used to identify the predictive factors for the local treatment response. Results. Total treatment response was achieved in 46.3% (n=25) of the patients. Local tumor progression was seen in 7.4% (n=4) of the patients, and the rate of intrahepatic distal recurrence was 46.3% (n=25). There were no significant differences in HPFS and OS between the three groups (p=0.56 and 0.90, respectively). Younger age

    Factors Affecting Optimal Titration Pressure of Continuous Positive Airway Pressure Device in Patients with Obstructive Sleep Apnea Syndrome

    Objective: To assess the effects of anatomical, clinical parameters, and pulmonary respiratory function on the therapeutic titration pressure of continuous positive airway pressure (CPAP) device in obstructive sleep apnea syndrome (OSAS).Methods: The study comprised 41 OSAS patients whose optimum CPAP titration pressures were measured. Each patient underwent an otorhinolaryngologic and thoracic examination, and data was recorded for height, weight, body mass index, neck-waist circumferences, Mallampati classification, tonsillar hypertrophy, hypopharyngeal collapse, soft palate-tongue base obstruction scores, peak nasal inspiratory flow and acoustic rhinometry measures, and CPAP device therapeutic pressures. Forced vital capacity, forced expiratory volume, FEV1/FVC ratio and peak expiratory flow values were noted.Results: Median CPAP optimal pressure cut-off value was determined as 9 mmH2O. Statistical analysis was made in two groups as CPAP titration optimal pressure ≤9 and >9 mmH2O. In the optimal pressure >9 group, neck and waist circumferences, hypopharyngeal collapse score, retropalatal and retrolingual lateral wall collapse scores were significantly higher (p<0.05). In multivariate and univariate model analysis, neck and waist circumferences, hypopharyngeal collapse score, retropalatal and retrolingual lateral wall collapse scores were observed to be significant in predicting high and low pressures in univariate model.Conclusion: For the prediction of optimal CPAP titration pressure in OSAS treatment, wide neck and waist circumferences, high hypopharyngeal collapse score and retropalatal and retrolingual lateral wall collapse grades may be determinative

    Metastatik safra yolu kanseri olan yaşlı hastalarda optimal tedavi yaklaşımları ve prognostik faktörler

    Introduction: There is a lack of evidence of the outcomes in elderly patients advanced stage biliary tract cancer due to the patients aged over 65 years are less than 25% in many prospective trials. We designed a retrospective multicenter study to evaluate the factors affecting treatment and survival in elderly patients with advanced-stage biliary tract cancer. Materials and methods: A total of 116 patients with advanced stage biliary tract cancer aged ≥65 years were included, and the treatment responses, survival, and toxicity rates were evaluated with respect to age groups Results: There was no significant difference between age and response to treatment, survival, or toxicity. The median progression-free survival and overall survival were 5.3, and 11.8 months respectively. Multivariate analysis indicated that ECOG PS (p<0.001 CI95% 1.5-3.7) and PNI (p<0.001 CI 95% 0.14-0.41) were significant independent prognostic factors for PFS. The independent prognostic factors for OS were choice of frontline regimen, NLR and PNI (p=0.007 CI 95% 0.71 – 0.94, p=0.006 CI 95% 1.2 – 3.1, p=0.001 CI 95% 0.35 – 0.91, respectively). Discussion: This study confirms the general prognostic relevance of inflammatory parameters and the importance of frontline treatment in elderly patients with advanced-stage biliary tract tumors. Additionally, getting older does not indicate that treatment will be avoided or that they will have a worse prognosis and suffer from more toxicities.Giriş: 65 yaş üzeri hastaların klinik çalışmaların %25’inden daha azını oluşturması nedeniyle biliyer sistem kanseri olan ileri yaş hastaların yönetimi konusunda kanıt eksiği bulunmaktadır. Bu amaçla, metastatik safra yolu kanseri tanılı yaşlı hastalarda tedaviyi ve sağkalımı etkileyen faktörleri değerlendirmek için retrospektif çok merkezli bir çalışma tasarladık. Gereç ve yöntemler: Çalışmaya 65 yaş ve üzeri, ileri evre safra yolu kanseri tanısı almış, 116 hasta dahil edildi ve yaş gruplarına göre tedavi yanıtları, sağkalım ve toksisite oranları değerlendirildi. Bulgular: Median yaşa göre gruplandırılıdğında; yaş ile tedaviye yanıt, sağkalım, toksisite arasında anlamlı bir fark bulunmadı. Tüm populasyonda medyan progresyonsuz sağkalım (PSK) ve genel sağkalım (GSK) sırasıyla 5.3, 11.8 aydı. Multivariate analizde, PSK için bağımsız prognostik faktörler preformans durumu(ECOG PS) (p<0.001 CI95% 1.5-3.7) ve Prognostik nutrisyonel indek (PNI) (p<0.001 CI 95% 0.14-0.41) olarak bulundu. GSK için ise bağımsız prognostik faktörler, birinci sıra tedavi seçimi, Notrofil Lenfosit oranı (p=0,007 CI %95 0,71 – 0,94) ve PNI (p=0,001 CI %95 0,35 – 0,91) olarak bulundu.     Rehabilitation for Addicted Patients: Erenköy BAHAR Model

    Rehabilitation is any action taken on an individual who has lost their physical or mental capabilities due to a disease or accident to recover their health or improve their capabilities in physical, mental, psychological, social and economic terms within their limitations. Rehabilitation application is divided into medical, social, occupational and psychiatric rehabilitation categories. Main purpose of rehabilitation application for addicted patients is the cessation of drug use in order to begin dealing with the psychological, legal, economic, social and physical damages done of the patients. The purpose of this study is to share several rehabilitation models applied on addicted patients of several countries, and to provide up-to-date knowledge on the development of rehabilitation models applied on addicted patients in our country with a relatively topical application example. In the practice of rehabilitation for addicted patients, we share the capacity and procedural operations, two years of research data and the experiences of what we believe is an important model for our country, the Erenköy Bağımlı Hastalar İçin Rehabilitasyon (BAHAR) Center’s model, for their individualized and integrated approach in recovery. The Center’s rehabilitation programs are cascaded into adaptation program, 0 to 3, 3 to 6, and 6 to 12 months long programs. In Erenköy BAHAR Center, patients’ recoveries in medical and spiritual sense were observed; and positive developments in social standing, family relations and occupational and social roles were recorded. It is seen that this center’s treatment methods for addiction are crucial for individual’s general wellbeing and functionality recovery, thus giving rise to the thought of establishing similar centers in our country. In conclusion, rehabilitation for addicted patients in our country is an emerging field and it requires more effort

    Prognostic factors for lymph node negative stage I and IIA non-small cell lung cancer: Multicenter experiences

    Surgery is the only curative treatment for operable non-small lung cancer (NSCLC) and the importance of adjuvant chemotherapy for stage IB patients is unclear. Herein, we evaluated prognostic factors for survival and factors related with adjuvant treatment decisions for stage I and IIA NSCLC patients without lymph node metastasis. Materials and Methods: We retrospectively analyzed 302 patients who had undergone curative surgery for prognostic factors regarding survival and clinicopathological factors related to adjuvant chemotherapy. Results: Nearly 90% of the patients underwent lobectomy or pneumonectomy with mediastinal lymph node resection. For the others, wedge resection were performed. The patients were diagnosed as stage IA in 35%, IB in 49% and IIA in 17%. Histopathological type (p=0.02), tumor diameter (p=0.01) and stage (p<0.001) were found to be related to adjuvant chemotherapy decisions, while operation type, lypmhovascular invasion (LVI), grade and the presence of recurrence were important factors in predicting overall survival (OS), and operation type, tumor size greater than 4 cm, T stage, LVI, and visceral pleural invasion were related with disease free survival (DFS). Multivariate analysis showed operation type (p<0.001, hazard ratio (HR):1.91) and the presence of recurrence (p<0.001, HR:0.007) were independent prognostic factors for OS, as well visceral pleural invasion (p=0.01, HR:0.57) and LVI (p=0.004, HR:0.57) for DFS. Conclusions: Although adjuvant chemotherapy is standard for early stage lymph node positive NSCLC, it has less clear importance in stage I and IIA patients without lymph node metastasis
