Ischemic Stroke of the Artery of Percheron with Normal Initial MRI: A Case Report
The artery of Percheron is a solitary trunk representing an uncommon anatomic variant that provides bilateral arterial supply to the paramedian thalami and the rostral midbrain. Occlusion of this artery results in bilateral thalamic and mesencephalic infarctions. The clinical diagnosis is difficult because the complex anatomy causes large clinical variability. We report a case of a comatose patient with normal early head-computed tomography and magnetic resonance imaging. A bilateral paramedian thalamic infarct due to an occlusion of the artery of Percheron was revealed two days later by a new head computed tomography. To our knowledge, this is the first report in the literature of a symptomatic patient presenting an acute Percheron stroke with normal early brain magnetic resonance imaging. Our case indicates that a normal initial magnetic resonance imaging cannot formally eliminate the diagnosis of acute stroke of the artery of Percheron. We discuss the causes of noncontributive brain magnetic resonance imaging at the onset of this acute Percheron stroke and the alternative diagnosis and therapy methods

Du XIXĂšme siĂšcle Ă nos jours, la rĂ©flexion sur ce que sont les animaux sâest enrichie dâapports scientifiques : thĂ©orie de lâĂ©volution, Ă©thologie, neurophysiologie, sciences cognitives. Mais la conscience animale reste toujours lâobjet de dĂ©bats importants dans la communautĂ© scientifique. Ainsi en 2012 un groupe de scientifiques de premier plan a Ă©prouvĂ© la nĂ©cessitĂ© de publier un manifeste intitulĂ© « DĂ©claration de Cambridge sur la Conscience », qui Ă©nonce quâ«âŠune convergence de preuves indique que les animaux non humains disposent des substrats neuro-anatomiques, neurochimiques et neurophysiologiques des Ă©tats conscients ainsi que la capacitĂ© dâexprimer des comportements intentionnels...».Les connaissances actuelles, dont cette expertise collective propose une synthĂšse, montrent que les animaux possĂšdent un large Ă©ventail de capacitĂ©s cognitives associĂ©es Ă des comportements plus ou moins complexes. Les formes de conscience Ă©tudiĂ©es chez les humains supposent des capacitĂ©s cognitives distinctes que lâon retrouve chez certains animaux. Peut-on en postuler que ceux-ci ont des formes de consciences Ă©quivalentes Ă celles de lâhomme, sans ĂȘtre forcĂ©ment identiques ?LâĂ©tude des niveaux et des contenus de la conscience chez les animaux est en passe de devenir un enjeu scientifique important en raison de la complexitĂ© du sujet et des controverses quâil ne manquera pas de susciter. Enfin, les acquis scientifiques dans ce domaine invitent Ă reprendre les rĂ©flexions morales concernant les relations que les hommes entretiennent avec les animaux (et particuliĂšrement avec les animaux domestiques

En quelques années, les bibliothÚques françaises ont développé des services de questions-réponses en ligne. Beaucoup d'établissements en sont encore à la phase de réflexion, d'autres démarrent, des réseaux s'élaborent. Dans le contexte actuel du repérage, de la sélection et de la validation de l'information pléthorique disponible sur Internet, les services de questions-réponses en ligne ont un rÎle primordial à jouer. En effet, ils valorisent l'offre des bibliothÚques dans le contexte du développement des services aux publics et des services à distance. Leur mise en place interroge le professionnel sous de multiples facettes : organisation du travail, politique de services, choix des outils⊠Ces différents aspects sont abordés, s'appuyant sur des expériences françaises et étrangÚres, qu'elles soient locales, coopératives et/ou collaboratives. Un éventail d'offres existe aujourd'hui, selon les objectifs, les besoins, les publics, les moyens mis à disposition et les technologies choisies. Le plan s'articule autour de quatre parties pragmatiques et opératoires : construire le projet, développer un savoir-répondre, organiser et travailler au sein d'une équipe et, enfin, adapter le service à l'environnement et à la nature de ses publics. Cet ouvrage collectif, coordonné par Claire Nguyen, se veut un guide utile à tous les professionnels qui veulent créer, maintenir ou faire évoluer un service de questions-réponses à distance
Different Transcriptional Control of Metabolism and Extracellular Matrix in Visceral and Subcutaneous Fat of Obese and Rimonabant Treated Mice
BACKGROUND: The visceral (VAT) and subcutaneous (SCAT) adipose tissues play different roles in physiology and obesity. The molecular mechanisms underlying their expansion in obesity and following body weight reduction are poorly defined. METHODOLOGY: C57Bl/6 mice fed a high fat diet (HFD) for 6 months developed low, medium, or high body weight as compared to normal chow fed mice. Mice from each groups were then treated with the cannabinoid receptor 1 antagonist rimonabant or vehicle for 24 days to normalize their body weight. Transcriptomic data for visceral and subcutaneous adipose tissues from each group of mice were obtained and analyzed to identify: i) genes regulated by HFD irrespective of body weight, ii) genes whose expression correlated with body weight, iii) the biological processes activated in each tissue using gene set enrichment analysis (GSEA), iv) the transcriptional programs affected by rimonabant. PRINCIPAL FINDINGS: In VAT, "metabolic" genes encoding enzymes for lipid and steroid biosynthesis and glucose catabolism were down-regulated irrespective of body weight whereas "structure" genes controlling cell architecture and tissue remodeling had expression levels correlated with body weight. In SCAT, the identified "metabolic" and "structure" genes were mostly different from those identified in VAT and were regulated irrespective of body weight. GSEA indicated active adipogenesis in both tissues but a more prominent involvement of tissue stroma in VAT than in SCAT. Rimonabant treatment normalized most gene expression but further reduced oxidative phosphorylation gene expression in SCAT but not in VAT. CONCLUSION: VAT and SCAT show strikingly different gene expression programs in response to high fat diet and rimonabant treatment. Our results may lead to identification of therapeutic targets acting on specific fat depots to control obesity
Pulmonary contusion : physiopathological aspects and therapeutic consequences
Lâassociation lĂ©sionnelle dâune contusion pulmonaire et dâun Ă©tat de choc hĂ©morragique est frĂ©quente et constitue un rĂ©el chalenge thĂ©rapeutique. La prise en charge de ce choc va nĂ©cessiter une rĂ©animation hĂ©modynamique dans laquelle le remplissage vasculaire tient une place centrale. Mais dans ce contexte de poumon contus, il devra ĂȘtre raisonnĂ© car dĂ©lĂ©tĂšre sur le plan pulmonaire, notamment en terme d'oedĂšme et d'altĂ©ration de la compliance. Ce remplissage devra donc ĂȘtre titrĂ©, basĂ© sur des objectifs tensionnels clairs et un monitorage hĂ©modynamique fiable. L'utilisation de solutĂ©s Ă haut pouvoir d'expansion volĂ©mique (sĂ©rum salĂ© hypertonique, colloĂŻdes) prĂ©sente un intĂ©rĂȘt, de mĂȘme que l'introduction prĂ©coce de vasopresseurs. Le monitorage hĂ©modynamique permettra de conduire cette rĂ©animation sur des objectifs de pression artĂ©rielle, sur des indices de prĂ©charge dĂ©pendance et sur la mesure de l'eau pulmonaire extravasculaire. Notre travail, basĂ© sur des Ă©tudes expĂ©rimentales et cliniques, a pour objectif de caractĂ©riser les modalitĂ©s actuelles de prise en charge dâune contusion pulmonaire, sur les plans hĂ©modynamiques et respiratoires.Pulmonary contusion is often associated with hemorrhagic shock, constituting a challenge in trauma care. For patients who have sustained lung contusions, fluid resuscitation should be carefully performed, because injured lungs are particularly vulnerable to massive fluid infusions with an increased risk of pulmonary edema and compliance impairment. Fluid administration should be included in an optimized and goal directed resuscitation, based on blood pressure objectives and hemodynamical monitoring. The use of fluids with high volume-expanding capacities (hypertonic saline, colloids) is probably interesting, as well as early introduction of vasopressors. Hemodynamic monitoring will allow to conduct resuscitation on blood pressure objectives, on preload parameters and on extravascular lung water measurement.Our work, based on experimental and clinical studies, objective to characterize the current modalities of ventilatory and hemodynamical aspect of pulmonary contusion care
